An estimated 40% of people with tuberculosis (TB) are missed due to underdiagnosis, and among those previously treated TB cases, about a quarter develop drug-resistant TB in Nigeria. The emergence of multi-drug-resistant TB (MDR-TB) is stalling the effort towards TB’s control and eradication. Few studies have examined the impact of the psychosocial effect and support systems for TB, particularly for MDR-TB. The project is a staff education initiative to address medication nonadherence in a behavioral health outpatient clinic. The practice-focused question underpinning this project was, Will a staff education program with essential components improve staff knowledge, skill, and attitudes, ultimately improving medication adherence among behavioral health patients in outpatient clinics? The staff education project aimed to address the gap in practice regarding the management of aggression in psychiatric patients at a local mental health clinic. Approximately 40% of the clinic’s patient population exhibited aggressive behaviors, including verbal threats, and physical altercations. However, nurses primarily relied on pharmacological interventions and physical restraints to manage these behaviors, which failed to address underlying causes, such as psychiatric conditions and environmental stressors. Summary This doctoral project developed a clinical practice guideline (CPG) to address the critical health issue of early identification of delirium among hospitalized high-risk patients. Hospital delirium is associated with prolonged hospital stay, increased nursing home or rehab use after hospitalization, and higher mortality. Evidence shows that systematic screening improves delirium identification among hospitalized high-risk adults. The growing prevalence of e-cigarette use is fueled by the false belief that they are safer than traditional cigarettes. African American adults aged 18 and older are disproportionately impacted by tobacco products, including e-cigarettes. Summary The type of project was a staff education designed to increase staff knowledge through the implementation of an evidence-based fall prevention program tailored to an acute inpatient crisis stabilization unit. Black/African American women face significant health disparities in hereditary breast and ovarian cancer syndrome (HBOC) due to systemic barriers to genetic counseling and testing, leading to poorer survival outcomes. Guided by the Social Determinants of Health (SDH) framework and Intersectionality Theory, this quantitative descriptive study examined disparities in access to genetic services, socioeconomic barriers, and healthcare biases. This Doctor of Nursing Practice project addressed the underutilization of heat therapy for lower back pain management at an outpatient clinic through an educational intervention aimed at enhancing multidisciplinary team knowledge. My project implemented a comprehensive educational program for 12 healthcare providers. The intervention included structured learning sessions with didactic presentations, case discussions, and hands-on demonstrations. Pre- and post-intervention assessments evaluated provider understanding of heat therapy's physiological mechanisms, clinical applications, safety considerations, and treatment protocols. Analysis revealed significant improvements in provider knowledge, with mean scores increasing from 33.33% pre-intervention to 92.25% post-intervention (p<0.001). Recommendations included integrating regular educational sessions into organizational development programs, developing standardized protocols, and creating patient education materials. The project demonstrated the effectiveness of structured educational interventions in improving provider knowledge while supporting broader implementation of non-pharmacological pain management strategies. These findings have implications for enhancing patient care through increased utilization of evidence-based heat therapy interventions

Black adolescents with Type 1 diabetes mellitus (T1DM) face significant health disparities, yet their unique treatment journeys remain largely underreported. This phenomenological qualitative study explored the lived experiences of Black adolescents with T1DM. The study addressed two research questions examining how these individuals experience and navigate T1DM self-management, self-efficacy beliefs, and perceived barriers and facilitators to optimal self-care. Virtual semi-structured interviews were conducted with 15 Black adolescents with T1DM recruited from a pediatric hospital in Washington, DC. This Doctor of Nursing Practice (DNP) project focused on enhancing provider knowledge and supporting the integration of FreeStyle Libre continuous glucose monitoring (CGM) technology in a family medicine and occupational health clinic. A gap in provider understanding of CGM technology and its role in diabetes management was identified. The intervention targeted a diverse group, including medical assistants, management, clerical staff, occupational health specialists, and medical providers. A pre- and post-education knowledge assessment was conducted, and results showed an increase in provider understanding. JC polyomavirus (JCV) causes progressive multifocal leukoencephalopathy in immunocompromised individuals and has been linked to cancer development. To-date, no information is available about the JCV seroprevalence in the healthy (nondiseased) population or whether this virus is ecologically connected to humans. The aim of this systematic literature review and meta-analysis was to the estimate the overall seroprevalence of JCV in the healthy human population by age, region, and testing method. A meta-analysis conducted on 24 studies revealed a pooled JCV seroprevalence of 60% (95% CI, 55% - 65%) with statistically significant effect size (z = 32.93, p < 0.001). Meta-regression and subgroup analyses with STATA18 showed that age and region had a strong positive predictive value on prevalence (R = .578, R2 = 0.334, F(2,52) = 12.543, p < 0.001) whereas the testing method had no significant effect on prevalence. The theories that grounded this study were the ecological model and the systems theory, which support the study result that JCV appears to be ubiquitous in the human population with JCV being acquired very early in life. The results of the systematic literature review indicated that JCV is transmitted via fecal-oral and respiratory routes within and outside of families. Most infants are infected with JCV by the age of 6 months and JCV seronegativity may not be indicative of a lack of an underlying JCV infection. JCV seropositivity increases with age, which is most probably by viral reactivation due to immune senescence. These findings have positive social change implications with JCV treatment, screening, and vaccine development.

The NurseOneTap Project is a staff education initiative designed to improve nursing documentation accuracy and efficiency through the implementation of a culturally sensitive electronic health record (EHR) system. This project addressed a critical gap in nursing practice—transitioning from paper-based documentation to EHR systems—by providing structured educational interventions to enhance staff proficiency, confidence, and documentation accuracy. A pre- and post-evaluation methodology was employed to assess improvements in system navigation, real-time data access, workflow optimization, and documentation accuracy. The purpose of this DNP project was to evaluate whether a staff education program could measurably improve healthcare professionals' knowledge of the Patient Health Questionnaire-9 (PHQ-9) for depression screening at the practicum site. Depression remains underdiagnosed in primary care, largely due to inconsistent use of standardized tools like the PHQ-9, which is essential for timely diagnosis and treatment.
