Occupational Therapy Publications

While optimizing quality of life(QOL) is a key goal of rehabilitation care,no previous study has reported on what ‘QOL’ means to youth with chronic health conditions. In addition, no qualitative studies have explored the relationship between QOL and self-determination(SD). Objectives of this qualitative study were to examine: what the terms ‘quality of life’ and ‘self-determination’ mean to youth with chronic conditions; the factors these youth think are linked with these concepts; the relationship they see between concepts, the types of future goals youth have and how they view the connection between their SD and these goals. A descriptive methodology was …

Situating Occupation In Social Relations Of Power: Occupational Possibilities, Ageism And The Retirement ‘Choice’, Debbie Laliberte Rudman

Occupational Therapy Publications

Introduction: Research attending to social relations of power can enhance understanding of the mechanisms through which occupational injustices occur and inform socially transformative practice. This study explored how power operates through ageism in ways that shape what people come to take for granted regarding occupation in relation to age, and what occupations are supported through socio- political conditions.

Methods: Narratives were collected using a two-stage interview process with 17 retired Canadians. A critical narrative analysis approach was employed to examine how these narratives revealed the complex ways occupations are negotiated within broader discourses and age relations.

Findings: Ageism mattered for …

Building Trust: A Critical Component Of Global Health, Obidimma Ezezika Jan 2015

Building Trust: A Critical Component Of Global Health, Obidimma Ezezika

Health Studies Publications

Trust is a concept that we often take for granted. But as much as it serves as glue for human relationships, trust lies, albeit subtly or sometimes hidden, at the crux of all global health interventions and is critical to their success or failure.1,2 The recent Ebola pandemic in West Africa shows how distrust among the community, public health officials and government institutions can lead to a massive failure in the delivery of health care and response to a disease outbreak. Media articles and commentaries attributed the slow response to the Ebola crisis to a lack of trust on many …

Unified Voxel- And Tensor-Based Morphometry (Uvtbm) Using Registration Confidence., Ali R Khan, Lei Wang, Mirza Faisal Beg Jan 2015

Unified Voxel- And Tensor-Based Morphometry (Uvtbm) Using Registration Confidence., Ali R Khan, Lei Wang, Mirza Faisal Beg

Medical Biophysics Publications

Voxel-based morphometry (VBM) and tensor-based morphometry (TBM) both rely on spatial normalization to a template and yet have different requirements for the level of registration accuracy. VBM requires only global alignment of brain structures, with limited degrees of freedom in transformation, whereas TBM performs best when the registration is highly deformable and can achieve higher registration accuracy. In addition, the registration accuracy varies over the whole brain, with higher accuracy typically observed in subcortical areas and lower accuracy seen in cortical areas. Hence, even the determinant of Jacobian of registration maps is spatially varying in their accuracy, and combining these …

Medical Biophysics Publications

INTRODUCTION: Sepsis-associated encephalopathy (SAE) is a state of acute brain dysfunction in response to a systemic infection. We propose that systemic inflammation during sepsis causes increased adhesion of leukocytes to the brain microvasculature, resulting in blood-brain barrier dysfunction. Thus, our objectives were to measure inflammatory analytes in plasma of severe sepsis patients to create an experimental cytokine mixture (CM), and to use this CM to investigate the activation and interactions of polymorphonuclear leukocytes (PMN) and human cerebrovascular endothelial cells (hCMEC/D3) in vitro.

METHODS: The concentrations of 41 inflammatory analytes were quantified in plasma obtained from 20 severe sepsis patients and …

A Method For 3d Histopathology Reconstruction Supporting Mouse Microvasculature Analysis., Yiwen Xu, J Geoffrey Pickering, Zengxuan Nong, Eli Gibson, John-Michael Arpino, Hao Yin, Aaron D Ward

Medical Biophysics Publications

Structural abnormalities of the microvasculature can impair perfusion and function. Conventional histology provides good spatial resolution with which to evaluate the microvascular structure but affords no 3-dimensional information; this limitation could lead to misinterpretations of the complex microvessel network in health and disease. The objective of this study was to develop and evaluate an accurate, fully automated 3D histology reconstruction method to visualize the arterioles and venules within the mouse hind-limb. Sections of the tibialis anterior muscle from C57BL/J6 mice (both normal and subjected to femoral artery excision) were reconstructed using pairwise rigid and affine registrations of 5 µm-thick, paraffin-embedded …

Bone and Joint Institute

© 2015 The Authors. Optimal component alignment in total knee arthroplasty has been associated with better functional outcome as well as improved implant longevity. The ability to align components optimally during minimally invasive (MIS) total knee replacement (TKR) has been a cause of concern. Computer navigation is a useful aid in achieving the desired alignment although it is limited by the error during the manual registration of landmarks. Our study aims to compare the registration process error between a standard and a MIS surgical approach. We hypothesized that performing the registration error via an MIS approach would increase the registration …

Introduction: Hip fractures are a leading cause of mortality and disability worldwide, and the number of hip fractures is expected to rise to over 6 million per year by 2050. The optimal approach for the surgical management of displaced femoral neck fractures remains unknown. Current evidence suggests the use of arthroplasty; however, there is lack of evidence regarding whether patients with displaced femoral neck fractures experience better outcomes with total hip arthroplasty (THA) or hemiarthroplasty (HA). The HEALTH trial compares outcomes following THA versus HA in patients 50 years of age or older with displaced femoral neck fractures. Methods and …

Patient socioeconomic status (SES) has been demonstrated to impact access to care for numerous healthcare interventions but little is known about its impact on HRA when compared to THA. The aim of this study was to investigate whether there are disparities in SES for patients receiving HRA or THA. A retrospective database review was conducted comprising 617 hip arthroplasty patients (310 HRA, 307 THA). Patient postal code was used as a surrogate marker for patients’ SES …

Forebrain Neurocircuitry Associated With Human Reflex Cardiovascular Control, J. Kevin Shoemaker, Ruma Goswami Jan 2015

Forebrain Neurocircuitry Associated With Human Reflex Cardiovascular Control, J. Kevin Shoemaker, Ruma Goswami

Bone and Joint Institute

© 2015 Shoemaker and Goswami. Physiological homeostasis depends upon adequate integration and responsiveness of sensory information with the autonomic nervous system to affect rapid and effective adjustments in end organ control. Dysregulation of the autonomic nervous system leads to cardiovascular disability with consequences as severe as sudden death. The neural pathways involved in reflexive autonomic control are dependent upon brainstem nuclei but these receive modulatory inputs from higher centers in the midbrain and cortex. Neuroimaging technologies have allowed closer study of the cortical circuitry related to autonomic cardiovascular adjustments to many stressors in awake humans and have exposed many forebrain …

A Technique Of Superficial Medial Collateral Ligament Reconstruction Using An Adjustable-Loop Suspensory Fixation Device, Shaneel Deo, Alan Getgood

Bone and Joint Institute

© 2015 Arthroscopy Association of North America. This report describes superficial medial collateral ligament reconstruction of the knee using a novel method of graft fixation with the ACL Tightrope RT (Arthrex, Naples, FL). After tibial fixation with either a standard interference screw or staple, femoral fixation of the semitendinosus tendon is performed with the adjustable-loop suspensory fixation device, which allows for both initial graft tensioning and re-tensioning after cyclical knee range of motion. This provides the ability for the graft to accommodate for resultant soft-tissue creep and stress relaxation, thereby allowing for optimal soft-tissue tension and reduction in laxity at …

Deficits In Audiovisual Speech Perception In Normal Aging Emerge At The Level Of Whole-Word Recognition., Ryan A Stevenson, Caitlin E Nelms, Sarah H Baum, Lilia Zurkovsky, Morgan D Barense, Paul A Newhouse, Mark T Wallace

Brain and Mind Institute Researchers' Publications

Over the next 2 decades, a dramatic shift in the demographics of society will take place, with a rapid growth in the population of older adults. One of the most common complaints with healthy aging is a decreased ability to successfully perceive speech, particularly in noisy environments. In such noisy environments, the presence of visual speech cues (i.e., lip movements) provide striking benefits for speech perception and comprehension, but previous research suggests that older adults gain less from such audiovisual integration than their younger peers. To determine at what processing level these behavioral differences arise in healthy-aging populations, we administered …

Prevalence Of Increases In Functional Connectivity In Visual, Somatosensory And Language Areas In Congenital Blindness., Lizette Heine, Mohamed A Bahri, Carlo Cavaliere, Andrea Soddu, Steven Laureys, Maurice Ptito, Ron Kupers Jan 2015

Prevalence Of Increases In Functional Connectivity In Visual, Somatosensory And Language Areas In Congenital Blindness., Lizette Heine, Mohamed A Bahri, Carlo Cavaliere, Andrea Soddu, Steven Laureys, Maurice Ptito, Ron Kupers

Brain and Mind Institute Researchers' Publications

There is ample evidence that congenitally blind individuals rely more strongly on non-visual information compared to sighted controls when interacting with the outside world. Although brain imaging studies indicate that congenitally blind individuals recruit occipital areas when performing various non-visual and cognitive tasks, it remains unclear through which pathways this is accomplished. To address this question, we compared resting state functional connectivity in a group of congenital blind and matched sighted control subjects. We used a seed-based analysis with a priori specified regions-of-interest (ROIs) within visual, somato-sensory, auditory and language areas. Between-group comparisons revealed increased functional connectivity within both the …
