Epidemiologic studies indicate blood glucose management in the hospitalized patient population is not adequate; with approximately 30% of patients reporting values >180mg/dl. Patients with increased blood glucose levels pose a higher mortality risk as well as increased risks for dehydration, hypotension, renal injury, decreased wound healing, immunosuppression, and an overall increase in hospital length of stay (AACE, 2015). Increasing cardiac device patient demands within a large Midwest healthcare institution resulted in significant increases in staff shift requirements and corresponding patient complaints. The quality improvement project aimed to decrease patient complaints by executing a standard baseline daily full time equivalent (FTE) staffing strategy for future schedules, increasing nursing FTE, and maintaining institutional staffing standards. Standard calculated daily nursing requirements targeted 17.0 FTE. Implementation occurred over a three month period including schedule reprocessing, a pending incremental nursing FTE request, and a restructuring of unit based scheduling and paid time off guidelines. This project focuses on the CNL competency leadership theme of Clinical Outcomes

Manager. “Clinical Outcomes Manager uses data to change practice and improve outcomes”

(AACN, 2013). With the assistance of my preceptor, lean team members, department managers,

and administration, this project will aim to reduce delayed patient transfers times between the

emergency department and inpatient units by 50%. This project goal will be achieved by writing

and setting in place a new guideline for 45 minute black-out transfer times. The anticipated goal

The ending data reflected what we expected in that we would see a significant incline but

not quite the 100% compliance rate we are looking for. Our highest compliance rate has reached

90% and we have contributed the margin error to new staff that requires the proper teaching and

Failure to recognize delirium compromises patient safety and may lead to harmful outcomes. By June 2015, nurses on medical-surgical units will screen 100-percent of patients for delirium according to an established protocol. The project mesosystem is a 411-bed, level I trauma center and teaching hospital; the microsystem is a 28-bed medical-surgical telemetry unit.

An audit of microsystem data collected over four months in 2014 revealed that delirium was consistently underidentified (10% in September, 26% in October, 34% in November, and 22% in December), suggesting that medical-surgical nurses were not equipped to effectively perform delirium screening.

This Clinical Nurse Leader (CNL) project involves the nursing staff and patients of a 23-bed post-surgical medical-surgical nursing unit at a Bay Area community hospital. The goal of this project was to improve patient care, patient satisfaction and staff workflow. Assessment of the microsytem revealed that from July 2013 through July 2014, the nursing unit scored below the unit’s goal 10 out of the 12 months for the HCAHPS question related to patient’s receiving help as soon as they pressed their call button.
