Inappropriate corticosteroid prescribing for acute upper respiratory tract infections (AURTIs) remains a prevalent issue in outpatient primary care despite limited evidence supporting their use in otherwise healthy adults. This Doctor of Nursing Practice (DNP) project aimed to reduce inappropriate corticosteroid prescribing for AURTIs by implementing an evidence-based clinical practice guideline over an eight-week period in a rural primary care setting.
The doctoral project employed pre- and post-intervention design. Providers were introduced to the corticosteroid guideline derived from the 5-Minute Clinical Consult, emphasizing appropriate use criteria for corticosteroids in AURTI cases (Unbound Medicine Inc., 2025). The integration of oral health assessments into primary care settings has been strongly advocated to enhance access to care for elderly individuals. Medical and dental professionals recognize the critical connection between oral health and systemic health. Physicians can play a key role in enhancing oral health care by incorporating an oral health assessment into routine general health appointments, such as the Medicare Annual Wellness Visit (AWV). This dissertation investigated the mechanisms and applications for optimizing color vision performance across diverse contexts. In a world where color cues augment critical decisions to optimize performance and safety in transportation (e.g., aviation, railway), military, law enforcement, medical diagnostics, and myriad occupational tasks, deficiencies in color perception pose significant challenges. By uniting four interrelated studies, this work explored intrinsic retinal processes, assistive technologies for deficiencies, and novel pathways that challenge conventional understandings of vision. The research bridged fundamental vision science with applications in occupational settings, low vision rehabilitation, and clinical diagnostics, ultimately aiming to optimize color performance.

Noninvasive, wearable biosensors capable of detecting stress biomarkers in sweat require electrodes that are flexible, conductive, insoluble in water, and highly sensitive at low concentrations of analyte. In this work, bovine serum albumin blocked and anti-cmab immobilized nitrogen doped graphene quantum dots integrated polyaniline/polystyrene composite fibermat electrodes (BSA/Anti-Cmab/N-GQDs/PANI/PS electrodes) were synthesized and evaluated as a potential platform for an electrochemical cortisol biosensor in sweat based systems. Polyaniline was utilized in order to provide electrical conductivity, while polystyrene served as a carrying polymer for mechanical support and its hydrophobicity. This quality improvement (QI) project examined the impact of structured debriefing training on nursing faculty perceptions of confidence, competence, and skill in simulation-based education (SBE) at a northeast Texas community college. The problem addressed was the lack of a standardized debriefing process, leading to inconsistent faculty practices and variable student learning outcomes. Shared decision-making (SDM) is the best practice in contraceptive counseling, yet it remains underutilized in primary care, particularly within underserved communities. Limited SDM use reduces patient autonomy, weakens satisfaction, and contributes to inconsistent documentation. This quality improvement (QI) project aimed to evaluate whether an educational intervention on SDM for contraceptive counseling improved provider self-efficacy and the electronic medical record (EMR) documentation of SDM practices. Background: Substance Use Disorder (SUD) and Alcohol Use Disorder (AUD) are major public health concerns, particularly in underserved populations. Screening, Brief Intervention, and Referral to Treatment (SBIRT) is evidence-based models that promote early detection and care. Integrating SBIRT into Family Nurse Practitioner (FNP) curricula prepares future providers to address substance use in primary care. Local Problem: Despite the effectiveness of SBIRT, its use in primary care remains inconsistent. Barriers, including limited training, low confidence, and unfamiliarity with referral resources, hinder implementation. Structured education and protocols are needed to support provider readiness. Atherosclerotic cardiovascular disease is the number one cause of death in the United States and the world. Every year, billions of dollars are spent in the US alone for direct treatments of this disease, especially on prescription medicines like statins. However, these medications have been unable to curtail global mortality rates, partially because they do not target the entirety of atherosclerotic pathophysiology. A key area of this pathophysiology is that atheroprone arteries typically include pro-inflammatory endothelial cells that exhibit decreased levels of apoAI-mediated cholesterol efflux and an increased release of adhesion molecules, such as VCAM-1. The fascia iliaca block (FIB) is an effective regional anesthesia technique used in various clinical settings, including operating rooms, emergency departments, and pain management clinics. Despite its advantages in reducing postoperative complications and minimizing opioid use, FIB remains underutilized among Certified Registered Nurse Anesthetists (CRNAs), primarily due to limited exposure and training. This doctoral project examines the implementation of an Objective Structured Clinical Examination (OSCE) as an educational intervention to enhance CRNA proficiency in performing FIB using both ultrasound-guided and landmark techniques.

Among the many aspects of preparation required for patients undergoing general anesthesia, preoperative optimization is key. One goal of preoperative optimization is the establishment of adequate preoperative intravascular fluid volume. Inadequate fluid volume can increase surgical risks and lead to adverse patient outcomes. Evidence has demonstrated that using point-of-care ultrasound (POCUS) to assess preoperative surgical patients can diminish the consequences associated with altered fluid status and increase the likelihood of hemodynamic stability. Recommendations regarding breastfeeding following maternal anesthesia are inconsistent and often not up to date with current best practice guidelines. Previous recommendations have led to mothers unnecessarily “pumping and dumping” breast milk has benefits for both mother and baby. These benefits include (a) increasing the infant’s access to colostrum, (b) encouraging mother-baby bonding, (c) improving the mother’s breastfeeding experience, (d) and providing significant health benefits to the infant. The purpose of this systematic review is to synthesize literature examining the relationship between using exercise as a preventative method for gestational diabetes development during pregnancy, and the effect it may have on lowering glycemic levels. This study explored the relationship between exercise, social support, and mental health in college students. Mental health issues such as depression, anxiety, and stress are prevalent among college students. The two components that are experimented with for their potential in decreasing the effects of negative mental health are exercise and social support. This project executed a cross-sectional design by surveying three groups of college students: cycle class participants, independent exercisers, and non-active students. Survey-based research was performed to identify risk factors and prevention tactics for finger injuries in boulderers. An IRB-approved survey was distributed and taken by 228 respondents, whose data was analyzed. Risk factors for finger injury were found to include years of experience and skill level. Results indicated stronger, more experienced boulderers were more likely to report history of finger injury than weaker, less experienced boulderers. Findings suggest boulderers may avoid injury by manipulating their grip type in specific ways.

Among college populations, students who are members of social Greek life organizations (i.e., social fraternities and sororities) drink more and experience higher rates of negative alcohol-related consequences than their non-affiliated peers. Although there has been success of alcohol interventions in general college populations, research has shown that college drinking interventions that are delivered to students in Greek life are not efficacious in reducing alcohol consumption or related consequences. Suicidal thoughts and behaviors (STBs) are a growing public health concern among youth. Bullying victimization is a robust risk factor for STBs and identity-based bullying (IBB) or bullying that occurs due to a person’s actual or perceived social-identity/identities that is ingrained in discrimination, prejudice, or bias, has been found to be more strongly associated with STBs compared to bullying that was unrelated to identity. Glioblastoma (GB), acute myeloid leukemia (AML), chronic lymphocytic leukemia (CLL), cholangiocarcinoma, and chondrosarcoma (CS) cancers all contain mutations in the gene isocitrate dehydrogenase 2 (IDH2). The mutant IDH2 enzyme exhibits transformation of alpha-ketoglutarate (αKG) into the oncometabolite D-2-hydroxyglutarate (D2HG) in the mitochondria of these cancers. Mitochondrial-mediated transfer between cancer cells and recipient cells is a significant event that impacts the physiology of the receiving cell, specifically the epigenetic landscape. Previous experiments indicating increased DNA methylation in mesenchymal stem cells exposed to IDH1 or IDH2 conditioned medium underscore the potential role of D2HG to alter methylation states. Ageism remains a pervasive and often underexamined form of discrimination, affecting older adults across both everyday and clinical contexts. This paper explores how cognitive schemas—mental frameworks that shape perception and experience—and stigma interact to reinforce a “we–them” divide between and older patients and healthcare providers, as well as between older and younger individuals in broader society. Through focus groups and listening to the voices of older patients, this study examines how assumptions of age-based decline are embedded in society and inform schemas of ageism. Self-Determination Theory (SDT) posits that the quality of students’ motivation regulation (i.e., autonomous versus controlled) shapes both learning and health behavior, yet few studies have examined the relations among motivation regulation, alcohol and cannabis use, and achievement. In this study, I surveyed 459 undergraduates at a mid-sized public university in the southeastern United States. Participants completed the Academic Motivation Scale-College version 28 to assess autonomous and controlled motivation regulation, a typical-week alcohol grid, a seven-day cannabis grid, and self-reported grade point average (GPA). Environmental sanitation is a fundamental component of infection prevention in long-term care facilities (LTCFs), where residents are at increased risk due to advanced age, underlying health conditions, and communal living arrangements. This project sheds light on mental health issues within the Black community and the need for faith communities to intentionally address mental health issues in communities of color. Black men are oppressed by an interplay of cultural, historical, societal, and economic factors that impact their well-being. Racial discrimination and oppression are non-medical drivers exacerbating mental health disorders among Black men. The influences of culture and faith fabricate barriers to help-seeking behavior, and mistaken perceptions of masculinity cause many men to suffer in silence. The increasing demand for hospice care in Monterey County is a clear indicator of the rapid growth in the need for end-of-life services driven by the aging population. The project was developed at the Department of Hospice of the Central Coast, which is under the Montage Health Foundation, a non-profit organization. The community served consists of elderly people receiving hospice and end-of-life care, as well as volunteers who provide administrative and indirect support. This capstone project addressed the problem of sexual health misinformation among youth in Santa Cruz County, which stems from cultural factors, limited parental communication, and inadequate comprehensive sexual health education. Implemented through the Santa Cruz County Public Health Department’s Teen Health Outreach (THO) program, the project evaluated the effectiveness of parent workshops designed to build the capacity of adults’ knowledge and confidence to discuss sexual and reproductive health topics with youth. Serving primarily Hispanic/Latino communities in Watsonville and Freedom, THO collaborates with local schools and organizations to deliver workshops. Gateway Center is a nonprofit organization that provides services for adults with developmental, cognitive, and physical disabilities. The organization identified a need to improve client engagement and staff support, as existing activities had become outdated and inconsistent. Cytauxzoon felis is a protozoan that causes an often-fatal illness in felines known as cytauxzoonosis, or bobcat fever. Ticks are vectors of this pathogen, with Amblyomma americanum being the most common vector. Previous studies have noted a correlation between peak tick season and peak cytauxzoonosis diagnosis. For cytauxzoonosis, minimal treatments exist, and scientists have not found a cure, which is why focus needs to be on preventing disease. The goal of this Doctor of Nursing Practice (DNP) quality improvement project was to enhance pharmacovigilance practices among non-medical mental health providers (NMMHPs) in an outpatient psychiatric setting. The project addressed the issue of limited recognition and reporting of adverse drug reactions (ADRs) by NMMHPs, which can lead to delays in clinical intervention and compromise patient safety. To tackle this problem, a quasi-experimental, pre–post intervention design was employed. Diabetic retinopathy (DR) is a leading cause of vision impairment that can be prevented if found and treated early. Inadequate provider knowledge and adherence to guidelines, and low referral rates for screening at the project site, highlighted the need for targeted training. Studies show that evidence-based education plans improve engagement in DR screening. This quality improvement (QI) project aimed to impact knowledge, self-efficacy, and compliance among healthcare providers (HCPs—nurse practitioners and physicians) and increase screening rates by implementing an American Diabetes Association (ADA) guideline-based education plan over 4 weeks.
