• Steroids are used in shock to minimize the impact of shock-induced dysregulation of inflammatory response – Hydrocortisone is preferred for its activity as a glucocorticoid and a mineralocorticoid • Available data on the use of hydrocortisone shows mixed benefit – Data to guide dosing is limited, use of 100 mg IV every 8 hours may have benefit over 50 mg IV every 6 hours • Use of steroids has been associated with potential risks, including gastrointestinal bleeding, hyperglycemia, hypernatremia, superinfection, etc.

Circulating tumor DNA (ctDNA) are short DNA sequences shed by tumor cells into the systemic circulation. Studies have shown potential utility of the test to predict relapse or recurrence following treatment in solid tumors, but sensitivity and specificity have varied widely, ranging from 19-100% and 80-100% respectively, in breast cancer specifically. Moreover, literature describing the utility of monitoring dynamic changes in ctDNA trends is limited. We aim to evaluate the correlation between ctDNA test, both single test as well as dynamic trends in value over time, with imaging findings.

Rationale: Idiopathic generalized epilepsy (IGE) accounts for approximately one-third of all epilepsies. The majority of people with IGE achieve seizure-freedom from treatment with anti-seizure medications (ASM). However, up to 25% of people with IGE continue to experience seizures despite adequate ASM trials. In patients with drug resistant IGE, neuromodulation with responsive neurostimulation (RNS) in the bilateral centromedian nucleus (CMN) of the thalamus may be an effective treatment option. Reconstruction of midfacial traumatic defects secondary to a gunshot wound (GSW) is one of the most challenging head surgeries. The high impact on the functional and aesthetic quality of life, and the small margin of error to achieve a successful outcome are significant hurdles in the surgical treatment of these cases. Here we report a 33-years old patient who suffered a GSW with an entrance wound penetrating between the soft and hard palate and an exit wound on the right malar region. Purpose: The etiology of lower urinary tract symptoms secondary to benign prostatic hyperplasia (LUTS/BPH) remains uncertain. The purpose of our study was to quantitatively analyze pelvic anatomic characteristics on magnetic resonance imaging (MRI) to assess for independent factors for symptoms.

Patient with documented COVID-19 infection five weeks prior to arrival and presented with pain, paresthesias, and weakness in the bilateral lower extremities. Symptoms began shortly after patient recovered from COVID-19 infection, however, patient developed ascending weakness extending into the hands with left sided chest pain, prompting patient to present for evaluation.

Background: Increased eosinophils in sinus contents are regarded as nonspecific allergic sinusitis. However there are only a few studies addressing the rate of fungal infection associated allergic fungal sinusitis in non-specific allergic sinusitis. This study was to identify the prevalence of allergic fungal sinusitis in the sinus content specimens with increased eosinophils. Background: The presence of neutrophils in muscularis propria is known to best correlate with clinical symptoms of acute appendicitis. Eosinophils present in the muscularis propria were noted in cases of acute appendicitis in 2000 (Carr, NJ. Annal of Diagnostic Pathology 2000;4:46–58), but the significance of their presence there has been unclear. The goal of this study was to investigate whether increased eosinophils in muscularis propria could be correlated with early symptoms of patients with clinical diagnosis of symptomatic appendicitis.

Background:Paneth cells with features ofneuroendocrine differentiation can be seen inmultiple organs and various tumours. Paneth cell-likegranules in clear cell renal cell carcinomas (RCC)have previously been reported, but are found noexpression of neuroendocrine markers. Case Diagnosis: Protective Effect of Previous Spine Surgery in Traumatic Cervical Spinal Cord Injury Case Description or Program Description: A 39-year-old female with previous C5-7 anterior cervical discectomy and fusion (ACDF) in 2017 presented following a fall from 20 feet while hiking. She was life flighted to an outside hospital and found to have multiple cervical spine fractures which were surgically repaired with C4-T2 fusion and C7 laminectomy.
