High fructose corn syrup (HFCS) is a commonly used sweetener in soft drinks and processed foods, and HFCS exacerbates inflammation when consumed in excess. Fructose, a primary component of HFCS; however, it is unclear whether fructose directly activates inflammatory signaling. Growth hormone secretagogue receptor (GHSR) is a receptor of the nutrient‐sensing hormone ghrelin. We previously reported that GHSR ablation mitigates HFCS‐induced inflammation in adipose tissue and liver, shifting macrophages toward an anti‐inflammatory spectrum. Since inflammation is primarily governed by innate immune cells, such as macrophages in the peripheral tissues and microglia in the brain, this study aims to investigate whether …

Objective: To investigate the tissue dissolution efficacy of different concentrations of sodium hypochlorite (NaOCl) using the GentleWave® technology (GW) and compare it with that of passive ultrasonic irrigation. This study will provide a novel in vitro model used to evaluate the tissue dissolution efficacy of different concentrations of NaOCl using the GentleWave® procedure.

Congenital heart disease (CHD) is a major health concern worldwide. This study focused on survival analysis and the factors influencing survival in infants with critical congenital heart disease (CCHD). A total of 79 infants diagnosed with CCHD were identified, with 48.1% (n=38) exhibiting duct-dependent pulmonary circulation, 35.6% (n=28) exhibiting duct-dependent systemic circulation, 6.3% (n=5) exhibiting critical non-duct-dependent, and 10.2% (n=8) exhibiting parallel circulation issues. Of the infants studied, 55.7% (n=44) were male, 78.5% (n=62) had a gestational age of ≥37 weeks, 62% (n=49) had a birth weight of ≥2,500 grams, 59.5% (n=47) exhibited normal fetal growth, 93.7% (n=74) experienced no …

Adolescence is a vulnerable period to reproductive health problems, especially regarding issues of sexuality. Healthy adolescent behavior is needed to achieve the SDGs targets by 2030. This study aimed to analyze personal factors such as attitudes, perceptions, and self-efficacy, as well as environmental factors like family, school, friends, community, and social media's role in adolescent reproductive health behavior and health promotion models. This bibliometric study examined the evolving research landscape on physician mental health from 2010 to 2023, analyzing key themes, publication trends, and global contributions. A total of 3,360 articles, including 3,137 original research and 223 reviews, were sourced from Web of Science and Scopus. Findings revealed a significant increase in research output, particularly during the COVID-19 pandemic, with burnout, depression, and anxiety emerging as dominant themes. The US accounted for most publications, while research from low- and middle-income countries remains limited, indicating a substantial global disparity. Assessing adolescent resilience may provide valuable insights into adult resilience. This study aimed to analyze the relationship between determinants and adolescent resilience in Surabaya City. The participants of this cross-sectional study were 277 adolescents aged 12–19 years who were in senior high school. The outcome was the resilience levels. Many districts and cities in Indonesia continue to have Public Health Development Index scores below the national average, highlighting the need for improved health governance. This study assessed the five-year impact (2019–2023) of a mentoring program for 149 District Health Offices in preparing annual work plans, focusing on stagnation or performance declines. Using a qualitative approach, the study analyzed annual work plan scores before and after technical assistance, conducted focus group discussions, and carried out in-depth interviews to identify challenges. Mental health issues are a prominent burden among adolescents and young adults in Southeast Asia, compounded by reluctance to seek help and low treatment rates. This mixed-method systematic review analyzed help-seeking profiles (preference, facilitators, and barriers) and evaluated implemented help-seeking programs. Data were sourced from electronic databases, hand-searched local databases, conference proceedings, and grey literature. Study selection followed PRISMA 2020 guidelines, focusing on 1) adolescents (10–19 years) and young adults (18–26 years) in Southeast Asia who were not receiving mental health care, 2) assessing help-seeking behavior, and 3) evaluating programs to increase help-seeking. The increased risk for psychosocial burden and suicidality in people with epilepsy compared to the general population is a well-established global public health concern. Suicidality risk is also increased in patients with functional seizures. The timely identification of patients at highest risk for psychosocial burden and self-harm is vital. This can pose a significant challenge for multidisciplinary clinicians caring for people with epilepsy. Pericardial effusions, though relatively rare, can lead to life-threatening complications such as cardiac tamponade. While viral etiologies are common culprits, rapid and accurate diagnosis remains challenging. We present the case of a 74-year-old male with a history of upper respiratory infection who developed sudden onset dyspnea and chest discomfort. Bedside point-of-care ultrasound (POCUS) revealed a large pericardial effusion, prompting urgent intervention. Despite initially stable vital signs, the patient rapidly deteriorated, necessitating emergent pericardiocentesis. Laboratory findings and pathology results eventually ruled out common viral causes, guiding diagnosis toward coxsackieviruses A and B, echovirus, adenoviruses, or influenza. Cerebral arteriovenous malformations (AVMs) are congenital vascular anomalies that can lead to severe complications, including hemorrhage and neurological deficits. This study compares the outcomes of microsurgical resection and stereotactic radiosurgery (SRS) for SM grade I and II AVMs. Out of a large multicenter registry, we identified 180 matched patients with SM grade I and II AVMs treated with either microsurgical resection or SRS between 2010 and 2023. The primary outcomes were AVM obliteration rates and complications; secondary outcomes included neurological status and functional outcomes measured by the modified Rankin Scale (mRS). Background: One percent of pancreatic adenocarcinoma (PDAC) patients are diagnosed with new onset diabetes (NOD) over the age of 50 years within 3 years. Therefore, NOD is a major factor for early diagnosis of PDAC. Research has focused on understanding the differences between NOD and type 2 diabetes, particularly in relation to PDAC. However, conflicting data exists regarding their impact on survival outcomes in PDAC patients. We performed this multi-center study to assess the prevalence and influence of NOD on clinical outcomes in patients with PDAC within a community-based hospital system.

PURPOSE OF REVIEW: The purpose of this review is to highlight the utility of echocardiography in the diagnosis and management of cardiomyopathies.
RECENT FINDINGS: Echocardiographic parameters function synergistically to guide decision-making ranging from early detection of disease and screening to risk stratification of complex disease. The collective wealth of information available from 2D/3D assessment, Doppler, diastology and strain makes echocardiography an invaluable decision aid.

Hypertension is one of the most chronic conditions in the United States and worldwide. Patients with lower socioeconomic class tend to have poor control of their blood pressure and higher morbidity and mortality rates. The scholarly project was conducted to identify if a nurse-led eight-to-10-week telehealth program improves blood pressure in the uninsured and underinsured community. Anterior cruciate ligament (ACL) injury adversely affects skeletal muscle, leading to muscle atrophy and weakness, significantly impacting clinical outcomes. This study aimed to determine if estrogen-related receptor gamma (ERRγ) overexpression in skeletal muscle could mitigate muscle atrophy after ACL injury. An animal model with selective overexpression of ERRγ in skeletal muscle (ERR-gamma transgenic mice, TG) and WT control mice were used for this study. Microglia actively survey the brain and dynamically interact with neurons to maintain brain homeostasis. Microglial Gi protein-coupled receptors (Gi-GPCRs) play a critical role in microglia-neuron communications. However, the impact of temporally activating microglial Gi signaling on microglial dynamics and neuronal activity in the homeostatic brain remains largely unknown. In this study, we used Gi-based designer receptors exclusively activated by designer drugs (Gi-DREADD) to selectively and temporally modulate microglial Gi signaling pathway. Cerebral arteriovenous malformations (AVMs) are congenital vascular anomalies that can lead to severe complications, including hemorrhage and neurological deficits. This study compares the outcomes of microsurgical resection and stereotactic radiosurgery (SRS) for SM grade I and II AVMs. Out of a large multicenter registry, we identified 180 matched patients with SM grade I and II AVMs treated with either microsurgical resection or SRS between 2010 and 2023. The primary outcomes were AVM obliteration rates and complications; secondary outcomes included neurological status and functional outcomes measured by the modified Rankin Scale (mRS). Plasma membrane rupture by Ninjurin-1 (NINJ1) executes programmed cell death, releasing large cellular damage-associated molecular patterns (DAMPs). However, the regulation and selectivity of NINJ1-mediated DAMP release remain unexplored. Here, we uncover that murine norovirus (MNoV) strategically co-opts NINJ1 to selectively release the intracellular viral protein NS1, while NINJ1-mediated plasma membrane rupture simultaneously bulk-releases various cellular DAMPs. Host caspase-3 cleaves the precursor NS1/2, leading to NS1 secretion via an unconventional pathway. An unbiased CRISPR screen identifies NINJ1 as an essential factor for NS1 secretion. Introduction: Burnout has been described as “a state of mental and physical exhaustion related to work or caregiving activities.” Among resident physicians, burnout is a serious concern that can lead to medical errors and negative patient outcomes. To combat burnout among physiatry trainees during the COVID-19 pandemic, an innovative wellness retreat was created and implemented at the University of Miami’s Physical Medicine and Rehabilitation (PM&R) Residency Program in March 2021 and February 2022.
