Prophylactic Resuscitative Endovascular Balloon Occlusion In The Management Of Placenta Percreta: A Case Report, Monica Rodriguez, Nicole Tenzel, Rachel Russo, Robert Cohen Jan 2025

Prophylactic Resuscitative Endovascular Balloon Occlusion In The Management Of Placenta Percreta: A Case Report, Monica Rodriguez, Nicole Tenzel, Rachel Russo, Robert Cohen

North Texas Research Forum 2025

Introduction: Placenta accreta spectrum disorders (PASD) are associated with severe maternal morbidity, including hemorrhage requiring massive transfusion, intensive care unit (ICU) admission, coagulopathy, organ ischemia or failure, and death. The adherent placenta results in continuous mixed arterial and venous bleeding, preventing the uterus from adequately contracting. This can result in a blood loss rate of 600 mL or more per minute, posing a risk of exsanguination in less than five minutes. Few reports exist on the prophylactic use of resuscitative endovascular balloon occlusion of the aorta (REBOA) to reduce obstetric hemorrhage and its application for prolonged occlusion times exceeding 60 …

Interferon-Gamma Receptor Signaling Regulates Innate Immunity During Staphylococcus Aureus Craniotomy Infection, Zachary Van Roy, Gunjan Kak, Rachel W. Fallet, Tammy Kielian Jan 2025

Interferon-Gamma Receptor Signaling Regulates Innate Immunity During Staphylococcus Aureus Craniotomy Infection, Zachary Van Roy, Gunjan Kak, Rachel W. Fallet, Tammy Kielian

Journal Articles: Pathology and Microbiology

A craniotomy is a neurosurgical procedure performed to access the intracranial space. In 3-5% of cases, infections can develop, most caused by Staphylococcus aureus biofilm formation on the skull surface. Medical management of this infection is difficult, as biofilm properties confer immune and antimicrobial recalcitrance to the infection and necessitate additional surgical procedures. Furthermore, treatment failure rates can be appreciably high. These factors, compounded with rapidly expanding rates of antimicrobial resistance, highlight the need to develop alternative treatment strategies to target and reverse the immune dysfunction that occurs during biofilm infection. Our recent work has identified CD4+ Th1 and Th17 …

High-Grade Avm. Educational Vignette And Clinical Cases, Marcos Devanir Silva Da Costa, Marcelo Augusto Acosta Goiri, Daniela Coelho, Juan Alberto Paz Archila, Talita Helena Martins Sarti, Feres Chaddad-Neto

Archives of Neurosurgery

The arteriovenous malformation (AVM) is a vascular malformation characterized by fistulas between arteries and veins forming a nidus, without interposition of capillary vessels and of which early draining vein is identified at the digital angiography. Potential reports of its existence date from the second century AD. Spetzler and Martin (S-M) proposed a classification considering three features and Lawton then added new predictors. A maximum of 10 score can be achieved by applying the modified S-M grade system. This classification guides decision-making and the possible choice of the multimodal treatment (surgery, radiosurgery and embolization). We discuss historical, pathological, clinical and therapeutic …

Cd4+ T Cell-Innate Immune Crosstalk Is Critical During Staphylococcus Aureus Craniotomy Infection, Gunjan Kak, Zachary A. Van Roy, Rachel W. Fallet, Lee E. Korshoj, Tammy Kielian Jan 2025

Cd4+ T Cell-Innate Immune Crosstalk Is Critical During Staphylococcus Aureus Craniotomy Infection, Gunjan Kak, Zachary A. Van Roy, Rachel W. Fallet, Lee E. Korshoj, Tammy Kielian

Journal Articles: Pathology and Microbiology

Access to the brain for treating neurological sequalae requires a craniotomy, which can be complicated by infection. Staphylococcus aureus accounts for half of craniotomy infections, increasing morbidity in a medically fragile patient population. T cells preferentially traffic to the brain during craniotomy infection; however, their functional importance is unknown. Using a mouse model of S. aureus craniotomy infection, CD4+ T cells were critical for bacterial containment, as treatment of WT animals with anti-CD4 exacerbated infection that was similar to phenotypes in Rag1-/- mice. Single-cell RNA-Seq (scRNA-Seq) revealed transcriptional heterogeneity in brain CD3+ infiltrates, with CD4+ cells most prominent that displayed …

Debating Member-Checks Of Exceptional Human Experiences, James Houran, Everton De Oliveira Maraldi, Beth M. Houran Jan 2025

Debating Member-Checks Of Exceptional Human Experiences, James Houran, Everton De Oliveira Maraldi, Beth M. Houran

International Journal of Transpersonal Studies Advance Publication Archive

This integrative essay reflects on member-checking and engaged research approaches in the study of exceptional human experiences (EHEs)—subjective accounts that often challenge conventional views of consciousness and physical reality. Member-checking, or inviting research participants to review data or interpretations, can improve credibility by aligning research with their lived experience. While benefits include transparency and insight, risks include confirmatory bias, emotional distress, power asymmetries, and logistical complexity. The evolving nature of EHEs further complicates participant validation. We therefore advocate a six-level "fit-for-purpose" system to balance rigor with ethical care and responsiveness. We also explore digital, asynchronous, and AI-assisted strategies to enhance …

Mental Health Stigma As A Barrier To Care, Whitney Grant Jan 2025

Mental Health Stigma As A Barrier To Care, Whitney Grant

Regis University Student Publications (comprehensive collection)

Abstract

Mental health stigma is a major barrier to care. The mental health stigma as a problem is relevant due to its influence on increasing the burden of mental health issues due to a lack of help-seeking, contributing to increased healthcare costs, and increasing adverse health outcomes. Currently there is no action consistently taken to assess or address this stigma in the primary care setting. Healthcare professionals can help to reduce the perceptions of stigma and lessen stigma as a barrier to care. A quality improvement project was implemented to evaluate if an education intervention by a primary care provider …

Materials and Methods: This is a single-blind, RCT conducted between 2018 and 2023 in patients 18 years or older who were undergoing surgeries …

Reproductive Autonomy In Immunological Intervention: Ethical Boundaries In Maternal-Fetal Immune Therapy, Marium Z. Rizvi Jan 2025

Reproductive Autonomy In Immunological Intervention: Ethical Boundaries In Maternal-Fetal Immune Therapy, Marium Z. Rizvi

Journal of Health Ethics

Recent advances in immunology have broken barriers in reproductive medicine through immune-modulating therapies that support fertility and pregnancy. Treatments such as intravenous immunoglobulin (IVIG) and intralipid infusions are increasingly offered to patients with Recurrent Implantation Failure (RIF) or Recurrent Pregnancy Loss (RPL), conditions that result in miscarriages and failed IVF treatments (Pillarysetti et al., 2023). Maternal-fetal immunology reveals a delicate interplay between immune tolerance and defense: the maternal system must accept a genetically foreign fetus while preserving immunological protection. Disruptions in this balance are implicated in implantation disorders and pregnancy failure. As a result, immune-based therapies have emerged as promising …

Growing old is perceived differently among world cultures. In some cultures, elders are revered and celebrated. However, in other cultures, growing old is dreaded and feared. Current Western culture depicts aging negatively through media, beauty marketing, and in general society. The elderly population is also a lost population as they are often forgotten about and overlooked. With aging comes with illness, depression, change in activities of daily living, financial stressors, isolation, societal changes, and death of loved ones, and eventually self. Chronic illness(es) can make it harder for the elderly to maintain …

Occupational Therapist's Role In Sensory Inclusive Space Design, Colleen Mcgill

SURF Posters 2025

The rise in individuals experiencing sensory processing difficulties has revealed the challenges they face in regard to engagement in different environments and social participation. Sensory processing difficulties can impact the individual's social connectedness to entertainment venues that confine larger crowds and overwhelming stimuli, causing a form of occupational deprivation. This form of occupational deprivation can not only affect the individual, but can also impact family interactions at social events. Occupational therapists have proven highly skilled when assessing and treating clients with sensory processing difficulties, as well as evaluating and adapting environments to promote successful participation. With that, there is a …
