Diagnostic Evaluation Of Fine Needle Aspiration Cytology In The Management Of Palpable Breast Lesions, S. H. Shah, Naila Kayani, S. H. Hasan, I. N. Soomro, Shahid Pervez, A. S. Hussainy

A total of 113 fine needle aspirates of the breast masses were evaluated in which the subsequent biopsy or mastectomy specimen were also available for histological examination. The age ranged from 16 to 80 years with a mean of 42 years. In benign conditions the mean age was 34.7 years while in malignant cases it was 48 years. The cytological diagnoses were compared with the histological results which revealed that the specificity and sensitivity of fine needle aspiration cytology in the palpable breast lesions was 86.1% and 89.2% respectively with a positive predictive value of 93% and efficiency of 88.2%. …

Studies uniformly conclude that union wage effects are largest for workers with low measured skills. Longitudinal analysis using 1989190- 1994195 Current Population Survey matched panels produces union premium estimates equivalent across skill groups, following appropriate sample restrictions and control for worker-specific skills. Evidence from the National Longitudinal Survey of Youth on aptitude scores confirms that union workers with high measured skills have relatively low unmeasured skills. Differential selection by skill class and skill homogeneity in union workplaces results from employer and employee sorting in response to wage standardization, union organizing where skills are homogeneous, and unionized employers' reluctance to hire …

One of the most pressing challenges for health programs in most sub-Saharan African countries is effectively addressing the increasing prevalence of HIV/AIDS. Recent evidence suggests that controlling sexually transmitted diseases (STDs) through undertaking preventive measures, early diagnosis, and treatment significantly slows the spread of HIV/AIDS. In regard to STI/HIV, Maternal and Child Health (MCH) and Family Planning (FP) clients are described as “low risk” groups. However, in a number of sub-Saharan African countries, the reported levels of STDs are significantly high enough to justify use of limited resources to target this group for STD services. Hyperlipidaemia is a powerful predictor of coronary artery disease, with a strong, independent, continuous and grades positive associationbetweencholesterol levels and risk of coronary events. Several large studies have shown the benefit of cholesterol reduction and there is clear evidence of the efficacy of statins in the reduction of events inprimazy and secondary prevention1. A 1% reduction in the total serum cholesterol reduces the frequency of fatal coronary heart disease by atleast 2%2. Recent work of cognitive and social psychologists has questioned the ability of subjects to accurately remember and report their experiences. This has particular significance for recreation research as the survey questions we ask of visitors change from more stable visit and group characteristics to reports of conditions, experiences and feelings. Strategies exist to encourage accurate recall, particlarly by minimizing the delay between event and report such as is provided by the Experience Sampling Method.

Attention Deficit Hyperactivity Disorder (ADHD) continues to be widely studied within the literature and researchers continue to investigate interventions and environments which allow for success. Whalen and Renker (1991) argue that multiple intervention methods are needed for success of children and adolescents in social situations. The purpose of this paper is to review the literature, and review interventions used to facilitate appropriate behaviors in children, aged six to eleven, with a diagnosis of attention deficit hyperactivity disorder.

It has been reported that among the 15 to 24- year-old population, the incidence of suicide has increased 200 percent since 1960. Background and Objectives: Using activities of daily living (ADLs) and instrumental activities of daily living (IADLs) as a focus, faculty at Eastern Virginia Medical School provide an aging simulation exercise for a mandatory fouth-year clerkship in geriatrics. The specific aims of the simulation are to 1) experience the physical frailties of aging, 2) develop creative problem-solving techniques, 3) identify feelings regarding the experience of functional loss, and 4) develop proactive clinical approaches to the care of the elderly.

In the newborn period, infants prenatally exposed to cocaine and other drugs show low scores on the Neonatal Behavioral Assessment Scale. Beyond that period, research is limited on the effects of prenatal drug exposure on neurobehavioral functioning. In this study we compared infants exposed to cocaine and other drugs and control infants from low socioeconomic backgrounds on measures of neurobehavioral functioning during neuromotor assessment at 1, 4 and 7 months of life. None of the measures of neurobehavioral functioning showed any significant group differences. OBJECTIVE: To study maternal and neonatal effects of combination nucleoside analog therapy administered to human immunodeficiency virus (HIV)-infected pregnant women for maternal indications.

METHODS: A multicenter, prospective observational study was undertaken at six perinatal centers in the United States and Canada that supported regional referral programs for the treatment of HIV-infected pregnant women. Demographic, laboratory, and pregnancy outcome data were collected for 39 women whose antiretroviral treatment regimens were expanded to include more than one nucleoside analog for maternal indications. OBJECTIVE: To determine voluntary human immunodeficiency virus (HIV) testing rates and factors influencing testing in a private obstetric practice.

METHODS: Antepartum patients were offered HIV testing after completing a self-assessment questionnaire. Perceived risks and demographics were correlated with testing rates.

RESULTS: Overall, 348/600 (58%) women consented to HIV testing. In a univariate analysis, patients with "any" perceived risk(s) were more likely to be tested. Single women and those with an at-risk partner(s) or a history of sexually transmitted disease (STD) were more likely to desire testing. These factors remained independently associated with voluntary testing in a multivariate regression model. No …

The data for this cross-sectional retrospective study are from surveys of 65 dairy-cattle herds in central New York, USA sampled between February, 1993 and March, 1995. The objective was to identify probability distributions of logarithmically transformed somatic-cell counts (linear score) for use in a simulation model of mastitis and milk quality. Probability density functions were estimated using maximum-likelihood estimators for the linear score of individual-cow composite milk samples culture negative and culture positive for the pathogens Streptococcus agalactiae, Streptococcus non-agalactiae, Staphylococcus aureus, and coagulase-negative staphylococci for the complete dataset and by bulk-tank somatic-cell count group (< 500 000, ≥ 500 000 SCC/ml). Potential problems with the use of viral vectors for gene therapy necessitate the development of efficient nonviral vectors. The association of transferrin, or the pH-sensitive peptide GALA, with cationic liposomes composed of 1,2-dioleoyl-3-(trimethylammonium) propane and its equimolar mixture with dioleoylphosphatidylethanolamine, under conditions where the liposome/DNA complex is negatively charged, drastically increased luciferase expression from pCMVluc. The percentage of cells transfected, measured by β-galactosidase expression, was also increased by about 10-fold. The zeta potential of the ternary complexes was lower than that of the liposome/DNA complexes. The intercistronic gene junctions of vesicular stomatitis virus (VSV) contain conserved sequence elements that are important for polyadenylation and transcription termination of upstream transcript as well as reinitiation of transcription of downstream transcript. To examine the role of the putative polyadenylation signal 3' AUACU75' at the gene junctions in polyadenylation and transcription termination, we constructed plasmids encoding antigenomic minireplicons containing one or two transcription units. Mummification was not intentional for most North American prehistoric cultures. Natural mummification occurred in the dry areas of North America, where mummies have been recovered from rock shelters, caves, and overhangs. In these places, corpses desiccated and spontaneously mummified. In North America, mummies are recovered from four main regions: the southern and southwestern United States, the Aleutian Islands, and the Ozark Mountains of Arkansas. This chapter is limited to a discussion of burial practices and a tabulation of the location of mummies in the southwestern United States with some comments on mummies of the southern United States, including the Ozarks.

Recent research demonstrates that silica phytoliths of dietary origin are associated with microwear of human teeth. Previous research has shown that severe enamel microwear and dental wear characterizes Archaic hunter-gatherers in the lower Pecos region of west Texas. Calcium oxalate crystals are especially common in Archaic coprolites. The vast majority are derived from prickly pear and agave, which were the dietary staples in west Texas for 6,000 years. The calcium oxalate phytoliths are harder than enamel. Therefore, calcium oxalate crystals are the most likely source of previously documented dental microwear and wear in the lower Pecos region.Am J Phys Anthropol …

The purpose of this study was to explore the relationships among alcohol consumption, sexual assault and rape, and nonsexual violence in a college population at a large Northeastern university. A 49-item questionnaire was designed to elicit responses from both victims and perpetrators. Of the 1,084 respondents: 518 were male, 566 were female, the majority were White (91.8%), and in the 18-21 age range. Unwanted sexual activity, rape, and violence were significantly related to alcohol consumption on the college campus studied. The incremental generation and disposal of hazardous waste, causing severe environmental pollution, has become one the world’s most important ecological problems starting with the Stockholm Conference in 1972. This paper examines two legislative reactions to pollution caused by hazardous wastes in the United States, under the Resource Conservation and Recovery Act 1976 (RCRA), and Argentina, in Law No. 24051. A recent report describes three types of prevention programs: universal, selected, and indicated (Institute of Medicine 1994). Universal prevention approaches are those that serve the entire population who share a general risk to the disorder without regard to specific risk status. Selected prevention approaches serve those whose precursors of problem behaviors are elevated but who have not yet manifested the problem behavior to be prevented. This article presents a demographic profile of grandparents, using the National Survey of Families and Households. Specific dimensions of grandparenthood addressed include grandparents' survival, the timing of grandparenthood, grandparents' involvement in other roles, surrogate parenting, and stepgrandparents. The data indicate considerable heterogeneity among grandparents of different genders and races or ethnicities. They also suggest modifications in previous descriptions of modern grandparenthood.

BACKGROUND: Skin-sparing mastectomy with immediate transverse rectus abdominis musculocutaneous (TRAM) flap reconstruction is being used more often for the treatment of breast cancer. Mammography is not used routinely to evaluate TRAM flaps in women who have undergone mastectomy. We have identified the potential value of its use in selected patients.

METHODS AND RESULTS: We report on four women who manifested local recurrences in TRAM flaps after initial treatment for ductal carcinoma in situ (DCIS) or DCIS with microinvasion undergoing skin-sparing mastectomy and immediate reconstruction. All four patients presented with extensive, high-grade, multifocal DCIS that precluded breast conservation. Three of four …

As with many other aspects of health status, it is most probable that prior to the European settlement of Australia in 1788 the eye health of Australian Aborigines and Torres Strait Islanders was excellent. In fact. their eye health was almost certainly better than that of Europeans of the time. Indeed, even those who do subscribe to this interpretation recognize the responsibility of each individual to show not only sensitivity and compassion but render assistance to those in distress. Pharmacologic hypnosis, morphine intoxication, and terminal sedation provide their own type of medical “salvation” to the terminally ill patient suffering unremitting pain. More and more states are enacting legislation that recognizes this need of the dying to receive relief through regulated administration of controlled substances.
