Bruton tyrosine kinase inhibitors (BTKis) and cell therapy have successfully been used to treat mantle cell lymphoma (MCL). However, therapy resistance inevitably emerges. Cancer cells can progressively develop stable resistance by traversing through a transient drug-tolerant persister (DTP) state. The mechanisms enabling DTP cells to reversibly adapt to therapies and evolve to acquire heterogeneity remain poorly understood, and characterizing DTP cells in MCL continues to pose a challenge for clinic translation. The advent of single cell DNA sequencing revealed astonishing dynamics of genomic variability, but failed at characterizing smaller to mid size variants that on the germline level have a profound impact. In this work we discover previously uncharacterized genomic dynamics in 18 cells from three human brains utilizing single cell long-read whole genome sequencing. This provides key insights into the dynamic of the genomes of individual cells and further highlights brain specific activity of transposable elements, but requires validation in larger studies.

Gliomas represent a heterogenous group of primary brain tumors with overlapping imaging phenotypes. Treatment typically includes surgery and/or chemoradiation, however this varies based on the overall lesion and clinical presentation. This heterogeneity in both lesion characteristics and management strategies contributes to a lack of reliable findings when evaluating treatment outcomes with conventional MRI. The overlapping imaging features of radiation necrosis and tumor progression post-treatment can be particularly challenging for radiologists. Persistent maladaptive drug-context associations are a key component of relapse vulnerability and pose a major obstacle in treating opioid use disorder. Although the dorsal hippocampus (dHPC) is critical for contextual memory formation, the synaptic mechanisms by which opioids alter hippocampal circuits to sustain these associations remain unclear. We previously found that repeated context-paired morphine exposure impairs dHPC long-term potentiation (LTP) through enhanced activity of small-conductance calcium-activated potassium (SK2) channels. Purpose: To report surgical outcomes in eyes with non-complex rhegmatogenous retinal detachment managed with primary scleral buckle (SB) at a single academic institution over a 10-year period, with a secondary focus to determine if outcomes varied based on macula detachment status, presence of posterior vitreous detachment, and location of retinal detachment and retinal breaks.

Problem: Airway management is the primary intervention for halting clinical decompensation in pediatric and neonatal patients. In cases where patients require invasive support, endotracheal intubation (ETI) is the preferred method of airway securement. Unfortunately, ETI is a challenging skill to develop, especially when caring for children and infants, resulting in increased rates of failed ETI attempts which can result in worse outcomes. Background

People living with HIV (PLWH) face a higher risk of cardiovascular disease (CVD), with significant sex differences in outcomes. However, it is unclear whether these differences are driven by distinct risk factor profiles or by a differential impact of shared risk factors. This study aimed to identify factors associated with CVD among PLWH and to specifically assess for effect modification by sex using a large, diverse nationwide database.

Unlike human physicians, veterinarians are unable to directly communicate with their patients and must utilize client-veterinarian interactions to achieve the desired level of patient care. Communication, client education, and trust have been previously identified as important aspects of veterinary clinical interaction, and this study aimed to assess how these variables influence client compliance, treatment plan adherence, and perceived patient outcomes in veterinary medicine. The impact of psychiatric disorder, especially depression, on the prognosis of sarcopenic patients is gaining prominence. Background: Infection and nonunion are common sequelae of open fractures. Studies have shown infection and nonunion rates ranging from 2% to 52% and 12% to 17%, respectively. The present article describes the rates of surgical site infection (SSI) and delayed union/nonunion following open fractures in a large contemporary series of patients from prospective clinical trials with adjudicated outcomes. Methods: A descriptive analysis was performed with use of patient data from the FLOW, Aqueous-PREP, and PREPARE-Open studies. Introduction Deaths related to drug overdose and suicide in the USA have increased 500% and 35%, respectively, over the last two decades. The human and economic costs to society associated with these - €- deaths of despair' are immense. Optimal nutritional therapy is important for infants and children with perioperative congenital heart disease and heart failure. Medical providers face physiological and metabolic challenges when administering enteral and parenteral nutrition to these patients. Complications related to enteral nutrition can increase morbidity and mortality, although outcomes are improved in those cases in which caloric and nutrient administration can be achieved. Agitation is a neuropsychiatric syndrome characterized by excessive motor and/or verbal behaviors, with or without aggressive behaviors. The prevalence of agitation in Alzheimer’s disease varies from 5% to over 50%. Multiple factors have been implicated in its pathophysiology, including disease stage, comorbidity with other symptoms (e.g., psychosis, anxiety/depression), and psychosocial factors. Ruling out delirium and identifying environmental triggers are fundamental steps in the management of agitation in Alzheimer’s disease. Increasing The Likelihood Of Using Mental Health First Aid: Applying The Theory Of Planned Behavior, Alexander E. Chan, Cheyenne Schad, Julie Slivka, Elise Young, Ghaffar A. Hurtado Choque

This study applies the Theory of Planned Behavior to examine what may drive intentions to help others after receiving Mental Health First Aid (MHFA) training. The study is unique in its application of theory to understanding MHFA outcomes, as prior studies tended to be atheoretical. Logistic regression was used to analyze the results of post-program surveys completed by MHFA participants. Results showed that participants who reported positive changes in subjective norms and attitudes were significantly more likely to report increases in intentions to help. However, when analyzed in a model containing all predictors, only attitudes remained significant.

Needs assessments are an important activity for Extension professionals, as they identify community assets and guide decisions about programs and services. A common challenge is a lack of needs assessment data. The Fermi Estimate Framework can help Extension professionals overcome the challenge of missing data by producing reasonable estimates to answer questions of interest. Here we share detailed, step-by-step instructions for using the Fermi Estimate Framework, accompanied by a real-world Extension example.

Extension played a significant role, in collaboration with other key community organizations, by disseminating information that debunked COVID-19 myths, and promoting vaccinations at the community level. In this article we present insights from the experiences of Extension faculty at the University of Missouri who were engaged in promoting COVID-19 vaccinations for meat processing plant workers. Findings from this quality improvement project highlight opportunities for leveraging Extension field faculty, and challenges to consider in future public health promotion efforts.

Conjunctival melanoma (CJM) is a rare and aggressive malignancy. Mainly attributed to its rarity, the genomic features of CJM have not been well characterized. In the present study, we sequenced the exomes of primary and metastatic CJM tumors with the aim to unravel their genomic landscape. The results of the study suggest that CJM is a molecularly distinct melanoma subtype with mixed genotype and targetable mutations. Notably, there were features overlapping with both mucosal, and mainly cutaneous melanoma. Of note, some genetic alterations were found in association with specific subsets of CJM, such as bulbar anatomical localization. CJM tumors had …

The dataset (Elsafty_Reports_of_Myeloid_Neoplasms_2024) consists of 14,441 comprehensive clinicopathologic correlations (CPCs) for myeloid neoplasms (MN) with corresponding laboratory results. Specialized online platform utilizing these laboratory results automatically generated the diagnostic and prognostic CPCs. These results include complete blood counts (CBC), peripheral blood smear (PBS) findings, blast/promyelocyte count with dysplasia screening by flow cytometry, and molecular results by NGS/PCR. Two Hematopathologists, who performed the CBC and PBS review, collected their 10,794 real-world reports of cases served in Egypt with molecular studies performed in the USA and Europe. Background: Infection and nonunion are common sequelae of open fractures. Studies have shown infection and nonunion rates ranging from 2% to 52% and 12% to 17%, respectively. The present article describes the rates of surgical site infection (SSI) and delayed union/nonunion following open fractures in a large contemporary series of patients from prospective clinical trials with adjudicated outcomes.

Methods: A descriptive analysis was performed with use of patient data from the FLOW, Aqueous-PREP, and PREPARE-Open studies. BACKGROUND: Expanding treatment for hypertension requires ensuring access to affordable, high-quality blood pressure (BP) lowering medications. We describe the design, implementation and evaluation of a BP-lowering medication drug revolving fund (DRF) embedded in the Hypertension Treatment in Nigeria (HTN) Programme in 60 primary healthcare centres (PHCs) in the Federal Capital Territory, Nigeria.

METHODS: We used the exploration, preparation, implementation and sustainment framework to describe the DRF design and implementation and the reach, effectiveness, adoption, implementation and maintenance framework to report implementation outcomes. Missed appointments remain a serious problem in mental health outpatient clinics. They break the flow of treatment, waste staff time, and worsen outcomes for patients. In this setting, the practice problem is the lack of an evidence-based system for reminding patients about their visits. This doctor of nursing practice (DNP) project focused on developing and evaluating a clinical practice guideline (CPG) for automated text message (SMS) appointment reminders. Treatment-resistant depression (TRD) is associated with worse clinical outcomes and longer course of illness. However, TRD is more difficult to model in animal phenotypes, suggesting that other experimental and translational models must be considered to properly address and research novel therapeutics. Reelin, an endogenous glycoprotein downregulated in depression, has shown rapid antidepressant-like effects akin to those of the N-methyl-D-aspartate receptor (NMDAR) antagonist ketamine. Interestingly, the antidepressant-like effects of both ketamine and reelin affect mechanistic target of rapamycin complex 1 (mTORC1) activity and that of its related downstream signalers. This staff educational project focused on implementing training on the tissue, infection/inflammation, moisture, edge (TIME) framework to address gaps in nurses’ knowledge and improve adherence to the TIME framework during wound assessment. Current evidence shows inconsistent use of standardized wound assessment among nurses, which can lead to inappropriate treatment, delayed wound healing, increased infection risk, and higher healthcare utilization. Abstract Rural African Americans experience a higher prevalence of Type 2 diabetes compared to their White counterparts, yet little research has explored the roles of food insecurity and educational attainment in this disparity. The purpose of this quantitative, correlational study was to examine whether food insecurity and educational attainment were associated with the prevalence of Type 2 diabetes among African Americans aged 18 to 65 years in rural Southern United States, using the World Health Organization’s social determinants of health framework. Introduction: Given concerns for antibiotic resistance, broader spectrum antibiotic agents such as ertapenem are being used for pre-operative prophylaxis. We hypothesize that pre-operative ertapenem prior to laparotomies and colectomies is superior at decreasing surgical site infections (SSIs) compared with other widely used antibiotic agents.

Methods: Medline, Embase, the Cochrane Library, and Web of Science were queried for studies until February 02, 2025. Studies were excluded if they did not evaluate ertapenem as a pre-operative antibiotic prophylaxis in patients undergoing laparotomy or colectomy, the outcome was not SSIs, or it represented a review of prior publications. OpenBCI is a low-cost, open-source platform for alternative brain-computer interface (BCI) software and hardware. This thesis evaluates OpenBCI’s electroencephalogram (EEG) and electromyography (EMG) capabilities by constructing and testing a 16-channel EEG system using the Ultracortex Mark IV headset and Cyton + Daisy biosensing board. The viability of the system was assessed through real-time BCI control and comparison to a clinical-grade EEG system. Real-time BCI control of an online falling-block game was tested via the use of eye blinks EMG (channels Fp1/Fp2) and head-tilt accelerometer inputs. Advocacy is at the heart of nursing practice, and advocating for the social determinants of health (SDOH) is crucial when providing care to patients from vulnerable populations. Service-learning is a strategy that can be used to teach students advocacy skills. Chapter 2 of this dissertation portfolio presents a concept analysis that explores the definition of advocacy using the Walker and Avant method (2019).
