Kenya: Identifying Rtis Remain Problematic: Prevention Is Essential, Frontiers In Reproductive Health

Since 1990, the Nakuru Municipal Council (Kenya) has implemented a multifaceted program to reduce the incidence of reproductive tract infections (RTIs), especially those that are sexually transmitted, including HIV/AIDS. Staff in the Council’s five health clinics use syndromic management guidelines, based on clients’ reported symptoms and clinical signs, to identify clients with RTIs. In 1998, the Population Council conducted a study to assess the accuracy of syndromic management and determine the best ways to integrate RTI management into existing antenatal (ANC) and family planning (FP) services. The various initiatives to eradicate FGC in Mali over the past two decades have had little impact on this traditional practice. This study assessed the use of health personnel to combat FGC, as recommended by the World Health Organization. Conducted in 1998 by the Association de Soutien au Developpement des Activites de Population, a nongovernmental organization, and the Ministry of Health, the study covered 14 urban and rural health centers in Bamako and Segou. The Population Council has supported a series of studies to improve the quality of postabortion care (PAC) in Egypt. A 1994 pilot study in two Egyptian hospitals showed that upgrading PAC and training physicians in manual vacuum aspiration (MVA), infection control, and counseling led to significant improvements in the care of postabortion patients. The 1997 study, conducted by the Egyptian Fertility Care Society with support from the Population Council, sought to institutionalize improved postabortion medical care and counseling procedures in ten hospitals. Until recently, the practice of female genital cutting (FGC) has been nearly universal in Egypt. However, a 1998 national survey found the first signs of a decline in the practice among adolescents since 1994. Nongovernmental organizations (NGOs) working in community development, health, and women’s rights have played a leading role in advocating eradication of FGC in Egypt. To document and assess the impact of anti-FGC programs, the Population Council conducted an assessment from August 1999 to February 2000. To raise public awareness of reproductive health (RH) issues, the Population Council’s Frontiers project and the Futures Group’s Policy project jointly organized four press briefings and provided background materials to key journalists from Arabic newspapers and magazines. From May 1999 to June 2000, project staff worked closely with 20 Egyptian journalists, including editors of women’s pages and senior editors. The press briefings covered youth, marriage patterns, contraceptive technology, and menopause. About 94 percent of Malian women aged 15–49 have experienced female genital cutting (FGC). In Mali, FGC is associated with serious gynecological and obstetric complications. In 1998, the National Center of Scientific and Technological Research of the Mali Ministry of Secondary and Higher Education and Scientific Research conducted an evaluation of programs to eradicate FGC. The study assessed the work of three national nongovernmental organizations working in Bamako and five regions of Mali. These NGOs had attempted to persuade traditional practitioners of FGC (“excisors”) to abandon the practice. All three NGOs employed outreach workers to educate excisors and community members …

This study was carried out to describe the major NGO community-based distribution (CBD) programs in Ghana, and to assess in some detail the functioning, quality of care, and performance of the Planned Parenthood Association of Ghana (PPAG) program. The study showed that national coverage by the NGO programs is extensive—virtually all the 110 districts in the 10 regions have at least one program. However, coverage within districts is sparse, as each program covers only a few communities. A patient’s adherence to self-management therapies in diabetes strongly influences health outcomes, health care utilization, and ultimately health care costs. Engaging in regular exercise therapy reduces risk factors associated with type 2 diabetes. The purpose of this randomized prospective study was to determine if a client-centered approach (CCA) toward diabetes education, verses a traditional-directive approach (TDA), would improve exercise participation, perceived control, intention to exercise, and glycemic control in patients with type 2 diabetes.

A total of 756 female adolescents, aged 13 to 18 years, from five public high schools in Antelope Valley Union High School District located in North Los Angeles County participated in this study between February 1999 and March 1999. The ethnic composition was 53.3% White/Anglo Americans, 19.3% Hispanics or Latinos and 10.2% Black/African Americans. The constructs of the Theory of Planned Behavior (attitude, subjective norm, and perceived behavioral control and their underlying beliefs) were assessed to determine the predictors of low-fat milk and regular soda consumption. A cross-sectional design with group-administered questionnaires was utilized.

Pregnancy is the single most important reason why girls fail to complete secondary school in the Caribbean. The Women's Centre of Jamaica Foundation (WCJF) Program was established in 1978 to enable adolescent mothers 16 years and under to return to school, and to delay second pregnancies until completion of their education.

This historical cohort study assessed the effect of the WCJF Program on the incidence of repeat pregnancies over the period 1995-1998, and on the educational attainment of program participants. Lead poisoning is one of the most common pediatric health problems in the United States. A direct result of industrialization, the toxic substance lead has contaminated our environment. Parents of young children should be aware of the hazards of lead, the environmental sources, the need for annual screening. the implications of screening results. and steps to reduce the chances of exposure. A 42-year-old logger (the victim) was killed when he was struck by a log rolling off a loaded truck at a sawmill. At about 6:10 a.m., the victim and the truck driver arrived at the sawmill with a load of logs on the 20-foot flatbed truck. The stack of logs was rounded above the height of the 53-inch standards and secured by only one chain on the middle of the load. In December 1998, a 31-year-old male (the victim) sawmill employee was killed when struck by a log that rolled off the top of the truck. He was preparing the truck to be unloaded when the log rolled off. While most of the logs received by the sawmill were delivered by independent loggers, the company did own one boom truck that their employees used to pick up timber. This was the truck the victim was driving the day of the incident. At the time of the incident, he was working alone.

Literature Review: Challenging Hiv-Related Stigma And Discrimination In Southeast Asia: Past Successes And Future Priorities, Joanna Busza

HIV and AIDS

Pervasive stigma has surrounded HIV/AIDS since the beginning of the pandemic. In Southeast Asia, as elsewhere, it has been accompanied by discrimination, affecting transmission patterns and access to care and support. This paper describes the regional experience of stigma and discrimination and offers a review of community-based interventions that have attempted to reduce them. The evidence presented comes primarily from unpublished literature and anecdotal evidence gained through interviews with project staff throughout the region.

HIV and AIDS

According to a 1998 UNAIDS/WHO report, HIV infection among women is the fastest-growing segment of the AIDS pandemic, especially in developing countries. Unfortunately, the HIV-prevention strategies put forward by the public health community—monogamy, condom use, and treatment of sexually transmitted infections—are not feasible for many women. Women urgently need a means within their personal control to protect themselves against HIV. Recognizing this need, a number of international and nongovernmental organizations and government agencies have called for the development of microbicides, products that women could use vaginally to prevent infection with sexually transmitted infections, particularly HIV.
