Therapeutic Effects Of Sildenafil Combined With Low-Dosage Imatinib On Pulmonary Hypertension In Five Dogs, Yu Sahashi, Yoshiaki Hikasa

Five dogs were diagnosed with pulmonary hypertension (PH) secondary to chronic degenerative mitral valve disease or ventricular septal defect. They were initially administered low-dose imatinib mesylate (3 mg/kg, q24h) without changes to the background therapy to treat PH. The low-dose imatinib improved the right ventricular (RV) function parameters including estimated systolic and mean pulmonary arterial pressures (sPA and mPAP) and clinical symptoms. On day 53 to day 168 after imatinib administration, as PH and clinical signs worsened, low-dose sildenafil (0.5 mg/kg, q12h) was administered in combination with imatinib. A five-month-old Siamese kitten was presented with acute respiratory distress. The kitten had respiratory acidosis. The result of the feline N-terminal prohormone of brain natriuretic peptide (NT-proBNP) test kit was abnormal. Thoracic radiography revealed patchy lung infiltration and cardiomegaly. Severe right heart enlargement and an intermediate probability of pulmonary hypertension were suggested from echocardiography. Pulmonary hypertension secondary to severe pneumonia was suspected. The kitten was treated with oxygen supplementation, antibiotics and pimobendan. The clinical signs, radiography and echocardiography returned to normal after 30 days of treatment.

The objective of this retrospective study was to distinguish between dogs with pulmonary hypertension (PH) and non-PH by using parameters obtained from thoracic radiographs. Data of 72 dogs of small breeds aged more than 1 year old and weighing less than 10 kg were retrieved. Echocardiography was used as a gold standard to diagnose PH and separate the dogs into two groups: PH (n=15) and non-PH (n=57). Seven formulae were created from measurements on right lateral and ventrodorsal radiographic views. Long-Term Effect Of Low-Dose Imatinib Therapy For Pulmonary Hypertension Due To Chronic Degenerative Mitral Valve Disease In Six Dogs, Zi Ping Leong, Shinji Arita, Yoshiaki Hikasa

Six dogs were diagnosed with pulmonary hypertension (PH) secondary to chronic degenerative mitral valve disease (CDMVD) on echocardiography. Imatinib (3 mg/kg, every 24 hours, PO) was initiated without any changes to the background therapy to treat the PH. Follow-up evaluations at 1, 3, 5 and 6 months revealed substantial clinical and hemodynamic improvements. One dog showed deterioration after the imatinib withdrawal to necessitate a restart therapy. No side effects were observed throughout the 6-month treatment course. Low-dose imatinib may provide a promising treatment alternative for CDMVD-associated PH in dogs.
