State Agency Promising Practice: Mandatory Situational Assessments In Tennessee, Allison C. Hall, Thinkwork! At The Institute For Community Inclusion At Umass Boston

Tennessee implemented its Employment First initiative in 2002 with a goal of making employment the first day service option for adults receiving supports from the Department of Mental Retardation Services. As part of the Employment First initiative, the state requires a periodic community-based work assessment for all individuals not currently employed in the community. State-contracted and independent individualized support coordinators ensure that individuals who are not in integrated employment participate in a community-based work assessment at least every three years. Since December 2005, employment providers can be trained in conducting situational job assessments and the importance of providing these opportunities.

The state of Florida has implemented a five-year employment initiative for people with ID/DD. The goal is to enable at least 50% of adults (ages 18 to 55) receiving APD-funded day services as of July 1, 2004, to achieve community employment by July 1, 2009. APD-funded services include adult day training, supported employment, and non-residential supports and services. Florida is specifically targeting a total of 25% of individuals who were in Adult Day Training (ADT) on July 1, 2004, to be employed by July 1, 2009. In North Carolina, counties have been consolidated into Local Management Entities (LMEs). These entities contract for services with community providers and provide oversight on access, utilization, best practices, and community collaborations. The Mecklenberg County LME established the Best Practices Community Committee, comprising service providers, individuals and family members, advocacy agencies, community partners, interested community volunteers, and LME staff. Sub-committees addressed several areas, including employment. Based on their recommendations, a pilot project that uses an outcome-based funding model for follow-along employment services was developed. In 2007, the State of Connecticut’s Department of Developmental Services (DDS) partnered with the self-advocacy group People First of Connecticut to develop Employment Idol, an innovative project for promoting employment as the preferred outcome for individuals with intellectual/developmental disabilities (ID/DD) in the state. Spinning off the concept of the popular television show American Idol, Connecticut’s Employment Idol showcases the employment success stories of a select group of individuals with ID/DD.

The State of Pennsylvania’s Office of Developmental Programs (ODP) created a monthly newsletter called The Employment Update, which covers state- and nationwide news about the employment of people with disabilities, including intellectual/developmental disabilities. The Employment Update is sent via email to state agency contacts and a large stakeholder community, including individuals with disabilities, service providers, state associations, employers, advocacy groups, family members, representatives from academia and others. The Employment Update contains information about employment trends, employment policy, trainings and conferences throughout Pennsylvania and nationwide. Oklahoma’s Developmental Disabilities Services Division (DDSD) realized the need for increased attention towards the goal of community-based employment for individuals they served. Initially, rates were based on a vendor’s costs of providing direct services such as job development and job coaching. It became increasingly apparent that claims for vocational services oftentimes reflected staff activities (e.g., job development, client assessment, and service delivery documentation), which may have been occurring without the direct involvement of the service recipient. In some situations, this resulted in long-term job development with little success in actually acquiring a job. In 1995, DDSD elected to focus on …

The University of Maine’s Center for Community Inclusion and Disability Studies (CCIDS), along with the Maine Department of Behavioral and Developmental Services (BDS) and the Bureau of Rehabilitation Services, developed the Maine Employment Curriculum (MEC). The comprehensive curriculum fosters best practices in employment supports for people with disabilities statewide by using a cadre of trainers who are supported by the Maine Employment Curriculum project staff. The Maine Employment Curriculum ultimately seeks an increase in the number of integrated, communitybased supports available and builds the capacity of the employment support provider community to achieve this goal.

In 2002, the Massachusetts Department of Developmental Services (DDS) developed a contractual requirement that employment service provider performance be tracked through outcome measures. As a result, DDS shaped its employment data collection system to focus on what it viewed as key outcomes for measuring success around employment. A confluence of factors including participation in the National Core Indicator project, a new Request for Responses (RFR) for Employment Support Services that emphasized performance measurement, and DDS’s intrinsic commitment to greater community employment supported the development of an employment data system that focused on individual outcomes. In Colorado, counselors from the Division of Vocational Rehabilitation are housed on-site in Community Centered Board1 offices so they can provide direct services to individuals with intellectual and developmental disabilities (ID/DD). The goal of the project was to serve 240 customers with ID/DD and provide 134 successful employment outcomes over a two-year period2. Streamlined services and enhanced communication emerged through a unique collaborative effort between the two entities.

This study explored Lao life histories, health, and social adjustment in the southern New England states of Rhode Island and Connecticut. In addition, it sought to examine whether there was a correlation between war experiences early in life and health in adulthood, a finding reported in previous research on the Hmong, another ethnic group from Laos. Overall, 99 Lao adults born in Laos or Thailand (mean age 43.5 +/- 10.8 years) completed orally administered questionnaires and were measured for blood pressure and various anthropometric markers. As huge numbers of baby boomers reach old age, an increased need for nursing home care in the future is inevitable. Older people face more complex health issues that often lead to debilitation or disability and thus the need for long-term care. The numbers of consumers needing long-term care services will more than triple during the next 30 years.
African Americans compose less than 10% of nursing home residents and even at age 84 and older, when the likelihood of nursing home placement is higher, proportionately fewer African Americans live in nursing homes. The underrepresentation of ethnic minorities groups in …

Background: Recent studies have noted myriad qualitative and quantitative inconsistencies between the medieval Black Death (and subsequent ‘‘plagues’’) and modern empirical Y. pestis plague data, most of which is derived from the Indian and Chinese plague outbreaks of A.D. 1900615 years. Previous works have noted apparent differences in seasonal mortality peaks during Black Death outbreaks versus peaks of bubonic and pneumonic plagues attributed to Y. pestis infection, but have not provided spatiotemporal statistical support. Our objective here was to validate individual observations of this seasonal discrepancy in peak mortality between historical epidemics and modern empirical data.

Research has extensively documented that a small number of Emergency Department (ED) patients make disproportionate use of emergency services (Okin et al., 2000). The term “Frequent Users” is used to describe patients who utilize ED services more than four times per year (Byrne et al., 2003). Frequent Users are often times patients with chronic medical, mental health, alcohol and drug problems, as well as other psychosocial issues (Fulde & Duffy, 2006). Past research has suggested that second language acquisition has a beneficial effect on the development of inhibitory control processes in children and adults. This has been referred to as the "bilingual advantage" and is most commonly quantified using the Simon task. Whether the bilingual advantage extends to precognitive mechanisms has not yet been examined. The goals of this study were to examine the bilingual advantage in university students; and to examine whether the bilingual advantage extends to the precognitive filtering mechanism of sensorimotor gating. Suicide is a deliberate act of annihilation against one's self due to a crisis of problem solving. Far too many youth are dying by their own hands resulting in suicide being the third leading cause of death for 10-19 year olds (CDC, 2005). Suicide is particularly problematic for adolescents due to the impulsivity inherent in adolescent development. Since 1980, the national suicide rate of African American youth has increased by 114 percent (CDC). Attention-Deficit/Hyperactivity Disorder (ADHD) is one of the most prevalent childhood disorders affecting 3 to 5% of school populations in the United States and other countries. Due to the behavioral and/or academic challenges of children with ADHD, they are at risk for grade retention, dropping out of high school, and teenage delinquency, which can lead to negative consequences in society. Children with ADHD are found in every school setting, including parochial schools. Service learning programs are becoming a part of curricula in universities throughout the United States. The University of Arkansas, Fayetteville, (UAF) established a service learning program that targeted the educational, health, social, and agricultural needs of a community. The focus of this research aimed to provide students, faculty, community members, school officials, and students with a template for crop budgets. These crop budgets are used to evaluate the costs and returns of producing multiple crops at a school. The objective of this chapter is to summarize the current standards of pain management in minor sports-related musculoskeletal injuries. This chapter also addresses the topical form of non-steroidal anti-inflammatory drug as an effective pain management option in an outpatient setting. Design: Quantitive systematic review of randomized controlled trials. Methods: The data was obtained through literature review of articles published in the last 10 years. Sitting is one of the first developmental milestones that an infant achieves. Thus measurements of sitting posture present an opportunity to assess sensorimotor development at a young age, in order to identify infants who might benefit from therapeutic intervention, and to monitor the efficacy of the intervention. Premarital partnerships among youth, including those that do not involve sexual relations, are widely discouraged in India. There is a widespread perception that youth do not find opportunities to mix and form romantic relationships and that premarital sex is negligible. These kinds of perceptions are often cited to argue against the provision of sex or family life education and sexual and reproductive health services to unmarried young people. The article discusses the importance of water treatment in providing quality and safe drinking water. A historical background of the U.S. federal regulation of drinking water quality is presented. It was found by the Public Health Service in 1969 that only 60 percent of the systems surveyed delivered water that met all the Public Health Service standards. Filtration and chlorination are also cited as an effective treatment techniques for protecting U.S. water supplies from harmful microbes. An overview of the process of water treatment is presented.

The purpose of this study is to analyze the International Olympic Committee’s (IOC) host city site selection process. The recent Beijing Olympics brought to the surface the political implications of the host city selection. Have the past IOC site selections been politically influenced? What factors do the IOC members consider to make their decision? I use data provided by Paul Poast from the University of Michigan that includes data from every Olympic candidate city since the 1960 Roman Games. Long QT syndrome (LQTS) is a life-threatening inherited cardiac condition. It is caused by a disturbance in the ion channel genes that control cardiac repolarization, resulting in ventricular arrhythmias that can lead to syncope, sudden cardiac arrest (SCA), or sudden cardiac death (SCD). Most deaths occur in children, adolescents, and young adults. Although congenital LQTS occurs in 1/3000 to 1/5000 individuals, to date, little research has been conducted on the psychosocial effects of LQTS. Mothers who are confronted with a new diagnosis of their children’s LQTS are likely to have never heard of the disease prior to diagnosis, and few …
