In order to evaluate the prevalence and risk factors of post-stroke depression, this study collected the data of 102 stroke patients from the outpatient clinic of the neurology and rehabilitation departments in a medical center in Taipei. The patients' objective data, such as age, infarction area of the brain, duration from the stroke attack to the time of our study, number of stroke attacks, antidepressant use, and rehabilitation program, were obtained from the history chart. Despite the great number of computer input devices available commercially, many disabled persons still rely on special equipment to operate computers. Providing those persons with an adequate human-computer interface should improve their productivity and efficiency. However, the specialized equipment needed is usually expensive, difficult to obtain and difficult to maintain. This study developed an economical apparatus that, flexibly integrated commercial computer mice and trackballs to tailor them for individual needs. A case of femoral neuropathy as a complication of flap excision for reconstruction of a defect due to buccal cancer excision is presented. A 51-year-old man developed numbness over his left anteromedial thigh and medial lower leg, left quadriceps weakness, and absence of left knee jerk after a flap was obtained from his anterolateral thigh. Electromyographic examination revealed evidence of denervation in the left quadriceps. Winging scapula is uncommon clinically and easily ignored. It not only leads to a limited range of motion of the upper extremities, but impairs functional activity and body appearance as well. Many etiologies can induce winging scapula, including trauma, infection, iatrogenic injury, congenital abnormality, and the impairment of periscapular nerves, muscles, bones and joints. Immobilization following spinal cord injury (SCI) is a condition known to be associated with osteoporosis, which leads to increased risk of fractures. The aim of this study was to survey the incidence of bony fractures and its associated factors among chronic SCI patients in southern Taiwan. We recruited 194 patients, including 153 men and 41 women. Their mean age was 46 years old. The average duration of SCI was 12 years. There were 110 paraplegics (56.7%) and 84 tetraplegics (43.3%). Limbus vertebral fracture results from the herniation of disc material into a vertebral body, and in the formation of one or more isolated bony fragments. It is mostly found in adolescents and young adults. Clinically it resembles an acute disc prolapse with low back pain and radiculopathy. Trauma plays a significant etiologic role in up to one half of cases reported in the literature. The diagnosis of limbus vertebral fracture can rarely be based on conventional radiographs. It has been estimated that the incidence of urinary incontinence among women older than 65 of age is about 30-60%. Central pontine myelinolysis(CPM) and extrapontine myelinolysis(EPM), sharing the same pathology, are the same disease, although different symptoms and signs developed according to different lesion sites. The etiology and pathogenesis of CPM/EPM remain unclear, meanwhile rapid correction of the serum sodium level for hyponatremic patient has proven to be one of the major causes of CPM/EPM. Therefore an appropriate correction of the sodium level could prevent the development of CPM/EPM. The incidence of hyponatremia after surgical removal of pituitary adenoma reported varies from 8.4% to 38.8%. The incidence of fracture is higher in patients with chronic spinal cord injury (SCI), particularly female and elderly patients, than in healthy people. Functional impairment due to SCI may lead to accidental falls and fractures, most of which are due to trivial injury or without known trauma. In addition, the inevitable osteoporosis in the paralyzed limbs of individuals with chronic SCI increases their risk of fracture. The lower limbs are more often involved than the upper limbs. Polyneuropathy is common in the field of rehabilitation medicine. Its symptoms include sensory impairment in the distal limbs, ataxia, tremor, and pinch weakness. Electrophysiology will show multiple neuropathies with distal involvement first. Sensory nerve impairment usually appears earlier than motor nerve impairment. In severe cases, muscle atrophy can also be seen.Due to the multiple etiologies of polyneuropathy, the treatments vary. We report a rare case of anti-MAG neuropathy. The objective of the present study was to investigate the growth and body composition of younger children with cerebral palsy, using anthropometric measures and bioelectrical impedance analysis (BIA). We also looked into the relative strength and consistency between different measurements in analyzing body composition. Thirty-nine patients, aged 3 to 13, with an established diagnosis of cerebral palsy, and age- and sex-matched control subjects, were recruited for this study. For patients with symptomatic end-stage coronary artery disease, who have had a poor response to percutaneous transluminal coronary angioplasty or coronary artery bypass grafting, transmyocardial revascularization (TMR) seems to be an alternative to relieve disabling ischemic symptoms. The purpose of this study was to evaluate, by cardiopulmonary exercise testing, the cardiopulmonary capacity after TMR.Ten patients were enrolled in the study. None of them showed significant exacerbation in pulmonary function after TMR. Their average aerobic capacity was about 5MET5; a lower anaerobic threshold and high functional aerobic impairment revealed an insufficient cardiopulmonary capacity. An exact identification among the subsets of pulmonary lupus is important in planning efficacious therapy. Asymptomatic pulmonary lupus needs no specific treatment, while prompt and aggressive treatment is mandatory for the potential life-threatening pulmonary version. We present a case of systemic lupus erythematosus(SLE)with lobar atelectasis and severely restricted lung impairment and illustrate how a pulmonary rehabilitation program aided therapy during the acute infectious episode. An individualized pulmonary rehabilitation program for SLE patients compromised with chest distress may thus be a prudent course of action.

Acute ischemic stroke is well known to be a single event. However, multiple simultaneous acute strokes involving more than one vascular territory are less studied. Large-artery disease of corresponding vessels and cardioembolism are thought to be the most important risk factors for multiple simultaneous infarctions. Diffusion-weighted imaging (DWI), which successfully discriminats acute from chronic infarct, is particularly useful in patients with multiple infarcts in more than one territory. Churg-Strauss syndrome (CSS) is a rare systemic vasculitis of unknown cause, occurring in patients with asthma and accompanied by hypereosinophilia. Pathologically, CSS is characterized by necrotising eosinophilic vasculitis which may involve multiple organ systems, commonly including the lungs, skin, and peripheral nervous system. Corticosteroids are the first-line therapy, with cyclophosphamide as adjuvant therapy. The overall prognosis is good.We report a case of CSS with the clinical pattern of asthma, hypereosinophlilia (＞10%), pulmonary infiltration, and mononeuritis multiplex. Aortic valve replacement (AVR) is a common surgical procedure to treat severe valvular heart disease. Major complications of AVR include perioperative myocardial infarction, thromboemboli, hemorrhage, prosthetic endocarditis, aortic dissection, prosthetic dehiscence, and prosthetic dysfunction. Ischemic myeloradiculopathy due to aortic dissection after AVR is another complication documented in the medical literature, but the combination of lower-extremity compartment syndrome and ischemic myeloradiculopathy is rarely reported. We describe the clinical course and recovery of a 57-year-old man with this unusual combination and discuss the proper management to reduce long-term disability.

In many cases acute back pain in aged women is caused by vertebral compression fractures. Although the indirect mortality rate among these patients is not as high as in those with hip fractures, the effect of acute back pain on patients’ daily lives and social activities should not be underestimated. This study planed to develop a functional performance measure in order to systematically describe and evaluate the ability and limitation of Taiwanese children in their daily life activities. This study includes two pilot studies investigating content validity of this newly developed test: The Pediatric Functional Performance Scales (PFP Scales).Parents of 49 children aged 2-12 years were interviewed face to face or through telephone in the first study. Spasticity of the upper limb muscles is a common complication following stroke. Injection of botulinum toxin can decrease upper limb spasticity. The effects of botulinum toxin treatment are usually evaluated by activities of daily living, range of motion (ROM), or modified Ashworth scale. However, in this study, we evaluated the effect of botulinum toxin injection by quantitative EMG, and investigated the relationship between EMG and conventional evaluation methods.Twenty patients with chronic stroke-related spasticity of the upper limb muscles received botulinum injections. Positive sharp waves (PSW) and fibrillations are frequently seen together in a variety of neuromuscular diseases and share the same clinical significance. However, PSW alone are also reported to occur in myotonic disorders, periodic hypokalemic paralysis, and local muscle trauma.Our patient, a 32-year-old man, fractured his left humerus in a traffic accident 3 weeks prior to presentation. Cerebellar stroke is uncommon in young patients. We report the case of a 22-year-old man with a delayed cerebellar infarction secondary to vertebral artery dissection caused by trauma. While riding a bicycle as part of an extreme sport, his left clavicle struck a bicycle handle. Evaluation in the emergency room led to a diagnosis of left shoulder contusion without fracture of clavicle or cervical spine. Headache, severe vomiting, and weakness of the right-sided limbs developed three days later, and he presented to emergency staff again. Osteoarthritis (OA) is a slowly evolving articular disease that appears to originate in the cartilage and affects the underlying bone, soft tissues, and synovial fluid. This condition usually occurs late in life, principally affecting the large weight-bearing joints such as the knee. Health-related physical fitness can be assessed with four main attributes: body composition, muscular strength and endurance, flexibility, and cardiorespiratory endurance. Regular exercise is thought to improve physical fitness, and objective physical fitness assessment and appropriate exercise prescriptions may help unfit individuals to become fit. Assessment of physical fitness among the general population can aid the understanding of general levels of physical fitness in the community, promote healthy habits, and reduce the burden of chronic illness caused by physical inactivity.
