The remodeling of connexin43-containing gap junctions (GJs), as defined by changes in size, distribution, or function, is a prominent feature of both the developing and diseased myocardium. During postnatal development of the mammalian ventricle, there is a progressive remodeling of GJs into intercalated disks. This change in GJ quantity and distribution is correlated with alterations in the velocity and anisotropy of impulse spread within differentiating myocardium. Orderly distribution of GJs is perturbed in many cardiac diseases, and reemergence of immature patterns of myocyte coupling may be a factor in the arrhythmogenic potential attending such pathologies. Seizures cause selective neuronal cell loss in vulnerable regions such as the hippocampus, cortex, thalamus or substantia nigra (SN) in human or experimental animals. While the HF can propagate seizures, the SN pars reticulata (SNR) regulates the spread ol seizures in an age-dependent fashion, and the pars compacta (SNC) affects extrapyramidal motor control. In mature animals, both structures are wlnerable to seizure-induced damage. In prepubescent rats, seizure susceptibility is reduced and CA 1 region rs primarily affected. The proto-oncogenes, Bcl-2 and Bax, encode specific proteins that inhibit and promote programmed cell death, respectively. In order to determine whether proto-oncogenes are …

HIPAA - the Health Insurance Portability and Accountability act of 1996 is the largest government action in healthcare since Medicare. Since the passage of the HIPAA legislation, estimated cost of implementation has ranged from $3.8 billion to $45 billion for the implementation of the privacy rules. This study examines the cost on implementing HIPAA in private physicians' offices in South Carolina. The study selected the medical specialties of family practice, general practice, obstetrics & gynecology, and pediatrics. The study compares and analyzes cost of rural vs. urban physician practices, by specialty, and by type of entity when implementing privacy rules.

In the 2000 case Pegram v. Herdrich, the U.S. Supreme Court decided that administrators of health benefits plans covered by the Employee Retirement Income Security Act (ERISA) of 1974 do not have a fiduciary duty towards beneficiaries when making "mixed decisions" involving treatment and eligibility components. This thesis, Rethinking Pegram: Why ERISA Health Benefits Administrators Should Have a Comprehensive Legal Requirement of Fiduciary Duty, challenges the Court's opinion on grounds of legal precedent and public health ethics. While the legal critique of Pegram is an elaboration of earlier scholarship, the public health ethics critique represents a unique contribution …

Introduction: Vision loss resulting from retinal ischemia is associated with several systemic and ocular diseases. In the retina, ischemia is thought to lead to neuronal cell death by initiating a series of events that include oxidative stress, glutamate excitotoxicity, and intracellular Ca2+ overload. Attempts to prevent ischemic retinal degeneration have included the use of NMDA receptor antagonists, Ca2+ channel blockers, and antioxidants. These treatments have had limited success due to incomplete retinal protection and development of adverse side-effects. Knee injuries are among the most common injuries seen in a physical therapy clinic. Nearly all rehabilitation programs following a knee injury involve quadriceps muscle strengthening. One popular and safe method of quadriceps strengthening often used is the wall slide. There is, however, debate and little research indicating whether or not changing the foot position used during wall slides alters the muscle activity during this exercise.

The purpose of the present study is to determine the effects of individual phonological awareness and phonics training emphasizing auditory, motoric, and alphabetic properties of phonemes with 3 children who did not make substantial gains following classroom phonological awareness intervention. Subjects were enrolled in the first grade and exhibited speech and/or language impairments. The individual phonological awareness program contained 3 parts: (1) phonological awareness, (2) phoneme-grapheme correspondence, and (3) decoding and spelling and employed a single subject multiple probe baseline across behaviors design. The purpose of this study was to determine if a significant difference exists in the relationship between measures of muscle size and strength among elite bodybuilders, powerlifters, and Olympic weightlifters. Fifteen male subjects between the ages of 16 and 48 years participated in the study. All three subject groups, Olympic weightlifters (OWL, n=5), powerlifters (PL, n=5), and bodybuilders (BB, n=5), were highly trained and currently involved in competition training. All test subjects were of similar body weight and weighed between 76-96 kilograms. Few studies have investigated the factors which influence adolescent substance abuse and the respective effects on treatment outcome. Objective: To determine how a clinician’s background knowledge, their tasks, and displays of information interact to affect the clinician’s mental model. Design: Repeated Measure Nested Experimental Design Population, Sample, Setting: Populations were gastrointestinal/internal medicine physicians and nurses within the greater Houston area. A purposeful sample of 24 physicians and 24 nurses were studied in 2003. Methods: Subjects were randomized to two different displays of two different mock medical records; one that contained highlighted patient information and one that contained non-highlighted patient information. The current state of health and biomedicine includes an enormity of heterogeneous data ‘silos’, collected for different purposes and represented differently, that are presently impossible to share or analyze in toto. The greatest challenge for large-scale and meaningful analyses of health-related data is to achieve a uniform data representation for data extracted from heterogeneous source representations. Body weight support (BWS) training provides a safe environment in which the individual can perform and practice mechanics of normal gait at a variety of speeds with or without body weight support, depending on the level of function. Backward walking is a common intervention in the rehabilitation of lower extremity injuries. Despite its popularity, there is limited research available on the EMG activity during backward walking at an incline when compared to forward walking. In this study, we recorded EMG activity in four muscles of the lower extremity and utilized motion analysis to evaluate the knee range of motion when walking forward and backward on a treadmill at 0 and 15 percent grade inclines.

Overall, our results indicated a greater increase in muscle activity during backward walking than forward walking. Walking backward at a 15 percent grade incline …

The purposes of this study are to assess whether University of North Dakota Physical Therapy (UND-PT) graduates desire a transitional Doctorate of Physical Therapy (t-DPT) degree and to determine their preferences regarding this proposed educational program.

The survey was sent to 1136 alumni of the UND-PT program spanning from its first graduating class in 1970 to the class of 2002. It consisted of 8 demographic questions and 12 questions regarding areas such as curriculum content, program implementation, and personal views about the Doctorate of Physical Therapy degree. Numerous studies have been done on the Plyo Press, with focus on the concentric phase of the exercise. However, for athletes eccentric training has always been a topic of interest. Most leg press machines have a concentric and an eccentric phase involved with the exercise. However, during the eccentric phase of the leg press the muscles are underloaded because of the force-velocity relationship of the human skeletal muscle and the fact that eccentric contractions require fewer motor units for the same force and workload. Aquaporin 0 (AQP0, MIP), a water channel protein found in the ocular lens fiber cells, has been proposed to maintain osmotic homeostasis by contributing to an internal circulatory system within the avascular lens. The formation of cataracts in patients with mutations in the AQP0 gene and in heterozygous AQP0 deficient mice suggests that AQP0 is necessary for establishing and preserving lens transparency. Previous structural characterization of AQP0 revealed that the C-terminus, a putative regulatory domain, is subject to many age-related posttranslational modifications. A descriptive correlational design was utilized to explore the relationship between preoperative self-efficacy and the postoperative outcomes of distance ambulated, brace application, discharge disposition and length of stay in surgical spinal fusion patients. Social cognitive theory and the concept of self-efficacy were used to provide the theoretical framework. A novel self-efficacy questionnaire was developed to measure the independent variable, it was tested for reliability and validity in a 16 patient pilot study. The pilot study was followed by a study of 52 post-operative patients. Preoperative self-efficacy significantly correlated to distance ambulated in the entire sample. Adolescents with chronic diseases must accomplish the management of their healthcare decisions and practices; those with epilepsy also deal with the uncertainty of seizure disorders and the stigma linked to this disease. This secondary analysis of data examined psychosocial variables for adolescents with epilepsy within the developmental-contextual framework proposed by Hohnbeck and Shapera (1999). No statistically significant differences were found between the mean Piers-Harris scores, self-assessment scores on the Children's Depression Inventory (CDI) or the Revised Family APGAR for the subgroups of males, females, younger (9 to 11.44 years) or older (11.45 to 14 years) adolescents. This qualitative research study examines some of the ways in which the healing process takes place in adults who have experienced dissociation, and who have most likely been psychologically traumatized before full language development. The impact of early trauma on human cognitive, socio-emotional, and spiritual development are explored within the context of participant narratives in order to develop an understanding of what may be helpful in developing effective teaching strategies for clinical and teacher education. One of the major challenges facing the pharmaceutical industry is the control of the amount of trace contaminants during the manufacture of organic substances. Lowering the level of contamination improves the quality of the product and reduces possible side effects.

One of the main techniques used to detect trace contaminants is UV-VIS spectroscopy, where the unique absorption fingerprint of a chemical reveals its presence.
