Introduction: Nurses are often on the frontline of disaster management, providing care to patients with emerging physical, mental, and emotional turbulence, and acting as educators for health promotion and disaster prevention in both rural and urban contexts. However, the literature suggests that nurses are inadequately prepared for disaster response. This study examined preparedness for disaster response among nurses in rural and urban primary healthcare settings in Tanzania.

Aim

To explore the International Network for Child and Family Centred Care (INCFCC) members' experiences and views on the long-term impact of COVID-19 on the nursing workforce.

Background

On the 11 March 2020, the World Health Organization declared COVID-19 a global pandemic. While some countries adopted a herd immunity approach, others imposed stricter measures to reduce the transmission of the virus. Hospitals in some countries faced an avalanche of extremely sick admissions, whereas others experienced an early surge in cases or were able to control the spread.

Extreme weather events including wildfires and hurricanes are becoming increasingly hazardous due to climate change, and often result in transient or permanent population displacements. Disaster-related disruptions in infrastructure, workforce, wages, and social networks can combine with population displacements to result in interruptions in health care access and prolonged impacts on morbidity and mortality. The data needed to make health systems and emergency management approaches more resilient to these hazards, and more responsive to the needs of affected populations, are sequestered in silos across private corporations and public agencies. With the recent COVID-19 pandemic, the importance of vaccine development, distribution, and uptake has come to the forefront of the public eye. Effectively fielding vaccines during an emergency-whether that emergency is a result of an infectious disease or not-requires an understanding of usual vaccine-related processes; the impact of outbreak, complex emergencies, mass gatherings, and other events on patients, communities, and health systems; and ways in which diverse resources can be applied to successfully achieve needed vaccine uptake. When communities are impacted by disasters, state officials must implement efficient and competent recovery strategies with the health and wellbeing of residents in mind. This research brief summarizes findings from a peer-reviewed study showing that few U.S. states explicitly integrate public health into their disaster recovery laws, and the types of public health requirements and authorizations vary substantially across states.

Mass casualty incidents (MCI) impose a substantial threat to healthcare. Despite the increasing frequency of MCIs many healthcare organizations and nursing programs do not adequately prepare nurses to be effective responders during an MCI. This evidence-based practice pilot project was purposed to increase new orienting emergency department nurses’ confidence and ability to effectively respond to an MCI. This project involved implementing an MCI educational program consisting of a disaster nursing framework to educate new orienting emergency department nurses on emergency preparedness and disaster response core competencies. This essay offers an intermediate discussion of select policy, strategic, operational, and tactical issues that demonstrate where and how the Commonwealth of Pennsylvania’s novel coronavirus response on the one hand, and homeland security frameworks and research on the other, converge or—more often so—diverge, and how to narrow this gap. Although typically framed as a pandemic owned by the public health sector, the COVID-19 response falls directly within the homeland security mission space, whose core missions include “Ensuring Resilience to Disasters.” In some respects, Pennsylvania’s response exemplifies best practices suggested by research. Providing education on medical care and triage during mass casualty incidents (MCI) can be challenging. Table top exercises lack the impact of a hands-on experience necessary to emphasize the scale of a real event, while full scale events are often time and resource intensive. We present a novel method using low-cost stuffed bears to expose learners to MCI triage methodology and medical care.

Background: Research on post-traumatic stress disorder (PTSD) following natural and human-made disasters has been undertaken for more than three decades. Although PTSD prevalence estimates vary widely, most are in the 20–40% range in disaster-focused studies but considerably lower (3–5%) in the few general population epidemiological surveys that evaluated disaster-related PTSD as part of a broader clinical assessment. The World Mental Health (WMH) Surveys provide an opportunity to examine disaster-related PTSD in representative general population surveys across a much wider range of sites than in previous studies.

Objective: Gathering and analyzing information on experience, training, and professional affiliation of disaster responders who will deal with functional needs clients in general shelters. Methods: The web-based survey administered in June, 2012 to 165 local Medical Reserve Corps (MRC) volunteers and 72 public health workers was part of a larger study designed to assess functional needs flow processes in general shelters. Descriptive statistics and simple logistic regression were used to analyze data.

Results: The response rate was 16.5%. The percent of survey respondents that had any disaster, medical, or functional needs training were approximately 80%, 70%, and 40% respectively. The …

Chronic conditions, disability limitations (mobility, cognitive, and sensory), and the need for assistance with activities of daily living are characteristics of elderly and medically fragile populations. Theory suggests that households with these vulnerability attributes are more likely to suffer storm-induced adverse and prolonged health consequences and, therefore, ought to evidence an increased propensity to evacuate prior to a severe storm event. Yet despite being more sensitive to storm disruption, the elderly and medically fragile populations are only slightly more likely to evacuate in the face of impending storms. The research was based on the textbooks used in the Introduction to Nursing, Psychiatric Nursing, and Community Health Nursing classes at four local nursing schools: Sinclair Community College, Kettering College of Medical Arts, Cedarville University, and Wright State University. Ultrasonography used by practicing clinicians has been shown to be of utility in the evaluation of time-sensitive and critical illnesses in a range of environments, including pre-hospital triage, emergency department, and critical care settings. The increasing availability of light-weight, robust, user-friendly, and low-cost portable ultrasound equipment is particularly suited for use in the physically and temporally challenging environment of a multiple casualty incident (MCI). Nurse practitioners need to be effective in responding to large-scale natural and man-made disasters. However, disaster core competencies for NPs are largely nonexistent. The purpose of this study was to assess NPs' level of disaster preparedness and determine how NPs acquired knowledge about disaster preparation. Findings suggest that a consistent national NP curriculum for disaster preparedness, including communication with other disaster agencies, is needed. With proper education, NPs can play a major role in disaster management.
