Background: Proteinuria is a known cardiovascular risk factor in hypertensive or diabetic patients as well as the general population. Worsening renal function (WRF) has been shown to influence outcomes in the heart failure population. The prognostic value of these markers for renal dysfunction in patients after myocardial infarction is unclear. Methods: The Survival and Ventricular Enlargement trial (SAVE) randomized 2231 patients with left ventricular dysfunction between 3-16 days (average 11 days) post-MI to receive captopril or placebo; those with a serum creatinine above 2.5mg/dl were excluded from SAVE. Evaluation Of The Effectiveness Of A Diabetes Managment Training Program For Unlicensed Assistive Personnel In Schools, Elaine M. Gomer

This study evaluated the diabetes management-training program for unlicensed assistive personnel (UAP). The purpose of the program was to prepare UAP to recognize and respond to the health care needs of students diagnosed with diabetes mellitus. Twenty UAP participated. Teaching strategies were based on Albert Bandura' s social learning theory~ Two instruments were administered before and after the program. One was a 1 0-question survey addressing self-efficacy; the other was a 10 question multiple-choice test measuring knowledge of diabetes. Results of paired t tests indicate consistent and significant improvement in both perceptions of self-efficacy and knowledge (p < .05). This program provides a framework for school nurses training UAP that assist in the health care of students with diabetes. It is important that school nurses capitalize on the contribution that informed confident UAP might contribute to the provision of safe quality care to students with diabetes.

Diabetes mellitus is a group of conditions characterized by hyperglycemia due to an inability to produce or properly utilize insulin. The majority of cases fall into two categories, Type I and Type 2. Type I results from the autoimmune destruction of pancreatic β-cells of the islets. The beta cells are the exclusive source of insulin and the patient becomes entirely dependent on exogenous insulin to survive. Patients with Type 2 are distinguished by insulin resistance, a condition that develops due to the inability of the body to effectively use the insulin being produced. Objective: Chronic disease comorbidities, on the rise in the U.S. and Virginia, represent a new challenge to the way medicine is practiced and prescribed. This descriptive study uses Virginia hospital discharge data to describe the prevalence and trends of chronic disease comorbidities present in the state's over-45 population during the years 2001 and 2004.Methods: Data collected by Virginia Health Information was utilized. Adults over the age of 45 years and who selected for race and location were included in this analysis, with an aggregate sample size of 813,336 (N=458,593 [2001]; N=364,743 [2004]). Background The reduction of diabetes complications depends on adherence to self-management behaviors. The current study was designed to examine the influence of multiple social-environmental levels of support on diabetic self-management behaviors in a lower income community. The eight distinct levels of social-environmental support studied were personal characteristics, physician and health team, family and friends, neighborhood, community, media and policy, community organizations, and the workplace. Methods An anonymous self-administered survey, consisting of questions about demographic features, hemoglobin AlC, self-management behaviors, and perceived support from multiple social-environmental levels, was completed by 36 diabetic participants from the Regions Family Physicians clinic. Obesity is a dangerous health condition affecting approximately 30% of all Americans and can be attributed to 300,000 deaths a year. Obesity has been found to disproportionately affect Blacks, Latinos, and those with lower SES. Although obesity is a problem for all age groups, its prevalence is highest among those 60-74 years of age. Studies have shown that there is an association between obesity and chronic conditions such as cardiovascular disease, arthritis, cancers, and type-2 diabetes. Research objectives. One source of falls in the elderly may be an inability to sufficiently adjust to transient postural perturbations or slips. Identifying useful predictors of fall potential, as well as factors that affect the ability of an individual to detect a movement of the standing support surface may provide insight into postural stability and methods to increase stability in elders. To do this, acceleration thresholds to short, precise, lateral platform translations and the resultant psychophysical responses of adults with early Type 2 diabetes to age-matched controls and young adults were measured.

This pilot study examined the relationship between perceived self-efficacy and regimen adherence on glycemic control among adolescents, aged 11 to 18 years, with Type 1 Diabetes Mellitus. Thirty-six adolescent-parent dyads participated in this study. Bivariate analysis using Pearson product moment-correlations identified no significant relationships among the variables. Regarding treatment regimen, adolescents on insulin pumps had a mean glycosylated hemoglobin level of 8.09% whereas adolescents on daily insulin injections had a mean glycosylated hemoglobin level of 9.26%. Bandura's theory of self-efficacy provided the theoretical framework for this study. The Self-Efficacy for Diabetes scale and Questionnaire of Self-Care Behaviors scale were used …

The relationship between chronic illness and sexuality has long been recognized, and diabetes mellitus is a chronic illness affecting 16 million individuals in the United States. The purpose of this descriptive comparative study was to explore sexuality in the insulin-dependent diabetic female. The research hypothesis that guided the study was there is no difference in perceived sexuality for women with insulin-dependent diabetes and women without chronic illness. Diabetic neuropathy is an extremely common medical problem because of the high frequency of diabetes and its complications. Diabetic neuropathy consists of not one, but many patterns of nerve injury, each potentially with its own discrete pathogenic mechanisms. Evidence is accumulating that indicates autoimmunity plays a role in diabetic peripheral neuropathy. However, the significance of immunopathogenesis of diabetic neuropathy is as yet unclear.

From a population of former clients of the diabetic unit at Baptist Memorial Hospital in Memphis, Tennessee, a sample of 21 individuals w e r e g i v e n the Assessment of Educational Needs of the Diabetic Patient Questionnaire. This study described three days of dietary intake, physical activity, and glycemic control in 14 female and eight male adult diabetics using Continuous Subcutaneous Insulin Infusion (CSII). Changes in weight after initiation of CSII were also described.
Dietary intakes were analyzed using the NUTREDFO computer program then compared to established standards. Both sexes had mean intakes of zinc, folate, vitamin B6, and magnesium below the Recommended Dietary Allowances (RDA, 1980) for age and sex. This study explored the effect of blood glucose self-monitoring on two dependent variables: (a) the diabetic client's self-esteem, and (b) the diabetic client's ability to apply knowledge about diabetes mellitus. The sample consisted of two groups of adult Type I diabetic clients. Group I consisted of ten clients who had been performing blood glucose self-monitoring for at least one year, and group II consisted of ten clients who had been performing urine testing for at least one year. The literature regarding angiographic patterns of arteriosclerosis obliterans (ASO) in the legs of diabetics and nondiabetics is reviewed. The medical records of 28 adult-onset diabetics and 35 nondiabetics with chronic ASO of the lower extremities who underwent angiography for the first time were analyzed for age, presenting symptoms, smoking history, history of hypertension and therapeutic fate of the affected limbs. The arteriograms were analyzed for basic disease patterns and degree of disease at specific segments of the arterial tree, based on the radiologist's findings. The problem for this research study was to ascertain if registered nurses of varying educational backgrounds and experience discriminated similar human needs as being altered by specific pathologic states. The two pathologic states studied were chronic obstructive pulmonary disease and diabetes mellitus, chosen for their high prevalence in the population. A population sample of 55 registered nurses practicing on medical units in five hospital settings were surveyed. This study was conducted to determine whether a functional relationship exists between human tissue and environmental levels of chromium and/or zinc and in individuals with maturity onset diabetes.
Three counties of Utah were selected as sites:
