The purpose of this study was to compare the performance of students exposed to two different instructional modalities for dental anatomy wax carving: CAI (computer-assisted instruction) using DVD technology, or traditional laboratory instruction. Students’ self-assessment scores were also compared to faculty scores, and students’ perceptions of their teaching modality were analyzed. Seventy-three first-year dental students (response rate 81 percent) participated in this randomized single blind trial, in which faculty graders were blinded to student group assignment. The dental profession is guided by normative principles that provide guidance to our leaders and practicing dentists in addressing the needs of patients and the profession, yet there is room for incorporating new ideas that help dental professionals meet their professional obligations. The purpose of this essay is to discuss the concept of “servant leadership,” especially in contrast with “self-serving leaders,” and to suggest that servant leadership is consistent with the high ethical and professional ideals of the dental profession. การรั่วซึมระดับไมครอนของสารยึดติดสามชนิดที่มีการปรับเปลี่ยนขั้นตอนการใช้งาน, ชุตินาฏ อินทกนก, ศิริวิมล ศรีสวัสดิ์

วัตถุประสงค์ เพื่อประเมินการรั่วซึมระดับไมครอนของโพรงฟันชนิดคลาสไฟว์ที่บูรณะด้วยเรซินคอมโพสิตเมื่อใช้ สารยึดติด 3 ชนิดตามคําแนะนําของบริษัทผู้ผลิต เปรียบเทียบกับเมื่อใช้สารยึดติดโดยการปรับวิธีการใช้ วัสดุและวิธีการ นําฟันกรามน้อยที่ถูกถอนของมนุษย์ 60 ปี กรอเตรียมโพรงฟันชนิดคลาสไฟว์ทําการบูรณะด้วย เรซินคอมโพสิต โดยสารยึดติดที่ใช้ ได้แก่ ออปติบอนด์เอฟแอล เคลียร์ฟิลเอสอีบอนด์ และเคลียร์ฟิลไตรเอสบอนด์ แบ่งเป็นกลุ่มที่ใช้ตามคําแนะนําของบริษัทผู้ผลิต เป็นกลุ่มควบคุมเปรียบเทียบกับกลุ่มที่ใช้สารยึดติด โดยมีการปรับ วิธีการใช้ หลังจากผ่านเครื่องเทอร์มอไซคลิง 5,000 รอบแล้วนํามาทดสอบการรั่วซึม ประเมินและวิเคราะห์ผลการ รั่วซึมด้วยสถิติทดสอบไคสแควร์ และวิลคอกซัน ชายน์ แรงค์ ที่ระดับความเชื่อมั่น 95% ผลการศึกษา เคลียร์ฟิลเอสอีบอนด์มีการรั่วซึมที่เคลือบฟันน้อยลงเมื่อเปรียบเทียบกับกลุ่มควบคุมเมื่อกัดผิว เคลือบฟันด้วยกรดฟอสฟอริกก่อนทาสารยึดติด แต่ไม่มีนัยสําคัญทางสถิติ เคลียร์ฟิลไตรเอสบอนด์ มีการรั่วซึมที่ เคลือบฟันลดลงอย่างมีนัยสําคัญเมื่อปรับเปลี่ยนวิธีการใช้งานเมื่อเปรียบเทียบกับกลุ่มควบคุม ออปติบอนด์เอฟแอล ที่ปรับเปลี่ยนวิธีการใช้งานมีการรั่วซึมที่เนื้อฟันน้อยลงอย่างมีนัยสําคัญเมื่อเปรียบเทียบกับกลุ่มควบคุม สรุป การใช้เคลียร์ฟิลเอสบอนด์ตามคําแนะนําของบริษัทผู้ผลิตเปรียบเทียบกับการใช้กรดฟอสฟอริกกัดที่ผิวเคลือบฟันมีประสิทธิภาพป้องกันการรั่วซึมที่เคลือบฟันไม่แตกต่างกัน ตรงข้ามกับเคลียร์ฟิลไตรเอสบอนด์ ซึ่งการรั่วซึมลดลงอย่างมีนัยสําคัญที่เคลือบฟันที่มีการใช้กรดกัดก่อน เปรียบเทียบกับกลุ่มที่ใช้ตามคําแนะนําของผู้ผลิตสําหรับออปติบอนด์เอฟแอล การปรับสภาพเนื้อฟันด้วยไพรม์เมอร์เพียงอย่างเดียวพบว่าลดการรั่วซึมที่เนื้อฟัน ลงอย่างมีนัยสําคัญ (ว ทันต จุฬาฯ 2552;32:23-38)

วัตถุประสงค์ เพื่อศึกษาอุบัติการณ์และรูปแบบความผิดปกติของขากรรไกรในผู้ป่วยไทยที่มีโครงสร้างกะโหลก ศีรษะประเภท II ในเพศชายและเพศหญิง รวมทั้งเปรียบเทียบความแตกต่างระหว่างเพศ วัสดุและวิธีการ กลุ่มตัวอย่างได้จากการคัดเลือกแบบเฉพาะเจาะจง จากผู้ที่มารับการรักษาทางทันตกรรมจัดฟัน ที่คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย จํานวน 100 คน (ชาย 50 หญิง 50 คน) อายุ 18-43 ปี มีค่ามุมเอเอ็นบีมากกว่าหรือเท่ากับ 6 องศา นําภาพรังสีวัดศีรษะด้านข้างก่อนการรักษาของกลุ่มตัวอย่างมา ลอกลาย วัดค่าที่แสดงลักษณะขากรรไกรบนและล่างในแนวหน้าหลัง ได้แก่ มุมเอสเอ็นเอ ความยาวขากรรไกร บน มุมแม็กซิลลารีเด็พ มุมเอสเอ็นบี ความยาวขากรรไกรล่าง มุมเฟเชียลเด็จ วัดค่าที่แสดงลักษณะในแนวดิ่ง ของโครงสร้างใบหน้าและขากรรไกร ได้แก่ มุมเฟเชียลแอ็กซิส มุมระนาบแฟรงค์ฟอร์ต ความสูงใบหน้าส่วนล่าง ความสูงใบหน้าส่วนหลัง มุมแมนดิบูลาอาร์ค นําค่าที่วัดได้เทียบกับค่าปกติของคนไทยที่ใช้ในคณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย เพื่อจําแนกตําแหน่งและความสัมพันธ์ของส่วนโค้งแนวฟันบนและล่างของกลุ่มตัวอย่าง เป็นสามกลุ่ม (โดยใช้การผ่านเกณฑ์ 2 ใน 3) ได้แก่ ตําแหน่งถอยหลัง ตําแหน่งปกติ และตําแหน่งยื่นไป ทางด้านหน้า ในแนวดิ่ง (โดยใช้การผ่านเกณฑ์ 3 ใน 5) จําแนกเป็นโครงสร้างกะโหลกศีรษะแบบสบลึก แบบสบ ปกติ และแบบสบเปิด หาอัตราการเกิดความผิดปกติของโครสร้างกะโหลกศีรษะชนิดต่างๆ และเปรียบเทียบความสัมพันธ์ระหว่างเพศกับลักษณะโครงสร้างโดยใช้ค่าสถิติไคสแควร์ ผลการศึกษา กลุ่มตัวอย่างมีโครงสร้างกะโหลกศีรษะในแนวหน้าหลังเป็นแบบขากรรไกรบนและขากรรไกรล่าง อยู่ในตําแหน่งปกติมากที่สุดทั้งในเพศชายและเพศหญิง (ร้อยละ 44 และ 46 ตามลําดับ) ลักษณะที่พบมากเป็น ลําดับสองในเพศชาย คือ ขากรรไกรบนยื่นร่วมกับขากรรไกรล่างอยู่ในตําแหน่งปกติ (ร้อยละ 24) ในเพศหญิง คือ ขากรรไกรบนอยู่ในตําแหน่งปกติร่วมกับขากรรไกรล่างอยู่ในตําแหน่งถอยหลัง (ร้อยละ 26) ลักษณะในแนวดิ่งพบ ว่ากลุ่มตัวอย่างมีลักษณะโครงสร้างกะโหลกศีรษะแบบสบปกติเป็นจํานวนมากที่สุด (ร้อยละ 46) ซึ่งใกล้เคียงกัน กับโครงสร้างกะโหลกศีรษะแบบสบเปิด (ร้อยละ 41) เมื่อพิจารณาความสัมพันธ์ระหว่างเพศกับลักษณะโครงสร้าง กะโหลกศีรษะทั้งแนวหน้าหลังและแนวดิ่งพบว่าไม่มีความแตกต่างอย่างมีนัยสําคัญทางสถิติ สรุป การศึกษาลักษณะโครงสร้างกะโหลกศีรษะประเภท II …

วัตถุประสงค์ เพื่อศึกษาสมบัติทางกลและทางกายภาพของอีลาสโตเมอริกลิเกเจอร์ที่เป็นผลิตภัณฑ์ทางการค้าและใช้ในงานทันตกรรมจัดฟัน วัสดุและวิธีการ การศึกษาสมบัติทางกลของอีลาสโตเมอริกลิเกเจอร์ที่เป็นผลิตภัณฑ์จากบริษัท 7 แห่ง โดยวัด ค่าแรงดึงของตัวอย่าง 20 วงต่อผลิตภัณฑ์ ณ เวลาเริ่มต้นและภายหลังจากแช่ในน้ํากลั่นที่ระยะเวลา 1 วัน 7 วัน และ 28 วัน ตามลําดับ ด้วยเครื่องวัดลอยด์ และประยุกต์วิธีการให้สอดคล้องกับมาตรฐานไอเอสโอ 21606 ทดสอบ ความแข็งผิวโดยวัดค่าความแข็งผิวของชิ้นทดสอบ 10 ชิ้นต่อผลิตภัณฑ์ก่อนและหลังแช่ในน้ํากลั่นที่ระยะเวลา 1 วัน 7 วัน และ 28 วัน ด้วยเครื่องวัดดูโรมิเตอร์แบบชอร์เอ และศึกษาสมบัติทางกายภาพของอีลาสโตเมอริกลิเกเจอร์ ในด้านการต้านทานต่อการติดสีของตัวอย่างผลิตภัณฑ์ โดยวัดค่าความต้านทานต่อการติดสีของชิ้นทดสอบก่อน และหลังแช่ในน้ําชาหรือกาแฟที่ระยะเวลา 1 ชั่วโมงและวันด้วยเครื่องสเปคโตโฟโตมิเตอร์เพื่อหาค่าความแตกต่างของสี หลังจากนั้นวิเคราะห์ผลค่าแรงดึงและความแข็งผิว โดยใช้สถิติการวิเคราะห์ความแปรปรวนทางเดียว และทดสอบ ความแตกต่างระหว่างค่าเฉลี่ยในแต่ละกลุ่มด้วยการเปรียบเทียบเชิงซ้อนชนิดบอนเฟร์โรนที่ระดับนัยสําคัญ 0.05 ผลการศึกษา ค่าแรงดึงของอีลาสโตเมอริกลิเกเจอร์ทุกกลุ่มหลังจากแช่ในน้ํากลั่นที่ระยะเวลา 1 วัน 7 วัน และ 28 วัน มีค่าต่ํากว่าค่าแรงดึงเริ่มต้นอย่างมีนัยสําคัญทางสถิติ (p < 0.05) โดยพบว่าที่ระยะเวลาการแช่ 1 วัน ค่าแรง ดึงจะลดลงประมาณร้อยละ 40 ถึง 60 ของแรงดึงเริ่มต้น แต่ค่าแรงดึงระหว่างทุกช่วงเวลาของการแช่ในน้ํากลั่น มีความแตกต่างกันอย่างไม่มีนัยสําคัญทางสถิติ (p > 0.05) ค่าความแข็งผิวแบบซอร์เอของชิ้นทดสอบระหว่างก่อน และหลังแช่ในน้ํากลั่นที่ระยะเวลาต่าง ๆ มีความแตกต่างกันอย่างไม่มีนัยสําคัญทางสถิติ (p > 0.05) ความต้านทาน ต่อการติดสีพบว่า ความแตกต่างของสีมีค่ามากกว่า 3.3 เมื่อชิ้นทดสอบผ่านการแช่น้ําชาหรือกาแฟเป็นเวลา 1 ชั่วโมง สรุป อีลาสโตเมอริกลิเกเจอร์มีค่าแรงดึงลดลงหลังจากแช่น้ํากลั่นเป็นเวลา 1 วัน แต่ค่าความแข็งผิวของชิ้น ทดสอบไม่เปลี่ยนแปลงทุกช่วงระยะเวลาของการแช่ในน้ํากลั่น นอกจากนี้ชิ้นทดสอบสามารถติดสีได้หลังจากแช่ใน น้ําชาหรือกาแฟที่ระยะเวลา 1 ชั่วโมง (ว ทันต จุฬาฯ 2552;32:11-22)

วัตถุประสงค์ ดัชนีแบบอย่างความเป็นวิชาชีพได้รับการประยุกต์สําหรับทันตแพทย์ไทย โดยผ่านกระบวนการ ปรับข้ามวัฒนธรรม การศึกษานี้มีวัตถุประสงค์เพื่อประเมินคุณสมบัติด้านความเที่ยงและความตรงของดัชนีฉบับภาษาไทย วัสดุและวิธีการ กลุ่มตัวอย่างชุด ก. ประกอบด้วยทันตแพทย์ที่ได้รับการสุ่มเลือกจํานวน 792 คน ดําเนินการเก็บ ข้อมูลด้วยแบบสอบถามทางไปรษณีย์ ประเมินความตรงเชิงปัจจัยด้วยเทคนิคการวิเคราะห์ปัจจัยเชิงสํารวจประเมินความตรงเชิงโครงสร้างด้วยวิธีความตรงร่วมกันของข้อความและวิธีความตรงแบ่งแยกของข้อความประเมินความเที่ยงประเภทความฟ้องภายในด้วยค่าสถิติครอนห์มาคอัลฟา และประเมินความเที่ยงประเภทแบ่ง ครึ่งด้วยสัมประสิทธิ์สเปียร์แมน-บราวน์ กลุ่มตัวอย่างชุด ข. ประกอบด้วยทันตแพทย์จํานวน 234 คน ที่ได้รับและ ส่งคืนแบบสอบถามซ้ํา เพื่อประเมินความเที่ยงประเภททดสอบซ้ําด้วยสัมประสิทธิ์สหสัมพันธ์ภายในชั้น ผลการศึกษา ผลการวิเคราะห์ปัจจัยเชิงสํารวจได้โครงสร้างปัจจัยประกอบด้วยมิติทั้ง 4 ด้าน ตามดัชนีต้นฉบับ ได้แก่ คุณวุฒิ หน้าที่รับผิดชอบ ความมีอิสระในการทํางาน และอํานาจแห่งตัวแทน รวมทั้งมิติที่ 5 ซึ่งเพิ่มขึ้นมา ในด้านคุณวุฒิ-หน้าที่รับผิดชอบ มิติส่วนใหญ่ของแบบจําลอง 5 ปัจจัยนี้มีความตรงร่วมกันของข้อความและ ความตรงแบ่งแยกของข้อความในระดับที่ดี ยกเว้นเพียงมิติด้านความมีอิสระในการทํางาน ผลการประเมิน ความพ้องภายในได้ค่าสถิติอัลฟาอยู่ระหว่าง 0.25-0.85 ค่าสัมประสิทธิ์สเปียร์แมน-บราวน์มีค่าระหว่าง 0.34-0.87 สัมประสิทธิ์สหสัมพันธ์ภายในชั้นของแต่ละข้อความมีค่าอยู่ระหว่าง 0.32-0.60 สรุป การประเมินคุณสมบัติด้านความเที่ยงและความตรง พบว่ามีระดับค่อนข้างต่ําอยู่บ้างในบางมิติของดัชนีโดยเฉพาะอย่างยิ่งดัชนีฉบับภาษาไทยยังมีโครงสร้างของปัจจัยไม่สอดคล้องตามดัชนีต้นฉบับ จึงสมควรต้องมีการพัฒนาคุณสมบัติทางการวัดผลของดัชนีต่อไป ก่อนที่จะนําไปใช้ประเมินความเป็นวิชาชีพของทันตแพทย์ไทยได้อย่างมีประสิทธิภาพ (2 ทันต จุฬาฯ 2552;32:53-68)

Oral manifestations such as candidiasis and hairy leukoplakia are common lesions found in patients infected with human immunodeficiency virus. Those who are on antiretroviral drugs may demonstrate various orofacial effects. Immune reconstitution reflects a patientûs improved immune response, but this may be accompanied by excessive response to previously exposed antigens, resulting in opportunistic infection. Common adverse effects of antiviral therapy include xerostomia and erythema multiforme, which are mostly related to use of protease inhibitors and nucleoside reverse transcriptase inhibitors. วัตถุประสงค์ เพื่อเปรียบเทียบระดับความเข้มข้นของไอระเหยของสารประกอบซัลเฟอร์ (วีเอสซี) ในช่องปากก่อน-หลังการแปรงฟันและลิ้นด้วยยาสีฟันแต่ละชนิด และเปรียบเทียบประสิทธิภาพในการลดกลิ่นปากระหว่างยาสีฟันสามชนิด วัสดุและวิธีการ กลุ่มตัวอย่างซึ่งปราศจากพยาธิสภาพในช่องปากและโรคทางระบบ 16 คน (ชาย 8 คน หญิง 8 คน อายุ 18-22 ปี) ได้รับการวัดระดับความเข้มข้นของวีเอสซี เพื่อหากลิ่นปากตอนเช้าด้วยเครื่องออริลโครมา จากนั้น ให้กลุ่มตัวอย่างทดลอง 4 ครั้ง โดยแปรงฟันและแปรงลิ้นด้วยยาสีฟัน 3 ชนิด (อภัยภูเบศร คอลเกตโททอล 12 ซิสเท็มมาดีปอิมแพค) และยาสีฟันหลอก 1 ชนิด แล้ววัดกลิ่นปากที่เวลา 0 30 60 90 และ 120 นาทีหลังการ แปรงฟันและลิ้น เปรียบเทียบระดับความเข้มข้นของวีเอสซีก่อน-หลังการแปรงฟันและลิ้น โดยใช้สถิติการทดสอบที่สําหรับกลุ่มตัวอย่างสองกลุ่มที่สัมพันธ์กัน หรือการทดสอบเชิงเครื่องหมายและลําดับที่แบบวิลคอกชน และเปรียบเทียบประสิทธิภาพในการลดกลิ่นปากระหว่างยาสีฟันทั้งสามชนิดกับยาสีฟันหลอก โดยใช้สถิติการวิเคราะห์ ความแปรปรวนทางเดียว การทดสอบความเท่ากันของค่าเฉลี่ยแบบโรบัสต์ หรือการทดสอบคริสตัล-วอลลิส ที่ ระดับนัยสําคัญ 0.05 ผลการทดลอง ระดับความเข้มข้นของวีเอสซีลดลงอย่างมีนัยสําคัญทางสถิติที่เวลา 0 30 60 90 และ 120 นาที หลังการแปรงฟันและลิ้นด้วยยาสีฟันแต่ละชนิด (p < 0.05) เมื่อเปรียบเทียบประสิทธิภาพระหว่างยาสีฟันทั้งสาม ชนิดกับยาสีฟันหลอก ไม่พบความแตกต่างอย่างมีนัยสําคัญทางสถิติ (p > 0.05) สรุป ยาสีฟันแต่ละชนิดมีประสิทธิภาพในการลดกลิ่นปากที่เวลา 0 30 60 90 และ 120 นาทีหลังการแปรงฟันและลิ้น ประสิทธิภาพในการลดกลิ่นปากระหว่างยาสีฟันทั้งสามชนิดกับยาสีฟันหลอกไม่แตกต่างกันอย่างมีนัยสําคัญทางสถิติ (ว ทันต จุฬาฯ 2552;32:1-10)

This handbook is the culmination of a multiphase, multidisciplinary research project that used grounded theory and participatory action research to illuminate ways that healthcare providers can work sensitively (in a trauma-informed way) with adult survivors of childhood sexual abuse.

The research identified nine Principles of Sensitive Practice: respect, rapport, taking time, sharing information and control, respecting boundaries, fostering mutual learning, understanding non-linear healing and demonstrating an understanding of trauma to patients. Specific guidelines were developed for a wide variety of issues pertinent to clinical practice such as, removal of clothing, touch, responding to disclosures of abuse, managing triggers among others. …
