วัตถุประสงค์ เพื่อศึกษาข้อมูลของผู้ป่วยสูงอายุในโครงการการบริการทางทันตกรรมแก่ผู้สูงอายุโดยไม่คิดมูลค่าเนื่องในวโรกาสที่พระบาทสมเด็จพระเจ้าอยู่หัวทรงเจริญพระชนม์พรรษาครบ 6 รอบ และนําไปเป็นข้อมูลเบื้องต้น ในการจัดเตรียมการให้บริการทันตกรรมผู้สูงอายุของคณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย วัสดุและวิธีการ ท่าการเก็บข้อมูลโดยการสํารวจเวชระเบียนผู้ป่วยที่เข้าร่วมโครงการฯ จํานวน 647 ราย ซึ่งมีอายุ ตั้งแต่ 60-88 ปี เกี่ยวกับ ประวัติทางการแพทย์ อาการสําคัญ สิ่งที่ตรวจพบทางคลินิก และการไปรับการรักษา ตามภาควิชาต่าง ๆ แล้วนําาข้อมูลมาวิเคราะห์ ผลการศึกษา จากผู้ป่วยที่สํารวจได้จํานวน 647 ราย (ขาย 300 ราย และหญิง 347 ราย) ผู้ป่วยช่วงอายุที่มารับ การรักษามากที่สุดคือ 60-64 ปี (ร้อยละ 41.0) ผู้ป่วยที่มารับการรักษาส่วนใหญ่ต้องการมาตรวจสุขภาพช่องปาก โดยไม่ระบุปัญหา (ร้อยละ 74.5) รองลงมาคือมีอาการเคี้ยวอาหารไม่สะดวกต้องการมาใส่ฟันปลอม (ร้อยละ 19.5) จากลักษณะที่ตรวจพบทางคลินิกพบว่าผู้ป่วยเป็นโรคปริทันต์มากที่สุด (ร้อยละ 43.7) รองลงมาคือฟันผุและสันเหงือก ว่าง (ร้อยละ 34.0 เท่ากัน) จากประวัติทางการแพทย์พบว่าสองในสามของผู้ป่วยเป็นโรคทางระบบ (ร้อยละ 68.9) ผู้ป่วยเป็นโรคความดันโลหิตสูงมากที่สุด (ร้อยละ 21.6) รองลงมาคือ เบาหวาน และ โรคหัวใจ (ร้อยละ 9.9 และ 9.7 ตามลําดับ) และพบว่าผู้ป่วยไปรับการรักษาจากภาควิชาทันตกรรมประดิษฐ์มากที่สุด (ร้อยละ 76.5) รองลงมาคือภาควิชาทันตกรรมหัตถการและปริทันตวิทยา (ร้อยละ 53.3 และ 50.7 ตามลําดับ) สรุปผู้ป่วยสูงอายุในโครงการนี้ส่วนใหญ่ต้องการการรักษาเกี่ยวกับการใส่ฟันปลอม รักษาฟันผุและรักษาโรคปริทันต์ ตามลําดับ การให้การรักษาแก่ผู้สูงอายุควรให้ความสําคัญในการสอบถามเกี่ยวกับโรคประจําตัวทางระบบและเพื่อระมัดระวังในการให้การรักษา

To determine the characteristics of new dental faculty and what factors influenced them to choose academic careers, a survey was sent to deans at all U.S. dental schools to be distributed to faculty with length of service of four years or less. Responses were received from 240 individuals. About half of the respondents had been in private practice for an average of eight years, and 20 percent had military experience averaging almost sixteen years. วัตถุประสงค์ เพื่อศึกษาค่าเฉลี่ยฟันผุถอนอุด และเปรียบเทียบค่าเฉลี่ยฟันผุถอนอุดระหว่างฟันน้ํานมและฟันถาวร กับอายุและเพศของเด็กที่มารับการรักษาที่ภาควิชาทันตกรรมสําหรับเด็ก คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหา วิทยาลัย ในช่วงปี พ.ศ. 2537-2542 และนํามาเปรียบเทียบกับการศึกษาในปี 2536 วัสดุและวิธีการ ทําการศึกษาย้อนหลังโดยบันทึกค่าเฉลี่ยฟันผุถอนอุดของเด็กอายุ 2-12 ปี จํานวน 1,650 คน ที่เข้ามารับการตรวจทางคลินิก และได้รับการถ่ายภาพรังสีชนิดไบท์วิ่งที่ภาควิชาทันตกรรมสําหรับเด็ก คณะทันต แพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย ในช่วงปี พ.ศ. 2537-2542 แล้วนําค่าเฉลี่ยที่ได้ไปวิเคราะห์โดยใช้สถิติชนิด ที-เทสต์ ผลการศึกษา ค่าเฉลี่ยฟันผุถอนอุดของฟันน้ํานมเป็นซี่ คน และด้าน/คน ของเด็กอายุ 2-6 ปี ในช่วงปี พ.ศ. 2537- 2542 ลดลงจาก 7.93 เป็น 6.38 คน และ 19.53 เป็น 15.53 ด้าน/คน ยกเว้นค่าเฉลี่ยฟันผุถอนชุดเป็นด้าน/คน ในปี พ.ศ. 2540 ซึ่งมีค่าสูงขึ้น ค่าเฉลี่ยฟันผุถอนอุดของฟันถาวรเป็น คน และด้าน/คน ของเด็กอายุ 7-12 ปี ใน ช่วงปี พ.ศ. 2537-2542 ลดลงจาก 3.26 เป็น 2.54 คน และ 5.55 เป็น 3.96 ด้าน/คน เมื่อนํามาเปรียบเทียบ กับผลการศึกษาในปี พ.ศ. 2536 พบว่าค่าเฉลี่ยเหล่านี้มีค่าสูงขึ้นยกเว้นค่าเฉลี่ยฟันผุถอนอุดของฟันน้ำนมเป็นซี่ คน ในปี พ.ศ. 2542 และค่าเฉลี่ยของฟันถาวรในปี พ.ศ. 2540 และ 2542 ความแตกต่างระหว่างเพศไม่มีความสัมพันธ์ กับค่าเฉลี่ยฟันผุถอนอุดของฟันน้ํานมและฟันถาวร อย่างมีนัยสําคัญทางสถิติ (p>0.05) ยกเว้นค่าเฉลี่ยฟันผุถอนอุต ของฟันน้ํานมเป็นคน ในปี พ.ศ. 2538 และ …

ผลของแคลเซียมไฮดรอกไซด์ที่ตกค้าง ต่อการรั่วซึมปลายราก, คุณเมตตจิตต์ นวจินดา, พีรยา ภูอภิชาติดํารง

วัตถุประสงค์ เพื่อศึกษาผลของแคลเซียมไฮดรอกไซด์ที่ตกค้างในคลองรากฟัน ที่มีต่อการเกิดการรั่วซึมที่บริเวณ ปลายรากฟัน ภายหลังการขุดคลองรากฟันนาน 30 และ 40 วัน วัสดุและวิธีการ การศึกษานี้ได้ทําการทดสอบการรั่วซึมด้วยการใช้สี และนํามาผ่านขั้นตอนการทําฟันใสเพื่อวัด ผลในห้องปฏิบัติการ กลุ่มตัวอย่างเป็นฟันถอนจากคนที่มีคลองรากฟันเดียว รากตรง จํานวน 60 ซี่ ขยายคลองราก ฟันถึงเครื่องมือขยายขนาด 40 จากนั้นทําการขยายโดยวิธีสเตปแบคถึงเครื่องมือขยายขนาด 60 ได้มีการแบ่งกลุ่ม ตัวอย่างเป็น 2 กลุ่มแบบสุ่ม คือ กลุ่ม 1 เป็นกลุ่มทดลองที่ใช้แคลเซียมไฮดรอกไซด์ใส่ในคลองรากฟัน และแบ่งเป็น อีก 2 กลุ่มย่อย คือ กลุ่ม A (15 ) ภายหลังการขุดคลองรากฟันเก็บฟันไว้นาน 30 วัน กลุ่ม C (15 ) ภายหลัง การอุดคลองรากฟันเก็บฟันไว้นาน 60 วัน กลุ่ม II เป็นกลุ่มควบคุมที่มีการแบ่งเป็นอีก 2 กลุ่มย่อย คือ กลุ่ม B (15 ) ภายหลังการอุดคลองรากฟันเก็บฟันไว้นาน 30 วัน กลุ่ม D (15 ) ภายหลังการขุดคลองรากฟันไว้นาน 60 วัน แล้วน้าฟันมาทดสอบการรั่วซึมบริเวณปลายรากฟันด้วยการซึมของสีอินเดียนอิงค์ แล้วนํามาผ่านขั้นตอนการ ทําฟันใส วัดระยะการซึมสีเป็นแบบเชิงเส้นผ่านกล้องสเตอริโอนําข้อมูลมาวิเคราะห์ทางสถิติ ผลการศึกษา ผลการวิจัยพบว่า ระหว่างกลุ่มที่ใช้แคลเซียมไฮดรอกไซด์และกลุ่มควบคุมไม่มีความแตกต่างกันอย่างมี นัยสําคัญทางสถิติ (p>0.01) ทั้งในกรณีที่ทดสอบการรั่วซึมภายหลังการขุดคลองรากฟันแล้วนาน 30 วัน และ 60 วัน สรุป การใช้แคลเซียมไฮดรอกไซด์ใส่ในคลองรากฟัน ไม่มีผลต่อการรั่วซึมที่บริเวณปลายราก หลังจากขุดคลองราก พันนาน 30 และ 60 วัน

วัสดุบูรณะแบบเซรามิคทั้งซี่, เฉลิมพล ลี้ไวโรจน์

ปัจจุบันพอร์ซเลนหรือเซรามิคได้ถูกพัฒนาให้มีความแข็งแรงพอที่จะนํามาใช้ในงานบูรณะฟันแบบ เซรามิคทั้ง อีกทั้งยังมีความสวยงามสูง สามารถเลียนแบบความเป็นธรรมชาติของฟันมนุษย์ได้ บทความนี้ได้ บรรยายถึงวิธีการต่าง ๆ ที่ใช้ในการพัฒนาพอร์ซเลนให้มีความแข็งแรงสูงพอที่จะรองรับแรงต่าง ๆ ในช่องปาก และ สรุปข้อบ่งใช้ของเซรามิคแต่ละระบบให้เหมาะสมกับงานทางทันตกรรมรวมทั้งการเลือกใช้ซีเมนต์ยึดครอบฟันเซรามิค เพื่อเป็นแนวทางสําหรับการปฏิบัติงานของทันตแพทย์

การศึกษาความต้านทานต่อการลอกออกเมื่อยึดซิลิโคน อิลาสโตเมอร์เข้ากับแผ่นโพลิยูรีเทนที่ขึ้นรูปจากโพลิยูรีเทนเหลว ด้วยการทาสารไพรเมอร์ 3 ชนิด, ปิยวัฒน์ พันธุ์โกศล, นิยม ธำรงค์อนันต์สกุล, จารุวรรณ จิรทองคำโชติ, นิรินทร์ รัตนภพ, สิริมา ลิ่มชัยชะนะ

วัตถุประสงค์ เพื่อศึกษาความต้านทานต่อการลอกออกเมื่อยึดซิลิโคน อิลาสโตเมอร์ เข้ากับแผ่นโพลียูรีเทนที่ขึ้นรูป จากโพลียูรีเทนเหลว ด้วยการทาสารไพรเมอร์ 3 ชนิด คือ 1205 เอส-2260 และเซ-4040 วัสดุและวิธีการ ขึ้นรูปแผ่นโพลียูรีเทนจากโพลียูรีเทนเหลวโดยใช้เบ้าหล่อยิปซัมเป็นแม่แบบ แบ่งการทดสอบออก เป็น 4 กลุ่มๆ ละ 30 ชิ้น โดย 3 กลุ่มแรก ทาไพรเมอร์ แต่ละชนิดได้แก่ ไพรเมอร์ 1205 เอส-226 และ เอ-4040 ตามลําดับ และกลุ่มที่เหลือเป็นกลุ่มควบคุม อัดทับแผ่นโพลียูรีเทนของแต่ละกลุ่มด้วยซิลิโคน อิลาสโตเมอร์ ชนิด ไตรอะซีทอกซีไซเลน แล้วตัดขึ้นทดสอบให้มีขนาดตาม เอเอสทีเอ็ม ดี 1876-72 และทดสอบค่าแรงต้านทานต่อ การลอกออก โดยใช้เครื่องวัดลอยด์ รุ่น แอลอาร์ 10 เค ตามข้อกําหนด เอเอสทีเอ็ม ดี 1876-72 ผลการศึกษา พบว่าแผ่นโพลียูรีเทนที่ทาด้วยไพรเมอร์ เอส-2260 เอ-4040 และกลุ่มควบคุมไม่เกิดการยึดติดระหว่าง แผ่นโพลียูรีเทนกับซิลิโคน อิลาสโตเมอร์ ส่วนในกลุ่มที่ทาด้วยไพรเมอร์ 1205 มีค่าของแรงยึดระหว่างโพลียูรีเทน กับซิลิโคนสูงจนเกิดการฉีกขาดในเนื้อซิลิโคน อิลาสโตเมอร์ สรุป ไพรเมอร์ 1205 สามารถใช้เป็นสารยึดแผ่นโพลียูรีเทนที่ขึ้นจากโพลียูรีเทนเหลวให้ติดกับซิลิโคน อิลาสโตเมอร์ ชนิดไตรอะซีทอกซิไซเลน เพื่อประโยชน์ในการประดิษฐ์อวัยวะเทียมได้

การกระตุ้นการทํางานของ Mmp-2 โดยสารหลังจาก แบคทีเรียที่เพาะเลี้ยงจากร่องลึกปริทันต์, เกษรา ปัทมพันธุ์, ธนธร ทั่งทอง, ธนภูมิ โอสถานนท์, วรภัทร หลิ่วจุลัสจรรย์, จินตกร คูวัฒนสุชาติ, ทัศนีย์ ดรงค์สุวรรณ, ประสิทธิ์ ภวสันต์

วัตถุประสงค์ การทดลองครั้งนี้ต้องการศึกษาผลของสารหลังจากแบคทีเรียที่เพาะเลี้ยงได้จากร่องลึกปริทันต์ของผู้ป่วย ในสภาวะที่ไม่มีออกซิเจน ที่มีต่อการกระตุ้นการทํางานของเอนไซม์ MMP-2 จากเซลล์เพาะเลี้ยงจากเนื้อเยื่อเหงือก ของมนุษย์ วัสดุและวิธีการ แบคทีเรียจากร่องลึกปริทันต์ของผู้ป่วยถูกเพาะเลี้ยงในสภาวะที่ไม่มีออกซิเจนเป็นเวลา 5 วัน สาร หลังจากแบคทีเรียทั้งที่ผ่านและไม่ได้ผ่านการต้ม ถูกนําไปกระตุ้นเซลล์เพาะเลี้ยงจากเนื้อเยื่อเหงือกของมนุษย์เป็น เวลา 48 ชั่วโมงในสภาวะที่ไม่มีซีรัม ระดับความเป็นพิษต่อเซลล์จะวัดด้วยเทคนิค MTT assay ส่วนการกระตุ้น การทํางานของเอนไซม์จะวัดโดยเทคนิค Zymography ในอีกการทดลองหนึ่ง การกระตุ้นเซลล์ด้วยสารหลังจาก แบคทีเรียจะทําในสภาวะที่มีหรือไม่มี actinomycin D และ cycloheximide เป็นเวลา 24 ชั่วโมง ผลการศึกษา สารหลังจากแบคทีเรียในระดับที่ไม่เป็นพิษต่อเซลล์ ทั้งที่ผ่านและไม่ผ่านการต้ม สามารถกระตุ้น การทํางานของ MMP-2 ผลของการกระตุ้นจะแปรผันตามความเข้มข้นของสารหลั่งที่ใช้ ผลของสารหลั่งในการกระตุ้น MMP-2 จะถูกยับยั้งได้ด้วย cycloheximide แต่ไม่ยับยั้งด้วย actinomycin D สรุป สารหลังจากแบคทีเรียจากร่องลึกปริทันต์ของผู้ป่วย สามารถกระตุ้นการทํางานของเอนไซม์ MMP-2 จากเซลล์ เพาะเลี้ยงจากเนื้อเยื่อเหงือกของมนุษย์ โดยการกระตุ้นนี้ไม่ต้องการการสร้าง mRNA ใหม่แต่ต้องการการสร้างโปรตีนใหม่

ฟันสึกอย่างรุนแรงเป็นสาเหตุหนึ่งที่ทําให้ตัวฟันสั้น ซึ่งเป็นปัญหาที่ทันตแพทย์พบได้บ่อย การตัดสินใจ ในการวางแผนการรักษาและขั้นตอนการบูรณะค่อนข้างสลับซับซ้อน บทความนี้นําเสนอถึงสาเหตุที่ทําให้เกิดตัวฟัน สั้น ตําแหน่งที่เกิดในช่องปาก ระดับความรุนแรงของการสึกของฟันและปัจจัยอื่น ๆ ที่มีผลกระทบต่อการวางแผน การรักษา รวมทั้งแนวทางเลือกในการบูรณะฟันสั้นด้วยวิธีต่าง ๆ เพื่อการเพิ่มการยึดอยู่ของครอบฟันตัวอย่างเช่น การเพิ่มความยาวของตัวฟันด้วยการทําศัลย์ปริทันต์ การดึงฟันด้วยแรงทางทันตกรรมจัดฟันตลอดจนการเพิ่มมิติ ในแนวดิ่งใบหน้าด้วยการบูรณะฟันชนิดติดแน่นหรือชนิดถอดได้ และอธิบายถึงข้อพิจารณาการให้การบําบัดรักษาที่เหมาะสมในแต่ละราย

การประเมินทัศนคติของนิสิตนักศึกษาทันตแพทย์ไทยต่อผู้ป่วยสูงอายุ, ชาญชัย โห้สงวน

วัตถุประสงค์ เพื่อประเมินทัศนคติของนิสิตนักศึกษาทันตแพทย์ไทยต่อผู้ป่วยสูงอายุ และเพื่อพัฒนาตัวชี้วัดมิติต่างๆ ของทัศนคติต่อผู้สูงอายุ วัสดุและวิธีการ กลุ่มตัวอย่างคือนิสิตนักศึกษาทันตแพทย์จากคณะทันตแพทยศาสตร์ 4 สถาบัน แบ่งเป็นชั้นปีที่ 1 จํานวน 188 คน และชั้นปีที่ 6 จํานวน 138 คน ใช้แบบสอบถามชนิดตอบด้วยตนเองเก็บข้อมูลทางด้านทัศนคติ ต่อผู้สูงอายุในมิติต่างๆ คือ ภาพลักษณ์ทั่วไปต่อผู้สูงอายุ ทัศนคติเชิงความชอบต่อผู้สูงอายุทั่วไป ทัศนคติต่อผู้ป่วย สูงอายุ และทัศนคติด้านความมุ่งมั่นก่อพฤติกรรม ใช้การวิเคราะห์ปัจจัยศึกษาคุณสมบัติของตัวชี้วัดทัศนคติ และเปรียบเทียบทัศนคติระหว่างกลุ่มนิสิตนักศึกษาทันตแพทย์ด้วยสถิตินอนพาราเมตริก ผลการศึกษา กลุ่มตัวอย่างนิสิตนักศึกษาทันตแพทย์ไทยมีทัศนคติในเชิงบวกต่อผู้ป่วยสูงอายุ ทั้งทัศนคติเชิงความ ชอบและทัศนคติด้านความมุ่งมั่นต่อพฤติกรรม โดยนิสิตนักศึกษาชั้นปีที่ 6 มีระดับทัศนคติเชิงความชอบต่อผู้ป่วย สูงอายุในระดับที่เป็นบวกมากกว่านิสิตนักศึกษาชั้นปีที่ 1 อย่างมีนัยสําคัญ (P<0.001) แต่มีระดับทัศนคติด้าน ความมุ่งมั่นก่อพฤติกรรมในระดับที่เป็นบวกน้อยกว่านิสิตนักศึกษาชั้นปีที่ 1 อย่างมีนัยสําคัญ (P = 0.03) สรุป นิสิตนักศึกษาทันตแพทย์ที่มีประสบการณ์ทางคลินิกแสดงระดับทัศนคติต่อผู้ป่วยสูงอายุแตกต่างไปจากนิสิตนักศึกษาที่ยังไม่มีประสบการณ์ทางคลินิก
