Chronic aphasia following a left middle cerebral artery (L-MCA) stroke is often associated with limited recovery beyond the acute phase, necessitating interventions that target neuroplastic mechanisms. This study examined the effects of somatosensory pneumotactile stimulation paired with cognitive-linguistic tasks on functional language performance in an individual with chronic aphasia. Using a single-subject pre-post design, the participant completed a 10-week intervention protocol involving repeated pneumotactile stimulation via the GALILEO system combined with language and motor tasks. Outcomes were assessed using standardized measures, discourse-based language samples, oral reading, repetition, and phonemic verbal fluency. Background and aims
Intra-individual variability in language performance has been proposed as a factor associated with treatment outcomes in chronic aphasia. However, the nature of linguistic variability and the degree to which it informs therapeutic success remains poorly understood. In this study, we sought to (1) assess person- and item-level factors associated with naming variability, practice effects, and item-based inconsistencies, (2) determine the relation between intra-individual naming variability, practice effects, and item-based inconsistencies at baseline and treatment outcome in aphasia, and (3) determine the treatment impact on the change in variability and consistency of naming responses.Method

Several decades of research have investigated the neural connections between stroke-induced brain damage and language difficulties. Typically, lesion-symptom mapping (LSM) studies that address this connection have relied on mass univariate statistics, which do not account for multidimensional relationships between variables. Machine learning (ML) techniques, which can capture these intricate connections, offer a promising complement to LSM methods. To test this promise, we benchmarked ML models on structural and functional MRI to predict aphasia severity (N = 238) and naming impairment (N = 191) for a cohort of chronic-stage stroke survivors. Individuals with post-stroke aphasia exhibit deficits in regulating vocal (i.e., laryngeal) pitch control during speech vowel production; however, it is not determined whether such deficits also exist when they control their supra-laryngeal speech articulators during word production. To address this question, 19 subjects with post-stroke aphasia and 20 controls were tested under an altered auditory feedback paradigm in which they received + 30% shifts in their vowel first-formant frequency during word production. Stroke rehabilitation often necessitates coordinated intervention services; however, few studies have examined stroke-based interprofessional education (IPE) interventions for graduate speech-language pathology (SLP) and occupational therapy (OT) students. The purpose of this mixed methods study was to examine graduate SLP and OT students' interprofessional collaboration competence before, immediately, and several months after participating in an IPE stroke workshop. Participants were 38 SLP and 14 OT students who took part in a 2-hour case study workshop, completed an online survey, and participated in a 1-hour virtual focus group. Background: Currently, there are no agreed quality standards for post-stroke aphasia services. Therefore, it is unknown if care reflects best practices or meets the expectations of people living with aphasia. We aimed to (1) shortlist, (2) operationalise and (3) prioritise best practice recommendations for post-stroke aphasia care.
Methods: Three phases of research were conducted. In Phase 1, recommendations with strong evidence and/or known to be important to people with lived experience of aphasia were identified. Among stroke survivors, linguistic and non-linguistic impairments exhibit substantial inter-individual variability. Stroke lesion volume and location do not sufficiently explain outcomes, and the neural mechanisms underlying the severity of aphasia or non-verbal cognitive deficits remain inadequately understood. Converging evidence supports the idea that white matter is particularly susceptible to ischaemic injury, and long-range fibres are commonly associated with verbal and non-verbal function. Here, we investigated the relationship among post-stroke aphasia severity, cognition, and white matter integrity. This study provides an investigation into the effectiveness of a modified version of Phonomotor Treatment (PMT) for treating apraxia of speech (AOS) in a telepractice model. A single-subject design was utilized to provide treatment to a participant with post-stroke apraxia of speech and aphasia. Treatment occurred two hours a day, five days a week, for four weeks. Targets were selected specifically for the participant after initial probing and testing. Data was taken to measure the chosen targets for acquisition, maintenance, and generalization. People with aphasia often experience activity limitations and participation restrictions despite wanting to engage in communication activities that are satisfying and meaningful. Communication partner support, the complexity of the communication environment, and the thoughts and feelings of people with aphasia have important implications for both language processing and communicative participation. The perspectives of PWA about the role that environmental and personal factors (e.g. thoughts and feelings) play in their communication experiences could inform clinical practice.

While converging research suggests that increased white matter hyperintensity load is associated with poorer cognition, and the presence of hypertension is associated with increased white matter hyperintensity load, the relationship among hypertension, cognition and white matter hyperintensities is not well understood. Discourse is a fundamentally important aspect of communication, and discourse production provides a wealth of information about linguistic ability. Aphasia commonly affects, in multiple ways, the ability to produce discourse. Comprehensive aphasia assessments such as the Western Aphasia Battery-Revised (WAB-R) are time- and resource-intensive. We examined whether discourse measures can be used to estimate WAB-R Aphasia Quotient (AQ), and whether this can serve as an ecologically valid, less resource-intensive measure. Language comprehension is often affected in individuals with post-stroke aphasia. However, deficits in auditory comprehension are not fully correlated with deficits in reading comprehension and the mechanisms underlying this dissociation remain unclear. Vocational rehabilitation is recommended; however, there are no clear guidelines informing vocational rehabilitation for people with communication disability. Emotion plays an integral role in communication and has long been considered relevant to aphasia rehabilitation. Speech-language pathologists (SLPs), however, may sometimes wonder how or whether to address emotional responses. The purpose of the present tutorial was to (1) present a framework that clinicians can use to understand individualized emotional responses in aphasia and (2) discuss examples of how the framework informs practical strategies that SLPs can use to provide emotional support to PWA. Purpose: To (a) compare physiological arousal and attentiveness during a confrontational naming task between participants with aphasia and a control group across four conditions that varied according to emotionality of presented stimuli and (b) explore relationships among physiological arousal, attentiveness, perceived arousal, and naming performance. We hypothesized that participants with aphasia would show lower levels of arousal and attentiveness than control participants and that emotional conditions would lead to increased physiological arousal and attentiveness.

Stroke, also sometimes referred to as a brain attack, occurs when there is a blockage in blood supply to the brain or when a blood vessel bursts within the brain (CDC, 2022). Rural populations have a higher incidence of stroke and higher stroke mortality in the United States (Howard et al., 2017). Although there is a higher incidence rate of stroke in rural populations, researchers have also found that these populations appear to have limited knowledge on stroke and its risk factors. Neuropsychological case studies have provided evidence that individuals with semantic, but not phonological, working memory (WM) deficits have difficulty producing phrases containing several content words. These findings supported the claim of a phrasal scope of planning at the grammatical formulation stage of production, where semantic WM supports the maintenance of lexical-semantic representations as they are inserted into slots in phrasal constituents. Recent narrative production results for individuals at the acute stage of stroke supported the role for semantic WM in phrasal elaboration while suggesting a role for phonological WM at a subsequent phonological encoding stage in supporting fluent, rapid speech. …

BACKGROUND: Stroke rehabilitation interventions are routinely personalized to address individuals' needs, goals, and challenges based on evidence from aggregated randomized controlled trials (RCT) data and meta-syntheses. Individual participant data (IPD) meta-analyses may better inform the development of precision rehabilitation approaches, quantifying treatment responses while adjusting for confounders and reducing ecological bias. The association between age and language recovery in stroke remains unclear. Here, we used neuroimaging data to estimate brain age, a measure of structural integrity, and examined the extent to which brain age at stroke onset is associated with (i) cross-sectional language performance, and (ii) longitudinal recovery of language function, beyond chronological age alone. A total of 49 participants (age: 65.2 ± 12.2 years, 25 female) underwent routine clinical neuroimaging (T1) and a bedside evaluation of language performance (Bedside Evaluation Screening Test-2) at onset of left hemisphere stroke. The purpose of this study was to identify behavioral and neural correlates of impaired speech auditory feedback processing in people with a history of left-hemisphere stroke (LHS). To do this, we used the altered auditory feedback (AAF) paradigm to externally induce speech errors by randomly shifting the pitch frequency of the online auditory feedback up or down at ±100 cents in 38 LHS and 27 neurologically intact control participants under two experimental conditions: 1) active vocalizations of a steady speech vowel sound “ah,” and 2) passive listening to the playback of the same self-produced vocalizations. Identifying effective and efficient rehabilitation tools is crucial to improve language outcomes for persons living with chronic aphasia. Speech entrainment has proved to be particularly successful in improving speech output in nonfluent aphasia. It is hypothesized that, for patients with aberrant oscillatory synchronization between anterior and posterior language regions of the left hemisphere, speech entrainment may act as an external gaiting mechanism to bolster an impaired efference copy and improve synchrony between these regions. Theoretical and empirical evidence supports this idea that speech production relies on anterior-posterior connectivity in the left hemisphere.

Chronic aphasia, a devastating impairment of language, affects up to a third of stroke survivors. Speech and language therapy has consistently been shown to improve language function in prior clinical trials, but few clinicially applicable predictors of individual therapy response have been identified to date. Consequently, clinicians struggle substantially with prognostication in the clinical management of aphasia. Background:
While meta-analyses confirm treatment for chronic post-stroke aphasia is effective, a lack of comparative evidence for different interventions limits prescription accuracy. We investigated whether Constraint-Induced Aphasia Therapy Plus (CIAT-plus) and/or Multimodality Aphasia Therapy (M-MAT) provided greater therapeutic benefit compared with usual community care and were differentially effective according to baseline aphasia severity.
Methods:
We conducted a three-arm, multicentre, parallel group, open-label, blinded endpoint, phase III, randomised-controlled trial. Background & objective: Contralesional 1-Hz repetitive transcranial magnetic stimulation (rTMS) over the right pars triangularis combined with speech-language therapy (SLT) has shown positive results on the recovery of naming in subacute (5–45 days) post-stroke aphasia. NORTHSTAR-CA is an extension of the previously reported NORTHSTAR trial to chronic aphasia (>6 months post-stroke) designed to compare the effectiveness of the same rTMS protocol in both phases. Purpose: To investigate how emotional arousal and valence affect confrontational naming accuracy and response time in people with mild to moderate aphasia compared with adults without aphasia. We hypothesized that negative and positive emotions would facilitate naming for people with aphasia but lead to slower responses for adults with no aphasia.
Method: Eight participants with mild to moderate aphasia, 15 older adults, and 17 young adults completed a confrontational naming task across three conditions (positive, negative, neutral) in an ABA case series design. Objectives:
The aim of this systematic review was to identify the presence and nature of relationships between specific forms of aprosodia (i.e., expressive and receptive emotional and linguistic prosodic deficits) and other cognitive-communication deficits and disorders in individuals with right hemisphere damage (RHD) due to stroke.
Methods:
One hundred and ninety articles from 1970 to February 2020 investigating receptive and expressive prosody in patients with relatively focal right hemisphere brain damage were identified via database searches.
