To Do Or Not To Do? Slow Orthodontic Extrusion For Implant Site Development In Esthetic Zone At The Anterior Maxilla, Hui-Chen Tsai, Chung-Chen Jane Yao, Cheing-Meei Liu
Taiwanese Journal of Orthodontics
Slow orthodontic extrusion (SOE) for implant site development, was introduced in 1991. It has been more than 20 years since this treatment was applied in our clinical practice. In this paper, we investigate surgeon's expectation of SOE for special requirements of immediate implantation by a clinical case. We also review case reports/case series searched from PubMed and inspect the power of evidence of these reports from the integration of data collection, and try to define the value of SOE. If the SOE cannot eliminate at least one surgical procedure, we need to consider the necessity of SOE before implant surgery.

This case report describes the management of an adult patient presenting with a skeletal Class III malocclusion, mandibular protrusion, upper incisor proclination and mandibular arch spacing by a modified surgery-first approach. A 26-year-old man had skeletal Class III and dental Class III malocclusion with concave facial profile, midface deficiency and mandibular prognathism with chin deviation to left. His dental manifestation presented anterior crossbite, upper incisors proclination and spacing in his mandibular arch. Orthodontic treatment of a Class II malocclusion case with single maxillary central incisor was presented in the case report. The treatment plan consisted of removing one maxillary central incisor instead of premolars. Bilateral maxillary lateral incisors were mesialized for space closure and substitution for maxillary central incisors. The morphology of maxillary lateral incisors was modified to mimic the maxillary central incisors. After treatment, both occlusion and esthetics was improved, and the patient was satisfied with the results.

Traditionally orthodontists have used the classification system developed by Edward H. Angle in 1900 to describe the malocclusion of an orthodontic patient. This classification is very limited with respect to the information it provides and should be replaced by a more detailed and inclusive description of a patient’s malocclusion as well as the underlying biology. In this review article we propose a new technique that ensures all pertinent information gets considered and we suggest a systematic approach to a more differentiated orthodontic diagnosis. Methods and materials: Firstly, the records of all patients who have completed fixed orthodontic treatment from 2012 to 2017 were identified and included in the study. Age at start of treatment, gender, decayed, missing filled teeth (DMFT), malocclusion classification, type of appliance used and type of treatment (extraction or non-extraction) were recorded. This case report described the management of congenital missing bilateral mandibular central incisors in a 16 years-old female patient who was diagnosed as Angle’s Class I molar malocclusion, canine Class II relationship by a treatment plan of orthodontic space regain. The options of fixed bridge, removable partial denture or dental implants to replace the missing teeth were well explained to the patient by prosthodontic consultation. After the orthodontic alignment, the treatment results demonstrated that the initial profile of the patient was maintained, the missing teeth was later replaced by removable partial denture. This 22-year-old female presents with skeletal Class III malocclusion, complicated by anterior cross bite, deep bite, and congenital missing of bilateral mandibular second premolars. The treatment modality was fullmouth fixed edgewise appliances. A favorable result of ideal overbite and overjet and closure of bilateral spaces of missing teeth were achieved. The patient was satisfied the improvement of function and esthetics after treatment.

A 7-year 10-month-old boy with skeletal Class III malocclusion was treated in two phases. In phase one, rapid maxillary expansion (RME) and facemask was applied for early intervention in a cervical vertebral maturation (CVM) stage of CS2. After RME and facemask treatment, decompensation of maxillary and mandibular incisors was noted, as well as an improved skeletal Class III jaw relationship with reduction of the A point-nasion-B point (ANB) angle. Maxillary expansion is a common treatment option for upper crowded dentition and posterior lingual crossbite in children and adolescents. As the midpalatal suture is not yet completely interlocked at these ages, the midpalatal suture can be opened via maxillary expansion to increase the length and width of the upper arch. This solves the problems of posterior crossbite and upper crowded dentition. Since the midpalatal suture is stiff in adults, many clinicians believe that adults require surgical intervention to achieve maxillary expansion. The maxillary six anterior teeth play important roles in esthetics and oral function. Treatments for missing maxillary lateral incisors either by space closure or space preservation need multiple clinical considerations. This case report presents an orthodontic treatment of a 33-year-old female patient who had a convex profile with lip incompetence, missing lower molars and poor prognosis of bilateral maxillary lateral incisors. The atypical extraction in treatment plan consisted of removing lateral incisors instead of premolars to retract the anterior teeth. Objective: The aim of this study demonstrates an improved method for determining the exact molar positions on the lateral headfilm. Previously this position was located by visual inspection of the headfilm. We are testing the precision of visual determination compared to using occlusogram measurements. A 13-year-old female patient presented with a chief complaint of anterior crossbite and chin prognathism. She had a skeletal Class III relationship with a brachyfacial vertical pattern. Anterior crossbite exhibited maximum intercuspation–centric relation shift, leading to an edge to-edge incisal position. Impaction in maxillary central incisor is not as common in comparison with maxillary canine, whereas it causes more occlusal and esthetic impact due to its visible location. This case reported a combined surgical and orthodontic treatment on two horizontally impacted maxillary central incisors with dilacerated roots. The treatment included 2 stages. In the first stage, the dental space was regained and surgical exposure with closed eruption technique was applied. Introduction: This in vitro study evaluates the home bleaching treatment effect on the surface corrosion of stainless steel orthodontic brackets. Materials and Methods: Three types of brackets, 3M Unitek (U), Dentaurum (D) and Tomy (T), were immersed in either one of six bleaching treatments at 37 ℃ for 28 days. After immersion, the amounts of iron, chromium, and nickel ions released from the brackets were detected using inductively coupled plasma mass spectrometry (ICP-MS). Class II division 2 malocclusion has low incidence rate of only 1% in Taiwan. The criteria of diagnosis and treatment consideration is more challenging as compared to other types of malocclusions. The consideration of orthodontic biomechanics could be sophisticated in facing the correction of the molar relationship and the angulation of the anterior teeth. This article discusses the clinical diagnosis and treatment mechanics of Class II division 2 malocclusion.

Anterior open bite is a complicated problem due to its multiple etiologies, including anatomical, environmental, and genetic factors. The complexity of skeletal class III deformity depends on the severity of bony discrepancy, especially with anterior open bite. The case reported the use of improved super-elastic Ti-Ni alloy wire (ISW), which was developed by Tokyo Medical and Dental University, to correct a case of Angle Class II division 2 case with unstable mandibular position. An adult 28-year old women came to our clinic with a chief complaint of poor dental alignment. Clinical examination revealed a Class II division 2 malocclusion. This report illustrates a 20 years old female presented a Class II division 2 malocclusion with deep overbite, gummy smile, canted maxillary anterior occlusal plane, low mandibular plane angle and bilateral scissors-bite. A two-phase treatment comprising functional orthopedic therapy and fixed orthodontic therapy is often proposed for pubescent patients with Class II division 1 malocclusion. Mandibular retrognathism is a more common underlying cause of Class II skeletal discrepancy than the maxillary prognathism. In such cases, an activator is a widely used functional orthopedic appliances to enhance the mandibular growth. High pull headgear with face bow has been a popular treatment modality for upper first molars distalization and anchorage enhancement. It can effectively inhibit the eruption of upper molars, enhance condylar growth to contribute a forward movement of the mandible and further improve the bite and lip incompetence. The compliance from patients is particularly important for favorable treatment outcome. The patient was an 11-year-old boy with features of Class II division 1 malocclusion. Clear aligner therapy (CAT) is an increasing popular alternative to conventional bracket therapy. This review summarized contemporary data regarding the history of CAT. The information regarding the advantages and disadvantages of CAT, biomechanical force and biological considerations, controlling orthodontic tooth movement (OTM) would be discussed in this review. The CAT is well accepted for their esthetics, comfort, good care of oral hygiene and periodontal status and suitable for interdisciplinary dental treatment. An Overview Of Digital Intraoral Scanners: Past, Present And Future- From An Orthodontic Perspective, Henry Hann-Min Hwang, Chi-Wei Chou, Yi-Jane Chen, Chung-Chen Jane Yao

Digital intraoral scanners(IOSs) have become the ongoing trend in contemporary digital orthodontics. This article aims to elaborate the past, present, and future of IOSs from an orthodontist’s perspective. We summarized the comparison between digital and conventional impression in literatures. Also, we discussed about the imaging principles and characteristics of different IOSs. Although unable to recommend the single best option, a checklist of consideration when choosing an IOSs was developed after trailing the mainstream commercial products. With technology revolution, embracing the IOSs may be essential for those interested in future digital orthodontic workflow.

Congenitally missing of maxillary lateral incisor is one of the most common agenesis anomalies. The purpose of this review was to present the orthodontic treatment of choice for a malocclusion with missing maxillary lateral incisors. Treatment plans consist of three categories: canine substitution, single-tooth implant and tooth supported restorations. Among which the least invasive option to achieve satisfying function and aesthetics is canine substitution. Factors such as the profile of the patient, the space analysis of dentition, and the color and shape of canine should be taken into consideration for treatment choice.
