Abnormal Uterine Bleeding: A Management Algorithm., John Ely, Colleen Stockdale, Elizabeth Clark, Noelle Bowdler May 2013

Abnormal Uterine Bleeding: A Management Algorithm., John Ely, Colleen Stockdale, Elizabeth Clark, Noelle Bowdler

Colleen Kennedy Stockdale

Abnormal uterine bleeding is a common problem, and its management can be complex. Because of this complexity, concise guidelines have been difficult to develop. We constructed a concise but comprehensive algorithm for the management of abnormal uterine bleeding between menarche and menopause that was based on a systematic review of the literature as well as the actual management of patients seen in a gynecology clinic. We started by drafting an algorithm that was based on a MEDLINE search for relevant reviews and original research. We compared this algorithm to the actual care provided to a random sample of 100 women …

Vulvar Cellular Angiofibroma: A Case Report, R. Kerkuta, Colleen Kennedy, J. Benda, R. Galask

Colleen Kennedy Stockdale

Cellular angiofibroma is a benign growth initially described in 1997, with few reports to date. A 31-year-old woman presented with a 3-year history of a small left labial mass, which had recently increased in size to 5 cm, and was clinically thought to be a lipoma. A simple excision was performed. Histologically, the mass was consistent with a cellular angiofibroma. Ten months later, the growth has not recurred. Cellular angiofibroma is a rare, benign mesenchymal lesion typically Occurring on the Vulva, and should be considered in the differential diagnosis of a painless, soft, vulvar mass. Dyspareunia increased during pregnancy, but remained elevated post partum. Compared with the historical nonpregnant …

Management Of Atypical Squamous Cells, Low-Grade Squamous Intraepithelial Lesions, And Cervical Intraepithelial Neoplasia 1, Lori Boardman, Colleen Kennedy May 2013

Management Of Atypical Squamous Cells, Low-Grade Squamous Intraepithelial Lesions, And Cervical Intraepithelial Neoplasia 1, Lori Boardman, Colleen Kennedy

Colleen Kennedy Stockdale

In the American Society for Colposcopy and Cervical Pathology 2006 Consensus Guidelines, several changes in the management of mildly abnormal cervical cytology and histology were made. The most notable changes involve the management of adolescents, pregnant women, and postmenopausal women. For adolescents, management of atypical squamous cells of undetermined significance and low-grade squamous intraepithelial lesions is conservative, eliminating the need for immediate colposcopy. For pregnant women, options have been made to allow for deferral of colposcopy until pregnancy completion, whereas for postmenopausal women, the new guidelines call for the option to rely on human papillomavirus DNA testing or repeat cytology …

Vaginal Microbiota Of Women With Frequent Vulvovaginal Candidiasis, X. Zhou, R. Westman, R. Hickey, M. Hansmann, Colleen Kennedy, T. Osborn, L. Forney

Colleen Kennedy Stockdale

Vulvovaginal candidiasis (VVC) is an insidious infection that afflicts a large proportion of women of all ages, and 5 to 8% of affected women experience recurrent VVC (RVVC). The aim of this study was to explore the possible importance of vaginal bacterial communities in reducing the risk of RVVC. The species composition and diversity of microbial communities were evaluated for 42 women with and without frequent VVC based on profiles of terminal restriction fragment polymorphisms of 16S rRNA genes and phylogenetic analysis of cloned 16S rRNA gene sequences from the numerically dominant microbial populations. The data showed that there were …

Meta Analysis: Effects Of Interventions On Premature Infants Feeding, H. Daley, Colleen Kennedy May 2013

Meta Analysis: Effects Of Interventions On Premature Infants Feeding, H. Daley, Colleen Kennedy

Colleen Kennedy Stockdale

Nipple feeding of premature infants presents a challenge to neonatal nurses who are trying to prepare the infants for eventual discharge from the hospital. To determine what empirical evidence there was to support interventions that positively influence feeding performance and feeding efficiency a meta-analysis is presented. The effects of NPO, nasogastric tubes, different nipples, breast versus bottle feeding, gestational age, oral support, oral stimulation, and nonnutritive sucking on volume intake are evaluated. The results suggest that nursing care should: (a) consider the infant's gestational age; (b) maturational ability and development; (c) keep track of and attempt to minimize the length …

Risk Factors For Painful Bladder Syndrome In Women Seeking Gynecologic Care, Colleen Kennedy, Catherine Bradley, R. Galask, I. Nygaard

Colleen Kennedy Stockdale

The aim of this cross-sectional study was to report risk factors for painful bladder syndrome in women. We surveyed 645 women presenting for care to general gynecology clinics and a vulvar specialty clinic using a standardized questionnaire and validated outcome measures. We used two definitions for painful bladder syndrome, based on the O'Leary-Sant interstitial cystitis symptom and problem indices. Of those women surveyed, 29.5% met broader criteria and 8.5% met more restrictive criteria for painful bladder syndrome. After adjusting for confounders, bladder pain was significantly associated with current smoking, irritable bowel syndrome, and a generalized pain disorder. Tobacco use, the …

Colleen Kennedy Stockdale

BACKGROUND: Leiomyosarcoma of the vulva is a rare gynecologic malignancy, comprising approximately 1% of vulvar cancers. CASE: A 36-year-old woman was referred for a slowly growing painless vulvar Mass that was initially thought to be a Bartholin's duct cyst but was cancerous upon biopsy. A modified radical vulvectomy was performed, and pathology revealed a grade I leiomyosarcoma. Thirteen months later, the tumor had not recurred. CONCLUSION: Vulvar cancer must be considered in patients with. a suspected Bartholin duct cyst that demonstrates. atypical features. A biopsy should be obtained if the mass appears firm or solid on palpation, is ulcerated or presents …

Vaginal Misoprostol Aids In Difficult Intrauterine Contraceptive Removal: A Report Of Three Cases., Whitney Cowman, Jean Hansen, Abbey Hardy-Fairbanks, Colleen Stockdale

Colleen Kennedy Stockdale

BACKGROUND: Intrauterine devices are the most used long-acting reversible contraceptive method worldwide. Under normal circumstances, removal of an intrauterine contraceptive (IUC) is an uncomplicated procedure requiring gentle traction on the string. STUDY DESIGN: We report three cases of nonvisible IUC strings where, following use of vaginal misoprostol, the IUC strings were visualized and the IUCs were removed intact with gentle traction. CONCLUSIONS: The uterotonic and uterocontractile effects following vaginal misoprostol facilitated removal in three cases of nonvisible IUC strings. We suggest that clinicians consider including vaginal misoprostol alone or prior to planned repeat office or procedure-clinic interventions for nonvisible IUC …
