This capstone was at the Cleveland clinic Hand Therapy Department. It focused on exploring, identifying and creating ways to enhance the use of occupation into evaluations and interventions for individuals with hand and other upper extremity conditions. This was done by identifying five evidence and occupation-based recommendations that could be implemented and explored at the Cleveland Clinic (research on outcome measures, updating a handout to address activities of daily living (ADLs) while wearing a splint, updated ADL kits, creating treatment cards, and created a manipulation board). This information was disseminated to the hand therapists for future use in practice.

Deoxyribonucleic acid (DNA) codes for proteins using a unique order of four major nitrogenous bases adenine, thymine, cytosine, and guanine. In several biological applications, such as the determination of DNA stability, production of biologic based drugs, and even taxonomic reorganization of species, it becomes necessary to understand the relative amounts of each nucleotide in a DNA sample, regardless of their sequence. However, most assays used to determine base composition use expensive equipment, harsh chemicals, and advanced computer software. Diabetes is a complex and chronic disease. Patients with diabetes may suffer complications such as microvascular and macrovascular diseases [1]. In 2018, the prevalence of diabetes was estimated to affect 34.2 million people, or 10.5% of the US population [2]. About 1 in 10 adults in Georgia has diabetes, a health condition that can be prevented with weight loss and lifestyle changes [3]. In 2018, diabetes-related emergencies accounted for 17 million emergency room visits, with 248,000 for hyperglycemia and 242,000 for hypoglycemia [4]. The purpose of this project was to increase health by implementing a personalized, clinic-based weight loss program for overweight/obese patients leading to a decrease in weight, BMI, and waist circumference and an increase in muscle mass by using resistance training (RT) exercises as an adjunct to diet, therefore helping to decrease comorbidities associated with obesity. In 2019, 57% of Adolescents who have substance or alcohol abuse disorders experienced at least one ACE, and 25 % experienced two or more ACEs; this means that over half of the patients admitted to an adolescent substance and alcohol abuse program have experienced childhood trauma (Gomez et al., 2017).
This study investigated ACE awareness amongst staff working with a population known to have high ACE scores. The intended purpose was to evaluate staff understanding of ACEs and their impact on patient satisfaction scores.

Background: Phlebotomy is an invasive procedure associated with pain and iatrogenic anemia. Minimizing phlebotomy for hospitalized children could improve their experience and avoid unnecessary tests. Objective: To describe: 1) the prevalence of phlebotomy-free days among children hospitalized with common conditions and 2) the association of phlebotomy-free days with clinical outcomes. Design/Methods: We performed a multicenter, cross-sectional study of children hospitalized 1/1/2018 to 12/31/2019 with an All Patient Refined Diagnosis Related Group (APR-DRG) for common infections across 38 hospitals in the Pediatric Health Information System (PHIS) database. We excluded patients with length of stay (LOS) < 2 days, medical complexity, interhospital transfers, and those receiving intensive care. We defined phlebotomy-free days (PFDs) as hospital days with no laboratory blood testing and measured the proportion of PFDs per hospital day (PFD ratio) for each condition and hospital. Hospitals were grouped into low, moderate and high average PFD ratios. Adjusted outcomes were compared across groups and included LOS, costs, and all-cause 14- and 30-day readmission rates. Results: We identified 126,135 patient encounters (Table 1). Bronchiolitis (N=31,302), non-bacterial gastroenteritis (N=20,430), and pneumonia (N=16,031) accounted for the greatest number of hospital days. Bronchiolitis (0.78) and pneumonia (0.54) had the highest overall PFD ratios, while bone and joint infections (0.28) and non-bacterial gastroenteritis (0.30) had the lowest overall PFD ratios. There was wide variation across hospitals and conditions in PFD ratios (Figure 1). We identified 8 hospitals with low, 21 with moderate, and 9 with high PFD ratios (Figure 1 and Table 1). There were statistically significant but small differences in the distributions of age, payer, and H-RISK among patients in the low, moderate, and high PFD hospital groups (Table 1). There were no differences in adjusted outcomes across low, moderate, and high PFD hospital groups (Table 2). Conclusion(s): Among children hospitalized with common infectious conditions, there was variation across conditions and hospitals in the proportion of PFDs per hospital day. Hospitals with low, moderate and high ratios of PFDs had no differences in outcomes. Our data suggest at least some laboratory overuse and opportunities to improve the experience and care of children hospitalized with infections.

Background: Bronchopulmonary dysplasia (BPD) is a significant cause of morbidity and mortality in premature infants with several known risk factors. Recent literature described the harmful effect of inflammation on the developing lung. Recently, we showed that late-onset sepsis (LOS) and necrotizing enterocolitis (NEC) causes acute lung injury (ALI) in preterm infants as measured by increases in pulmonary severity scores (PSS). The aim of this study was to investigate the association between LOS-induced ALI and BPD outcomes. Background: The assessment of pain in children with severe neurologic impairment (SNI) can be more challenging than in neurotypical children.
Objective: To describe the type, number of classes, and duration of post-operative pain medications for procedures common among children with SNI. Secondarily, to describe the variability in pain management strategies across children’s hospitals, specifically for opioid administration.

Worldwide migraines affect millions of people everyday causing significant impact on their lives. • Often times, patients have failed several first-line therapies for migraine prophylaxis. • The purpose of this systematic literature review is to evaluate the effectiveness and tolerability of TENS devices vs. CGRP antagonists in the application of migraine prophylaxis. • PubMed and ClinicalKey were searched with key terms, 17 sources were selected that were published after 2016. • Sources included meta-analysis, literature review, and randomized control trials. • Data shows that TENS devices as well as CGRP antagonists were effective and safe therapy options for migraine prophylaxis. …

Introduction: Plantar Fasciitis is a common condition in adult patients. The purpose of this literature review is to determine if there is any evidence for alternative treatments in the regimen of plantar fasciitis that has not been resolved with the standard treatment. Methods: The review was done using Pubmed, CINAHL and SportDiscus. Research was included on the following alternative treatments of PF: corticosteroid(CS) injections, extracorporeal shockwave(ESWT), dry needling, platelet-rich plasma (PRP), autologous blood injection (ABI), cupping, custom insoles, and ultrasound therapy (US).
