Mineral trioxide aggregate (MTA) has been shown to promote regeneration of periradicular tissues when used as a root-end filling material. The purpose of this study was to compare the effect of fresh MTA with set MTA on hard tissue healing following periradicular surgery.

Under general anesthesia, the root canals of twenty-four mandibular premolars in four 2-year-old beagle dogs were filled with MTA. Two weeks later the root-ends of half of the samples were surgically exposed and resected to the level of set MTA within the canals. การติดตามและพัฒนาความรู้ทางการรักษา คลองรากฟันของทันตแพทย์ไทยกลุ่มหนึ่ง, วีระ เลิศจิราการ, ภควัตร ชาคริยานุโยค, วิชชา พิชิตพิเชษฐกุล, ชาญชัย สุนพคุณศรี

วัตถุประสงค์ เพื่อศึกษา เปรียบเทียบขั้นตอน ในการรักษาคลองรากฟันของทันตแพทย์ไทยกลุ่มหนึ่งที่ปฏิบัติอยู่ ในปัจจุบัน กับขั้นตอนที่ปฏิบัติขณะที่ศึกษาอยู่ในโรงเรียนทันตแพทย์ วัสดุและวิธีการ ส่งแบบสอบถาม 600 ชุด ไปยังกลุ่มทันตแพทย์ไทยทั่วประเทศโดยการสุ่ม (Convenience Sampling) จากรายชื่อสมาชิกของทันตแพทยสมาคมแห่งประเทศไทยฯ และทําการเปรียบเทียบข้อมูลในสมัยเรียน กับในปัจจุบันที่ปฏิบัติในทางคลินิก ผลการศึกษา พบว่าทันตแพทย์ในประเทศไทยกลุ่มที่ตอบแบบสอบถามมีวิธีการรักษาคลองรากฟันในปัจจุบัน แตกต่างจากวิธีการรักษาคลองรากฟันที่ใช้ปฏิบัติกันในขณะศึกษาอยู่ในโรงเรียนทันตแพทย์ในระดับปริญญาตรี อย่างมีนัยสําคัญ (P<0.001) ทั้งในขั้นตอนการล้างคลองรากฟันและการใส่ยาในคลองรากฟัน รวมทั้งพบว่ามี ทันตแพทย์ถึง 91.95% ซึ่งติดตามพัฒนาความรู้ใหม่ ๆ อยู่ตลอดเวลา สรุป ทันตแพทย์ในประเทศไทยกลุ่มที่ตอบแบบสอบถามมีการติดตามและพัฒนาความรู้อยู่ตลอดเวลา จากการฟัง บรรยายต่าง ๆ ซึ่งจัดโดยสมาคม ชมรม หรือมหาวิทยาลัยต่าง ๆ ซึ่งแสดงให้เห็นว่า การจัดอบรมระยะสั้น มีความ สําคัญต่อการพัฒนาความรู้ของทันตแพทย์ในปัจจุบัน

ฟันเดนซ์อีแวกจิเนทัสเป็นฟันที่มีความผิดปกติของผิวเคลือบฟัน ซึ่งมีลักษณะเป็นปุ่มนูนยื่นออกมาจาก ผิวเคลือบฟัน มักพบว่าเนื้อเยื่อในฟันตายเพราะปุ่มฟันหักทําให้ แบคทีเรียแทรกซึมเข้าไปได้ ก่อให้เกิดพยาธิสภาพ ที่ปลายรากฟันและอาจทําให้มีรอยโรคในช่องปากร่วมด้วย โดยทั่วไปมักจะเกิดในฟันที่ปลายรากฟันยังไม่ปิด ใน รายงานนี้แสดงผลการรักษาคลองรากฟันเดนซ์อีแวกจีเนทัสในฟันกรามน้อยที่ 2 ล่างขวาซึ่งมีรอยโรคในช่องปาก ร่วมด้วย โดยใช้ แคลเซียมไฮดรอกไซด์ เพื่อกระตุ้นให้มีการเสริมสร้างการปิดที่ปลายรากฟัน และอุดคลองรากฟัน ด้วยวิธี Lateral condensation จากการติดตามผลการรักษาเป็นเวลา 1 ปี 6 เดือน พบว่ามีการสร้างกระดูกที่บริเวณ ปลายรากฟัน ฟันใช้งานได้ดี ไม่มีรอยโรค หรือพยาธิสภาพเกิดขึ้นใหม่

Objective The purpose of this study was to compare the periodontal status of rural and urban Thai population who were at high risk in stress. Materials and Methods The cross-sectional study for severe periodontal destructive condition was carried out in over 35 years old Thai population. All first, second molars and central incisors of the total 1167 rural farmers from 4 villages in Payao province and 654 urban bank employees and secondary school teachers in Bangkok were examined using CPITN index. ความพึงพอใจของผู้ป่วยต่อประสิทธิภาพของฟันปลอมบางส่วนถอดได้ข้างเดียวชนิดโครงโลหะ, ร่าโพ โรจนกิจ

วัตถุประสงค์ เพื่อให้ทราบถึงความพึงพอใจของผู้ป่วยต่อประสิทธิภาพของฟันปลอมบางส่วนถอดได้ข้างเดียวชนิด โครงโลหะ ที่ใช้ทดแทนฟันกรามที่ขาดหายไป 1-2 ซี่ ว่าช่วยในการบดเคี้ยวได้เพียงใด และเป็นข้อมูลประกอบการ ตัดสินใจสําหรับทันตแพทย์ที่จะเลือกทําฟันปลอมชนิดนี้ให้แก่ผู้ป่วย วัสดุและวิธีการ จัดส่งแบบสอบถามทางไปรษณีย์ จํานวน 150 ชุด ให้กับผู้ป่วยที่ได้รับการใส่ฟันปลอมบางส่วน ถอดได้ข้างเดียวชนิดโครงโลหะ ที่ผู้วิจัยเป็นผู้ทําให้ แบบสอบถามแบ่งออกเป็น 4 ส่วน คือ ข้อมูลทั่วไปของผู้ตอบ แบบสอบถาม ข้อมูลทางทันตกรรมของผู้ตอบแบบสอบถาม ข้อมูลเกี่ยวกับความพึงพอใจต่อประสิทธิภาพของ ฟันปลอมที่ใช้อยู่ และสรุปความเห็นในการเลือกชนิดของฟันปลอมในอนาคตของผู้ตอบแบบสอบถาม โดยนํามา วิเคราะห์เป็นอัตราร้อยละ และหาความสัมพันธ์ของความพึงพอใจรวมทั้งหมดกับความพึงพอใจในด้านต่าง ๆ ของ ชิ้นฟันปลอมทางสถิติด้วยเปียร์สัน ไค สแควร์ (Pearson Chi-square test) ที่ระดับความเชื่อมั่นร้อยละ 95 (α = 0.05) ผลการศึกษา แบบสอบถามส่งกลับมาและสามารถนํามาวิเคราะห์ได้ 121 ชุด โดยระดับความรู้ของผู้ตอบแบบ สอบถามเฉลี่ยอยู่ในระดับปริญญาตรี มีอายุเฉลี่ย 50.5 ปี และรายได้เฉลี่ย 30,966 บาทต่อเดือน จํานวนชิ้นงาน ฟันปลอมบางส่วนถอดได้ข้างเดียวที่ผู้ตอบแบบสอบถามใส่อยู่มีทั้งหมด 166 ชิ้น โดยผู้ป่วยมีความพอใจประสิทธิภาพ ของฟันปลอมในภาพรวมอยู่ในเกณฑ์ดี และความพึงพอใจในภาพรวมทั้งหมดต่อชิ้นฟันปลอม มีความสัมพันธ์ กันกับความพอใจในด้านต่าง ๆ ของชิ้นฟันปลอมอย่างมีนัยสําคัญทางสถิติ (α = 0.05) เมื่อทดสอบด้วยเปียร์สัน ได สแควร์ และผู้ป่วยถึงร้อยละ 62 เลือกที่จะใส่ฟันปลอมแบบเดิมในกรณีที่เลือกทําฟันปลอมใหม่ในอนาคต สรุป ความพึงพอใจของผู้ป่วยต่อประสิทธิภาพของฟันปลอมบางส่วนถอดได้ข้างเดียวชนิดโครงโลหะอยู่ในเกณฑ์ดี และถึงแม้จะให้เลือกชนิดของฟันปลอมในอนาคต ผู้ป่วยส่วนมากยังตัดสินใจที่จะใช้ฟันปลอมชนิดนี้ต่อไป ดังนั้นจึงน่า จะเป็นทางเลือกหนึ่งของทันตแพทย์ที่จะทําฟันปลอมชนิดนี้ให้แก่ผู้ป่วยได้ในกรณีที่เหมาะสม

การรักษาทางทันตกรรมในผู้ป่วย ซิสเท็มมิกสเคลอโรซิส : รายงานผู้ป่วย, สุวิมล ปิติพานิช

ซิสเท็มมิกสเคลอโรซิส จัดเป็นความผิดปกติที่เกิดจากภูมิต้านเนื้อเยื่อตนเอง รอยโรคที่พบในความผิดปกติ นี้คือมีภาวะพังผืดขึ้นในหลายอวัยวะ เป็นผลให้ผู้ป่วยเกิดความบกพร่องของสภาพร่างกายและการทํางานของ อวัยวะที่เกิดความผิดปกติเหล่านั้น ซิสเท็มมิกสเคลอโรซิสที่เกิดขึ้นกับอวัยวะของช่องปากในผู้ป่วยมักทําให้เกิด ปัญหาเกี่ยวกับสุขอนามัยในช่องปาก แต่การรักษาทางทันตกรรมในผู้ป่วยกระทําได้ยาก เนื่องจากช่องปากที่แคบ เล็กกว่าปกติ บทวิทยาการนี้เป็นการรายงานผู้ป่วยด้วยโรคซิสเท็มมิกสเคลอโรซิส ซึ่งผู้ป่วยมีปัญหาเกี่ยวกับโรคฟันผุ โรคเหงือกอักเสบและการใส่ฟันปลอมที่ไม่พอดีกับช่องปาก ในบทวิทยาการจะได้กล่าวถึงลักษณะอาการของโรคใน ผู้ป่วย วิธีดําเนินการรักษาทางด้านทันตกรรม การทําความเข้าใจกับผู้ป่วยถึงปัญหาที่ผู้ป่วยประสบจากการเกิดโรค และความสําคัญของการรักษาอนามัยของช่องปาก นอกจากนี้ยังบรรยายถึงปริทัศน์วรรณกรรมของโรคซิสเท็มมิกสเคลอโรซิสอย่างละเอียดไว้ด้วย

ผลของสารสกัดจากส่วนวุ้นและส่วนยางจาก ว่านหางจระเข้ต่อการเพิ่มจํานวนของเซลล์สร้างเส้นใย ที่เพาะเลี้ยงจากเนื้อเยื่อเหงือกและเซลล์สร้างเคอราตินในห้องปฏิบัติการ, พสุธา ธัญญะกิจไพศาล, ดำรง ดำรงค์ศรี, นฤมล เจริญเวชธรรม, สุภีพันธุ์ บุณยรัตนสุนทร, สุรีรัตน์ อุดมกิจธนสาร

วัตถุประสงค์ เพื่อศึกษาผลของสารสกัดจากว่านหางจระเข้ที่มีต่อการเพิ่มจํานวนของเซลล์สร้างเส้นใยและเซลล์ สร้างเคอราติน CCL-25 วัสดุและวิธีการ เซลล์ถูกทดสอบด้วยสารสกัดจากส่วนวุ้นและส่วนยางที่ระดับความเข้มข้นของโปรตีนต่าง ๆ กัน เป็นเวลา 24 ชั่วโมง จากนั้นจํานวนเซลล์จะถูกคํานวณโดยวิธีการย้อมเซลล์ด้วยสีเมทิลีนบลู และผลการทดลองจะถูกวิเคราะห์ทางสถิติแบบ One-way Analysis of Variance ผลการศึกษา สารสกัดจากส่วนวุ้นของว่านหางจระเข้ที่ระดับความเข้มข้นของโปรตีนตั้งแต่ 1-50 ไมโครกรัมต่อ มิลลิลิตร สามารถกระตุ้นการเพิ่มจํานวนเซลล์สร้างเส้นใยอย่างมีนัยสําคัญทางสถิติแต่ไม่มีผลต่อเซลล์สร้างเคอราติน ส่วนสารสกัดจากส่วนยางที่ระดับความเข้มข้นของโปรตีน 20-400 ไมโครกรัมต่อมิลลิลิตร สามารถกระตุ้นการเพิ่ม จํานวนเซลล์ของเซลล์สร้างเส้นใย ในขณะที่ความเข้มข้นของโปรตีนในสารสกัดจากส่วนยางของว่านหางจระเข้ที่ 20 ไมโครกรัมต่อมิลลิลิตร กระตุ้นการเพิ่มจํานวนของเซลล์สร้างเคอราติน อย่างมีนัยสําคัญทางสถิติ (p<0.05) สรุป สารสกัดจากส่วนวุ้นของว่านหางจระเข้ที่ระดับความเข้มข้นของโปรตีน 1-50 ไมโครกรัมต่อมิลลิลิตร และสาร สกัดจากยางที่ระดับความเข้มข้น 20-400 ไมโครกรัมต่อมิลลิลิตร สามารถกระตุ้นการเพิ่มจํานวนของเซลล์สร้างเส้นไย

การให้บริการทางทันตกรรมในคลินิกทันตกรรม โรงพยาบาลคณะทันตแพทยศาสตร์จุฬาลงกรณ์มหาวิทยาลัย, สุวิมล ปิติพานิช, มลิวรรณ วงศ์สิทธาจารย์

วัตถุประสงค์ เพื่อสํารวจข้อมูลของผู้ป่วยที่มารับบริการทางทันตกรรมในคลินิกทันตกรรมบริการ คณะทันต แพทยศาสตร์ จุฬาฯ และนําไปกําหนดเป็นนโยบายและวางแผนการให้บริการที่เหมาะสม และสอดคล้องกับ ความต้องการของผู้ป่วย วัสดุและวิธีการ การศึกษาทําโดยสํารวจจากแฟ้มประวัติและรายงานบันทึกการรักษาของผู้ป่วย ในช่วง 6 เดือน ระหว่างเดือนกรกฎาคม-ธันวาคม พ.ศ. 2542 จํานวน 6,354 ราย นําข้อมูลการให้บริการทางทันตกรรมที่ได้มา คํานวณเป็นร้อยละ และเปรียบเทียบความแตกต่างของชนิดการให้บริการแบ่งตามช่วงอายุและเพศ ผลการทดลองและสรุป พบว่าการให้บริการทางทันตกรรมที่มากที่สุด คือ การอุดฟันคิดเป็นร้อยละ 40.84 การ ขูดหินน้ําลายและรักษาโรคปริทันต์คิดเป็นร้อยละ 23.17 และการบําบัดรักษาเร่งด่วนคิดเป็นร้อยละ 13.95 แสดงว่าผู้ป่วยมีปัญหาเกี่ยวกับโรคฟันผุและโรคปริทันต์มากที่สุด และผู้ป่วยส่วนหนึ่งจะมาหาทันตแพทย์เมื่อปวด จึงควรรณรงค์เรื่องทันตกรรมป้องกันและให้ผู้ป่วยมารับการตรวจฟันทุก 6 เดือน

รายงานผู้ป่วยหญิงวัยกลางคน 2 รายที่มีรอยโรคไลเคนนอยด์ที่บริเวณเยื่อเมือกกระพุ้งแก้มที่สัมผัสกับ วัสดุอุดอมัลกัม การเกิดของโรคเป็นแบบค่อยๆ ดําเนินไป รอยโรคมีลักษณะเป็นฝ้าสีขาวมีอาการปวดแสบปวด ร้อนเมื่อรับประทานอาหารรสจัด การรักษาด้วยการรื้อวัสดุอุดอมัลกัมออก แล้วเปลี่ยนเป็นวัสดุคอมโพสิตเรซิน พบว่ารอยโรคและอาการปวดแสบปวดร้อนหายไป จากการติดตามผลการรักษาเป็นระยะ ๆ เป็นเวลา 1 ปี 6 เดือน ไม่พบรอยโรคปรากฏอีกเลย

The Effect Of Endodontic Solutions On Resorcinol-Formalin Paste, Ronald Nicholas Vranas

This study determined if any of four endodontic solutions would have a softening effect on resorcinol-formalin paste and if there were any differences in the solvent action between these solutions. Resorcinol, formalin and zinc oxide were mixed and allowed to set for 30 days. The solutions tested were 0.9% sodium chloride, 5.25% NaOCl, chloroform and Endosolv R. Seven samples per solution were tested and an additional seven samples served as controls.
