Murder, She Wrote Or Was It Merely Selective Nontreatment?, George P. Smith Ii

Scholarly Articles

This article will both explore and thereby establish the medical, ethical, and legal validity of selective nontreatment of severely handicapped newborns. A construct for principled decision-making, tied to a basic recognition of the right of self-determination, as shaped by compassion and validated principles of triage and cost-benefit analysis, will be seen as the most effective means for the states-and not the federal government-to evaluate the intensely complex issues associated with allocating scarce medical resources to defective infants. Governmental intrusions into the familial decision- making forum in these circumstances must be kept to a minimum and allowed only in grave cases.

Intrauterine devices are the most widely used of all reversible contraceptive methods worldwide. Modern IUDs are among the most effective and long-acting methods of family planning and are acceptable options for many women. This monograph presents highlights of the clinical performance of the Copper T 380 over eight years, including the latest data on effectiveness, expulsions, and continuation rates. It incorporates data from Population Council and World Health Organization studies, and work by scientists at Family Health International and in several countries. Rex protein, the posttranscriptional regulator of human T-cell leukemia virus type I (HTLV-I), is required for the control of viral structural protein expression and virus replication. Rex is a phosphoprotein found predominantly in the cell nucleolus, whose function is thought to be regulated by its nucleolar localization and phosphorylation. Therefore, we investigated the in vivo phosphorylation of Rex protein in more detail. Phosphorylation of Rex occurred in all HTLV-I-infected cell lines examined in vivo, primarily at serine residues and to a very small extent at threonine residues. Regional Research Institute for Human Services

Tribal colleges are providing interdisciplinary education and professional training for human services occupations at the undergraduate level. These programs also promote cultural competence as a result of required courses in tribal languages, history, culture and law. The Sinte Gelska University program is described as an example of the role of tribal college education in emphasizing cultural identity using a culture-specific instructional style and teaching method. These programs in tribal colleges will augment the availability of competent service to native Americans.

This commentary on assessment training in scientist-practitioner and practitioner-scholar programs examined potential differences between these training models on assessment instruments included in the curriculum, assessment instructors, modes of interpretation, anticipated clients, and service delivery styles. Over time it appears that the potential for difference in assessment training inherent in Boulder and Vail model values has been minimized. If these models for training professional psychologists are to be effective in preparing students for assessment tasks with additional populations of consumers in new service delivery systems, attention to these potential differences in assessment training offers promise for more responsible and competent practice …

Over the past years, the use of epidural anesthesia as an intervention for pain relief has increased in the labor and delivery setting. Exploration of the literature has not identified what effects the addition of this technology has had on the normal childbirth process and those participating in childbirth, specifically the new father. The studies in this issue of Quality/Calidad/Qualité describe three efforts to involve women in the development of reproductive health care materials in different settings in the developing world. In Cairo, a women's health collective produced a comprehensive reference book for women; in Peru, a woman's group, with the extensive involvement of their nonliterate audience, developed a series of illustrated teaching materials; and in the South Pacific, an all-woman production crew produced three motivation and teaching videos developed in response to the expressed needs of Pacific Island women. Parasitological studies of archaeological sites can be used to interpret past behavior and living conditions. During the 1980s problem-oriented research into prehistoric- and historical-period parasitism developed and resulted in thefield of archaeoparasitology. Archaeoparasitology attempts to integrate parasite data into archaeological theory and interpretation. Within the last decade, four major archaeoparasitological laboratories emerged. They developed interpretive frameworks that apply parasitological data to a remarkable variety of prehistoric behaviors. Parasite remains can be used to reconstruct aspects of diet, health, and other behaviors such as transhumance and trade. A naturally occurring outbreak of Mycobacterium bovid infection in captive wild elk (wapiti) in Montana was confirmed by mycobacteriologic examination. Twenty-eight of 143 elk responded to M. bovis purified protein derivative (PPD) tuberculin injected intradermally in the cervical region (SCT). The results of comparative cervical tuberculin skin tests conducted within 9 days of SCT revealed greater responses to M. bovis PPD tuberculin than to M. avium PPD tuberculin in 23 of 28 elk responding. At necropsy, several grossly visible tuberculous lesions were observed in the parenchyma of the lung, thoracic lymph nodes, and submandibular lymph nodes. Microscopic examination of appropriately …

The magainins, peptide antibiotics secreted by the frog Xenopus laevis, have previously been shown to permeabilize phospholipid vesicles. To elucidate the mechanism of permeabilization, we have conducted detailed kinetic studies of magainin 2 amide (mgn2a)hduced release of 6-carboxyfluorescein from vesicles of phosphatidylserine. The results show that dye release occurs in (at least) two stages-an initial rapid phase, with t1/2 ≈ 3 s, followed by a much slower phase that approaches zero leakage rate before all the dye is released. Light-scattering studies showed that mgn2a does not cause gross changes in vesicle structure. The peptide was found to rapidly …

Management and organizational theories are commonly used to design nursing administration studies. This article proposes a nursing theory as a viable alternative and demonstrates its utility in the conduct of nursing administration research. The purpose of the research was to determine the explanatory power of the environmental stimuli of medical condition, nursing condition, nursing intensity, and medical severity on length-of-stay (LOS). Twenty-three percent of the variance in LOS was accounted for by the environmental stimuli with nursing acuity contributing 14%. The focal stimulus, medical condition (diagnosis-related groups), uniquely explained only one percent of the variance. The theory of social organizations …

We present evidence that insulin-like growth factor II (IGF-II) mediates growth in early mouse embryos and forms a pathway in which imprinted genes influence development during preimplantation stages, mRNA and protein for IGF-II were expressed in preimplantation mouse embryos, but the related factors IGF-I and insulin were not. IGF-I and insulin receptors and the IGF-II/mannose-6-phosphate receptor were expressed. Exogenous IGF-II or IGF-I increased the cell number in cultured blastocysts, but a mutant form of IGF-II that strongly binds only the IGF-II receptor did not. Reduction of IGF-II expression by antisense IGF-II oligonucleotides decreased the rate of progression to the blastocyst …
