Background: Hospitalized patients suffer negative outcomes related to lack of mobilization activities such as falls with injury or infection. Mobility requires a multi-disciplinary team approach including Registered Nurses (RNs), Certified Nursing Assistants (NA-Cs), and Physical/Occupational Therapists (PT/OTs) to promote best patient outcomes. The Johns Hopkins Culture of Mobility toolkit provides evidence-based practices for integrating multi-disciplinary mobilization efforts for hospitalized patients. This toolkit had not been implemented on a busy orthopedic post-operative nursing unit in one large hospital.

Background: Chemotherapy-immunotherapies are high risk medications that can include multi-drug and multi-day regimens with narrow therapeutic indexes and high risk for toxicities. Susceptibility for errors exists if adherence to protocol is inconsistent or if pertinent patient specific information is omitted. Prior to the creation of the "SmartPhrase" in the Electronic HealthRecord (EHR) pre-administration safety checks were on paper, not included in the EHR, not standardized among clinicians, and not completed every time. Chart audits revealed a need for consistent, centralized documentation to enhance patient safety while minimizing professional liability. Background: Patients who suffer from alcohol or opioid use disorder face unpleasant and potentially life-threatening complications related to substance withdrawal when hospitalized. Evidence suggests that the use of a standardized tool to rate withdrawal severity and direct treatment, including score-driven medication dosing to alleviate withdrawal, leads to improved clinical outcomes. However, clinical nurses working in the inpatient medical-surgical specialty in a large medical center questioned whether all nurses were effectively using evidence-based practice (EBP) tools to assess and treat alcohol or opiate withdrawal severity.

Background: The lack of leadership support and research knowledge are reported barriers nurses experience in conducting hospital-based research.

Purpose/aims: The purpose was to determine if nursing leadership's knowledge and support of nursing research could be improved through an interactive intervention and to identify their research needs.

Methods/Approach: Design: A translational research study (Doctor of Nursing Practice project) utilizing the Barriers to Nurses' Participation in Research Questionnaire was conducted. Background: Delays in discharging patients can impact hospital and emergency department (ED) throughput. The complex discharge process makes it difficult to ensure that patients are set up for successful post-hospital care regimens. The focus of this project was to improve discharge times and flow throughout the hospital to align with national standards of providing the right care, in the right place, at the right time. Background: A virtual journal club (VJC) is a place where nurses can read, synthesize, and discuss research and evidence-based practice articles. There is a well-known time lag of disseminating this knowledge from bench to bedside. In 2016, a 300-bed community hospital constructed a VJC to educate nurses on the latest studies. In 2016 and 2017, this VJC was used via an intranet-based platform with little success. Background: Registered Nurses (RNs) working 12-hour shifts are regularly exposed to high stress situations which has intensified during the COVID-19 pandemic. Chronic stress exposure may lead to changes in health habits and impact sleep, exercise or diet, augmenting risk for adverse health outcomes and taking a negative emotional toll. Background: Most medical-device related pressure injuries develop when skin or underlying tissues are subjected to a sustained pressure or shear from medical devices. Dressings have shown substantial biomechanical effectiveness in alleviating facial tissue deformations and stresses by providing localized cushioning to the tissue at risk. Our units (DSU and SDU) provide for the monitoring and care of patients with moderate or potentially severe physiologic instability requiring technical support but not necessarily artificial support such as patients on BIPAP management. Background: Delayed recognition of in-hospital cardiac arrest (IHCA) events can affect survival rates. At the project site, there was a failure to rescue in the medical-surgical population. Key elements of success include staff identifying at-risk patients combined with early interventions to prevent deterioration of cardiac arrest. Survival and favorable outcomes for IHCA (i.e., neurological sequelae) were shown to be highly dependent on several factors, such as a skilled resuscitation team, prompt initiation of cardiopulmonary resuscitation and defibrillation, and organizational structures to support resuscitation care; therefore, nurses are an integral part of an IHCA response.

Chemotherapy-related cardiotoxicity (CTRC) is associated with significant morbidity and mortality in long-term cancer survivors. Left ventricular [LV] Global longitudinal strain (GLS) is a sensitive parameter that can be used for earlier detection of contractile function changes in these patients. Recent literature suggested certain genotypes may influence CTRC. Our goal was to assess longitudinal myocardial function and identify any correlation between certain genetic variations and LV function regarding CTRC in our pediatric population.

Clinical Presentation:

We discuss 3 patients, 8-10 years old, with significant tricuspid valve dysfunction during initial days of admission for multi-system inflammatory syndrome in children (MIS-C) associated with coronavirus 2019 (COVID-19) infection.

All 3 patients had COVID antibodies and presented in uncompensated shock needing aggressive fluid resuscitation and vasoactive support. Elevated acute inflammatory markers, acute kidney injury (2/3 patients), troponin leak and repolarization abnormality on ECG were present on admission and resolved by discharge. All were treated with intravenous immunoglobulin (IVIG) infusion, high-dose steroids and prophylactic low molecular weight heparin. They were discharged home on aspirin and steroid taper.

Two cases of {S,D,L} anatomically corrected malposition of great arteries (ACMGA) are presented with differences in conal anatomy. Case 1: A fetal echocardiogram (echo) performed at 28 weeks gestation due to multiple anomalies revealed atrial situs solitus, D-loop ventricles, a large conoventricular septal defect (VSD) and an overriding, anterior and leftward aorta. The diagnosis of double outlet right ventricle (DORV) vs ACGMA was entertained. Postnatal echo revealed {S,D,L} segmental anatomy with ventriculoarterial concordance consistent with ACGMA and a muscular VSD. Infants with univentricular physiology are at risk of poor weight gain and reduced ventricular function, both of which have been independently associated with worse outcomes. Since nutritional status has been correlated to ventricular function in other populations, we evaluated the relationship between nutritional status and ventricular function including speckle-tracking strain during this period.

Methods

Thirty term infants (median age 55 days, 13 females) with univentricular physiology prior to stage II palliation were included with data obtained at the time of their initial hospital discharge.
