การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการจัดการภาวะฟอสเฟตในเลือดสูงตามแนวคิดทฤษฎีการเปลี่ยนแปลงพฤติกรรมและการสัมภาษณ์เพื่อเสริมสร้างแรงจูงใจผ่านทางเทเลเฮลท์ในกลุ่มผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายที่ได้รับการบำบัดทดแทนไตโดยการฟอกเลือดด้วยเครื่องไตเทียม (โปรแกรม TMT) ในสถานการณ์การระบาดของไวรัสโคโรนา 2019 การศึกษานี้เป็นการศึกษาเชิงทดลองแบบสุ่มและมีกลุ่มควบคุม มีผู้เข้าร่วมวิจัยที่ตรงตามเกณฑ์ทั้งหมด 80 คน ได้รับการสุ่มเข้ากลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 40 คน ด้วยวิธีสุ่มแบบชั้นภูมิร่วมกับการใช้บล็อก โดยกลุ่มทดลองได้รับโปรแกรม TMT เปรียบเทียบกับกลุ่มควบคุม ซึ่งได้รับการดูแลแบบปกติตามมาตรฐานการรักษาในศูนย์ฟอกเลือดด้วยเครื่องไตเทียม วิเคราะห์ข้อมูลโดยใช้สถิติ Linear regression เพื่อเปรียบเทียบค่าเฉลี่ยของตัวแปรและสถิติ Fisher’s exact test เพื่อเปรียบเทียบตัวแปรเชิงกลุ่มระหว่างกลุ่มทดลองและกลุ่มควบคุมที่ระดับความเชื่อมั่นร้อยละ 95 ใช้ระยะเวลาในการศึกษา 24 สัปดาห์ ผลการศึกษาจากการวิเคราะห์ข้อมูลแบบ Intention to treat พบว่าระดับฟอสเฟตในเลือดของกลุ่มทดลองลดลงมากกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (ผลต่างค่าเฉลี่ย = -1.03, 95% CI = -1.77, -0.29) พฤติกรรมการบริโภคอาหารที่มีส่วนประกอบของฟอสฟอรัสในกลุ่มทดลองดีกว่ากลุ่มควบคุม (ผลต่างค่าเฉลี่ย = 37.76, 95% CI = 23.54, 51.98) ระดับความพร้อมในการเปลี่ยนแปลงพฤติกรรม ความเชื่อมั่นในการรับประทานยาจับฟอสเฟตและพฤติกรรมการรับประทานยาจับฟอสเฟตอย่างต่อเนื่องของกลุ่มทดลองดีกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ p-value < 0.001, 0.025 และ 0.001 ตามลำดับ แม้ว่าความรู้เกี่ยวกับการป้องกันและจัดการภาวะฟอสเฟตในเลือดสูงของทั้งสองกลุ่มไม่แตกต่างกัน (ผลต่างค่าเฉลี่ย = 3.86, 95% CI = -0.57, 8.29) จากผลของการศึกษาแสดงให้เห็นว่าโปรแกรม TMT สามารถนำมาประยุกต์ใช้เพื่อช่วยลดระดับฟอสเฟตในเลือดให้กับผู้ป่วยได้

การวิจัยนี้เป็นการศึกษาเชิงวิเคราะห์แบบติดตามไปข้างหน้า มีวัตถุประสงค์เพื่อศึกษาความถูกต้องและความน่าเชื่อถือของโมเดลไดนามิกส์ของการแพร่เชื้อวัณโรค ซึ่งประกอบด้วย 1) the Wells-Riley equation 2) Rudnick & Milton model 2 รูปแบบ 3) the Applied SEIR models และ 4) Issarow et al model ทำการศึกษาในกลุ่มตัวอย่าง คือ ห้องขังของเรือนจำความมั่นคงสูง 3 แห่ง จำนวน 985 ห้อง ใช้สถิติ Poisson regression ในการวิเคราะห์ความสัมพันธ์ระหว่างค่าความน่าจะเป็นของการแพร่เชื้อวัณโรคและอัตราเสี่ยงของการเกิดวัณโรคปอด วิเคราะห์ค่าความสอดคล้องด้วยการวิเคราะห์สหสัมพันธ์และสัมประสิทธิ์สหสัมพันธ์ภายในชั้น วิเคราะห์ความเหมาะสมของโมเดลด้วยเกณฑ์ของอาไคเคะและเกณฑ์ของเบส์ วิเคราะห์ความตรงเชิงพยากรณ์ และหาค่าพื้นที่ใต้กราฟ ผลพบว่า มีอุบัติการณ์ของผู้ป่วยวัณโรคปอดในผู้ต้องขังอย่างน้อย 1 คน ในห้องขัง 138 ห้อง จากห้องขัง 985 ห้อง (ร้อยละ 14.01) ค่าความน่าจะเป็นของการแพร่เชื้อวัณโรคที่ได้จากโมเดลไดนามิกส์ทั้ง 5 รูปแบบ มีความสัมพันธ์ทางบวกกับอัตราอุบัติการณ์ของวัณโรคปอดอย่างมีนัยสำคัญทางสถิติ ผลการประเมินความเหมาะสมของโมเดลไดนามิกส์ทั้งในแง่ความถูกต้องและความน่าเชื่อถือ พบว่า the Wells-Riley equation และ Issarow et al model มีความเหมาะสมในการประเมินความเสี่ยงของการเกิดวัณโรคปอด นอกจากนี้ยังพบว่า ค่าอัตราการระบายอากาศแบบรอบต่อชั่วโมง จำนวนผู้ป่วยวัณโรคในห้องขังและค่าความชุกของผู้ป่วยเสมหะบวกที่หนาแน่นในแดนส่งผลต่ออุบัติการณ์ของการเกิดวัณโรคปอด สรุปผลการศึกษานี้ แสดงข้อมูลเชิงประจักษ์ที่พบว่าโมเดลไดนามิกส์ทั้ง 5 รูปแบบสามารถทำนายความเสี่ยงของการเกิดวัณโรคปอดได้อย่างถูกต้องและปัจจัยด้านการระบายอากาศและการมีผู้ป่วยวัณโรคในห้องขังเป็นปัจจัยที่สำคัญในการควบคุมวัณโรคในบริบทของเรือนจำ

Mathurameha (MT) is a Thai traditional herbal formulation consisting of 26 herbs used for diabetes over 30 years. The study was to identify inhibitory compounds in Mathurameha against pancreatin amylase using the HPTLC-bioautographic technique. The results showed that MT inhibited pancreatin amylase activities, and the most active compounds were identified as andrographolide and gallic acid. The hypoglycemic effect of MT was determined using cell-based assays in L6 myotubes, 3T3-L1 adipocytes, and Caco2 cells. The study revealed that MT water extract (MTW, 125-1000 µg/mL), gallic acid (12.5-50 µM), and andrographolide (12.5-50 µM) significantly promoted glucose uptake in L6 myotubes and inhibited …

การวิจัยนี้ เป็นการศึกษาแบบ Ecological study ที่ผสมผสานระหว่าง Multiple-group กับ Time-trend design โดยมีวัตถุประสงค์ เพื่ออธิบายอัตราคลอดในวัยรุ่นอายุ 15-19 ปี ระดับอำเภอในประเทศไทยทางภูมิศาสตร์ วัตถุประสงค์เฉพาะ: (ก) เพื่อระบุจุดร้อน จุดเย็น และค่าผิดปกติเชิงพื้นที่ (ข) ระบุความสัมพันธ์ระหว่างอัตราคลอดในวัยรุ่นฯ และปัจจัยบริบทเชิงพื้นที่ และ (ค) ตรวจสอบว่าความสัมพันธ์ที่ระบุแตกต่างกันไปตามพื้นที่และเวลาหรือไม่ ดำเนินการศึกษาใน 878 อำเภอทั่วประเทศ ยกเว้นในเขตกรุงเทพมหานคร และระยะเวลาการศึกษาคือช่วง 10 ปี ระหว่างปี พ.ศ. 2552-2561 ใช้ซอฟต์แวร์ Arc GIS Pro 3.0 เพื่อวิเคราะห์และสร้างแผนที่อัตราคลอดในวัยรุ่นฯ และตรวจสอบรูปแบบเชิงพื้น จุดร้อน จุดเย็น และค่าความผิดปกติ ใช้ Ordinary Least Squares (OLS) regression and Geographically Weighted Regression (GWR) เพื่อประเมินความสัมพันธ์ระหว่างอัตราคลอดในวัยรุ่นและปัจจัยบริบทแวดล้อม การวิเคราะห์ข้อมูลแบ่งเป็น 2 ช่วง คือ ช่วงปี 2552-2554 และช่วงปี 2556-2561 ผลการศึกษาพบว่า อัตราคลอดในวัยรุ่นอายุ 15-19 ปี ระดับอำเภอในประเทศไทย ทั้งสองช่วงเวลามีรูปแบบกระจายแบบไม่เป็นระเบียบหรือกระจายแบบไม่มีทิศทาง (random) และพบกลุ่มอำเภอเล็ก ๆ ที่มีนัยสำคัญของอัตราคลอดในวัยรุ่นสูง (จุดร้อน) หรือต่ำ (จุดเย็น) ในปี พ.ศ. 2552-2554 และปี พ.ศ. 2556-2561 โมเดล GWR อธิบายความสัมพันธ์ระหว่างปัจจัยบริบทเชิงพื้นที่และอัตราคลอดในวัยรุ่น ได้ดีกว่าโมเดล OLS ช่วงปี 2552-2554 โมเดล GWR อธิบายความแปรผันของอัตราคลอดในวัยรุ่นระดับอำเภอได้ร้อยละ 23.6 พื้นที่ทางภูมิศาสตร์ที่มีตัวแปรอิสระทำนายอัตราคลอดในวัยรุ่นที่แข็งแกร่งและอ่อนแอในประเทศไทย ได้แก่ ความเหลื่อมล้ำทางรายได้ ค่าใช้จ่ายเฉลี่ยต่อคนต่อเดือน วัยรุ่นได้รับการศึกษาภาคบังคับ …

Human serum albumin (HSA), primarily derived from human plasma, is limited by supply constraints and safety concerns due to potential contamination with blood-borne pathogens. Given the global shortage, hundreds of tons of HSA are required for clinical and cell culture applications. To address this, our study explored the production of recombinant HSA using Nicotiana benthamiana as an alternative production platform. The transient expression of recombinant HSA in plants was optimized by adjusting the harvesting days, plants aged and the OD600 of Agrobacterium. Translation of CAR T cell therapy for AML has been challenging due to the lack of target specificity. One promising target is TIM3, which is overexpressed on AML cells and absent in majority of healthy hematopoietic cells. Previous reports have shown that each of CAR’s structural designs impacts CAR functionality. Here, we evaluated the influence of non-signaling domains on a preclinical TIM3-targeting CAR construct. การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาผลฉับพลันของการวิ่งเร็วแบบก้าวกระโดดร่วมกับการจำกัดการไหลเวียนเลือดที่มีผลต่อพลังกล้ามเนื้อและความสามารถในการเร่งความเร็วของนักวิ่งระยะสั้นเยาวชนชาย กลุ่มตัวอย่างเป็นนักวิ่งระยะสั้นเพศชาย อายุ 15-16 ปี จำนวน 13 คน โดยกลุ่มตัวอย่างทุกคนจะได้รับการทดลองครบทั้ง 4 เงื่อนไข ได้แก่ การวิ่งเร็วแบบก้าวกระโดดร่วมกับการจำกัดการไหลเวียนเลือด 60%ของแรงดันการปิดกั้นการไหลเวียนเลือดแดง (Aortic occlusion pressure, AOP) การวิ่งเร็วแบบก้าวกระโดดร่วมกับการจำกัดการไหลเวียนเลือด 40%AOP การวิ่งเร็วแบบก้าวกระโดดอย่างเดียว และเงื่อนไขควบคุม โดยทำการทดสอบเวลาและความเร็วในการวิ่งระยะ 20 และ 50 เมตร ความสามารถในการกระโดดในแนวราบ ความสามารถในการกระโดดในแนวดิ่ง และคลื่นไฟฟ้ากล้ามเนื้อจากกล้ามเนื้อ เรกตัส ฟีเมอร์ริส (Rectus femoris) แกสตรอคนีเมียส (Gastrocnemius) และ โซลีอุส (Soleus) ก่อนและหลังการทดลองนาทีที่ 4, 8 และ 12 ในแต่ละเงื่อนไข วิเคราะห์ข้อมูลทางสถิติโดยทดสอบความแตกต่างของค่าเฉลี่ยของตัวแปรตามด้วยการวิเคราะห์แปรปรวนสองทางชนิดวัดซ้ำ (Two-way analysis of variance with repeated measure) กำหนดระดับความมีนัยสำคัญทางสถิติที่ .05 ผลการวิจัยพบว่า ค่าเฉลี่ยเวลาและความเร็วในการวิ่งระยะ 20 และ 50 เมตร ไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 ทั้งในช่วงเวลาการทดสอบและเงื่อนไข ขณะที่ร้อยละของแอมพลิจูดของคลื่นไฟฟ้าของกล้ามเนื้อโซลีอุส (Soleus) สูงสุดเพิ่มขึ้นหลังการทดลองนาทีที่ 8 ในการวิ่งเร็วแบบก้าวกระโดดร่วมกับการจำกัดการไหลเวียนเลือด 60%AOP และหลังการทดลองนาทีที่ 12 เมื่อเทียบกับหลังการทดลองนาทีที่ 8 ในการวิ่งเร็วแบบก้าวกระโดดอย่างเดียว นอกจากนี้ยังพบว่าร้อยละของพลังสูงสุดเพิ่มขึ้นหลังการทดลองนาทีที่ 8 และ 12 ในการวิ่งเร็วแบบก้าวกระโดดร่วมกับการจำกัดการไหลเวียนเลือด 60% ขณะที่อัตราส่วนระหว่างเวลาที่เท้าลอยจากพื้นต่อเวลาที่เท้าสัมผัสพื้นเพิ่มขึ้นหลังการทดลองนาทีที่ 8 ส่วนระยะทางการกระโดดแนวราบเพิ่มขึ้นหลังการทดลองนาทีที่ 12 เมื่อเทียบกับก่อนทดลองในการวิ่งเร็วแบบก้าวกระโดดร่วมกับการจำกัดการไหลเวียนเลือด 60%AOP อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 สรุปผลการวิจัย การวิ่งเร็วแบบก้าวกระโดดร่วมกับการจำกัดการไหลเวียนเลือด 60%AOP สามารถกระตุ้นการทำงานของกล้ามเนื้อ soleus ได้เมื่อเทียบกับการออกกำลังกายในเงื่อนไขอื่น อาจส่งผลให้สมรรถภาพด้านความสามารถในการกระโดดแนวราบ และตัวแปรความสามารถในการกระโดดแนวดิ่ง ได้แก่ พลังกล้ามเนื้อ เวลาที่เท้าสัมผัสพื้น …

การวิจัยในครั้งนี้มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการฝึกการก้าวเท้าโดยใช้ตาราง 25 ช่องที่บ้านที่มีต่อความสามารถในการเคลื่อนไหว การเดิน การทรงตัว สมรรถภาพทางกายและคุณภาพชีวิตในผู้ที่เป็นโรคพาร์กินสัน กลุ่มตัวอย่างเป็นผู้ป่วยพาร์กินสันที่เข้ารับการรักษาที่ศูนย์รักษาโรคพาร์กินสันและกลุ่มโรคความเคลื่อนไหวผิดปกติ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย อายุ 50-75 ปี มีระดับความรุนแรงของโรค (Modified Hoehn & Yahr stages) อยู่ในระยะที่ 2, 2.5 และ 3 จำนวน 36 คน โดยวิธีการแบ่งกลุ่มแบบจับคู่ (Matched pair) โดยแบ่งตามเพศ อายุ ระยะเวลาการดำเนินของโรค (ปี) และระดับความรุนแรงของโรค โดยเป็นกลุ่มทดลอง จำนวน 18 คน และกลุ่มควบคุม จำนวน 18 คน ในกลุ่มทดลองจะได้รับการฝึกด้วยโปรแกรมการฝึกการก้าวเท้าที่บ้าน เป็นเวลา 12 สัปดาห์ สัปดาห์ละ 3 ครั้ง ครั้งละ 60 นาที กลุ่มควบคุมดำเนินชีวิตประจำวันตามปกติ ทำการทดสอบก่อนและหลังการทดลอง โดยทำการทดสอบความสามารถในการเคลื่อนไหวด้วยเครื่องวิเคราะห์การเคลื่อนไหว Quantitative Timed up and go test (QTUG) with Kinesis แบบทดสอบทางคลินิกสำหรับผู้ที่เป็นโรคพาร์กินสัน ด้านกิจวัตรประจำวันและด้านการเคลื่อนไหว (The unified Parkinson’s disease rating scale; UPDRS II & III) ทำการทดสอบการเดินด้วยเครื่องวิเคราะห์ใต้ฝ่าเท้าแบบบาง (Strideway) และแบบประเมินการเดินติด (Freezing of gait questionnaires; FOG-Q) ทำการทดสอบการทรงตัวขณะอยู่นิ่งด้วยเครื่อง Biodex balance system และประเมินการทรงตัวขณะเคลื่อนไหวด้วย Mini-BESTest ทำการทดสอบสมรรถภาพทางกายโดยใช้แบบทดสอบสมรรภาพทางกายสำหรับผู้สูงอายุ (Senior physical fitness) และด้านคุณภาพชีวิตทดสอบโดยใช้แบบสอบถามด้านคุณภาพชีวิตสำหรับผู้ที่เป็นโรคพาร์กินสัน (Parkinson’s disease questionnaire 8; PDQ-8) …

The Myopia Outcome Study of Atropine in Children (MOSAIC) is an investigator-led, double-masked, randomized controlled trial investigating the efficacy and safety of 0.01% atropine eye drops for managing myopia progression in a predominantly White, European population.

The aim of this dissertation was to explore how adults with a Lifelong Disability (LLD) access and experience rehabilitation therapy services across their lives. Many LLD diagnoses such as cerebral palsy and spina bifida are considered non-progressive; however, there are common challenges that occur as people with LLD become adults, including increased pain, fatigue, and functional loss. Both patellofemoral dislocation and anterior cruciate ligament (ACL) ruptures have a significant impact on an athlete’s ability to regain function and return to play. Surgical repair of the medial patellofemoral ligament (MPFL) and ACL ligament reconstructions result in a sequala of quadriceps weakness. Persistent quadriceps weakness following reconstruction can create a cascade effect of muscular atrophy and weakness resulting in significant knee and leg impairments. Firefighting is considered one of the most dangerous occupations in the workforce. Musculoskeletal injuries are consistently the most frequent, and most costly, documented non-fatal injuries in the firefighting setting. An increase in job stress, job exposure, and deprived sleep secondary to job-related duties are plausible explanations for the frequency in musculoskeletal injury. The aims of this dissertation were to ascertain not only how occupational exposure via firefighter emergency call volume may influence outcomes contributing to job stress and objective and subjective sleep metrics, but also its influence on the incidences of musculoskeletal pain.

Lesbian, gay, and bisexual individuals (LGBI) have a higher rate of mental health disorders than the general population. The mental health disparities experienced by LGBI are positively correlated with higher internalized homonegativity (IH) levels, which impacts their functioning at the unconscious, cognitive, and interpersonal levels. Thus, effective interventions targeting the mental health of LGBI must consider IH's intrapsychic, cognitive, and interpersonal nature. An integrative psychotherapy approach addressing the mental health and IH of LGBI is needed to address the intrapsychic, cognitive, and interpersonal factors at play.

Improperly polymerized self-adhesive resin cements (SARCs) can lead to adverse effects such as periodontitis or peri-implantitis. This in vitro study investigated the effect of 2 light-polymerizing units (LPUs) on the physical and chemical properties of 2 SARCs under varying simulated clinical scenarios and the responses of primary human gingival fibroblasts (hGFs) exposed to such procedures. Dental cement discs of SARCs (depth=2 mm, diameter=5 mm) were prepared with cylindrical acrylic molds. Heel counters are (usually) a rigid piece of material embedded in or molded around the upper of the rearfoot. Usually, these articles promise “secret knowledge” of what each wear pattern means and what horrible injury you’re therefore due. Individual companies often claim that certain wear patterns mean you need specific shoes or orthotics, which, lucky for you, they happen to be selling.
