Tooth transposition is a rare but disturbing problem for patients in esthetic and functional aspects. In this report, we describe a case with unilateral transposition of the maxillary canine and premolar. The patient received full mouth non-extraction orthodontic treatment with upper 1st premolar substitution for upper canine. Crown morphology modification and gingival plastic surgery for harmonious anterior esthetics and function were performed. Patient was satisfied with the treatment outcome.

In order to achieve better facial esthetics, functional occlusion, maintaining tissue health and stable treatment results, orthognathic surgery is required in certain orthodontic patients. While planning orthognathic surgery, we should inform patients benefits and risks associated in this treatment modality. Possible side effects including the post-operational temporary physiological influences to the patient should be mentioned.Thus, patients will be less anxious and deal with those problems more comfortably. The most common post-operation influences of orthognathic surgery is hypomobility. Severe root resorption with excessive tooth mobility may compromise the capability of the teeth to sustain occlusal loading, and possibly lead to extraction. Apical external root resorption (AERR) is considered as one of the risks associated with orthodontic treatment. Some clinical factors have been shown to correlate with orthodontic treatment, whereas the determinants for predicting AERR consistently were not found. Therefore, orthodontists must monitor the amount of root resorption during the course of orthodontic treatment. Multiple idiopathic root resorption (MIRR) is a rare condition. Fixed maxillary molar distalization appliance is an effective and noncompliance way for space regain. But with the increased confidence on skeletal anchorage, maximum anchorage preparation was planned with miniscrew in recent years. In this article, we cited two clinical cases to introduce an intraoral noncompliance appliance for maxillary molar distalization. RAMDA (rapid molar distalizing appliance) assist with skeletal anchorage is our modified technique used in these cases. From superimposition, we found that average rate of distalization was 1-2 mm per month. However, distal tipping of molar and anterior anchorage loss were also noted when using miniscrew as indirect anchorage.

Unilateral posterior crossbite case is quite complicated and difficult for orthodontist. The etiology may include any combination of dental, skeletal and neuromuscular functional components. This case is a 23 years old female with severe class II skeletal pattern and right side posterior teeth complete buccal crossbite. With the aid of TADs (temporary anchorage device) and full coverage biting plate, unilateral posterior buccal crossbite was corrected in an efficient way. Without an orthognathic surgery, the results of good dentition control in vertical direction and large amount of overjet reduction can still be obtained.

Objective: To retrospectively investigate the prevalence and patterns of tooth agenesis in permanent dentition of bilateral cleft lip and palate (BCLP) children in Southern Taiwan. Patients: 156 Taiwan children (99 males and 57 females) with BCLP aged from 4 to18 years were selected from the craniofacial records between July, 1983 and June, 2008 in the Department of Orthodontics, Kaohsiung Chang Gung Memorial Hospital. This case report presented a11 y/o Chinese female with Class I skeletal pattern and normal mandibular plane angle.Multiple teeth were impacteddue to a dentigerous cyst over anterior region in the maxilla. After the enucleation of the cyst, the crown portion of 11 was exposed but with improper position and angulation. How to accelerate orthodontic tooth movement has been an interesting issue to orthodontists and researchers. Ways being proposed to accelerate orthodontic tooth movement (OTM) include medication, mechanical stimulation, low level laser, corticotomy, periodontal accelerated osteogenic orthodontics, and surgery-first orthognathic surgery. Those invasive surgical procedures are used successfully on orthodontic patients for acceleration of OTM. Other non-invasive procedures, though with promising results obtained from animal experiments, await further clinical confirmations. This article reviewed recent clinical and animal studies, and the mechanism of medication, mechanical stimulation and surgical approaches for their effectiveness on OTM.

AIM: Computer-aided craniofacial reconstructions have been developed to allow surgeons to accurately simulate and predict the surgical outcome on complicated 3-D craniofacial structures. We have developed a new computerized method of 3-D simulation and prediction in orthognathic surgery (CASPOS) that not only enables clinicians to plan and simulate the surgical procedures, but also provides a bony "guiding splint" to allow surgeons to precisely position the bony segments into the planned location. The incidence of inferior alveolar nerve injury in this procedure is 9% ~85%. The clinical symptoms are numbness over the patients’ lower lip and chin. Objective: Dentofacial abnormalities, such as bimaxillary protrusion, can be distressing both socially and psychologically. The aim of this study is to identify the cephalometric features of bimaxillary protrusion and highlight the measurements that characterize bimaxillary protrusion when compared to a normal occlusion group. Materials and methods: Lateral cephalograms of 60 pretreated bimaxillary protrusion young adults were compared with 60 untreated young adults with normal occlusion. Using clip type self-ligation bracket combined with nickel-titanium arch wires can easily relief dental crowding and achieve arch expanding for adult patients with severe dentition crowding. This 25-year-old female was diagnosed as skeletal Class III with maxillary deficiency and severe dental crowding, anterior and posterior crossbite. We used clip type self-ligation bracket with non-tooth extraction, non-surgery, and non mini-implant insertion treatment. Successfully improved facial profile and achieve the harmonious proportion of vertical facial 's appearance.

Many factors can affect the post-treatment stability of either intermaxillary relationship, dental occlusion or facial profile, which include pre-operative preparations, types and range of surgical procedure, amount of bone movement, types of fixation, condylar position during fixation and the ability of soft tissue adaptation. Elastomeric chains were introduced to the orthodontic profession in the 1960s’, and are now an integral part of every day practices. They are used to generate light continuous forces for correcting rotations, consolidating spaces, and retraction canines. They are inexpensive, easily applied, relatively hygienic and require little or no patient cooperation. Elastomeric chain, however, are nor without their disadvantages. When exposed to an oral environment, they absorb water and saliva, permanent stain, and suffer breakdown of internal binds leading to permanent deformation. Posterior complete buccal crossbite, also called scissors-bite or Brodie Bite, is usually difficult to be corrected. In most cases with the occlusion of complete buccal crossbite, the mandibular teeth are lingually tilted, and the maxillary teeth erupt past their antagonists. When using the conventional orthodontic treatment to correct the scissors-bite, extrusion of the molars and subsequent occlusal plane canting frequently occur. The etiologies of skeletal Class III malocclusion include excessive growth of the mandible, insufficient growth of the maxilla, or the combination of both. The ideal treatment for severe mandibular prognathism is a combination of orthodontic treatment with orthognathic surgery to setback the mandible. However, nonsurgical approach using mini-implant anchorage could distalize the mandibular dentition and improve the occlusion if the skeletal discrepancy is mild to moderate. Patient with class III malocclusion and anterior crossbite combined with mandibular prognathism with deviation can be corrected through orthognathic surgery combined with conventional orthodontics. The aim of this study is to evaluate the shear bond strengths of metal brackets using two different fluoridereleasing adhesives (Light bond and Enlight) activated by two photoactivation systems [a conventional halogen light-curing system (HLC), and a plasma arc light curing system (PALC)]. Methods: Forty extracted premolars were divided into four groups of ten. Stainless steel brackets were bonded to the teeth using either a halogen light with a 20-second curing time or a plasma arc light with a 4-second curing time. The treatment timing is the most important factor in the correction of skeletal Class II malocclusion. The treatment effect in contribution of mandibular growth in the circumpubertal growth period is better than that in the pre-pubertal growth period in use of no matter what kind of orthodontic appliance. This report is a case of skeletal Class II malocclusion with mandibular deficiency. By using functional appliance (Activator) and fixed orthodontic appliance in the circumpubertal growth period, the malocclusion and retruded mandibular profile had been successfully improved.

Periodontal accelerated osteogenic orthodontics (PAOO) is a clinical procedure that combines selective alveolar corticotomy, particulate bone grafting, and the application of orthodontic forces. This procedure is theoretically based on the bone healing pattern known as the regional acceleratory phenomenon (RAP). Recently, partial evidences supported that PAOO had some advantages: 1. This case report is a 31-year-old man with Angle’s class I malocclusion (ANB: 1;wits: -3mm ) , upper and lower anterior crowding, missing lower right second premolar, and ankylosis of lower right first molar (beneath occlusal plane 7 mm). Orthodontic treatment consisted of extraction of #15 (upper right second premolar), and two buccal tipping maxillary third molars ( #18 & #28). Supraerupted maxillary molars usually result from early loss of the opposing mandible molars. This may induce insufficient intermaxillary space and compromised prosthesis. The best way to deal with supraerupted maxillary molars is no doubt to intrude them. For the minor supraeruption of maxillary molars, simply wearing a posterior bite plate could apply vertical force to intrude them. But the larger supraeruption, combination with temporary anchorage device usage will supply more stable anchorage force to intrude maxillary molars back to the normal position. To manage moderate mandibular prognathism combined with anterior open bite in adult cases,the dento-alveolar growth has to be guided and carefully utilized during orthodontic treatment, especially when the patients have acceptable profile before treatment and tend to deny the needs of orthognathic surgery. Purpose: The purposes of this study was to evaluate the registration error and its reproducibility for implementation of the laser-scanned dental model images onto the CBCT scan data by using the manual 3D superimposition. Materials and methods: Twelve arches out of 8 consecutive patients were enrolled in this study. Both CBCT and dental impression were taken as orthodontic records and the dental casts were then laser-scanned into dental model images with STL file format.
