Rehabilitation Practice and Science
Phenylketonuria (PKU) is a rare amino acid metabolism genetic disorder. Tetrahydrobiopterin (BH4) metabolism defects cause variant PKU. BH4 coenzyme deficiency disables phenylalanine metabolism into tyrosine, causing brain damage and intellectual impairment. Patients follow a low-phenylalanine diet for life, increasing the risk of obesity, hypertension, and metabolic syndrome. A 24-year-old male patient with BH4 deficiency underwent a pulmonary function test and cardiopulmonary exercise test (CPET). Forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) were within normal values, with an 80.68% ratio. Academy (AA) is a seven/eight-week program implemented by select University of Nevada, Las Vegas Doctorate of Physical Therapy (DPT) students to promote health and wellness at a Title I elementary school and help bridge the health education gap that underserved youth populations may be facing. Increasing numbers of people suffer from obesity and severe mental illness co-morbidly. If these conditions remain untreated, they can harm an individual's health and well-being. Unfortunately, there are very few programs currently available that target both management and improvement of these conditions simultaneously. Objective: To evaluate the association between baseline cognitive function, intervention dropout, adherence and 3-month weight loss (WL) when controlling for confounding demographic variables.
Methods: 107 (Mage = 40.9 yrs.), BMI in the overweight and obese range (BMI = 35.6 kg/m2), men (N = 17) and women (N = 90) completed a 3-month WL intervention. Participants attended weekly behavioral sessions, comply with a reduced calorie diet, and complete 100 min of physical activity (PA)/wk. PURPOSE:
To identify the accuracy of Body Mass Index (BMI) to categorize body weight in a sample of children with spina bifida and Down syndrome as compared to typically developing peers.
METHODS:
A secondary analysis of 32 children with spina bifida, Down syndrome or no chronic illness. A calculated BMI was plotted on the Centers for Disease Control and Prevention age- and sex-specific BMI growth charts to determine each child’s weight status. Background: Asthma affects approximately 6 million children in the United States and can greatly impact quality of life and occupational engagement. Although occupational therapists are well-equipped to address participation limitations, insufficient evidence exists to support the role of occupational therapists in asthma treatment.

BACKGROUND. Beige adipose tissue is associated with improved glucose homeostasis in mice. Adipose tissue contains β3-adrenergic receptors (β3-ARs), and this study was intended to determine whether the treatment of obese, insulin-resistant humans with the β3-AR agonist mirabegron, which stimulates beige adipose formation in subcutaneous white adipose tissue (SC WAT), would induce other beneficial changes in fat and muscle and improve metabolic homeostasis.

Introduction: Obesity is linked to altered activation in reward and control brain cir-cuitry; however, the associated brain activity related to successful or unsuccessful weight loss (WL) is unclear.
Methods: Adults with obesity (N = 75) completed a baseline functional magnetic res-onance imaging (fMRI) scan before entering a WL intervention (ie,3-month diet and physical activity [PA] program). We conducted an exploratory analysis to identify the contributions of baseline brain activation, adherence behavior patterns, and the asso-ciated connections to WL at the conclusion of a 3-month WL intervention. Nineteen overweight or obese females completed three 10-minute experimental trials including a self-selected pace shallow water walking trial, a matched heart rate response land walking trial, and a self-selected pace land walking trial. Energy expenditure (kcal·min-1)was computed from expired gases assessed via indirect calorimetry. Results showed energy expenditure was lower (p= 0.046) during shallow water walking (6.46 ± 1.38 kcal·min-1) compared to matched heart rate response land walking trial (7.26 ± 1.29 kcal·min-1), with no significant difference in between shallow water and self-selected pace land walking (6.92 ± 1.61 kcal·min-1). …

Background
Obesity prevalence is increased in children with developmental disabilities, specifically in children with spina bifida and Down syndrome. Energy expenditure, a critical aspect of weight management, has been extensively studied in the typically developing population, but not adequately studied in children with developmental disabilities.
Objective
Determine energy expenditure, fat-free mass and body fat percentile and the impact of these findings on recommended caloric intake in children with spina bifida and Down syndrome.

Postural control is a key aspect in preventing falls. The aim of this study was to determine if obesity affected balance in community-dwelling older adults and serve as an indicator of fall risk. The participants were randomly assigned to receive a comprehensive geriatric assessment followed by a longitudinal assessment of their fall history. The standing postural balance was measured for 98 participants with a Body Mass Index (BMI) ranging from 18 to 63 kg/m2, using a force plate and an inertial measurement unit affixed at the sternum. Objectives: The purpose of this study was to compare the effects of resistance training (RT) on body composition, muscle strength, and functional capacity in elderly women with and without sarcopenic obesity (SO).
Methods: A total of 49 women (aged $60 years) were divided in two groups: without SO (non-SO, n=41) and with SO (n=8). Both groups performed a periodized RT program consisting of two weekly sessions for 16 weeks. According to the World Health Organization (WHO, 2017), more than 1.9 billion adults age 18 and older are overweight and over 650 million are obese. Data collected through the Centers for Disease Control indicates the prevalence of U.S. adults who are overweight and obese is estimated at 36.5% and the national childhood obesity rate is more than 12.7 million (17%), with approximately one in six school-aged children reaching the obesity level (Ogden, Carroll, Fryar, & Flegal, 2015). Background: Obesity and poor health habits impact youth’s health and occupational participation. Occupational therapy’s role in preventing and treating obesity continues to emerge in the research literature. This article explores the impact of a community-based program emphasizing health and wellness for female youth.
Methods: Five girls 11 to 13 years of age participated in the healthy occupations program. Before and after the program, the participants engaged in an individual semi-structured interview and completed the Canadian Occupational Performance Measure and the CATCH Kids Club Questionnaire. Eighty one subjects were examined for the relationship between BMI, body fat, arterial oxygen saturation and arterial PO2 with the subjects at rest and after 5 minutes of walking on a treadmill ergometer at 3 mph at a 3% grade. They had BMIs between 19 and 50. All subjects were free of cardiovascular disease and had normal blood pressure making it safe for them to participate in mild exercise. They were all able to walk for at least 10 minutes without cardiovascular complications and were not taking any medications that altered the autonomic nervous system. The age was between 22 …

The effects of obesity on the body are complex and numerous especially when combining the effects on an already multi-faceted and multi-systems action such as human ambulation. This study summarizes a wide range of research performed, which investigated the effects of obesity on human ambulation. The effects are broken down into categories including osteoarthritis, replacement surgery, center of mass, stepping cycle, muscular activation, and specifics of the hip, knee and ankle joints as well as foot changes. Purpose: Rural women have high prevalence of obesity and prehypertension. Obesity, if associated with poor physical function, may have implications for applying activity guidelines for women volunteering for lifestyle modification. This study examined associations of body mass index (BMI) and percent body fat with measures of 1-mile walk time, post-walk perceived exertion, and 10-repetition chair stands in rural women ages 40-69.
Methods: Cross-sectional baseline data were collected using standardized methods from 289 rural women with prehypertension who volunteered for a lifestyle clinical trial for reducing blood pressure. ANOVAs and linear regression were used for analysis.

Resistance exercise has great potential to aid in the management of breast cancer-related lymphedema (BCRL), but little is known regarding the acute response of performing resistance exercises with the affected limb. Obesity and reduced active leisure involvement are prevalent problems among core members with developmental disabilities living at L’Arche Tahoma Hope. As a result, core members may be less likely to participate in meaningful activities. Due to a variety of factors, L’Arche experienced difficulty with ways to increase everyday active leisure. A dance DVD and accompanying manual were created to help ameliorate the prevalence of obesity among core members and provide a fun, active leisure option. PURPOSE: The present case study examined the potential use of the AlterG Anti-Gravity Treadmill as treatment modality for extreme obese individuals. The special effect of the treadmill reduces ground reaction forces during walking through the use of lower body positive pressure. METHODS: One extremely obese (BMI 69.2) Caucasian 44 year-old female utilized the AlterG Anti-Gravity Treadmill during a 14 wk walking program to pilot test the protocol. RESULTS: At the conclusion of the study, exercise tolerance time increased 3-fold (initially 20 min increased to 65 min) while caloric expenditure increased 4-fold (initially 91.0 kcal/session increased to 442.7 kcal/session) without an …

The purpose of this mixed methods study was to describe the relationship between self-efficacy and social support among overweight and obese adolescents at a weight-management camp. Twenty-three participants completed a pre- and post-camp questionnaire based on modified versions of the GSE Scale and the SSQSR. Additionally, seven females and six males engaged in focus group discussion at the beginning and at the end of camp. Statistical analysis of the modified GSE Scale and SSQSR surveys was completed. BACKGROUND: This study investigated the associations of fitness and fatness with metabolic syndrome in rural women, part of a recognized US health disparities group.
METHODS: Fitness, percentage body fat, BMI, and metabolic syndrome criteria were assessed at baseline in 289 rural women with prehypertension, ages 40-69, enrolled in a healthy eating and activity community-based clinical trial for reducing blood pressure.
RESULTS: Ninety (31%) women had metabolic syndrome, of which 70% were obese by BMI (≥30 kg/m²), 100% by percentage body fat (≥30%), and 100% by revised BMI standards (≥25 kg/m²) cited in current literature. Hierarchical logistic regression models, adjusted for …
