Introduction

United States poison centers are encountering increasing numbers of calls pertaining to edible cannabis exposed patients following state decriminalization. The emergence of these edible products has led to numerous emergency department visits in both adults and children following exposure. Background: The median raphe cyst is a rare, benign, congenital condition characterized by small, fluid-filled cysts that develop along the ventral midline of male external genitalia. These cysts are usually asymptomatic during infancy and childhood but tend to grow in size as the patient grows. These can become cosmetically concerning or symptomatic due to swelling, irritation, and discomfort. They may also present social and psychological stress to patients and families. Background: Cellulitis is a common skin infection that classically presents with unilateral lower extremity erythema, swelling, and pain. The diagnosis is made clinically. Many conditions may mimic cellulitis that are collectively referred to as “pseudocellulitis.” These include stasis dermatitis, deep vein thrombosis, atopic dermatitis, psoriasis, and gout. The Alt-70 model is a validated clinical prediction tool for differentiating cellulitis from pseudocellulitis. It is unclear if it is utilized or if it has changed clinical practice in the emergency department setting.

Introduction Elliptical excisions are the most common method to treat benign and malignant tumors in dermatologic surgery. This standardized design is traditionally 3 to 4.5 times longer than the excisional width to prevent unsightly protrusions of skin after primary closure, known as “standing cone,” “Burow’s triangles” or “dog ear” deformities. This results in a scar significantly longer than the original tumor that may be less easily concealed.

Objective To compare the final repair length of a tissue conservative method to remove skin tumors and close resultant defects against predicted repair length of traditional elliptical excision.

Background: Inpatient dermatology is an important yet evolving field. There has been a linear decrease in the number of dermatologists providing inpatient care and in the number of medical centers with an inpatient dermatology service. Medical centers with inpatient dermatology services are unevenly distributed geographically, with most in larger metropolitan areas. There is still a great need for dermatologic expertise in the inpatient setting. บทความวิชาการนี้เป็นการสังเคราะห์องค์ความรู้เกี่ยวกับสมรรถนะของพยาบาลเฉพาะทางการบำบัดทดแทนไตและแนวทางการพัฒนาคุณภาพการดูแลผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายโดยทบทวนวรรณกรรมและเอกสารทางวิชาการที่เกี่ยวข้องทั้งในและต่างประเทศรวมถึงแนวปฏิบัติทางคลินิกและมาตรฐานวิชาชีพพยาบาลเพื่อนำเสนอกรอบสมรรถนะหลักที่จำเป็นสำหรับพยาบาลเฉพาะทางในการให้การดูแลผู้ป่วยที่ได้รับการบำบัดทดแทนไตทั้ง3รูปแบบได้แก่การฟอกเลือดด้วยเครื่องไตเทียม การล้างไตทางช่องท้อง และการปลูกถ่ายไต  ผลลัพธ์ที่ได้จากการศึกษาพบว่าสมรรถนะหลักของพยาบาลเฉพาะทางการบำบัดทดแทนไตประกอบด้วย 9 หัวข้อสำคัญ ได้แก่ 1)สมรรถนะด้านการปฏิบัติการพยาบาลทางคลินิก 2)สมรรถนะด้านเทคโนโลยีและทักษะการใช้เครื่องมือฟอกไต 3)สมรรถนะด้านการสอนและการให้คำปรึกษา 4)สมรรถนะด้านจริยธรรมและความเป็นมืออาชีพ 5)สมรรถนะด้านการใช้เทคโนโลยีสาร 6)สมรรถนะด้านการสื่อสารและการทำงานเป็นทีม 7)สมรรถนะด้านการป้องกันและควบคุมการติดเชื้อ 8)สมรรถนะด้านคุณภาพและงานวิจัย 9) สมรรถนะด้านผู้นำและบริหารจัดการการพัฒนาคุณภาพการดูแลสามารถดำเนินการผ่าน4แนวทางหลักคือการพัฒนาศักยภาพบุคลากรอย่างต่อเนื่องผ่านการฝึกอบรมและการพัฒนาทางคลินิกการจัดทำและใช้แนวปฏิบัติทางคลินิกที่เป็นมาตรฐานการพัฒนาระบบติดตามประเมินผลและตัวชี้วัดคุณภาพและการส่งเสริมการทำงานเป็นทีมสหสาขาวิชาชีพ ซึ่งส่งผลให้ผู้ป่วยได้รับการดูแลที่มีคุณภาพ ลดภาวะแทรกซ้อน และมีคุณภาพชีวิตที่ดีขึ้น

ในยุคแห่งการเปลี่ยนผ่านสู่เทคโนโลยีดิจิทัล ปัญญาประดิษฐ์ (Artificial Intelligence: AI) มิได้เป็นเพียงเครื่องมือที่ช่วยวิเคราะห์ข้อมูลเท่านั้น แต่กำลังพลิกโฉมบทบาทของวิชาชีพพยาบาลอย่างรอบด้าน บทความนี้มีวัตถุประสงค์เพื่อวิเคราะห์ผลกระทบของปัญญาประดิษฐ์ต่อบทบาทของพยาบาลระดับปฏิบัติการขึ้นไปผ่านกรอบแนวคิด 3 มิติ ได้แก่ มิติด้านการปฏิบัติงาน มิติด้านความสัมพันธ์และจริยธรรม และมิติด้าน อัตลักษณ์ทางวิชาชีพ  ผลการวิเคราะห์พบว่า มิติด้านการปฏิบัติงาน ปัญญาประดิษฐ์ปรับบทบาทพยาบาลจากผู้ปฏิบัติตามกระบวนการสู่ผู้วิเคราะห์และกำกับการใช้ข้อมูลเพื่อสนับสนุนการตัดสินใจทางคลินิกมิติด้านความสัมพันธ์และจริยธรรมแม้ปัญญาประดิษฐ์จะเพิ่มความแม่นยำในการประเมินและเฝ้าระวังภาวะสุขภาพแต่ยังมีข้อจำกัดในด้านความฉลาดทางอารมณ์ ซึ่งไม่สามารถทดแทนความเข้าอกเข้าใจ การสัมผัสและเอาใจใส่ต่อผู้ป่วยได้ สำหรับมิติด้านอัตลักษณ์ทางวิชาชีพ การประยุกต์ใช้ปัญญาประดิษฐ์นำไปสู่การปรับขอบเขตความเชี่ยวชาญของพยาบาลโดยจำเป็นต้องพัฒนาสมรรถนะด้านดิจิทัลและการคิดเชิงวิพากษ์ควบคู่กับการดูแลที่ยึดผู้ป่วยเป็นศูนย์กลาง บทความนี้เสนอแนวคิดว่า พยาบาลควรเป็นผู้ใช้ปัญญาประดิษฐ์อย่างมีวิจารณญาณ โดยยึดหลักแนวคิดการบูรณาการระหว่างศักยภาพมนุษย์และปัญญาประดิษฐ์เพื่อยกระดับบทบาทของพยาบาลให้ สามารถคิด วิเคราะห์ และสร้างนวัตกรรมการดูแลที่ผสมผสานระหว่างเทคโนโลยีและหัวใจของความเป็นมนุษย์ได้อย่างสมดุล

บทความวิชาการนี้มีวัตถุประสงค์เพื่อสังเคราะห์บทบาทของพยาบาลในการป้องกันภาวะแทรกซ้อนและการฟื้นฟูสมรรถภาพหัวใจในผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลันกลุ่มเสี่ยงต่ำภายหลังได้รับการรักษาด้วยการถ่างขยายหลอดเลือดหัวใจ โรคกล้ามเนื้อหัวใจตายเฉียบพลันเป็นภาวะที่รุนแรงที่สุดในกลุ่มอาการกล้ามเนื้อหัวใจขาดเลือดเฉียบพลัน เกิดจากการปริแตกของคราบไขมันบริเวณผนังหลอดเลือด ส่งผลให้เกิดการก่อตัวของลิ่มเลือดและการอุดตันของหลอดเลือดหัวใจ การรักษาด้วยการถ่างขยายหลอดเลือดหัวใจเป็นวิธีการรักษาที่มีประสิทธิภาพสูง ช่วยให้สามารถเปิดหลอดเลือดหัวใจได้อย่างรวดเร็ว โดยมีระยะเวลาเฉลี่ยตั้งแต่ผู้ป่วยมาถึงโรงพยาบาลจนสามารถเปิดหลอดเลือดหัวใจได้ประมาณ 54 นาที ส่งผลให้ความรุนแรงของโรคลดลง ผู้ป่วยฟื้นตัวได้เร็ว และมีระยะเวลาการนอนโรงพยาบาลสั้นลง ผู้ป่วยบางรายจึงสามารถจัดอยู่ในกลุ่มเสี่ยงต่ำและสามารถจำหน่ายออกจากโรงพยาบาลได้ภายใน 48–72 ชั่วโมง พยาบาลจึงมีบทบาทสำคัญในการดูแลผู้ป่วยตั้งแต่ระยะวิกฤตจนถึงระยะเตรียมความพร้อมก่อนจำหน่าย โดยการพยาบาลสามารถแบ่งออกเป็น 3 ระยะ ได้แก่ การป้องกันภาวะแทรกซ้อนใน 24 ชั่วโมงแรก การฟื้นฟูสมรรถภาพหัวใจและการปรับเปลี่ยนพฤติกรรมเสี่ยงในช่วง 24–48 ชั่วโมง และการส่งเสริมการฟื้นฟูสมรรถภาพหัวใจอย่างต่อเนื่องในช่วง 48–72 ชั่วโมง การดำเนินบทบาทของพยาบาลในแต่ละระยะมีส่วนสำคัญในการลดภาวะแทรกซ้อนส่งเสริมการฟื้นฟูสมรรถภาพหัวใจ และเตรียมความพร้อมก่อนจำหน่ายจากโรงพยาบาล ซึ่งจะช่วยลดการกลับเป็นซ้ำของโรคและส่งเสริมคุณภาพการดูแลผู้ป่วยอย่างต่อเนื่อง

วัตถุประสงค์:เพื่อศึกษาผลของโปรแกรมการให้ข้อมูลแบบรูปธรรม-ปรนัยต่อพฤติกรรมการป้องกันการพลัดตกหกล้มของผู้ป่วยวัยผู้ใหญ่ที่รับการรักษาในโรงพยาบาลศิครินทร์ หาดใหญ่

แบบแผนงานวิจัย:การวิจัยกึ่งทดลอง ชนิดสองกลุ่มวัดก่อนและหลังการทดลอง

วิธีดำเนินการวิจัย:กลุ่มตัวอย่าง คือ ผู้ป่วยวัยผู้ใหญ่ทั้งเพศชายและเพศหญิงที่เข้ารับการรักษาในโรงพยาบาล ศิครินทร์ หาดใหญ่ โดยกำหนดขนาดกลุ่มตัวอย่างด้วยโปรแกรม G*Power ได้กลุ่มตัวอย่าง จำนวน 50 คน แบ่งเป็นกลุ่มทดลองที่ได้รับโปรแกรมการให้ข้อมูลแบบรูปธรรม-ปรนัยและกลุ่มควบคุมที่ได้รับการพยาบาลตามปกติ กลุ่มละ 25 คน เครื่องมือที่ใช้ในการวิจัย คือ โปรแกรมการให้ข้อมูลแบบรูปธรรม-ปรนัย ที่ผู้วิจัยสร้างขึ้นตามแนวคิดทฤษฎีการปรับตนเองของ Leventhal & Johnson และแบบสอบถามพฤติกรรมการป้องกันการพลัดตกหกล้ม จำนวน 12 ข้อ ผ่านการตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิ จำนวน 3 คน มีค่าดัชนีความตรงตามเนื้อหาเท่ากับ 1 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และเปรียบเทียบความแตกต่างของคะแนนเฉลี่ยพฤติกรรมการป้องกันการพลัดตกหกล้มระหว่างกลุ่มด้วยสถิติ Independent t-test

ผลการวิจัย:พฤติกรรมการป้องกันการพลัดตกหกล้มกลุ่มที่ได้รับโปรแกรมการให้ข้อมูลแบบรูปธรรม-ปรนัยหลังได้รับโปรแกรมสูงกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติ (p < .001) และกลุ่มที่ได้รับโปรแกรมการให้ข้อมูลแบบรูปธรรม-ปรนัยมีคะแนนเฉลี่ยพฤติกรรมการป้องกันการพลัดตกหกล้ม (=7.32, SD=5.30) สูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ (=1.92, SD=4.39) อย่างมีนัยสำคัญทางสถิติที่ระดับ .001 (p < .001)

สรุปผล:โปรแกรมการให้ข้อมูลแบบรูปธรรม-ปรนัยส่งผลให้ผู้ป่วยวัยผู้ใหญ่มีพฤติกรรมการป้องกันการพลัดตกหกล้มดีขึ้น ทั้งในการเปรียบเทียบระหว่างกลุ่มและก่อน-หลังการทดลอง จึงควรนำไปใช้เพื่อลดความเสี่ยงการพลัดตกหกล้มในสถานพยาบาล

วัตถุประสงค์:เพื่อศึกษาผลของโปรแกรมการให้ข้อมูลการดูแลผู้ป่วยต้อกระจกที่เข้ารับการผ่าตัดต้อกระจกและใส่เลนส์แก้วตาเทียมต่อพฤติกรรมการดูแลตนเองและความพึงพอใจของผู้ป่วยโรงพยาบาลศิครินทร์ กรุงเทพ

แบบแผนงานวิจัย:การวิจัยกึ่งทดลองแบบศึกษากลุ่มเดียว (วัดภายหลังการทดลอง)

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยที่อายุ 60 ปีขึ้นไป เตรียมเข้ารับการผ่าตัดต้อกระจกและใส่เลนส์แก้วตาเทียมที่ศูนย์ตา โงพยาบาลศิครินทร์ กรุงเทพ โดยคัดเลือกกลุ่มตัวอย่างแบบเจาะจงที่มีคุณสมบัติตามเกณฑ์คัดเข้าจำนวน 34 คน กลุ่มตัวอย่างได้รับโปรแกรมการให้ข้อมูลการดูแลผู้ป่วยต้อกระจกที่เข้ารับการผ่าตัดต้อกระจกและใส่เลนส์แก้วตาเทียมซึ่งเป็นโปรแกรมที่ผู้วิจัยสร้างขึ้น ผู้วิจัยดำเนินการเก็บรวบรวมข้อมูลโดยใช้แบบประเมินพฤติกรรมการดูแลตนเองของผู้ป่วยหลังได้รับโปรแกรม โดยดำเนินการเก็บข้อมูลเป็น 2 ระยะ คือ หลังผ่าตัด 7 วัน และ 1 เดือน และแบบประเมินความพึงพอใจของผู้ป่วยหลังได้รับโปรแกรมเมื่อหลังผ่าตัด 1 เดือน เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลทั้ง 2 ชุดมีค่าดัชนีความตรงตามเนื้อหา เท่ากับ 1.00 โดยมีค่าสัมประสิทธิ์อัลฟาของครอนบาค เท่ากับ .84 และ .80 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา (ค่าเฉลี่ย และ ร้อยละ) และการทดสอบค่าแบบกลุ่มตัวอย่างไม่เป็นอิสระต่อกัน (Dependent T-Test)

ผลการวิจัย:คะแนนเฉลี่ยพฤติกรรมการดูแลตนเองของผู้ป่วยผ่าตัดต้อกระจกภายหลังได้รับโปรแกรมการให้ข้อมูลการดูแลผู้ป่วยต้อกระจกที่เข้ารับการผ่าตัดต้อกระจกและใส่เลนส์แก้วตาเทียมเมื่อหลังผ่าตัด 1 เดือน มีความแตกต่างจากหลังผ่าตัด 7 วัน อย่างมีนัยสำคัญทางสถิติที่ระดับ (t = 4.61, p< .05) และคะแนนเฉลี่ยความพึงพอใจของผู้ป่วยต้อกระจกที่เข้ารับการผ่าตัดต้อกระจกและใส่เลนส์แก้วตาเทียมหลังได้รับโปรแกรมการให้ข้อมูลการดูแลอยู่ในระดับมากคิดเป็นร้อยละ 91.20

สรุป:โปรแกรมการให้ข้อมูลการดูแลผู้ป่วยต้อกระจกที่เข้ารับการผ่าตัดต้อกระจกและใส่เลนส์แก้วตาเทียม ส่งผลให้ผู้ป่วยมีพฤติกรรมการดูแลตนเองที่ดีขึ้นรวมถึงมีระดับความพึงพอใจอยู่ในระดับมากเมื่อภายหลังการผ่าตัด 1 เดือน

Inherited retinal diseases (IRDs) affect millions of people worldwide. Majority of IRDs are caused by degeneration of rod and cone photoreceptor cells (PRCs) due to gene mutations. The overarching goal of my dissertation is to model and evaluate the molecular role of various gene candidates involved in IRDs such as cone rod dystrophy (CRD) and retinitis pigmentosa (RP).

Postpartum depression screening among Hispanic women remains an important but complicated issue, as differences in how the EPDS is used and current barriers to care may affect early identification and treatment. This poster, Postpartum Depression Screening Among Hispanic Women: Insights from EPDS Literature, explores how the Edinburgh Postnatal Depression Scale (EPDS) has been evaluated across studies involving Hispanic populations. Using a literature review approach, eight peer-reviewed articles were organized into an annotated bibliography and matrix to identify key trends in screening practices and outcomes.

T cell therapy has proven challenging for pancreatic ductal adenocarcinoma (PDAC), partly due to heterogeneous expression of tumor-associated antigens (TAAs). To address tumor heterogeneity and mitigate immune evasion, an ex vivo expanded, polyclonal, T helper 1 cell-polarized T cell product targeting five TAAs—PRAME, SSX2, MAGEA4, Survivin and NY-ESO-1—was developed. These antigens were chosen based on their tumor specificity, oncogenicity, immunogenicity and level of expression. Thymidine kinase 2 deficiency (TK2d) (MIM 609560) is an ultra-rare, autosomal recessive mitochondrial myopathy caused by TK2 variants, leading to mitochondrial DNA depletion and/or multiple deletions. People with thymidine kinase 2 deficiency experience progressive myopathy, bulbar weakness and respiratory insufficiency, often losing the ability to walk, eat and breathe independently. Single nucleotide variants (SNVs) and copy-number variant (CNV) deletions involving TBX4 have been associated with pulmonary arterial hypertension, ischiocoxopodopatellar syndrome, and lethal lung developmental disorders (LLDDs). Thus far, all large CNV deletions encompassing entire TBX4 have been found to have arisen de novo. Here, we present a three-generation family with three neonate siblings who died within 35-66 days due to histopathologically diagnosed LLDD. Whole-genome sequencing identified an ~108-kb CNV deletion encompassing TBX4 in all three infants. Peanut allergy is a significant rising public health problem affecting both children and adults. Early peanut introduction has been adopted in multiple westernized nations as a preventative strategic measure to reduce the risk of developing peanut allergy, although implementation faces barriers. Multiple therapies to treat peanut allergy have been developed, including oral immunotherapy and omalizumab as approved treatments to provide protection against reactions from accidental exposure and reduce the risk of anaphylaxis, offering options beyond strict avoidance. Background and objectives: Intimate partner violence (IPV) and child physical abuse often co-occur, but the frequency and risk factors for abusive injuries in IPV-exposed children remain unknown. Pediatric tracheobronchial mucoepidermoid carcinoma (MEC), also called pulmonary MEC, is a rare malignant primary lung neoplasm that is typically found in the large airways. Patients may present with symptoms such as cough, pneumonia, fever, hemoptysis, dyspnea, wheezing, chest pain, weight loss, and fatigue. These nonspecific symptoms (or lack of symptoms in some cases) often result in a delayed diagnosis. Chest CT typically demonstrates findings consistent with bronchial obstruction and diagnosis is made via histopathology. Objectives: Syria has undergone significant socio-political turmoil since 2011 as internal conflict displaced portions of the population, destroyed infrastructure, and destabilized the economy. In the context of multilevel healthcare system disruptions, there have been increasing reports of substance use. This study seeks to evaluate patterns of substance use in Northwestern Syria.

Methods: Trained staff administered questionnaires to community members in Azaz, Syria. Questions focused on the substance use prevalence, societal and public health impacts, and perspectives on treatment availability.

Introduction: This Phase I study evaluated the safety and tolerability of concurrent niraparib, an oral poly(ADP-ribose) polymerase (PARP) 1/2 inhibitor, with definitive radiotherapy in women with locally advanced or platinum-sensitive metastatic cervical cancer. Dose escalation followed a Bayesian Optimal Interval (BOIN) design with a target dose-limiting toxicity (DLT) rate of 30%.
