House Staff Quality Improvement and Patient Safety Conference (2016-2019)

Current Performance:

The rates of AC in patients 18 years or older who visited general internists & family physicians not seen by cardiologists from 4/30/2013 to 4/30/2015, cardiologists from 11/29/2012 to 11/29/2014 and all 3 specialties from 1/30/2014 to 1/30/2016 were derived from the EHR.

A 12-question survey was shared using RedCap with these physicians to examine potential reasons for not prescribing AC.

Introduction

❖ Uveal melanoma is the most common primary intraocular malignant tumor in adults.

❖ Up to half of all patients develop systemic metastases, with liver involvement in >90% of patients.1

❖ Various liver-directed, locoregional therapies (i.e. chemoembolization, immunoembolization, radioembolization, and ablation) have played a significant role in prolonging the lives of patients with metastatic uveal melanoma.2

❖ Elevated bilirubin levels are typically considered a relative contraindication for lobar hepatic arterial embolization treatment, given mainly the increased risk of precipitating hepatic failure.

❖ Gilbert's syndrome, also known as benign unconjugated hyperbilirubinemia, is a hereditary disorder of bilirubin conjugation. …

Introduction

The volume and complexity of knowledge today has exceeded any single individual's ability to manage it consistently without error despite advances in technology, years of training, and specialization of functions and responsibilities. Checklists have been employed since the 1930s in aviation and high-risk industries to prevent accidents caused by human error. Only recently has their use been introduced to surgery after the World Health Organization (WHO) heightened awareness of the significant number of deaths, about a half million worldwide, that occur each year as a result of avoidable surgical error.1 With over 275,000 surgical procedures performed daily, an …

Department of Otolaryngology - Head and Neck Surgery Posters

BACKGROUND

  • Errors in medical care have consistently been linked to communication failures among team members. TeamSTEPPS® is a team training system which was co-developed by the Department of Defense and the Agency for Healthcare Research and Quality (AHRQ) which provides a full curriculum of ways to improve patient safety through four main skills: communication, mutual support, leadership and situation monitoring1.
  • Training in these skills through TeamSTEPPS® has been shown to improve patient safety and outcomes by reducing destructive barriers within the healthcare field such as poor communication, conflicts, ambiguous responsibilities of each team member, hierarchical constraints …

  • Proton Therapy Trials May Provide More Options for Patients with Non-Small Cell Lung Cancer: Proton therapy offers the potential to deliver a higher radiation dose to lung tumors and a lower dose to healthy tissues than does standard radiation therapy. However, the effectiveness of proton therapy has not yet been documented in clinical trials for patients with non-small cell lung cancer (NSCLC), the most common type of lung cancer. Two ongoing trials of proton therapy aim to demonstrate its effectiveness and may change the standard of care for NSCLC.

Introduction:

Most congenital heart disease [CHD] is diagnosed and treated in early childhood with parents responsible for decision-making. The adolescent assumes this role in preparation for transition to an adult congenital heart program. We studied the knowledge base of our adolescent CHD patients and their parents.

Methods:

Established CHD patients, >11 y.o. and their parents, were independently surveyed in the outpatient clinic. Participation was voluntary. Cardiomyopathy, electrophysiology and transplant patients were excluded. Scores were assessed as full, partial or incomplete.

Results:

Most (98% parents; 83% adolescents) reported that their cardiologist had provided education. Adolescents provided a full (49%) and partial …

Background:

Esophagectomy is a procedure that carries considerable morbidity. Many studies have evaluated factors to predict patients at risk and improve clinical outcomes. The aim of this study was to determine whether the SAS predicts complications, length of stay, and anastomotic leak for patients undergoing esophagectomy at a high-volume institution.

Use Of Propensity Score Matching To Identify A Strong Association Between Health Care Provider Advice Not To Smoke And Quit Attempts Among Mid-Adolescent Smokers, Russell K. Mcintire, Phd, Mph May 2016

Use Of Propensity Score Matching To Identify A Strong Association Between Health Care Provider Advice Not To Smoke And Quit Attempts Among Mid-Adolescent Smokers, Russell K. Mcintire, Phd, Mph

College of Population Health Posters

Background:

  • Nearly 1 out of 7 U.S. adolescents is a current cigarette smoker by the time they leave high school. 1
  • Although almost one third of adolescents who saw a healthcare provider in the past month were advised to quit or avoid tobacco,2 research on the influence of provider advice on quit attempts among adolescent smokers has shown mixed results. 3
  • These results may be due to selection bias—adolescent smokers who get advice may be different from those who do not on a range of factors including age of initiation, smoking frequency, attitudes toward smoking, exposure to other anti-tobacco …

Abstract:

Objective The recognition of wound-related complications following pediatric tracheotomy is on the rise. These complications can have a significant impact on clinical course as well as on hospital healthcare costs to the tracheotomy dependent child and family

Methods A tracheotomy database was created in 2000 and reviewed for incidence of major and minor complications.

Setting Tertiary Care Pediatric Referral Center

Results We identified a post-tracheotomy complication rate of 19.9%, of which the most common complication was wound breakdown (65% of complications).

Conclusion Rate of post-tracheotomy wound-care complications is 12.9%. Wound description is subjective and prone to inter-observer variability. A …

Department of Surgery Posters

Background:

The purpose of this study is to review the efficacy of Target Specific Oral Anticoagulants (TSOAC) versus vitamin K antagonist (VKA) for post-surgical anticoagulation in patient with primary subclavian vein thrombosis. Rivaroxaban, a factor Xa inhibitor, was the first TSOAC approved by the FDA in 2012 for the treatment and prevention of deep vein thrombosis. However, the efficacy and safety of TSOAC for prevention of recurrent deep vein thrombosis after revascularization of upper extremity compression syndromes is largely unknown.
