Pulmonary aspergilloma can cause life-threatening haemoptysis. Surgical resection and/or bronchial artery embolization (BAE) are established treatment modalities, but both can be risky in frail patients with comorbidities. Spontaneous regression of aspergilloma with antifungal agents alone is rare. We report an elderly gentleman with a background history of treated pulmonary tuberculosis, who presented with haemoptysis due to a large left upper lobe aspergilloma. He declined surgery and BAE due to perceived risks. Oral voriconazole and later isavuconazole were prescribed, which led to clinical improvement and significant reduction in the size of the aspergilloma over 6 months. Antifungal agents might be a …

Background: Healthy lifestyle factors, including diet, may affect brain amyloid beta (Aβ) load. This study examines dietary patterns as moderators of the relationships among symptoms of depression, anxiety, and brain Aβ load. Method: A cross-sectional study of cognitively unimpaired older adults (n = 524) from the Australian Imaging, Biomarkers, and Lifestyle study assessed dietary patterns, depressive and anxiety symptoms, and brain Aβ load. Moderation and simple slope analyses were conducted. Results: The Dietary Approaches to Stop Hypertension (DASH) diet moderated the relationship between depressive and anxiety symptoms and brain Aβ load. The aim of this study was to analyse the Western Australian (WA) Safety Regulatory System (SRS) database to assess compliance of the WA mining sector regarding workers exposure to welding fumes and to identify trends over time. De-identified data analysed to assess the impact of reducing workplace exposure standards (WES) for general welding fumes on industry compliance. Historical trend analysis shows a shift from 100% compliance to 100% non-compliance, based on mean values and 95% confidence intervals, with exposure levels remaining consistent over time. Background and aim: Mechanistic data suggests several flavonoids may improve kidney function, but no large-scale prospective cohort study has been conducted. As such, we performed a prospective population-based study on flavonoid intakes, chronic kidney disease (CKD) risk, and all-cause mortality among participants with CKD. Methods: We examined associations between a Flavodiet score (FDS), individual flavonoid-rich foods and subclasses (based on ≥2 24-h dietary assessments) and a) CKD risk among 109,711 adults, and b) all-cause mortality among 3,287 participants with CKD at baseline, aged 40–69 years using multivariable Cox regression analyses. This quality improvement (QI) project sought to increase postpartum depression (PPD) screening rates at a single pediatric primary care practice setting by implementing an educational intervention for pediatric providers. Postpartum depression is a common and debilitating mental health condition affecting many mothers worldwide. Postpartum depression impacts many women in the United States and affects all ethnicities, although rates may vary based on demographic and socioeconomic factors. Improving Hiv Pep Patient Follow-Up Appointment And Laboratory Testing In A Telehealth Setting: A Quality Improvement Project, Ralston Lockett

The ongoing HIV epidemic in the United States highlights the need for innovative prevention strategies, including non-occupational post-exposure prophylaxis (nPEP). This Doctor of Nursing Practice (DNP) project evaluated a telehealth-based intervention aimed at increasing HIV PEP follow-up appointments and laboratory completion rates, while also enhancing provider knowledge. Dual interventions included a patient reminder system and provider education. While provider knowledge significantly improved post-intervention, based on Cohen’s d effect size of 0.62, clinical significance was demonstrated by follow-up appointment rates declining from 44% pre-intervention to 41% post-intervention. This evaluation project aimed to assess whether the nurse practitioner-led preoptimization care management program can increase patient satisfaction by improving the quality metrics, such as same-day ambulation post-surgery, discharge to home, and compliance in preoperative education. The evaluation retroactively reviewed 46 charts of primary total knee and hip arthroplasty patients, male (38%) and female (62%), aged 43 to 84. Using Fisher’s exact test to examine the Net Promoter Scores (NPS) for six months of the PCM program, there was no significant increase in patient satisfaction scores, despite PCM patients averaging 89% compared to non-PCM patients at 83%, based on an …

Invasive aspergillosis (IA), caused by Aspergillus fumigatus, is a life-threatening infection in immunocompromised individuals, necessitating novel host-directed therapies. Macrophages are pivotal in antifungal defense, yet their function can be dysregulated; the influence of the adipokine adiponectin (APN) on this process is not fully understood. This thesis investigated the role of APN signaling in alveolar macrophage (AM) and bone marrow-derived macrophage (BMDM) responses to A. fumigatus and assessed the therapeutic potential of the APN receptor agonist, AdipoRon. Schizotypy has been linked with many of the proposed endophenotypes of schizophrenia, but research is limited on the relationship between schizotypy and dermatoglyphic fluctuating asymmetry. The present study sought to examine relationships between schizotypy, dermatoglyphic fluctuating asymmetry, handedness, and season of birth. 146 undergraduate students were identified as schizotypes (n = 52) or controls (n = 94) based on the Multidimensional Schizotypy Scale and had their finger and palm prints collected along with completing additional measures of schizotypy and handedness. This study examined how direct experiences with COVID-19 infection and vaccination and indirect experience through government and media information predict individuals' intention to receive a COVID-19 booster dose. Drawing on secondary risk theory, we conducted a nationally representative door-to-door survey of 1000 adult Singapore residents in mid-2024. We used structural equation modeling to test whether risk perceptions and efficacy beliefs mediate the effects of experience and information exposure on booster intention. OBJECTIVE: Dystonia is one of the most prevalent movement disorders, characterized by significant clinical and etiological heterogeneity. Despite considerable heritability (~25%), the etiology in most patients remains elusive. Moreover, understanding correlations between clinical manifestations and genetic variants has become increasingly complex.

METHODS: Exome sequencing was conducted on 1924 genetically unsolved, mainly late-onset isolated dystonia patients, recruited primarily from two dystonia registries (DysTract and the Dystonia Coalition). Rare variants in genes previously linked to dystonia (n = 406) were examined, confirmed via Sanger sequencing, and analyzed for segregation when possible.

AIMS: The PANORAMA-HF trial demonstrated significant N-terminal pro-B-type natriuretic peptide (NT-proBNP) reductions in paediatric patients with left ventricular systolic dysfunction with sacubitril/valsartan or enalapril treatment over 52 weeks. This post hoc analysis aims to correlate changes in NT-proBNP levels with clinical outcomes in PANORAMA-HF patients receiving either sacubitril/valsartan or enalapril. In addressing art therapy education, Taylor (1991) suggested that undergraduate art therapy education is controversial in that professional entry into the field is at the masters' level. Cannabis use among young adult college students has risen steadily over the past decade, raising concerns about the associated health, academic, and psychological consequences. While cannabis use motives and readiness to change have each been linked to use patterns and outcomes, limited research has examined how these factors interact. The scope of practice for Licensed Professional Counselors (LPCs) and counseling residents (LPC-R) in integrated behavioral health (IBH) settings is grounded in accredited, theory-informed education and training. The benefits of IBH models are well-documented, however there is limited research that addresses how counselors develop professionally and clinically function, while navigating within pediatric inpatient environments. This qualitative dissertation used a focused ethnographic design to explore the lived experiences of ten purposively sampled counselors (n = 10) through demographic surveys, field observation, and semi-structured interviews. Purpose: This pilot study of a diet and physical activity intervention (HEALTH4CLL) was conducted to reduce fatigue and improve physical function (PF) in patients with chronic lymphocytic leukemia (CLL).

Methods: The HEALTH4CLL study used a randomized factorial design based on the multiphase optimization strategy (MOST). Patients received diet, exercise, and body weight management instructional materials plus a Fitbit and were randomized to undergo one of 16 combinations of 4 evidence-based mHealth intervention strategies over 16 weeks. Purpose: There is a paucity of literature on germline pathogenic variants (gPVs) in patients with invasive lobular carcinoma (ILC). This study characterizes the landscape and compares clinicopathologic variables and treatment outcomes between those with and without gPVs.

Methods: A prospectively maintained institutional database was used to identify all patients diagnosed with nonmetastatic ILC who had germline genetic testing. Clinicopathologic characteristics and time to recurrence, contralateral cancer, and death were compared for patients with and without gPVs. Conditional hazard ratios, computed by Cox proportional hazards models, described associations between clinicopathologic factors, including gPV status, and cancer events.

Purpose: This study aims to identify radiomic features from contrast-enhanced CT (CECT) scans that differentiate osteoradionecrosis (ORN) from normal mandibular bone in head and neck cancer (HNC) patients treated with radiotherapy (RT).

Materials and methods: CECT images from 150 patients with confirmed ORN diagnosis (2008-2018) at MD Anderson Cancer Center (MDACC) were analyzed (80 % train, 20 % test). Radiomic features were extracted using PyRadiomics from manually segmented ORN regions and automated contralateral healthy mandible regions. Correlation analysis (r > 0.95) reduced features for model training. A random Forest (RF) classifier with Recursive Feature Elimination identified discriminative features.
