Myofascial pain syndrome is very common in clinical practice. It is characterized by the existence of myofascial trigger points (MTrPs), which is a hypersensitive spot in a taut band of skeletal muscle fibers. In general, active MTrPs should be treated conservatively (non-invasive treatments including physical therapy) prior to the consideration of aggressive therapy (invasive treatments including injection and surgery). The most important strategy of MTrP therapy is to find out the etiological lesion that causes the activation of MTrPs and to treat the underlying pathology. This study aimed to assess the prevalence, severity, and correlation between non-cerebral palsy mental retardation and scoliosis in a special education institute in Taipei. Adam's forward bending test was used to clinically screen 237 mental retardation non-cerebral palsy patients for scoliosis. Those patients with positive forward bending tests were referred for standing spinal radiographs, so that the Cobb angle could be quantified to allow evaluation of the severity of the scoliosis. Of the 237 students, 41 (17.3%) had positive signs in the screening test (female:16.19%, male:18.18%). We described a case of radiation myelopathy (RM). The correlations of evolutional changes in magnetic resonance image (MRI) findings in three years with clinical symptoms and treatment responses in different stages of the disease were discussed. We also describe some special rehabilitative consideration for RM patients. Cord enlargement with contrast-enhancement showed in MRI findings corresponding to rapid neurological deterioration. Some neurological improvement after anti-coagulation and I steroid therapy was observed in this stage. Cord atrophy without contrast-enhancement was showed in following MRI. Spontaneous intracranial hypotension (SIH) is a syndrome with feature of low cerebrospinal fluid pressure (CSF) resulting from CSF leakage without any history of dural puncture, surgery, or penetrating trauma. The most characteristic feature of the syndrome is a postural headache. The headache is accentuated when the patient is in the upright position and relieved in the recumbent position. Associated symptoms include stiff neck, nausea, vomiting, tinnitus, hyperacusis, deafness, photophobia, diplopia, nystagmus, visual loss, vertigo, anorexia, malaise, and cranial nerves palsy. ”The study aimed to depict the influence of bolus temperature, volume, and viscosity in swallowing function by a self-designed phagometer. Two micro switches were used to detect submental muscle activity and pharyngeal motion. They were anchored on a posterior-type cervicothoracic orthosis with multiple adjustable joints to ensure the well contact between the switch and the skin. Acute low back pain is a very common disease in Rehabilitation clinics. Although acute low back pain can be successfully treated with epidural block, there is few researches for the efficacy of caudal epidural block on acute low back pain. Seventeen patients with acute low back pain were included and managed with caudal epidural block. Allergic reactions to wasp stings range from large local reactions to life-threatening anaphylaxis. Of the anaphylactic shock occurring outside the hospital, wasp stings was concluded to be the second cause following drug allergy. Cerebral infarction and transient ischemic attack have been reported in the setting of acute anaphylactic shock due to wasp stings. However, there are fewer reports about the presentation of large vessels occlusion outside the brain after wasp stings. Early diagnosis is difficult and thus treatment is often too late to prevent severe neuro- logical sequelae.A 46 years old male was admitted to our emergency department due to fever and general weakness. Under the impression of sepsis due to Staphylococcus aureus, he was admitted for further antibiotic treatment. Fever subsided one day after parenteral antibiotic usage. In the last decade, the pathophysiology of MTrPs has been much clarified based on clinical and basic science research work. To evaluate the immediate effects of acupuncture therapy in myofascial pain syndrome (MPS), 13 subjects (11 females and 2 males), aged from 34 to 59 years old were recruited and all had suffered MPS anywhere from 1 to 180 months. Hereditary hemorrhagic telangiectasia (HHT) or familial telangiectasia, also called Osler-Weber-Rendu syndrome, is an autosomal dominant systemic vascular disorder characterized by recurrent hemorrhage all over the whole body. The estimated incidence is approximately 1 to 2 per 100,000 people. Major clinical manifestations include frequent epistaxis, mucosal telangiectases and visceral arterio-venous malformations. To determine whether women participated in sport, even these young nulliparous female athletes, are more prone to have urinary incontinence in Taiwan. Four hundred and forty- six young female students were included in this study. All of them were nulliparous. Our subjects consisted of 260 varsity athletes, 106 athletes of high school, and 80 varsity non-athletes. They were requested to complete questionnaires on the type of major sport, intensity of training, menstrual history, sport injury, and urinary incontinence. The purpose of the study was to investigate the interaction between pain, pain behavior, anxiety, and muscle tension during electrodiagnosis, and to examine the anxiolytic and pain-relieving effects of traditional Chinese music. Two measuring tools, including electrodiagnosis pain behavior checklist (EPBC) and electromyography muscle tension scale (EMTS) were developed to objectively assess the subject's pain and muscle tension, respectively. The purpose of the study was to measure the peak pressure under the metatarsal heads (MTH) of young men and women in walking with high resolution pressure system and to determine their reliability. The peak pressure of sixteen health men and women, aged from twenty-one to thirty-five years-old, were measured at the first, second and third metatarsal head by high resolution pressure system for ten repetitions using first-step method. Spinal cord infarction is a rare disease. It is even more uncommon that spinal cord infarction occurs in a patient without any cardiovascular disease. Anterior spinal artery syndrome is much more common than the posterior spinal artery syndrome due to the difference in collateral supply. We present two patients with spinal cord infarction. In both patients, the clinical picture was stereotyped: sudden onset of weakness of limbs, sensory loss below the level of infarction and sphincter dysfunction. Treatment for the two patients was supportive and they had a good recovery over a period of weeks.

The clinical appearance of aortic dissection is characterized by the sudden, acute chest or abdominal pain, accompanied by coldness in the limbs and sweating-like symptoms of shock. Absent pulse or asymmetric pulse was observed in some patients. Primary antiphospholipid syndrome is a rare disease of vasculopathy, which is usually found in the stroke partient with onset during childhood and young adulthood. We report a female patient who had a history of multiple spontaneous abortions in the past. However, the diagnosis was not made until a cerebrovascular accident happened to her. After then, she started to receive anticoagulant therapy and rehabilitation program. No further cerebrovascular accident occurred in the following 22 months. By this report, we remind that a female patient with history of frequent abortion needs to be investigated carefully to search the possibility of antiphospholipid syndrome.

The purpose of this study was to survey the handedness performance in the children with cerebral palsy (CP). One hundred and eighty two cerebral palsied children were referred for study by the primary schools in the Taipei city. Handedness Inventory with two factors was developed and sent to their parents. Response to the survey was 116 (116/182 = 64%) with 101 of them being analyzed in this study. The mean handedness quotient was 34.39. A survey of effectiveness of home based team rehabilitation at Tai-Chung City was done form January to December 2001. Nineteen subjects (76.0% of total samples) were adopted for analysis. The rehabilitation programs included intervention once per week for six months and three times evaluation of ADL function. The results showed that 1) Both the first three months home based team rehabilitation and the following three months intervention had effectiveness on Barthel ADL index; the scores improved from 42.11±27.90 to 46.32±29.67 and 51.58±30.42 after 3 months and 6 months intervention; 2) Severity had inverse correlation to the score of Barthel ADL …

Extracorporeal shock wave therapy (ESWT) has shown success in the alleviation of painful heels and other orthopedic conditions such as tennis elbow, calcifying tendinopathy of the shoulder, and nonunion fractures of the long bones. Bilateral peripheral facial palsy (BPFP) is a rare disease. It occurs in less than one percent of patients with facial palsy. The average annual incidence rate is 0.13 to 0.2 persons per million. The etiologies of BPFP differ a little from those of unilateral peripheral facial palsy. The diagnosis depends on a series of complete examinations. We reported two cases with BPFP.
