The prevalence of Helicobacter pylori (H. pylori) infection among dyspeptic patients was common in developing countries. H. pylori is capable of colonizing the gastric mucosa, and the infections often lead to the development of various gastrointestinal complaints. Human colonization by H. pylori and intestinal parasites is also common. BACKGROUND Helicobacter pylori (H. pylori), a bacterium that infects approximately half of the world's population, is associated with various gastrointestinal diseases, including peptic ulcers, non-ulcer dyspepsia, gastric adenocarcinoma, and gastric lymphoma. As the burden of antibiotic resistance increases, the need for new adjunct therapies designed to facilitate H. pylori eradication and reduce negative distal outcomes associated with infection has become more pressing. Characterization of the interactions between H. pylori, the fecal microbiome, and fecal fatty acid metabolism, as well as the mechanisms underlying these interactions, may offer new therapeutic approaches.

Periprosthetic joint infection (PJI) is a rare but devastating complication after total joint arthroplasty. An estimated 7-12% of patients have negative cultures despite clear clinical evidence of infection. One oft-cited reason for this occurrence is the administration of antibiotics in the weeks prior to obtaining cultures. This article reviews the influence of antibiotics on the diagnosis of PJI. Specifically, we examine the effect of prophylactic and therapeutic antibiotic administration on the diagnostic accuracy of microbiological cultures as well as serum and synovial biomarkers. Infection with Helicobacter pylori is the single greatest risk factor for developing gastric adenocarcinoma. In prospective, population-based studies, seropositivity to the uncharacterized H. pylori proteins Hp0305 and Hp1564 was significantly associated with cancer risk in East Asia. However, the mechanism underlying this observation has not been elucidated. Here, we show that Hp0305 and Hp1564 act in concert with previously ascribed H. pylori virulence mechanisms to orchestrate cellular alterations that promote gastric carcinogenesis. In samples from 546 patients exhibiting premalignant gastric lesions, seropositivity to Hp0305 and Hp1564 was significantly associated with increased gastric atrophy across all stomach conditions. The rate of successful treatment for Helicobacter pylori infections, with the clarithromycin triple therapy, is only 75%. The triple therapy, which consists of a proton pump inhibitor and two broad-spectrum antibiotics such as clarithromycin and amoxicillin, is becoming less effective due to the rise of strains with resistance against these antibiotics. In the search for narrow spectrum drugs for the treatment of H. pylori infections, a high-throughput screen was performed to identify selective compounds against H. pylori. Helicobacter pylori colonizes 50% of the world’s population, whereby glycoproteins and Lewis Y-containing lipopolysaccharides contribute to its pathogenesis. We investigated whether the HopE porin is glycosylated, if the glycan is Lewis Y, and if this is mediated by the putative oligosaccharide transferase HP0946 or the O-antigen ligase WaaL. Western blotting was performed on outer membranes with anti-HopE antibodies, anti-Lewis Y antibodies and fucose-binding BambL lectin to ascertain HopE glycosylation. We discovered that HopE is likely glycosylated by a non-Lewis Y fucose-containing glycan and neither HP0946 nor WaaL are the transferase. Helicobacter pylori and Epstein-Barr virus (EBV) are two well-known contributors to cancer and can establish lifelong persistent infection in the host. This leads to chronic inflammation, which also contributes to development of cancer. Association with H. pylori increases the risk of gastric carcinoma, and coexistence with EBV enhances proliferation of infected cells. Further, H. pylori-EBV coinfection causes chronic inflammation in pediatric patients. We have established an H. pylori-EBV coinfection model system using human gastric epithelial cells. The type IV secretion system (T4SS) is a versatile nanomachine that translocates diverse effector molecules between microbes and into eukaryotic cells. Here, using electron cryotomography, we reveal the molecular architecture of the Helicobacter pylori cag T4SS. Although most components are unique to H. pylori, the cag T4SS exhibits remarkable architectural similarity to other T4SSs. Our images revealed that, when H. pylori encounters host cells, the bacterium elaborates membranous tubes perforated by lateral ports. Sub-tomogram averaging of the cag T4SS machinery revealed periplasmic densities associated with the outer membrane, a central stalk, and peripheral wing-like densities. Helicobacter pylori is a genetically diverse bacterial species that colonizes the stomach in about half of the human population. Most persons colonized by H. pylori remain asymptomatic, but the presence of this organism is a risk factor for gastric cancer. Multiple populations and subpopulations of H. pylori with distinct geographic distributions are recognized. Genetic differences among these populations might be a factor underlying geographic variation in gastric cancer incidence. Background The aim was to investigate the reinfection rate of H. pylori during a follow-up period of 12 months in adults who had undergone eradication therapy. Methods One hundred-twenty patients; 116 with gastritis, 3 with duodenal ulcer and 1 gastric ulcer, were studied. Their mean age was 41 ± 13 years (range 18–77) and male: female ratio of 2:1. The Role Of Outer Inflammatory Protein A (Oipa) In Vaccination Of Thec57bl/6 Mouse Model Infected By Helicobacter Pylori, Mohaddese Mahboubi, Tahereh Falsafi, Majid Sadeghizadeh, Fatemeh Mahjoub

Background/aim: Outer inflammatory protein A (OipA) is an important adhesin of Helicobacter pylori. Our goal was to assess the role of OipA in protection of C57BL/6 mice against H. pylori. Materials and methods: C57BL/6 mice were mucosally immunized with recombinant OipA protein, OipA + propolis, propolis, and phosphate-buffered saline. After vaccination, anti-OipA IgA was measured. Mice were challenged three times with 5 x 107 CFU of the H. pylori B19 strain. Two weeks later, bacterial colonization and inflammation in the stomach was analyzed using standard methods. Results: The CFU number in the OipA group was significantly (P < 0.05) lower than that of the control. The CFU number in the OipA + propolis group was higher than those of the OipA and propolis groups. IgA titers were significantly higher (P < 0.0001) in the OipA group compared to the control and OipA + propolis groups. Propolis did not play an adjuvant effect but it interfered with the efficient vaccine effect of OipA. Conclusion: Results show the effect of vaccination by OipA in protection of the mouse model and the importance of OipA in H. pylori pathogenesis. OipA may be proposed as a suitable oral vaccine candidate against H. pylori infection; however, further study is required to determine adjuvant or adverse effects of propolis toward OipA.

Bacteria utilize complex type IV secretion systems (T4SSs) to translocate diverse effector proteins or DNA into target cells. Despite the importance of T4SSs in bacterial pathogenesis, the mechanism by which these translocation machineries deliver cargo across the bacterial envelope remains poorly understood, and very few studies have investigated the use of synthetic molecules to disrupt T4SS-mediated transport. Here, we describe two synthetic small molecules (C10 and KSK85) that disrupt T4SS-dependent processes in multiple bacterial pathogens. Background/aim: The severity of disease outcome in dyspepsia has been attributed to Helicobacter pylori virulence genes. The aim of this study was to determine the distribution of H. pylori virulence genes (cagA, babA2, and dupA) and to determine whether or not there arises a significant correlation with clinical dyspepsia outcomes. Materials and methods: H. pylori genotypes cagA, babA2, and dupA were identified by polymerase chain reactions from gastric biopsy samples in 105 H. pylori-positive patients.
