Roles Of Albumin In Patients With Chronic Liver Disease: Evidence-Based Medicine, Akarawut Juntrapirat, Sombat Treeprasertsuk

This article details the roles of albumin in patients with chronic liver disease especially cirrhosis according to evidence-based information. In addition, this review includes the summary of several guidelines for albumin use in patients with chronic liver disease including those who are undergoing large volume paracentesis (more than 5 litre), those with spontaneous bacterial peritonitis (SBP), and those with hepatorenal syndrome (HRS), which occurred mainly in decompensated cirrhotic patients and acute liver failure. The physiologic functions of albumin and the other potential indications of albumin in cirrhotic patients are also mentioned.

Purpose
Albumin is recommended for the treatment of spontaneous bacterial peritonitis (SBP) in patients at high risk for mortality. We assessed adherence to guidelines for administration of albumin for SBP in clinical practice at a private tertiary care hospital.
Methods
A retrospective clinical analysis of all cases of SBP diagnosed at a tertiary referral center from January 1, 2006, to December 31, 2012, was performed. Albumin provides the vital scaffolding for growth and tissue repair and supports oncotic blood pressure and hemodynamics. In hemodialysis patients, albumin aids with fluid removal by drawing excess fluid from edematous tissues back into the blood where it can then be removed by a dialyzer. The hyperglycemia seen in dialysis patients with Type II diabetes progressively damages kidney glomeruli, which permits albumin seepage into the urine, thus lowering serum albumin. The conceptual framework underpinning this research is the van't Hoff theory of osmotic pressure. Dementia is a syndrome associated with declining cognitive function that has a variety of types and causes, and is encountered frequently in general medical practice. Researchers are actively exploring possible risk factors for dementia. The St. Louis University Mental Status (SLUMS) exam is a dementia-screening exam used in primary care visits to detect cognitive impairment that may be a sign of dementia.
