Prenatal alcohol exposure (PAE) is associated with structural and physiological changes that impact the central nervous system and can result in persistent negative consequences in a broad spectrum of cognitive and behavioral domains including deficits in spatial learning and memory and behavioral flexibility. The current study focuses on examining the behavioral and mechanistic consequences of PAE, sex and housing. Rats were tested in the Morris Water Task where they were trained to navigate to a goal location and tested in a pool shift manipulation. Practice Development Trajectories Of Experienced Nurses: Adaptive Experts, Routine Experts, And Non-Experts, Dee Mulcahy

This qualitative study examined practice development among experienced, acute care nurses (n = 9) with at least 10 years of experience. Using an interpretive biographical approach, with participants’ written practice narratives and in-depth interviews, the study focused on how the work environment and life experiences influenced or impeded practice development. Participants were classified as routine- or adaptive experts (n = 3 of each type) with one intermediate case and two not readily classifiable. Antecedents to becoming a nurse included family influences, personal interests, altruistic values, and financial need. Experience was necessary but not sufficient for adaptive expertise. Compared …

The partnership model in which universities collaborate with community colleges for the prelicensure Bachelor of Science (BSN) education has been identified by the National Academy of Medicine (NAM) as one of five viable academic progression models for nursing. The current literature lacks evidence on the success of these academic progression models for replication and improvement in an effort to increase the BSN-educated workforce. Opioid addiction is a serious and persistent global health issue. The federal Centers for Disease Control and Prevention (CDC) reported that between 1999 and 2016, more than 630,000 people in the United States died of an overdose of a prescription opioid or illicit drug (CDC 2018). Extant research has suggested that for nearly a century, New Mexico has experienced some of the highest rates of prescription and illicit opioid death in the nation (Goldstein and Herrera, 1995; Landon, 2003; Shah et al., 2008). The purpose of this study was to investigate the experiences and perceptions of persons living with narcolepsy from their perspective. An anonymous, online survey that included both open-ended and structured questions was administered to adults with narcolepsy with or without cataplexy (N = 72; 65 F) recruited through the auspices of the Narcolepsy Network (NN). A total of 69 participants responded to at least one open-ended question about experiences of diagnosis, treatment, and living with the condition. Measuring Language Development In Children With Down Syndrome Who Use Aac, Ji Sun Park

Purpose: This study examined the inter-observer agreement (IOA) and within-observer agreement as well as the clinical potential of newly proposed measures that are designed to monitor language progress of children with Down syndrome who use AAC. Measures were explored based on the Graphic Symbol Utterance and Sentence Development Framework.

Method: Participants included 8 preschoolers with Down syndrome. Four graduate student observers coded 13 measures across 57 intervention sessions. Each session was coded by two observers for IOA, and all sessions were recoded for within-observer agreement. Statistical analyses were completed on utterance level and session level.

In 2016, the CareLink New Mexico behavioral health homes program began enrolling Medicaid recipients with the goal of increasing care coordination, improving access to services, and decreasing long-term costs of care for adults with serious mental illness (SMI) and children with severe emotional disturbance (SED). To evaluate these aims, a retrospective interrupted time series study using Medicaid claims data was designed. First, a comparable subset of non-enrolled individuals was selected from the pool of Medicaid recipients with SMI or SED using propensity score matching. Using creative autoethnographic testimonio (CAT), a story is told about the injustices within the learning environment and work environment of higher education toward a person with disabilities: sleep apnea, learning disabilities, negative mental health. The author explores the health difficulties of addressing sleep deprivation while attending graduate school and working full-time as a professional. With sleep apnea impacting his health, his mental health declines. As his health declines, there is an increase in discrimination, hostility, oppression, bullying, and toxic masculinity. Commonly used standardized tests, like the Western Aphasia Battery-Revised (WAB-R), are not sensitive to higher level discourse deficits, leading to certain individuals not meeting diagnostic criteria for aphasia. Consequently, individuals with aphasia are excluded from receiving potentially beneficial services and/or from being included in research. In a large sample of persons with stroke-induced aphasia, this study analyzed discourse samples using Main Concept, Sequencing, and Story Grammar (MSSG) Analyses to examine macrostructural discourse characteristics of persons with latent aphasia (PWLAs) compared to persons with no underlying brain injury (PNBIs) and persons classified as having anomic aphasia (PWAAs) by the WAB-R. Hydrogen sulfide (H2S) is a gasotransmitter critical in maintaining microcirculation homeostasis. Impaired microcirculation occurs in multiple disease states such as peripheral vascular disease, diabetes mellitus (DM), and hypertension. Early detection and identification of patients with DM who are at risk for heart attack, stroke and amputation due to microvascular disease is crucial. Human skin is an accessible vascular bed that provides an opportunity to non-invasively measure H2S, which could be used as a biomarker to evaluate microvascular health.

In this dissertation I develop and use comparative genomic technologies to investigate the novel SARS-CoV-2 virus, assembly the first genome of the black lace domestic angelfish and identify germline genetic variants associated with altered breast cancer-specific survival. This study explored five New Mexican parents’ perceptions of changes to the public school-based communication and language services for their children with autism spectrum disorder (ASD) during the Covid-19 pandemic and the subsequent advantages and disadvantages of these changes. The parents participated in semi-structured interviews between October 2020 and February 2021. All of the parents reported their children’s services had eventually gone completely remote, but one had secured some in-person instruction time for her child by the time of her interview. Background: Proximal fifth metatarsal fractures are common. The authors previously performed a study examining the inter- and intraobserver reliability of the Lawrence and Botte Classification, which showed poor interobserver reliability of 16.67%. A novel approach was proposed for the classification of fifth metatarsal fractures using a mortise ankle radiographic view. Methods: The observers from the authors’ prior study reviewed non-weight bearing mortise views of 20 patients with isolated fifth metatarsal base fractures. Radius shaft fractures often require operative treatment after skeletal maturity to restore functional range of motion. If the radial bow is not anatomically restored, pain and instability can occur from bony impingement and disruption of the distal radioulnar joint (DRUJ). We describe a 25-year-old woman with a radius shaft malunion and DRUJ instability. Eight years prior, at the age of 17, she underwent open reduction and internal fixation for a radial shaft fracture with subsequent reinjury at 10 weeks postoperatively. Atypical femur fractures are rare, low-energy fractures that involve a specific constellation of radiographic findings. These fractures have been well described in adult osteoporotic patients on long-term bisphosphonates; however, little to no literature exists on atypical femur fractures in pediatric patients on long-term bisphosphonates. The use of bisphosphonates as treatment of osteogenesis imperfecta is common to reduce fracture rate and improve bone mineral density. We describe a 15-year-old adolescent boy with type I osteogenesis imperfecta on long-term bisphosphonate therapy. Distal radius fractures are common injuries, accounting for 17.0% of all emergency department visits. Operative treatment is an option when indicated. Volar plating has become the most frequently used mode of fixation. Although fixed angle volar locking plates allow for reliable and stable fixation, complications have been reported. Complications include flexor and extensor tendon rupture, intra-articular screw penetration, malreduction, loss of reduction, carpal tunnel syndrome, implant failure, and complex regional pain syndrome. We propose seven principles to avoid these preventable outcomes and tips to succeed.

A 41-year-old man presented to the emergency department after crashing his dirt bike and sustaining bilateral subtalar fracture dislocations. The left talus underwent open reduction internal fixation. At his first postoperative visit, the patient was found to have bilateral peroneal tendon subluxation (PTS). This case report discusses the cause of PTS and the challenges associated with accurate identification and diagnosis of this injury. We review the diagnostic and therapeutic challenges associated with injury, the negative impact on clinical outcomes, and the association between PTS and hindfoot trauma.

Introduction: The treatment algorithm for tibial plateau fractures continues to evolve as surgeon experience increases. Initial surgical procedures have been fine-tuned over time. Methods: This is a retrospective chart review of 132 patients with low-energy tibial plateau fractures (Schatzker I–IV) treated at a level-1 academic trauma center between January 2008 and February 2018. This study analyzes patient outcomes managed with primary operative intervention within 48 hours of admission. The average age was 45 years (range, 19 to 84 years). There were 92 men and 40 women. Medial opening wedge high tibial osteotomy (HTO) can provide symptom relief, improved ligamentous stability, and lower failure rates in patients with symptomatic medial knee osteoarthritis, ligament instability, and varus alignment after ligament reconstruction. It has been reported that correcting coronal plane abnormalities with a medial opening wedge HTO has resulted in altered posterior tibial slope angle (PTSA). Background: Gunshot injuries frequently involve the foot and ankle. The standard of care for treatment of gunshot injuries to the foot and ankle is not clear. To better understand these injuries, we completed a retrospective review of foot and ankle gunshot injuries at a level-1 trauma center seen as orthopaedic consults over a 2-year period. Methods: Orthopaedic consults for foot and ankle gunshot injuries from June 1st, 2018, through May 31st, 2020, were reviewed. Tibial overgrowth after pediatric tibial fractures has been reported in the literature. Multiple factors can affect the amount of overgrowth, including age and gender of patients, type of fracture (open vs closed), degree of communion, reduction techniques, and fixation method. We describe a 10-year-old boy with a 2-cm overgrowth after sustaining an isolated open tibia fracture with an intact fibula, which is more than what has been previously described in the literature for tibia fractures. This overgrowth was associated with asymmetrical growth of the distal tibial physis, asymmetrical growth between tibia and fibula, and ankle valgus deformity.
