Background. The relative roles of obesity, insulin resistance, insulin secretory dysfunction, and excess hepatic glucose production in the development of non-insulin-dependent diabetes mellitus (NIDDM) are controversial. We conducted a prospective study to determine which of these factors predicted the development of the disease in a group of Pima Indians.
Methods. A body-composition assessment, oral and intravenous glucose- tolerance tests, and a hyperinsulinemic-euglycemic clamp study were performed in 200 non-diabetic Pima Indians (87 women and 113 men; mean [+-SD] age, 26+-6 years). The subjects were followed yearly thereafter for an average of 5.3 years.

Although The Gambia Family Planning Association (GFPA) has always been interested in promoting contraceptive use in the Greater Banjul Area (GBA), insufficient funds and the need to prioritize program expansion in rural areas hindered this. The GFPA operates two clinics in GBA, open to few workers. The government has three Family Planning and Maternal and Child Health clinics in Banjul, also used by few workers. Factory managers, trade union leaders, and the GFPA believe that a well-organized family planning (FP) education and service delivery program could benefit employees and employers. Effect Of Protein Kinase C Inhibitors And Activators On Corneal Re-Epithelialization In The Rat, A. Hirakata, A. G. Gupta, A. D. Proia

PURPOSE: To examine the ability of protein kinase C (PKC) inhibitors and activators to influence the rate of corneal re-epithelialization in the rat. METHOD: Rat corneas with 3 mm diameter central epithelial abrasions were organ-cultured in control medium or in medium with inhibitors or activators of PKC. RESULTS: In control corneas, the defect was completely re-epithelialized by 25 hr. In the presence of the PKC inhibitors staurosporine (100 nM), sphinganine (50 mumol/l), or H-7 (100 mumol/l) there were significantly larger epithelial defects than in controls after 5-25 hr of incubation. Patients with undiagnosed haemostatic defects seen at The Aga Khan Hospital and Fatimid Blood Transfusion Centre during the period of 7 years (1 985-1 992) were screened with routine tests including bleeding time (BT), whole blood clotting time (CT), platelet count, activated partial thromboplastin time (APTT), prothrombin time (PT) and 5 molar urea test. Nine patients had a positive 5 molar urea test indicating factor XIII deficiency. Rest of the screening tests were normal in these patients. High incidence of consanguinity was observed in affected families. Clinical features included excessive bleeding from umbilical stump, bruising, post-traumatic bleeding, epistaxis, melaena and …

Intake and coefficient of absorption of nutrients wefe measured in 72 children dufing acute diarrhoea and 2 weeks after fecovery. No diarrhooeal pathogens could be identified in 18 (25%) childfen (group I). Aetiology of diafrhoea was identified in fest of the 54 children (group II). Absofption of oalofie, fat and cafbohydrate during the recovery stage were similaf in all 72 children, In group I, absorption of nutrients improved from acute stage to recovery stage as follows: fat from 62 to 85%, calofies from 68 to 86% and carbohydrate from 81 to 91%. This essay serves as the foreword to Implementing U.S. Health Care Reform, a symposium held in 1993.
The exact specifications of the new health care system depend on the package that President Clinton will send to Capitol Hill and the changes that Congress will make in the reform package. Some of the basic structures and organizing principles of the new system that are being considered by the President are already the subject of intense public scrutiny.

The purpose of this study was to evaluate the effects that aerobic training has on adolescents and young adults with Down syndrome. Fourteen individuals with Down syndrome (mean age = 17.7 yr) participated in a 10-wk walking/jogging exercise training study. A pre- and post-training walking treadmill test was performed to determine the following parameters: peak oxygen uptake (VO2, absolute and relative), minute ventilation (VE, l•min-1), heart rate HR, b•min-1), RER (VCO2/VO2), and time and grade to exhaustion. Following the pre-training evaluations, subjects were assigned to a control group (N = 4) or an exercise group (N= 10). The exercise group …
