Background: The role of mineralocorticoid receptor antagonists (MRAs) in acute myocardial infarction (MI) remains controversial, with conflicting evidence from landmark trials. We aimed to assess the impact of MRAs on mortality and cardiovascular outcomes post-acute MI.

Methods: We systematically searched PubMed, Embase and Cochrane CENTRAL databases up to February 2025 for randomised controlled trials comparing MRAs with placebo or standard care in adults experiencing acute MI. Primary outcome was all-cause mortality; secondary outcomes included cardiovascular mortality, heart failure, recurrent MI and ventricular arrhythmia. Data were pooled using random-effects models, with heterogeneity explored via subgroup analyses and meta-regression.

BACKGROUND: Renewed interest in anterior cruciate ligament (ACL) preservation has led to increased focus on tear location within the knee joint in treatment decisions, with primary ACL repair reserved for proximal tears. Retrospective studies have reported varying tear locations in adults when assessed on magnetic resonance imaging (MRI), with few studies comparing tear location on MRI versus intraoperative findings.

PURPOSE: To prospectively determine the distribution of ACL tear location assessed on MRI versus intraoperative evaluation to test the hypothesis that < 15% of ACL tears are proximal type avulsions.

BACKGROUND: Tailored implementation addresses the inherent dynamic complexity and heterogeneous nature of implementation practice. In general, tailored implementation involves setting implementation objectives, identifying determinants, matching strategies to those determinants, and developing an evaluation plan. How matching a specific implementation strategy to a determinant is done remains largely unknown. This study aimed to provide an overview of methods for matching strategies that have been applied in research and practice.

Gambling advertising has become increasingly prevalent across various media platforms such as TV, social media, real-life advertising, etc. With advertisements designed to influence attitudes, intentions, and behaviors of potential and existing consumers, this review aims to compile existing research to examine the measurable effects of gambling advertising. Studies indicate that young people and problem gamblers are especially vulnerable to these advertisements, which normalize gambling and contribute to increases in gambling behavior. This effect on behavior is dose-dependent; increased exposure to advertising is associated with greater gambling engagement. Intimate partner violence (IPV) refers to physical, sexual, or psychological harm caused by a current or former partner. While IPV can affect anyone, women are particularly vulnerable and disproportionately affected. Physical IPV often includes experiences of repeated trauma to the head, face, and neck, including strangulation. These injuries can result in a range of consequences, including comorbid conditions such as depression, PTSD, anxiety, and cognitive impairments, along with neurological effects like reduced brain connectivity and white matter integrity. People in love often crave their beloved, and some people who enjoy chocolate crave chocolate. This study explored the differences in craving, pleasantness, and arousal for the beloved and for chocolate. It also measured whether people exhibited a greater attentional bias for their beloved or chocolate. It is hypothesized that participants would experience greater craving, pleasantness, and arousal for their beloved than for chocolate. Additionally, some difference was expected for attentional bias toward the beloved and chocolate, but the directionality was not predicted. Adolescent psychiatric hospitalizations continue to rise, yet there is limited research on which daily practices inside inpatient units genuinely support engagement and recovery. This project examined how structure, staff tone, and therapeutic programming influence adolescents’ participation and emotional stability within a real inpatient setting. The study focused on three questions: how structured routines affect recovery, what challenges staff face when supporting adolescents with complex needs, and how adolescents’ engagement patterns reflect their perception of care. Postoperative anaemia is an overlooked complication of cardiac surgery that is associated with adverse clinical outcomes. Although small clinical trials suggest that postoperative treatment with i.v. iron improves haemoglobin recovery and reduces transfusion utilisation, appropriately powered randomised controlled trials are necessary to definitively evaluate the efficacy of treatment on clinical outcomes of importance to patients, clinicians, and healthcare systems. A comprehensive approach to perioperative anaemia management demands a renewed focus on both prevention and treatment to improve patient outcomes.

Replacing implanted medical hardware due to infection often requires one or more revision surgeries. Each surgery triggers a tissue injury response and disrupts the established bacterial biofilm. However, the complex tissue response to reinjury and biofilm disturbance is not well understood. Our results show that with an existing infection, immunological niches such as the bone marrow, lymph nodes, and circulating blood further upregulate pro-inflammatory programs in response to revision. Rather than reducing bacterial burden, this heightened inflammation provokes virulence factor expression and tissue damage, including bone osteolysis and muscle fibrosis. Gastrointestinal stromal tumors (GISTs) are rare mesenchymal neoplasms typically involving the upper gastrointestinal tract. Most GISTs are driven by activating mutations in the KIT or PDGFRA genes, which are primary molecular markers for diagnosis and treatment. In addition, CD117 and DOG1 (discovered on GIST 1) are important immunohistochemical diagnostic markers of GISTs. Esophageal GISTs are exceptionally rare, and those driven by BRAF gene fusions are even more uncommon. This report describes a 69-year-old woman who underwent surgical resection of an esophageal GIST harboring an MKRN1-BRAF gene fusion, offering insight into future treatment and surveillance strategies.

INTRODUCTION: Bone regeneration following injury depends on osteoprogenitor cells derived predominantly from the periosteum. Incomplete regeneration has been attributed to both cell-extrinsic factors (e.g., environment, inflammation, mechanical instability) and cell-intrinsic factors (e.g., impaired proliferation or differentiation of stem cells). In the digit amputation mouse model, amputation through the third phalanx (P3) supports complete regeneration, while amputation through the second phalanx (P2) results in callus formation and scarring. A 68-year-old male with a history of metastatic bladder cancer and urinary obstruction presented with intractable hiccups and chest pain is presented in this case. He was found to have a unilateral pleural effusion in addition to ipsilateral renal metastasis abutting the diaphragm. Imaging revealed empyema. Thoracentesis revealed positive cultures and a lack of malignant cells on cytology. Comorbid urinary obstruction and urinary tract infection suggested the rare diagnosis of an infected urinothorax. Thoracentesis also revealed pleural fluid creatinine greater than serum creatinine, meeting the most specific diagnostic criteria for urinothorax and establishing the diagnosis. Background: Current randomized controlled trials are investigating the efficacy and safety of mechanical thrombectomy (MT) in patients with medium vessel occlusion (MeVO) stroke. Whether best medical management (MM) is more efficient than unsuccessful vessel recanalization during MT remains unknown.

Methods: This was a retrospective cohort study using data from 37 academic centers across North America, Asia, and Europe between September 2017 and July 2021. Only patients with occlusion of the distal branches (M2 and M3) of the middle cerebral artery territory were included. BACKGROUND: Percutaneous coronary intervention (PCI) for heavily calcified chronic total occlusion (CTO) remains among the most challenging procedures in interventional cardiology. Hydrodynamic contrast recanalization (HDR) is a novel crossing technique for CTO that uses intraplaque contrast microinjections in the proximal CTO to facilitate the passage of polymer jacketed wires. The use of HDR combined with rotational atherectomy (RA) during CTO PCI in heavily calcified arteries has not been described.

Approximately 80% of invasive breast cancers are classified as human epidermal growth factor receptor 2 (HER2)-negative; however, many of these tumors have detectable levels of HER2 surface expression. Trastuzumab deruxtecan (T-DXd) is a HER2-directed antibody-drug conjugate with a membrane-permeable payload that is cytotoxic to both HER2-expressing tumor cells and neighboring cells via the bystander antitumor effect. T-DXd has shown significant antitumor activity in clinical trials for patients with HER2-positive (immunohistochemistry [IHC] 3+ or IHC 2+/in situ hybridization [ISH]+) breast cancer. BACKGROUND: Decades of research have identified a strong association between heavy cannabis use and schizophrenia (SCZ), with evidence of correlated genetic factors. However, many studies on the genetic relationship between cannabis use and psychosis have lacked data on both phenotypes within the same individuals, creating challenges due to unmeasured confounding. We aimed to address this by using multimodal data from the All of Us Research Program, which contains genetic data as well as information on SCZ diagnosis and cannabis use.

Due to their high economic value, essential oils (EOs) are increasingly subject to adulteration, posing significant challenges for quality assessment and control. Owing to its excellent molecular specificity, Raman spectroscopy has been widely employed for the analysis and evaluation of EO products. In this study, we collected a total of 2700 Raman spectra comprising rose essential oil (REO, n = 900), geranium essential oil (GEO, n = 900), and their mixtures (n = 900). We first constructed a conventional convolutional neural network to distinguish spectra corresponding to varying EO mixing ratios. BACKGROUND: Patients with COVID-19 are at risk of thrombotic and cardiovascular complications. Existing risk estimates derive from early COVID-19 experiences. More recent event rates, temporal trends, and the association between vaccination status and cardiovascular outcomes remain unclear.

METHODS: The CORONA-VTE Network (COVID-19 Registry to Assess Frequency, Risk Factors, Management, and Outcomes of Arterial and Venous Thromboembolic Complications) multicenter registry included patients with COVID-19 from March 2020 to February 2024. A composite of venous and arterial thrombotic events and a composite of adjudicated cardiovascular events were assessed at 90 days after COVID-19 diagnosis. Introduction: Minimally invasive robotic technique is gaining popularity due to its technical advantages when compared with conventional laparoscopy. Major hepatic lobectomy, however, remains a technically demanding procedure and requires extensive training to ensure safety and efficacy, especially when undertaken using minimally invasive approach.1–4 With focus on robotic surgery techniques, in this video we describe our standardized technical approach of robotic anatomical left hepatectomy with enbloc caudate lobe resection. Premature ovarian insufficiency (POI) is a common late effect of chemotherapy among survivors of adolescent and young-adult cancer (AYAC). Given on the widespread use of Chinese herbal medicine (CHM) in restoring individual health, we performed a cohort-based nested patient-control study with the aim of exploring the effect of blending CHM into routine care on the reduction of chemotherapy-induced POIrisk among females with AYAC.

An increasing number of people are turning to generative artificial intelligence (AI) tools and AI-assisted chatbots to manage mental health concerns. This data slice presents findings from a national survey of U.S. adults aged 18-49 (N = 1,805) conducted in October 2025. Among respondents, 35.2% reported using AI tools more than once a week for mental health support. Among those who had ever seen a human mental health professional, 28.4% reported visiting human providers less often since beginning to use AI for the same purpose. Autism and motor abilities have been found to be closely related. Culture affects motor development as well as various diagnostic criteria for an autism diagnosis, yet cultural factors are rarely considered in research and in clinical diagnostics. Objectives: To assess the prevalence and identify sociodemographic predictors of regular fast-food consumption (R-FFC) among United Arab Emirates (UAE) adults to inform public health nutrition responses to growing diet-related ill health in the region. Design: This is a descriptive cross-sectional study using purposive, convenience sampling. INTRODUCTION: Use of digital health technology can improve patient health outcomes; however, not all patients have the knowledge and skills to download a health app and access a patient portal. Providing digital onboarding support to hospitalized patients has potential to overcome some barriers to accessing needed education in the community, including both having the time and a location to receive education. To address this, our team developed the Jefferson Digital Onboarding Taskforce (JeffDOT), a group of staff and students who approach hospitalized patients and provide one-on-one teaching on how to sign up for and use a patient portal.
