Objectives Relatively few young adults with fragile X syndrome (FXS) meet the normative expectations of adulthood, although there is heterogeneity in this regard. The present study examined the preparation for the transition to adulthood experienced by young adults with FXS and their parents.

Methods Thirty-nine young adults with FXS and their primary caregiving parents participated. Those with FXS were in their last year of, or had recently left, high school or a formal transition program. Parents completed surveys about the formal transition planning process. We examined the role of the young adults’ adaptive behavior skills in transition planning.

Objectives

Main Concept Analysis is a discourse-based measure that captures the essential information conveyed during narrative speech and may serve as a functional indicator of communication ability in aphasia. This study will examine whether Main Concept composite scores derived from the Cinderella narrative task are associated with aphasia severity and identify the linguistic and cognitive factors that influence Main Concept performance. Effective rehabilitation tools are essential for improving language outcomes in chronic aphasia. Speech entrainment is a behavioral treatment that has shown promise in enhancing speech output in nonfluent aphasia, potentially by acting as an external mechanism to synchronize anterior and posterior language regions in the left hemisphere. Transcranial alternating current stimulation has been hypothesized to enhance functional connectivity between brain regions by amplifying endogenous oscillations. This proof-of-concept study explored whether high-definition tACS (HD-tACS) could improve speech fluency in nonfluent aphasia when paired with speech entrainment. The authors reviewed the pathophysiology, clinical genetics, phenotype, and comprehensive clinical management of Fragile X-associated Tremor/Ataxia Syndrome (FXTAS), a neurodegenerative disorder affecting FMR1 premutation carriers (55–200 CCG repeats).

Participants: 

A multispecialty and multidisciplinary team of authors with backgrounds in psychiatry, pediatrics, clinical psychology, speech and language pathology, and neurosciences.

Evidence:

People with Phelan–McDermid syndrome (PMS) have reduced speech and language abilities, yet little research has profiled the communication abilities in this population. The purpose of this study was threefold: identifying the language and communication profiles of school-aged children with PMS, identifying genetic contributions to language and communication abilities, and determining the feasibility of remote data collection for research purposes. Dementia has a significant impact on individuals, families, communities, and the economy. Alzheimer’s disease (AD) accounts for 60–80% of dementia cases. The APOE e4 allele is well known as the primary genetic risk factor for AD and dementia. Studies have also shown that sleep apnea increases the risk of dementia. Some research further suggests a possible interaction between sleep apnea and the APOE in dementia development; however, this relationship remains unclear. This study investigates the use of a speech verification system (SVS) technology, a form of automatic speech recognition (ASR), in the assessment of children's reading skills. Despite the growing integration of ASR systems in educational assessment, significant challenges persist, particularly due to the acoustic, pronunciation and dialectal variability inherent in children's speech. Our research evaluated the consistency between human rater (HR) and speech verification system (SVS) scores produced from SoapBox Labs across three linguistic tasks—phoneme blending, expressive vocabulary and word reading. Background

The FMR1premutation (FXPM) is a common genetic variant (∼1:151 females) linked to increased risk for neurodegenerative disease. Midlife cognitive phenotypes remain poorly defined.

Objective

To characterize episodic memory performance in midlife FXPM women and examine potential risk moderation via genetic (i.e., FMR1CGG repeat expansion size) and environmental (i.e., college degree attainment) influences.

Methods

Eighty-eight FXPM women and 84 matched controls, aged 30–55, completed the Loewenstein-Acevedo Scales for Semantic Interference and Learning (LASSI-L), a measure of specific episodic memory processes sensitive to subtle Alzheimer's disease (AD)-related cognitive and neuropathological changes.

Aphasia severity after stroke varies widely and is not fully explained by lesion size or location. We propose that incorporating the brain’s sensorimotor-association (S-A) gradient, reflecting cortical hierarchy, offers a biologically grounded framework for predicting language outcomes. In 236 individuals with chronic left-hemisphere stroke, we examine whether the hierarchical position of lesions in language-specific (LS) and domain-general (DG) areas modulates aphasia severity. We parcellate lesions using a cortical atlas and weigh them by each region’s S-A rank. Invisible Print? Reading Skill Predicts Children's Learning Of Novel Spoken Words, Taylor J. Bryant, Dawna Duff, Bethany Bell, Lisa Fitton, Suzanne M. Adlof

Research indicates that when literate children and adults hear a new word that they have never seen in print, they create an expectation for how the word may be spelled. However, studies have not yet examined the extent to which orthographic knowledge plays a role in children's spoken vocabulary learning when words are taught without print. The current study used structural equation modelling to examine the relation between children's word reading ability and their ability to learn novel words from spoken instruction. Children (N = 335; 7–9 years; 52% female; <  1% American Indian/Alaskan Native, 33% Black, 1% Multiracial, <  1% Native Hawaiian or Pacific Islander, 2% Other; 57% White; 6% did not report race; 2% Hispanic/Latino, 97% Not Hispanic/Latino; <  1% did not report ethnicity) completed norm-referenced language and reading assessments and experimental measures of spoken word learning. Word reading ability and vocabulary knowledge uniquely predicted spoken word learning with respective medium-small and medium-large effects after controlling for phonological memory, general language ability, and nonverbal cognition. Thus, orthographic skill has important effects on vocabulary learning, unaccounted for in previous research, that should be addressed in current theory and practice.

Lesion-symptom mapping techniques are essential to determine brain regions critical for language functions. However, high collinearity in neuroimaging and behavioural data remains a challenge for distinguishing neural substrates supporting multiple language domains (shared variance) and those subserving specific language functions (unique variance). Here, we employed a novel approach to multimodal lesion-symptom mapping using multivariate partial least squares regression to delineate the latent structure of lesion-behavioural mapping in aphasia and decompose the shared and unique neural determinants of language impairments. ‘Brain age’ is a biological clock typically used to describe brain health with one number, but its relationship with established gradients of cortical organization remains unclear. We address this gap by leveraging a data-driven, region-specific brain age approach in 335 neurologically intact adults, using a convolutional neural network (volBrain) to estimate regional brain ages directly from structural MRI without a predefined set of morphometric properties. Six distinct gradients of brain aging are replicated in two independent cohorts. Individuals with post-stroke aphasia exhibit deficits in regulating vocal (i.e., laryngeal) pitch control during speech vowel production; however, it is not determined whether such deficits also exist when they control their supra-laryngeal speech articulators during word production. To address this question, 19 subjects with post-stroke aphasia and 20 controls were tested under an altered auditory feedback paradigm in which they received + 30% shifts in their vowel first-formant frequency during word production. Age-Related Hearing Loss (ARHL), or presbycusis, affects two-thirds of U.S. adults over 70 and is linked to cognitive decline and an increased risk of dementia. This study examines associations between white matter integrity and hearing and cognitive function in healthy aging using diffusion tensor imaging (DTI). Convolutional neural networks (CNN) show great promise for translating decades of research on structural abnormalities in temporal lobe epilepsy into clinical practice. Three-dimensional CNNs typically outperform two-dimensional CNNs in medical imaging. Here we explore for the first time whether a three-dimensional CNN outperforms a two-dimensional CNN for identifying temporal lobe epilepsy-specific features on MRI. Using 1178 T1-weighted images (589 temporal lobe epilepsy, 589 healthy controls) from 12 surgical centres, we trained 3D and 2D CNNs for temporal lobe epilepsy versus healthy control classification, using feature visualization to identify important regions. Among stroke survivors, linguistic and non-linguistic impairments exhibit substantial inter-individual variability. Stroke lesion volume and location do not sufficiently explain outcomes, and the neural mechanisms underlying the severity of aphasia or non-verbal cognitive deficits remain inadequately understood. Converging evidence supports the idea that white matter is particularly susceptible to ischaemic injury, and long-range fibres are commonly associated with verbal and non-verbal function. Here, we investigated the relationship among post-stroke aphasia severity, cognition, and white matter integrity. Phonological processing is an important contributor to decoding and spelling difficulties, but it does not fully explain word reading outcomes for all children. As orthographic knowledge is acquired, it influences phonological processing in typical readers. In the present study, we examined whether orthography affects phonological processing differently for children with current reading difficulties (RD), children with a history of reading difficulties who are currently presenting with typical word reading skills (Hx), and children with typical development and no history of reading difficulties (TD). Background: Emergent research indicates that women carrying the FMR1 premutation (FXpm) may encounter difficulties in language skills, including increased dysfluencies, a reduction in the quantity and diversity of words produced, and increased word retrieval errors with age (Sterling et al., 2013; Bredin-Oja et al., 2021). Objectives: The present study aims to assess semantic and phonological lexical access skills in FXpm women as well as to examine the impact of cognitive load (time pressure) on lexical access.

Purpose – This study aims to investigate the impact of two types of factors on IQ test performance in children with variation in language ability. The two types of factors studied here are: a) test characteristics (i.e., complexity of test directions) and test-taker characteristics (i.e., language ability). Method – Four participants, ages 4-7, completed norm referenced assessments for language and intelligence. Results – Descriptive results suggest no discernable trends between performance on language assessments and nonverbal IQ measures. While converging research suggests that increased white matter hyperintensity load is associated with poorer cognition, and the presence of hypertension is associated with increased white matter hyperintensity load, the relationship among hypertension, cognition and white matter hyperintensities is not well understood. Background

Cognitive impairment has multiple risk factors spanning several domains, but few studies have evaluated risk factor clusters. We aimed to identify naturally occurring clusters of risk factors of poor cognition among middle-aged and older adults and evaluate associations between measures of cognition and these risk factor clusters.

Methods

We used data from the National Health and Nutrition Examination Survey (NHANES) III (training dataset, n = 4074) and the NHANES 2011–2014 (validation dataset, n = 2510). Individuals with aphasia, a disorder caused by damage to the brain’s language network, confront a variety of social and emotional struggles. While leaving their cognition largely intact, aphasia tremendously impacts a person’s ability to communicate confidently, fracturing their social network and contributing to feelings of loneliness and frustration. Language comprehension is often affected in individuals with post-stroke aphasia. However, deficits in auditory comprehension are not fully correlated with deficits in reading comprehension and the mechanisms underlying this dissociation remain unclear. This distinction is important for understanding language mechanisms, predicting long-term impairments and future development of treatment interventions. Using comprehensive auditory and reading measures from a large cohort of individuals with aphasia, we evaluated the relationship between aphasia type and reading comprehension impairments, the relationship between auditory versus reading comprehension deficits and the crucial neuroanatomy supporting the dissociation between post-stroke reading and auditory deficits. The purpose of this study was to explore general health literacy, hearing loss health literacy, and confidence levels among SLPs with different levels of academic training and on-the-job experience related to hearing loss and to understand how SLPs felt their training and experience contributed to their hearing loss health literacy development. Across two aims, we evaluated predictors of SLPs’ hearing loss health literacy and SLP beliefs of how hearing loss health literacy is gained.
