Objective: To describe a patient with the critical illness polyneuropathy superimposed on diabetic neuropathy and to highlight the importance of early rehabilitation after critical illness. Methods: A 42-year-old man with a previous history of diabetic polyneuropathy developed flaccid tetraplegia with respiratory failure after septic shock due to a liver abscess. This clinical manifestation, combined with the result of an electrodiagnostic study, suggested the acute exacerbation of polyneuropathy. Because a pre-existing polyneuropathy history complicated a differential diagnosis, we performed a comprehensive work-up to rule out other systemic diseases or metabolic disorders that may cause polyneuropathy. Hiring in-home foreign caregivers for long-term disabled patients is common in Taiwan. Caregivers help relieve the stress of caregiving on the patient's family members, and patients who receive care at home can also interact with their family. The Osaka Medical College (OMC) brace is an underarm spinal orthosis for the treatment of scoliosis. It consists of a plastic pelvic section with a pad extended from the lateral bar to correct the convexity of the curve. Background and Purpose: Whole body vibration (WBV) training is a novel exercise intervention that is used in both athletic and general populations with the aim of improving muscle strength and balancing ability. Amyloidosis is defined as the extracellular deposition of fibrillar proteins in different organs and tissues. The clinical features of amyloidosis include cardiomyopathy, hepatomegaly, proteinuria, macroglossia, and dysphagia. We report the case of a 52-year-old man with myeloma-associated amyloidosis who complained of progressive dysphagia, dysarthria, and macroglossia. Videofluoroscopic swallowing study revealed severe reduction in tongue movements, aspiration after swallowing, and retention in the valleculae and pyriform sinuses. The patient underwent swallowing therapy for 6 months; the therapy included compensatory neck flexion, vocal cord adduction exercises, supraglottic swallow, and the Mendelsohn maneuver. This is a casereport about a 53-year-old female patient with a history of chronic back pain, suffered from complete paraplegia shortly after spinal manipulation. CT and MRI images consistently showed ossification of the ligamenta flava in conjunction with bilateral facet joints hypertrophy at the T10-11, T11-12 and L1-2 levels. Spinal cord injury with T12 paraplegia ASIA-A was thus diagnosed. Posterior laminectomy at the T10, T11, L1 and L2 levels and interbody fusion from T10 to L2 spines for fixation were performed. A 37-year-old female had an occlusion of right middle cerebral artery (MCA) due to traumatic internal carotid artery (ICA) dissection without significant neck injury after a traffic accident. Although this phenomenon has been documented in recent studies, it remains a relatively rare event. Imbalance in automatic cardiovascular function has been shown to increase the risk of ventricular arrhythmias and sudden death in patients with coronary artery disease and after myocardial infarction. The study evaluated preoperative heart rate variability (HRV) as an independent prognostic factor for hospitalized patients undergoing coronary artery bypass surgery (CABG).In this single-blind study, we enrolled 25 patients scheduled to undergo CABG. For decades, ultrasonography has shown a high degree of accuracy in the evaluation of soft tissue tumors. In this study, we report the condition of a 80-year-old male patient who had been suffering from left thumb pain for 2 years. The patient had initially been diagnosed with de Quervain's disease, but responded poorly to conservative treatment. An ultrasonographic examination unexpectedly identified a well-margined solid tumor with marked vascularity. Suspicion of a neurogenic tumor led to an excision surgery and finally pathology revealed a Schwannoma. Following excision, the symptoms were entirely ameliorated. This is a report of a 65-year-old male patient with incomplete spinal cord injury caused by a traffic accident. He was robust prior to this injury. He was hospitalized due to tetraplegia caused by the accident, and did not lose consciousness at the time of injury. A cervical spine MRI was performed and showed cervical spondylosis with a C4-5, C5-6 herniated intervertebral disc. Numerous serotonergic drugs have been increasingly used in recent years to treat depression, pain, insomnia and obesity. However, concomitant use of these drugs may cause serotonin syndrome. The three main clinical features of serotonin syndrome are neuromuscular abnormalities, autonomic hyperactivity, and mental status changes. A wide variety of the clinical presentations range from: very mild symptoms such as tremor, diarrhea, muscle rigidity, or clonus, and hyperreflexia; to fatal consequences such as disseminated intravascular coagulation or multiple-organ failure. Delayed spinal epidural hematoma (SEH) after a spinal compression fracture is a very rare but important cause of symptomatic spinal cord compression. We report a patient with spinal cord compression and cauda equina syndrome due to delayed SEH after a traumatic T12 compression fracture. A 74-year-old male patient had severe back pain without neurologic deficits after a traumatic T12 compression fracture. He developed progressive bilateral lower extremity weakness, sensory loss, and sphincter dysfunction 2.5 months after the injury. Ankylosing spondylitis (AS) rarely coexists with tophaceous gout in the spine. We present a case of a 57-year-old businessman with AS who did not have previous history of hyperuricemia or gouty arthritis. He had a three-month history of progressive bilateral lower limb weakness caused by spinal cord compression (rooted on a tophaceous gout), leading to incomplete paraplegia. He received surgical decompressive laminectomy, which removed the hypertrophic ligamentum flavum and tophus in the level of thoracolumbar junction. The purpose of this study was to evaluate the effects of taping on motor recovery of paretic upper limb in chronic stroke patients. Ten chronic stroke patients with upper limb paresis were recruited. They all received regular rehabilitation programs before this study. Taping was applied to the upper trunk and paretic shoulder of those patients in addition to regular 3-week rehabilitation. Brain abscess is a neurological emergency that requires immediate high-dose antibiotics or surgical intervention. Brain abscess at the site of a previous hemorrhagic stroke that did not receive surgical intervention is rare. Here, we report the case of a 48-year-old man who developed a brain abscess as infective transformation of a preceding right putaminal hemorrhage, 22 days after the stroke. The patient recovered after surgical treatment and systemic antibiotics therapy. Macrodystrophia lipomatosa is a rare congenital anomaly of localized gigantism, characterized by progressive overgrowth of all mesenchymal components, particularly fibroadipose tissue. The anomaly occurs frequently in the distribution regions of the median nerve in the upper extremity and the plantar nerve in the lower extremity. It is usually noticed at birth with predominately unilateral distal involvement of the digits. The cause of this condition is still unknown. Speech sound errors have been noted as the most common speech disorder among children. However, recent studies about pronunciation errors among preschool children are lacking. Our aim was to evaluate the characteristics and frequency of pronunciation errors among pre-school children with phonological disorder in Taiwan. A 37-year-old male suffered from hemorrhagic stroke with left side hemiplegia and neurogenic bladder. On admission, due to post-renal acute renal failure with hydronephrosis, he received indwelling catheterization but felt very uncomfortable. However, he was physically unable to perform clean intermittent catheterization (CIC) due to his hemiplegia. Two novel devices were designed to help him perform CIC using one side of his body. As in our experience with this case, hemiplegic patients may now be able to perform CIC by themselves to achieve adequate bladder drainage using one-sided limbs under the assistance of the novel orthoses.

Titers of antistreptolysin-O (ASO) possess the diagnostic value in discriminating post-streptococcal reactive arthritis. Patients are labeled as ”unclassified arthritis” (UA) while hard to be classified as having a well-defined arthropathy.
