Determinants Of Head And Neck Cancer Survival In Nevada For 1995-2008, Xiao Q. Li

Head and neck cancer (HNC), also known as oral cavity and pharyngeal cancer, comprises a group of cancers that arise in the head or neck region, such as the nasal cavity, sinuses, lips, mouth, salivary glands, and pharynx. Use of tobacco, alcohol, or a combination of the two, are major risk factors for head and neck cancer. Approximately 75% of oral cancers are attributable to smoking and/or alcohol consumption. More recently, the human papilloma virus (HPV) has also been associated with the development of head and neck cancer.

Many people in the United States have untreated dental disease due to a lack of dental insurance, a lack of oral health knowledge, and a lack of priority placed on dental health. Despite an increase in dental service use by Medicaid recipients as a result of local programs, children enrolled in Medicaid often have low rates of use of dental services. The Surgeon General’s Call to Action report in 2000 established the problem of oral health disparities as an important public health issue. Because of its widespread prevalence and preventable nature, the problem of untreated dental caries and poor oral health has been called a “neglected epidemic.” As in other areas of health, the poor often carry a disproportionate burden of oral health problems. Correlates Of Past Year Dental Health Visits Among Black Men: From The Black Men's Health Study Of Indiana, Shauna Spring Stapleton

Objective: This study aimed to examine correlates of past year dental health visits among Black Men of Indiana.

Methods: A 2011 health needs assessment was taken from a convenience sample of 1,444 Black men from 12 Indiana counties. Participants represented a wide range of socioeconomic backgrounds. Utilizing logistic regression analysis, compensating for clustering by county, predisposing and enabling factors were explored to determine their correlation to dental health visits within the previous year. Predisposing factors included age, sex, marital status, educational level, and employment status. Background: Adult oral health has been identified as a major public health problem by the United States Surgeon General. A substantial number of adults between the ages of 20 and 64 have untreated tooth decay. There are limited treatment options for adults without dental insurance. Low-income adults must turn to free/low cost clinics, or private dentists that accept Medicaid to receive dental care. However, these resources are often limited. When the resources are not available these adults are left to deal with untreated tooth decay which can lead to worsening conditions and other health issues.

Despite the declining prevalence of dental caries over recent decades, caries remain the most common chronic childhood disease, especially in high-risk (low socio-economic status) populations. This appears to be true even when parents are aware of good oral health habits. The purpose of this study was to gather oral hygiene data on high-risk children in Nevada and to assess parental attitudes and education concerning their children’s oral health. Using visual examinations and a dmft (decayed, missing, filled, teeth) severity index, 235 lower SES children were screened and given treatment recommendations. Background: Elevated blood lead levels (EBLL) are declining in the United States, although some population subgroups continue to exhibit significant health disparities. A childhood lead poisoning prevention program was recently started in Nevada, and many efforts have been made to support this program and increase the screening rates.

Methods: To expand the potential pool of children screened for EBLLs, a pilot study was performed to evaluate lead concentrations in extracted deciduous teeth using Graphic Furnace Atomic Absorption Spectrometry (GFAAS), Inductively Coupled Plasma Mass Spectrometry (ICP-MS), and X-Ray Fluorescence (XRF) analysis.

Over 51 million school hours are missed annually by school-aged children due to a dental problem or visit, with 117 hours missed per 100 children (NIDCR, 2002). Approximately half a million of California's 7.2 million school-age children missed at least one day of school in 2007 because of dental issues such as toothaches (Pourat, & Nicholson, 2009). Disparities in utilization of oral health services have been shown to exist through epidemiological investigations and data analysis. Specifically, disparities exist for utilization of preventative dental care (PDC) and unmet dental need (UDN). The disparities in utilization of PDC and UDN are associated with demographic and insurance variables.

To address the oral health disparities, the University of Nevada, Las Vegas-School of Dental Medicine (UNLV-SDM) was created to improve the health of the citizens of Nevada through innovative programs of oral healthcare services to the community. A self-perpetuating cycle of poor health literacy and poor oral health knowledge and behavior affects approximately 90 million people in the United States, most especially those from low-income groups and other at-risk populations such as those with addiction. Poor oral health can result from lack of access to regular preventive dental appointments, lack of access to restorative care when dental diseases are treatable, and low oral health knowledge that leads to poor oral health self-care behaviors.
