Ethnicity Matters: The Socioeconomic Gradient In Health Among Asian Americans, Emily S. Ihara

This study examines the relationship between socioeconomic indicators and health status among Asian Americans using data from the 2001 California Health Interview Survey (CHIS), a population-based random-digit-dial survey with race-ethnic supplemental samples. Multivariate logistic regression analyses show that the inverse relationship between socioeconomic position and health status is similar for Asian Americans when measured as an aggregate group compared to Whites. However, when specific Asian American ethnic groups are examined, the relationship varies greatly. For example, among Chinese Americans and Vietnamese Americans, education is a significant predictor of poor health status, but household income is more significant among Korean Americans. …

"I'M Glad You Asked": Homeless Clients With Severe Mental Illness Evaluate Their Residential Care, Katherine Tyson Mccrea, Lesa Spravka
The Journal of Sociology & Social Welfare
Homeless clients with severe mental illness can offer considerable insight about their residential care, but there are significant methodological challenges in eliciting their service evaluations: maximizing participation, facilitating self-expression, and preserving clients' natural meanings. This study addresses those challenges and presents qualitative data residential care staff obtained from 210 clients. While clients prioritized meeting their subsistence needs, they emphasized attaining inner well-being and mutually respectful relationships, and that group services needed to reduce confrontational interactions in order to be helpful. For after-care services, clients sought sustained relationships with staff grounded in client initiative, combining respect for their autonomy with psychosocial …

Using Social Construction Theory As A Foundation For Macro-Level Interventions In Communities Impacted By Hiv And Addictions, David Allen Patterson, Robert H. Keefe
The Journal of Sociology & Social Welfare
Many professionals working with people living with HIV and alcohol and other drug addictions rely heavily on micro and mezzo-level interventions. The authors argue that although these approaches are effective for helping people living with some social problems they are too narrow for working effectively with HIV-positive and alcohol and other drug-addicted individuals. The authors use social construction theory to analyze the social problems of HIV/AIDS and addictions and make recommendations for macro-level interventions that may help curtail the dual problems of HIV and addictions.

Prior to January 1, 1997, individuals with drug- or alcohol-related disabilities could qualify for federal public assistance through the Supplemental Security Income (SSI) program. During the welfare reforms of the Clinton administration, this policy was changed, resulting in lost income and health care benefits for many lowincome substance abusers. This paper examines the historical underpinnings to the elimination of drug addiction and alcoholism (DA&A) as qualifjing impairments for SSI disability payments. Following this, empirical evidence is presented on the effect this policy change had on the subsequent economic security of former SSI DA&A beneficiaries. This paper presents an exploratory study of alcohol and drug use in two rural communities in Plateau State, Nigeria. The aim is to raise awareness of the rural alcohol and drug problem. The paper examines the patterns of alcohol consumption and drug use, and their perceived functions for substance use among rural farmers in Nigeria. The study shows the common use of marijuana and alcohol in addition to prescription drugs. There is also evidence of multiple or combinational drug use. Concepts from the procedural justice literature in social psychology are examined that offer useful guidance for social work researchers with interests in investigating informal adjudications, speciality treatment courts, and other areas of the administrative process previously neglected in mental health services research. These theoretical concepts are offered as an alternative to the therapeutic jurisprudence framework being adopted by some social workers in the field of law and mental health. The Use Of Critical Ethnography In Managed Mental Health Care Settings, Cassandra L. Bransford

How social workers in managed mental health care settings exercise their professional authority may have profound consequences for the provision of ethical and value-based services to vulnerable populations. Building upon Gidden's theory of structuration, this article describes the use of critical ethnography as a specific research methodology that may support social workers in the exercise of their authority. This article examines the historical roots of critical ethnography and provides a detailed examination of its underlying assumptions and research procedures. The article concludes with a case example of a critical ethnography conducted within a managed mental health care setting.

Nineteenth Century Review Of Mental Health Care For African Americans: A Legacy Of Service And Policy Barriers, Tony B. Lowe
The Journal of Sociology & Social Welfare
The need to focus on service and policy barriers to mental health service delivery for African Americans remains critical. The purpose of this article is to review nineteenth century care as a method for understanding contemporary service and policy barriers. A case study strategy is used to compare the efforts of Pennsylvania and South Carolina using primary and secondary sources to document these developments through a political economy perspective. These findings suggest that the prevailing social, political and economic realities have created mental health disparities along racial lines. Existing barriers are likely rooted in this same reality.

Job insecurity has been increasing since the 1980s. While researchers have found job insecurity to be negatively associated with multiple indicators of well-being for workers and their families in cross sectional studies, less is known about the long term effects of prolonged job insecurity. Specifically, there is a need to collect measures of both insecurity and its consequences at multiple time periods. The current study followed workers for 3 1/2 years to assess the effects of chronic job insecurity on psychological distress. Results indicate that while workers reported increased feelings of security over time, there were longer term negative effects …

Public opinion polls conducted from 1936 to 2002 found that Americans support both euthanasia and physician-assisted suicide. Although public opinion regarding end-oflife decisions appears to have been influenced by the events of the times, Americans have consistently favored the freedom to end one's life when the perceived quality of life has significantly diminished, either by one's own hand or with the assistance of a physician. This paper indicates that existing policy regarding euthanasia and physician-assisted suicide conflicts with the American public's attitudes regarding the matter, as well as examines implications for social workers who serve clients facing end-of-life decisions.

This study investigated the effect of patient insurance status upon physicians' decisions to write do-not-resuscitate orders (DNRs). Ninety-four physicians completed a questionnaire consisting of demographic data and a case vignette. In addition to the main research question, the study explored the effect of religious affiliation on writing DNRs and performing "slow codes." Results indicate that insurance status has a significant effect upon the likelihood of writing a DNR, with physicians more likely to write DNRs for patients covered by public (i.e., government-funded, as compared to private) insurance. Religious affiliation was also significant, with greater church attendance associated with a lesser …

Writings about hope within gerontological literature assume social workers already possess hope that they can use in their practice. The purpose of this article is to challenge this assumption and to examine ways in which social workers can sustain hope in personal life, in their agencies, and in the reform of larger social structures that impact older adults. The authors examine culture change in nursing homes as an emerging approach that can be more fully developed by applying the strengths perspective to interpersonal work with elders, agency change and broader structural change.

Although efforts are being made to decrease the number of new HIV infections in Thailand, less support is give to the growing population that is already affected by the disease. This qualitative study explores the roles of Buddhist temples in the treatment of AIDS in Thailand, specifically the perspectives of both Buddhist monks and persons who are living with AIDS on HIVIAIDS and the care provided at the temples. Three major themes were derived from the interviews: (1) temple as a last choice; (2) temple as a support group; and (3) the role of Buddhism and monks at the temple.

Older men are much less likely to be aware of community services available to them and they are less likely to utilize services generally. This underutilization is affected by the way in which social services are organized and how practitioners function within them. Since there are greater numbers of elderly women and women utilize services more readily, practice tends to be female-centered. This paper reports on the first comprehensive study of self-help groups in Hong Kong. Initial findings from the quantative and qualitative data suggest that self-help group participation has an impact on intrapersonal, interpersonal and community/political empowerment. Based on existing data, this study has resulted in the development of a hypothetical model encompassing the interrelationships among self-help group participation, social support, social learning, leadership and empowerment, for testing in future research.

In a qualitative study conducted in Ontario, Canada, family members and friends of ill people defined a normal territory in which care from health professionals could not be counted on to be timely, effective or empathic. Under these conditions relatives and friends took on considerable responsibility, both for providing care and for securing care from health professionals. Yet considerable variation was apparent in this study in the sense respondents had of their own capacities to provide and secure care. As formal systems for the protection of children have evolved in this country, certain barriers to achieving justice within the child protection system have emerged concomitantly. Specifically, these barriers involve ambiguous definitions of abuse and the appearance of social inequality and bias within the child protection system. One means of surmounting these barriers to justice is family group conferencing (FGC). Support for this assertion comes from the integration of the restorative justice model and procedural justice theory. When applied to the practice of FGCs in child protection, the integration of these theoretical perspectives provides a strong rationale for the use …
