The purpose of this study was to determine hospital executive management’s perceptions of how turbulence in the politico-legal sector of the macroenvironment impacted the strategic management systems of Tennessee hospitals. In particular, how did Federal and State funding restrictions (Medicare and TennCare) impact the strategic planning and implementation process of their hospitals? The study was also designed to gain insight regarding specific changes to strategic management systems that may have resulted from these funding restrictions.

Noise in the hospital environment can have negative effects on both patients and hospital staff. Unwanted noise can be disturbing and often annoying thus, interfering with patients' sleep and obstructing work performance of the hospital staff. A research study was conducted in the hospital unit to identify and develop methods of intervention for intermittent noise and their source. Routine staff work activity generated noise levels above EPA hospital noise recommendations. A sound level meter and video camera was used to capture noise between the work-shifts. The purpose of this exploratory study was to describe women's perceptions of the care they received in a birth centre, compared to their previous experiences in a hospital. Australian statistics indicate that five percent of childbearing women now choose to receive care in a birth centre setting. Clinical outcomes of birth centre care are now well documented, but there is limited empirical data about women's experiences of this model of care. Seventeen women, who had recently given birth in a birth centre, and had previously experienced care in a hospital setting, were interviewed about their care experiences. Using content analysis, …

The purpose of this descriptive study was to describe the evaluation of a 'Patient Management Program'. This managed care type of program combined a Pre-Admission Clinic and an Early Discharge Program. It was established as a twelve month pilot program at a major Western Australian public teaching hospital in January 1995. It aimed to provide a more efficient health service by replacing part of elective surgical patients' inpatient care with outpatient services. The first component of the Program was the Pre-Admission Clinic, which allowed patients to undergo pre-operative assessment and testing prior to admission to hospital, thus facilitating same day …

The problem of increasing health care costs and decreasing access to health care services is a major crisis in the U.S. today. One factor contributing to the rising cost of health care is the inappropriate use of emergency rooms, (ERs). In response to escalating costs a local health care corporation implemented a telephone triage program called the After Hours Program, (AHP). The goal of the program was to reduce inappropriate utilization of emergency rooms, and to educate patients to recognize and treat specific health problems independently, i.e., home treatment.

Major mental disorder, with prolonged periods of dysfunction that require long term care, is an issue of concern amongst mental health professionals. A descriptive quantitative study was conducted to determine what factors nurses considered when discharge planning, and how these factors compared with factors identified in the literature as being effective in planning for discharge. The study examined the relationship between selected demographic variables, occupational stress, and strategies. Although it is often assumed that a person will have increased knowledge after being given new information, it is important to determine how much information the person can recall and comprehend. The study sample was drawn from a paediatric hospital in Australia. The primary purpose of this study was to evaluate parents' understanding of discharge advice. The study is significant both to nursing practice and to hospital credibility. Medical records (1979-85) at the study hospital showed that approximately 100 children per year returned to hospital with secondary haemorrhage after discharge, post tonsillectomy/adenoidectomy. The purpose of this study was to test Rogers' principle of integrality by asking: Does a varied harmonic auditory environment increase the perceived restfulness of patients, who are hospitalised in order to heal following surgery? This study modified a previous study (Smith, 1986) which showed that varied harmonic sound had a more positive effect on the perceived restfulness of healthy subjects than did quiet ambience. This experimental study tested 22 post-operative inpatients, using a non-equivalent control group design with pretest and posttest. Varied harmonic sound was provided by audiotaped sequences of music and narrative; quiet ambience was achieved by maintaining …

The purpose of this research was to determine what factors, if any, led to the development of a preferred provider organization and if these factors were related to sponsorship. A survey instrument was developed and mailed to a random sampling of 90 existing preferred provider organizations' executive directors for their completion. A Two-Factor Contingency Table Analysis was compiled. Chi-square, and the Contingency factor, C, were computed and tested at the .05 level of significance.
