Syncope is a serious but potentially preventable occurrence commonly afflicting the elderly population. The objectives of this study were to estimate the incidence of syncope in the community-dwelling elderly population, to identify the clinical risk factors that predispose some elderly persons to syncope, and to describe situational factors and the morbidity associated with syncope. Study participants were members of the Project Safety cohort, representing a probability sample of non-institutionalized residents of New Haven, Connecticut aged 72 years or older. Age-restricted housing lists and utilities listings were used to generate an index of community dwellings from which 5,936 households were identified. …

Information on these topics enables nutrition educators to develop strategies to improve knowledge and to tailor educational efforts which affect a large number of persons. Poor health in America today is an ever present reality. This quasi-experimental study examined the effect of an educational program on the attitudes of hypertensive African-American clients regarding their adherence to a prescribed medical regimen.
Despite increased public awareness and improved treatment in hypertension, poorly controlled hypertension continues to be a significant problem especially among blacks and other minority populations in the United States (Shea, Misra, Ehlich, Field, & Francis, 1991). Case management services are an expanding program in the Texas Department of Mental Health and Mental Retardation (TXMHMR) system's treatment for chronically mentally ill individuals (CMI). CMI’s are individuals who are unable to maintain a stable adjustment to community life because of persistent and serious emotional disabilities. Twenty infertile couples participated in the present study to investigate whether infertile couples receiving medical treatment experience changes in their emotional, marital and sexual adjustment as a result of the length of time they have been in treatment. The couples were divided into three groups based on how long they had been seeking medical attention for infertility. The instruments administered were: SCL-90-R (Symptom Checklist), Dyadic Adjustment Scale (DAS) and Index of Sexual Satisfaction (ISS). Due to the presumed associations of dopamine (DA) receptor supersensitivity phenomena in both long-term neuroleptic-treated tardive dyskinetic rats and neonatal 6-hydroxydopamine (n6-OHDA)-lesioned rats, we studied the influence of haloperidol on n6-OHDA-lesioned rats. At 3 days after birth rats received 6-OHDA-HBr (200 $\mu$g, bilateral intracerebroventricularly; desipramine pretreatment, 20 mg/kg, 1h) or vehicle. Two months later haloperidol (1.5/kg/day $\times$ 2 days/week for 4 weeks, then 1.5 mg/kg/day, every day for 10 months) was added to the drinking water. Spontaneous oral activity of intact and n6-OHDA-lesioned rats receiving haloperidol was reached and maintained at significantly higher levels after 15 weeks of haloperidol treatment. …

Previous studies of the developing nervous system have shown that cell-cell and cell-matrix interactions are involved in a variety of processes such as the proliferation, migration and differentiation of neurons. While many cell surface molecules have been identified, the signal transduction mechanisms through which they alter cellular responses are poorly understood. Recent studies have described a new and large family of enzymes, protein tyrosine phosphatases (PTPases), that may play a key role in transduction of cell surface events. The problem of increasing health care costs and decreasing access to health care services is a major crisis in the U.S. today. One factor contributing to the rising cost of health care is the inappropriate use of emergency rooms, (ERs). In response to escalating costs a local health care corporation implemented a telephone triage program called the After Hours Program, (AHP). The goal of the program was to reduce inappropriate utilization of emergency rooms, and to educate patients to recognize and treat specific health problems independently, i.e., home treatment.

Anorexia nervosa is now being viewed as a multidimensional disorder in terms of predisposing factors such as genetics, biology, and environment. Results from twin studies suggest that part of a susceptibility to AN may lie in genetic factors (Holland et al., 1984, 1988; Treasure and Holland, 1991). However, the nature of a genetic contribution, if any, remains unclear. Neurochemical alterations have been found to be associated with AN, but it is difficult to assess what role these neurochemical changes play in the etiology of the disorder since they may precipitate, accompany, or follow weight loss (Fava et al., 1989). Moraxella (Branhamella) catarrhalis is the third-most-frequently isolated microorganism associated with acute exacerbations of chronic bronchitis in patients during their stay at the Mountain Home VA Medical Center (MHVAMC). In order to develop a practical, epidemiologically-meaningful typing method for M. (B.) catarrhalis, we tested two methods based on analysis of chromosomal DNA for typeability, reproducibility, and ability to differentiate between unrelated strains (discriminatory power, D). M. (B.) catarrhalis isolants from MHVAMC from 7/1/87-6/30/88 were grown overnight in broth and embedded in agarose. DNA was isolated by standard methods. In an effort to elucidate the mechanism by which indomethacin (IN) attenuates the stimulatory effect of estradiol (E$\sb2$) on rabbit splenic red pulp macrophages (RPM), thirty-nine female New Zealand White rabbits were divided into 10 groups: ovariectomized (OVX), OVX/IN at 0.1 and 5.0 mg/kg body weight (bw)/day; sham OVX (SOVX), SOVX/IN at 0.1 and 5.0 mg/kg bw/day; OVX/25 mg E2, OVX/25 mg E$\sb2$/IN at 0.1 and 5.0 mg/kg bw/day; intact Control. The purpose of this study was to examine and analyze parent care as experienced by middle-aged Filipino women, particularly those who have resided in the United States for over 20 years. There is little information known about this topic; therefore, a grounded theory study design was utilized. Open-ended, semi-structured interviews of 29 Filipino women were conducted in English and/or Tagalog, and at times a combination of both languages. Data were transcribed verbatim from the audio-recorded interviews and analyzed using a constant comparative method of analytic induction. Identification of variables which contribute to both positive and negative patient experience in managed care systems is crucial to their effectiveness. Using historical methodology, this study examined the concept of creativity in Western nursing from a cultural feminist perspective. An exploration of the concept was accomplished by means of a review emanating from the English literature of Western nursing and its allied disciplines of education and psychology. Two distinct views of creativity were delineated--the classical and the contemporary. The classical understanding required the completion of a major product or attainment in order for creativity to be ascribed. In contrast, the contemporary view was that of a novel, unrestrained thought process. It required no creative work or attainment for its ascription. The …

The use of free choice drawings and contingent questions as prompts to enhance oral language development in young children with moderate to profound hearing losses was the focus of this descriptive and experimental study. The effects of contingent queries (scaffolding) on drawing and oral language were measured using a single subject experimental design in which a staggered baseline was followed by treatment replicated across two sets of students at two levels of language abilities. An extensive coding system for dependent drawing and language variables was developed and utilized for analysis.

Agenesis of the corpus callosum is a disorder which has been the focus of clinical research on various tasks involving interhemispheric transfer of information. However, therapeutic intervention for individuals with this disorder has not been located in the literature by this researcher. Speech-language therapy for a five-year-old male incorporating interaction with songs was employed to activate various portions of the congenitally separated cerebral hemispheres and to encourage information transfer through non-traditional ways to facilitate syllable production.

We embarked on a study to define the role of protein-tyrosine phosphatases (PTPases) in both normal melanocytes and melanoma cells, searching for specific differences that might provide clues to the malignant transformation process in these cells and to the role of PTPases as possible tumor suppressor genes. We used Northern blotting and RNase protection assays to measure mRNA expression levels, immunoprecipitation and immunoblotting to measure protein expression, and a bioassay to measure protein PTPase activity in normal melanocyte and melanoma cell lines.
Our experiments showed that all major PTPases are equally expressed at both mRNA and protein levels, except for …

Several mathematical models are developed to describe the effects of chemotherapy on both cancerous and normal tissue. Each model is defined by either a single homogeneous equation or a system of heterogeneous equations which describe the states of the normal and/or cancer cells. Periodic terms are added to model the effects of the chemotherapy. What we obtain are regions, in parameter space (dose and period), of acceptable drug regimens.
The models take into account various aspects of chemotherapy. These include, interactions between the cancer and normal tissue, cell specific chemotherapeutic drug, the use of non-constant parameters to aid in modeling …

This study assessed the relationship of cognitive and noncognitive variables in relationship to academic success of students from selected allied health majors: respiratory therapist (RTT), radiologic technology (RT), surgical technology (ST) and medical records technician (MRT) from hospital based and community college programs in Virginia. Student academic success was defined as academic (classroom) grade point average, clinical grade point average, cumulative grade point average and passing status. This study tested if the availability of third party payment for prescriptive drugs as part of employer sponsored health insurance was associated with a change in the utilization of four types of health services in an independent practice association (IPA). The study employed the individual determinants component of the Andersen-Newman theoretical framework for health services utilization. In the United States, heart disease is currently the number one cause of death in the adult population (U.S. Department of Health and Human Services, 1991). Unlike many of the major causes of death, heart disease is largely preventable through changes in health related behaviors. Although a portion of heart disease risk is attributed to genetics, the primary cause of heart disease development lies in the eating habits, exercise behaviors, and smoking habits of the individual.

Recent innovations in patient care delivery and nursing governance require an empowered workforce. However, little is known about what combination of organizational, leadership, and personal characteristics maximize the prediction of registered nurse empowerment. The purpose of this study was to develop and test an explanatory model of registered nurse empowerment. The predictor variables included personal characteristics of registered nurses (age, experience, organizational tenure, education, and position level of clinical and/or management responsibility), perceptions of organizational culture, and connective leadership. This is an external validation study of the Alcohol and Drug dependence scales of the Millon Clinical Multiaxial Inventory II ([MCMI-II], Millon, 1987). The Structured Clinical Interview for the DSM-III-R ([SCID], Spitzer, Williams, Gibbon, & First, 1990) was administered to a sample of 73 adults who presented for treatment at a substance abuse facility to establish all possible substance abuse diagnoses based on the nosology of the revised Diagnostic Statistical Manual for Mental Disorders (3rd ed. Muscle contraction is thought to be accomplished by sliding of myosin filaments along actin filaments. Although actomyosin naturally uses ATP as an energy source, it can also use other nucleoside triphosphates (NTP) as substrates. In this work, the dependence of rate and equilibrium constants of the various steps in the muscle contraction mechanism upon nucleotide structure was investigated to unravel the dependence of the mechanical properties of the muscle upon changes in different biochemical steps of the mechanism.
