Introduction and objectives: The opioid overdose epidemic is exacerbated by the emergence of Xylazine as an illicit drug adulterant. Xylazine, a veterinary sedative, can potentiate opioid effects while also causing toxic and potentially fatal side effects. This systematic review aims to assess the impact of Xylazine use and overdoses within the opioid epidemic context.

Method: A systematic search was conducted following PRISMA guidelines to identify relevant case reports, and case series related to Xylazine use. Oftentimes, illegal drugs are cut with additional substances, known as cutting agents. These cutting agents fall into two categories: Diluents and adulterants. Diluents have no physiological effect on the user and simply allow the distributor to give the perception of “more product made.” Common examples of diluents are usually everyday household commodities (i.e., sugar or corn starch). On the other hand, adulterants are used to mimic or enhance the drug's physiological effects (i.e., caffeine in cocaine). The behavioral and neurochemical effects of cocaine are well established, and it is one of the most widely abused illicit drugs. Illicit cocaine is often adulterated with levamisole, which is an anthelmintic that was withdrawn from the U. S. market in 2000. It has been hypothesized that levamisole, unlike other common adulterants which are added as simple bulking agents, has effects of its own which may be responsible for its use as an adulterant.
