This project explores the values of home blood pressure monitoring and includes a pamphlet for use at the Colchester Family Medicine office. It has information on how to check blood pressure, which cuffs to buy, and how to enter home measurements into Epic so the physician can easily view them.

Recently UVM Health Network Family Medicine practices have implemented “The Genomic DNA Test” pilot program to begin a concerted effort toward offering genetically informed primary care to all patients. The program aims to increase the number of participating primary care providers stepwise over time. However, some providers may find integration of genomic testing and discussion of clinical genetics issues with patients to be challenging given the relatively recent change toward emphasis on these topics in medical training curricula. This project aimed to develop a better understanding of patient and provider needs at all UVMMC family medicine and urgent care offices in regards to the new Cures Act. Data was gathered via a 8 question multiple choice survey and we had 24 provider responses. Most providers have found that they are receiving more phone calls from patients with questions in regards to their medical notes or results since the enactment of the Cures Act. Providers mentioned common patient questions are about imaging results, CBC, and electrolytes. Nearly half of providers noted they have changed the way in which they …

Primary and secondary prevention are vitally important mainstays of primary care. Every well visit at Newtown Primary Care involved some discussion of preventive health screening and services and relied on patient recall or EHR document search for last screening/intervention dates. We aimed to find ways to make use of wait times for in-office visits that could also potentially improve efficiency during patient encounters and help educate patients about preventive health measures.

Seasonal Affective Disorder (SAD) is a cyclical pattern of depression-like symptoms that occur during the winter months. In Vermont, SAD is estimated to affect up to 10% of individuals. An informational flyer was developed that explained the symptoms of SAD, what the treatment options are, and then linked individuals with local resources for wellness activities in Chittenden County using QR codes in order to prevent SAD. Refugees experience profound hardship in their home-country, displaced country and throughout the period of re-settlement. Experiences like direct or indirect exposure to traumatic events, torture, sexual abuse and stigmatized refugee identity post-resttlement are major risk factors for the development of mental health coniditions inlcuding PTSD, depression and psychosis. Despite major risk factors for the development of mental health conditions, culturally-specific mental health stigma persists as a major barrier to addressing mental health among refugees. This project emphasizes techniques and self-education that can be utilized to improve mentah health screening and identification among refugee patients in Vermont.

At Milton Family Practice in Milton, VT, individuals diagnosed with pre-diabetes are referred to a clinical nutritionist and sent home with patient instructions about blood sugar and carbohydrates. For those who decline a meeting with the clinical nutritionist, the patient instructions are not an adequate substitute. This project focused on the creation of a SmartPhrase to be added to patient instructions outlining common high-carbohydrate foods and options for lower-carb alternatives.

Intimate partner violence (IPV) is a significant public health problem throughout the United States. The aim of this project was to implement a validated IPV screening form into the work flow of the Newtown Primary Care office and increase awareness about IPV throughout the office. The Hits, Insults, Threatens, Screams (HITS) screening form was implemented and resources from local domestic violence agencies were obtained for the office. Due to multiple factors, patients at ambulatory settings tend to experience long appointment wait times, negatively impacting patient satisfaction and overall well-being. One proposed solution is to use pre-visit data collection to simplify check-in and screening processes at the office. This project surveyed patients at Brookfield Primary Care & Pediatrics regarding quantitative wait time data and qualitative perceptions on the proposed solution. It was found that a majority of patients are open to using pre-visit online forms to reduce wait times.

UVMMC Primary Care is transitioning to the Primary Care Mental Health Integration (PCMHI) model of care, in which behavioral health and/or psychiatric treatment is provided within a primary care setting via in-house consulting clinicians. There are currently no standardized informational materials to educate patients on the details of the PCMHI program, which is a new treatment model for most patients and clinicians involved. Development of educational materials, as well as broader public health implications of the PCMHI model of care, are addressed.

Using reputable online resources, we created a handout containing a list of exercises that can be completed safely at home without equipment. Many patients with disordered eating will not present with objective manifestations such as low BMI, physical exam findings, or electrolyte abnormalities. Among patients that meet DSM-V criteria for an eating disorder, the majority do not fit criteria for anorexia nervosa or bulimia nervosa, but instead fall into a category known as "Otherwise specified feeding and eating disorders." As such, it can be difficult to identify these patients. This project proposes adding 2 validated screening questions to outpatient intake forms, or incorporating them into patient visits, to identify disordered eating among high-risk groups.

Rational barriers contribute to resistance towards initiating, restarting or maintaining lifestyle/behavioral modification. Such barriers include lack of confidence in the proposed intervention(s), exaggerated perspectives, unsubstantiated beliefs and feelings of futility/hopelessness in the setting of past failures or relapse. Lifestyle and behavioral modification are core elements of preventative health and mental health—identifying and addressing erroneous thought processes, perspectives or beliefs that contribute to hesitancy may empower patients to trust the process and put an honest effort towards making those changes.
