Behavior Of Restenosis After Distal Unprotected Bifurcation Left Main Coronary Artery Stenting, Ezz-Eldein El-Sawy, Khaled Al Merry, Ahmed Hassanin, Ehab Al-Hefny
Al-Azhar International Medical Journal
Background: There is controversy concerning the optimal stenting strategy in distal unprotected left main coronary artery (ULMCA) stenosis. Aim of the Work: To compare provisional stenting (PS), double kissing (DK) crush, and T and protrusion (TAP) stenting techniques in distal ULMCA stenosis. Patient and Methods: This cohort study enrolled 150 patients scheduled for percutaneous coronary intervention (PCI) and stenting with PS, TAP, and DK crush techniques. Quantitative coronary angiography (QCA) assessment was done for distal left main (LM), left circumflex (LCX), and left anterior descending (LAD) arteries pre-PCI, post-PCI, and during follow-up. Results: Post-PCI QCA revealed a significantly higher median …

Introduction: Prevalence of inguinal hernia worldwide is 15%–45% at different ages that requires surgical repair. Currently, inguinal hernia repair with a mesh is the common method through surgical procedure. The current type of mesh including different materials, and surgical options for mesh fixation include, sutures, tacks or staples, self-fixing meshes and fibrin, or other glues. Aim: to assess and compare the efficacy of mesh fixation using fibrin glue, tacks and sutures during laparoscopic repair of inguinal hernia. Patients and Methods: Randomized study included 15 adult male patients presented with inguinal hernias at Al-Azhar university hospitals using laparoscopic trans-abdominal pre-peritoneal repair. …

A Study On Effectiveness Of Preoperative Versus Postoperative Misoprostol Administration In Cesarean Section To Evaluate Primary Postpartum Hemorrhage, Ahmed Karraam, Emad Atalla, Abdelsamea Khalifa
Al-Azhar International Medical Journal
Background: Caesarean section (CS) is a frequently performed as a surgical operation, accounting for approximately one-third of all such procedures. Cesarean section rates continue to grow year after year around the world. A Caesarean section can cause a number of maternal and foetal lethal complications, the most serious of which is postpartum haemorrhage (PPH). Aim and objectives: The study's goal was to examine the safety and efficacy of preoperative vs postoperative misoprostol for reducing postpartum bleeding after cesarean surgery. Subjects and methods: This prospective single-blinded study included 100 women administrated for cesarean section. They had been randomly assigned to either …

Long-Segment Fixation With Posterolateral Fusion Versus Short-Segment Fixation With Interbody Fusion For Management Of High-Grade Lumbar Spondylolisthesis, Mostafa Aboelkhir, Mostafa Elmaghraby, Ahmed Rewehy
Al-Azhar International Medical Journal
Background & Purpose: Lumbar spondylolisthesis frequently causes a sagittal imbalance of the spine because it frequently co-occurs with other abnormalities, including forward slip and kyphosis. Spinopelvic sagittal balance is critical in spondylolisthesis assessment and treatment. The traditional fixation placement method, referred to as "short segment fixation," involves placing pedicle screws into the lower and slipping vertebral bodies. The upper vertebrae received additional pedicle screws, resulting in a long segment fixation. This study was conducted to assess the results of long-segment with posterolateral fusion fixation (Long-segment PLF) versus short-segment fixation with interbody fusion (Short-segment PLIF) for the treatment of high-grade lumbar …

The Role Of Transpedicular Approach In Thoracolumbar Junction Burst Fracture With Significant Canal Compromise, Mohamed Ali El-Gaidi, Ahmed H. Omar, Ahmed S. Kamel Abdelwahed
Advanced Spine Journal
Background Data: The thoracolumbar junction TLJ (T11–L2) is one of the most commonly injured spinal zones, representing more than 50% of all thoracic and lumbar spine fractures. Of all fractures in the TLJ, 40% are burst fractures. The ventral cord decompression at the TLJ after significant canal compromise is technically demanding. The midline laminectomy corridor does not allow adequate access to the vertebral body without significant neurological morbidity. Purpose: This study aims to evaluate the safety and efficacy of transpedicular (TP) decompression in the management of complete TLJ burst fracture with significant canal compromise. Study Design: A retrospective clinical case …

10-Minute Ebd: Should Chipped Or Worn Dental Sealants Be Observed, Touched Up, Or Restored?, Elliot Abt Dds, Ms, Msc
The Journal of the Michigan Dental Association
This column discusses the management of chipped or worn dental sealants in a 22-year-old patient seeking a second opinion. The author explores the need for occlusal restorations, sealant repair, or no treatment for such cases. A literature search, guided by a PICO question, reveals strong evidence supporting sealant effectiveness in preventing caries. The Cochrane review found an 88% reduction in caries odds with sealants. Clinical guidelines and systematic reviews from organizations like the American Dental Association (ADA) further endorse sealant use. The column emphasizes the importance of universal sealant application and highlights challenges in their adoption, such as concerns over …
Eye Safety In Dentistry, Marie T. Fluent Dds, Cdipc
Eye Safety In Dentistry, Marie T. Fluent Dds, Cdipc
The Journal of the Michigan Dental Association
In this article, Dr. Fluent underscores the importance of eye safety from vision loss that ended her clinical dental career. The article emphasizes the prevalence of ocular injuries in occupational settings, with about 2,000 U.S. workers suffering job-related eye injuries daily. Dental professionals face multiple eye hazards, including flying debris, chemicals, and infectious materials. Protective eyewear is crucial, with the CDC and OSHA recommending its use.
A significant concern is the "bottom gap" in protective eyewear, which exposes dental personnel to risks. The article introduces a new ANSI standard, ANSI/ISEA Z87.62-2021, to standardize eye and face protection against bloodborne pathogens …

Background: Diffusion-weighted imaging (DWI) investigates the microstructure of biological tissue. The variation obtained with this approach, and the estimation of ADC values for the apparent diffusion coefficient, are helpful MRI techniques that may be utilized for non-invasive tissue assessment . Aim of Work: The goal of this research is to see how valuable both qualitative and quantitative DWI assessments are in distinguishing among spinal tumors, as well as to find a cut-off ADC value among benign and malignant tumors. Patients and Methods: Studied at the Radio-Diagnosis Department of Sayed Galal hospital, National Cancer Institute. Using 1.5T MRI unit. Sagittal T1WI, …

Background: Forearm fractures are the most prevalent kind of fracture in children, accounting for 13 to 40% of all fractures. After distal radial fractures and supracondylar humeral fractures. Aim of the Work: The purpose of the study was to assess the outcomes of conservative and percutaneous pinning for the management of diaphyseal fractures of the radius and ulna in youngsters. Patients and Methods: Thirty patients aged 5 to 12 years old with closed displacement fractures of the shaft of both forearm bones were separated into two groups in this research at Al Azhar University Hospital and El Talaba Hospital.
