Improving Cardiopulmonary Resuscitation Education In Undergraduate Nursing Programs, Robin E. Roberts
Improving Cardiopulmonary Resuscitation Education In Undergraduate Nursing Programs, Robin E. Roberts
Electronic Theses and Dissertations
Ongoing competency validation of cardiopulmonary resuscitation (CPR) skills is not a routine practice in nursing school. Basic Life Support training and certification is obtained before or during nursing school with little opportunity for practice. Poor quality cardiopulmonary resuscitation skills can affect the confidence level of the performer and the patient outcome. The aim of this study was to explore the effect of training nursing faculty on the adoption of cardiopulmonary resuscitation technology into nursing school curriculum to provide improved cardiopulmonary resuscitation skills to nursing students. Using a pretest-posttest, this study followed the Kirkpatrick model of evaluation of continuing education, which …

Lazarus Syndrome After Aortic Aneurysm Repair, Kevyn Niu, Jamie Lee Aldakkour, Charles Huyghues-Despointes, Yizhi Lin
HCA Healthcare Journal of Medicine
Introduction
Lazarus syndrome is defined as the spontaneous return of circulation after cessation of cardiopulmonary resuscitation (CPR). Though there have been multiple cases of Lazarus syndrome documented in the literature, it is a significantly underreported phenomenon with less than 100 cases reported in the literature since the first case in 1982.
Case Presentation
After elective aortic aneurysm repair, an 88-year-old with a do-not-resuscitate directive had cardiac arrest, briefly showing post-mortem respiration and pulse. Despite resuscitation efforts including pharmacological intervention and CPR, he passed away within an hour. This case highlights complexities in end-of-life care and warrants exploration of post-mortem physiological …

Cardio-Pulmonary Resuscitation Without The Pulmonary? A Benchmark Project, Fernando A. Rios
MSN Capstone Projects
Cardiac Arrest occurs outside of the hospital setting more than you think. According to MyCares.net, 15% of OHCA happened in a public setting in 2020. Even if you are a trained healthcare professional, it may not be accessible at times in these situations due to different factors such as equipment availability, trained assistance, etc... Having an alternative option to conventional mouth-to-mouth rescue breath CPR would encourage trained and untrained bystanders to perform life-saving interventions to patients until local EMS responders can arrive. The covid pandemic affected everyone inside and outside the medical world. The general population got a real …

Background: Code status orders are important features of patient-centered clinical decisions, patient autonomy, and end-of-life care. Despite proper documentation of “do not resuscitate” (DNR) code status, hospitalized patients may be subjected to cardiopulmonary resuscitation (CPR) efforts that go against their wishes.
Purpose: The objective of this study was to identify and describe the population of hospitalized patients receiving discordant resuscitation efforts at a Midwest academic medical center utilizing electronic health records (EHR).
Method: The study included EHR records between 01/01/2011 and 01/01/2021 for hospitalized patients 19 years and older who experienced cardiac arrest (ICD-10 I46) and were documented as DNR. …

Research Symposium
Introduction: Cardiac tamponade occurring after cardiac surgery is rare but life threatening and requires simultaneous resuscitation and emergent sternotomy by the intensive care team. A simulated scenario using innovative mannequin with sternotomy wound has the capability of reproducing cardiac arrest associated with postoperative tamponade. We evaluated the face validity of this innovative mannequin, the confidence level and crisis resource management skills of the team during sternotomy to manage postoperative cardiac tamponade.
Methods: The simulation case scenario was developed using innovative sternotomy mannequin for children’s hospital cardiac intensive care unit (CICU) teams. The case involved a 3-year old male, intubated, mechanically …

Exploring Moral Permissibility Of Nurse Participation In Limited Resuscitation, Felicia Stokes
Exploring Moral Permissibility Of Nurse Participation In Limited Resuscitation, Felicia Stokes
Electronic Theses and Dissertations
This dissertation offers a novel approach to support nurses when they face conflict between clinicians and families or alternate decision-makers over potentially inappropriate end-of-life goals of care. This dissertation will provide a normative analysis of the moral permissibility of limited resuscitation, with arguments supported by analyses of families’ and nurses’ perspectives and actions in the EoL decision-making process. Limited resuscitation is a cardiopulmonary resuscitation effort where full pharmacologic and mechanical intervention is not used, or the length of the resuscitative effort is shortened. It is typically associated with deception because it is performed without the knowledge of patients and families. …

Cardiopulmonary Resuscitation Using High-Fidelity Simulators, Rebecca Walker
Cardiopulmonary Resuscitation Using High-Fidelity Simulators, Rebecca Walker
MSN Capstone Projects
Simulation and high-fidelity simulators (HFS) have become an important part of nursing education and its implementation into the curricula. It has become important in enhancing their knowledge and the development of competent nurses by reinforcement of skills that have been previously learned. HFS also increases students' confidence and reinforces their skills and knowledge while introducing cardiopulmonary resuscitation (CPR). The literature suggests that overall the students have greater retention of CPR skills if HFS is used instead of classical CPR training with low-fidelity simulators (LFS). However, some studies suggest that using HFS does not lead to retention of CPR skills or …
Family Presence During Cardiopulmonary Resuscitation: Clinical Practice Guidelines, Poonam Kalkat
Family Presence During Cardiopulmonary Resuscitation: Clinical Practice Guidelines, Poonam Kalkat
Walden Dissertations and Doctoral Studies
Healthcare providers can be skeptical if not resistant to patient-family centered care if standards of care are not written in a policy or guideline. The 2009 Emergency Nurses Association position statement supports giving families the option to be present during resuscitative measures to meet their emotional needs. However, healthcare organizations are frequently not implementing this statement into their practice. Based on peer-reviewed articles retrieved through an in-depth literature search and using the Appraisal of Guidelines for Research and Evaluation (AGREE) II model, the gap in practice was addressed by creating a clinical practice guideline (CPG). The practice-focused questions addressed what …

According to the American Heart Association (AHA), rates of successful resuscitation after out of hospital cardiac arrest (OHCA) vary across the country. Amongst 132 counties in the United States, the rates of CPR survival to hospital discharge ranges between 3.4%-22.0%, and the rates of CPR survival with functional recovery ranges from 0.8%-20.1%. This large degree of variability between regions has been improved through programs that educate Emergency Medical Service (EMS) departments on ways to improve outcomes through an evidence-based lens. The Medic One EMS department in Seattle and King County, Washington developed a resuscitation academy (RA) that improved cardiac arrest …

Despite highly trained healthcare professionals being readily available to respond to In-Hospital Cardiac Arrest (IHCA), a patient experiencing an IHCA often remains a devastating event for the patient. Each year in America, 13.5% of all deaths result from sudden cardiac arrest. Even with healthcare professionals certified in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS), the survival rates following an IHCA are not inspiring. The median survival rate to hospital discharge of an adult experiencing IHCA is 26.4% and 49.5% for the pediatric population. Despite a continued focus on improving CPR quality from the American Heart Association (AHA), …

West Florida Division GME Research Day 2020
Pneumoperitoneum is most commonly associated with a perforated viscus. When free air is seen on imaging, the most common course of action is an emergent exploratory laparotomy. However, pneumoperitoneum may not always be indicative of a perforated bowel. In the case described below, we will discuss a patient who developed acute peritonitis and pneumoperitoneum after undergoing chest compressions during cardiopulmonary resuscitation (CPR) and positive pressure mechanical ventilation.

Introduction: State-level legislation requiring CPR education prior to high school graduation is associated with an increased likelihood of community-level CPR training. CPR Legislation has also been shown to be associated with increased bystander CPR. We hypothesized that states with CPR Legislation would be associated with higher survival following out-of-hospital cardiac arrest (OHCA).
Methods: Utilizing 2014 Medicare Claims data for emergency department visits and inpatient stays, we identified OHCA via ICD-9-CM code. CPR Legislation data was collected through online statute review. Exposure to CPR Legislation was assessed using the patient state of residence. All disposition categories were considered survival to discharge …
