A 32-year-old male was killed when he was caught in a rotating power take-off (PTO) shaft. He had been involved in farming for many years and often helped his friends who owned a dairy farm. The farm worker was helping one of the owners of the farm load corn from the silage wagon into the feeding trough. Both the farm worker and the owner were experienced in operating the equipment on the farm and had worked together on this type of task before. Outpatient consultation-liaison (C-L) psychiatry clinics are valuable settings for research and teaching endeavors. However, little is known about psychiatric symptoms and health status of persons treated in such settings. In this study, 80 persons seen in an outpatient C-L psychiatry clinic were compared with 100 persons seen in a mood disorder clinic on a variety of self-report instruments. Outpatient C-L clinic patients were found to have significantly poorer health status than mood clinic patients on the following domains as measured by the RAND instrument: general health perception, pain, physical functioning, and role functioning due to physical problems. Clinic-Based Investigation Of The Typology And Self-Reporting Of Fgm In Egypt, Population Council

This clinic-based study of female genital mutilation (FGM) was designed and carried out by the Egypt Fertility Care Society to gather detailed information on the types of FGM practiced in Egypt. The study involved interviews with clinic clients to elicit information about their experience and attitudes concerning FGM, and gynecological examinations by specially trained OB/GYN physicians. In all, 93% of the women in this study were found to have some type of FGM. The study's findings were incorporated in the final report of the 1995 Egyptian Demographic and Health Survey, with the aim of contributing to the scientific understanding of …

Most national MCH-FP programs in sub-Saharan Africa are shifting their programmatic strategies toward a broader reproductive health service approach that includes integrating STI and HIV/AIDS services into existing MCH-FP programs. This is based on the belief that integration will lead to more cost-effective and better-quality service, and maximize use of existing resources. There is an assumption that the basic infrastructure for the MCH-FP programs can absorb the extra demands of the STI/HIV/AIDS services with minimal additional inputs and that staff providing MCH-FP services require only minimal training to be able to provide STI/HIV/AIDS services. This report reviews the literature on men and family planning (FP) in Bangladesh. A variety of topics are covered: male fertility preferences, opinions about male methods of contraception, family planning (FP) decision-making, and recent efforts to involve Bangladeshi men in FP programs. Men’s desired family size in Bangladesh is small; only one-third of respondents in the most recent DHS survey wanted more than two children. The same survey revealed that knowledge of at least one modern method of FP is practically universal and that attitudes toward contraception are overwhelmingly favorable. The family planning (FP)/maternal and child health (MCH) program in Bangladesh has achieved success in the recent past, mostly through a large-scale government service-delivery system with support and cooperation from donors and nongovernmental organizations. There is concern about the financial and social sustainability of the program. Other issues include achievement of replacement-level fertility within a stipulated period and improvement of MCH-FP service quality. India, the second most populous country in the world, experienced an accelerated decline in fertility during the 1980s. As a result, India’s total fertility rate (TFR) in 1993, 3.5 births per woman, is below the level presumed in the population projections of the United Nations and the World Bank. This favorable development has occurred as India is attempting to reorient its family planning program to focus on the reproductive health of women and the health and welfare of children. We are now in the second decade of the AIDS epidemic in the United States. As of October 31, 1995, a total of 311,381 U.S. citizens had t died from AIDS, another 189,929 had been diagnosed with AIDS (1), and it is estimated that nearly l million persons are infected with HIV, the virus that causes AIDS (CDC). Despite the best efforts of biomedical researchers, we still have neither a cure nor a vaccine to prevent this deadly disease. Although occupational epidemiological studies and animal experimentation provide strong evidence that radon-222 (222Rn) progeny exposure causes lung cancer, residential epidemiological studies have not confirmed this association. Past residential epidemiological studies have yielded contradictory findings. Exposure misclassification has seriously compromised the ability of these studies to detect whether an association exists between 222Rn exposure and lung cancer. Misclassification of 222Rn exposure has arisen primarily from: 1) detector measurement error; 2) failure to consider temporal and spatial 222Rn variations within a home; 3) missing data from previously occupied homes that currently are inaccessible; 4) failure to link …

Increasing attention has been paid to quality of care (QOC) in family planning (FP) during the last few years, and there have been several initiatives to strengthen QOC in FP in the developing world. This paper reviews the quality of the FP program in India’s public and private sectors, and examines six elements: choice of methods, information given to clients, technical competence, interpersonal relations, continuity of care and follow-up, and appropriate constellation of services. However, while there is strong support for public policies aimed at economic recovery and human resource development in the Philippines, there is still a lack of consensus on the need for public policy to moderate population growth and on the role of a government-sponsored family planning program in overall population and development activities. This paper examines systemic contexts for symbol use among the Maa-speaking Il Chamus in the Lake Baringo region of west-central Kenya. The systemic context for symbols and material culture consists of the environmental constraints and behavioral responses that characterize pastoralist life in East Africa. The author's interest in this problem developed in response to Ian Hodder’s work among the Il Chamus, Pokot, and Tugen in the Baringo District. Unlike Hodder, however, the author argues that symbols and their use in East Africa can be more productively explained from a materialist perspective. In the past eight years, many operations research (OR) programs were implemented in Indonesia to increase the availability, accessibility, and acceptability of services desired by family planning (FP) users. OR has also been employed to improve quality and sustainability by studying the performance, efficiency, and effectiveness of services delivered by providers. On its face, Daubert v. Merrell Dow Pharmaceuticals was about as easy a case as the Supreme Court gets. The plaintiffs claimed that their birth defects were caused by the anti-nausea drug Bendectin, which their mothers had used during their gestation. In response to a motion for summary judgment by the defendant, the plaintiffs presented affidavits of eight expert witnesses who offered their opinions - based on a variety of studies - that Bendectin was indeed the culprit. The federal district court that heard the motion granted summary judgment to the defendant, and the Ninth Circuit affirmed. Both lower courts …

The Indonesia Norplant® Removal Assessment Study was prepared for the World Bank and USAID/Indonesia. All women interviewed had Norplant (contraceptive implant) inserted for five or more years before the interview. For the entire sample, 8.2 percent had not yet had Norplant removed at the time of the interview. This report indicates a very high continuation rate for each Norplant insertion cohort from 1987 through 1991. Almost all Norplant users continue through the fourth year. Between the end of the fourth year and the end of the fifth year, there is a sharp drop in use from 90 percent to 66 …

In 1994, the Population Council, Manila, conducted an operations research (OR) project entitled “DMPA Monitoring and Follow-up Studies.” The activity was undertaken to provide accurate and timely support to the DMPA Reintroduction Program of the Department of Health. Commonly known as Depo-Provera, DMPA stands for Depo-Medroxy Progesterone Acetate, an extremely effective injectable contraceptive given every three months. The global nature of the threat posed by new and reemerging infectious diseases will require international cooperation in identifying, controlling, and preventing these diseases. Because of this need for international cooperation, international law will certainly play a role in the global strategy for the control of emerging diseases. Recognizing this fact, the World Health Organization has already proposed revising the International Health Regulations. This article examines some basic problems that the global campaign against emerging infectious diseases might face in applying international law to facilitate international cooperation. A total of 250 new women teachers participated in a longitudinal study of the influence of negative affectivity (NA) on the relation of self-report work-environment measures to psychological outcomes. Three "neutrally worded" work-environment measures were specially constructed to minimize confounding with NA. The work-environment measures were moderately related to postemployment depressive symptoms, job satisfaction, and, among Whites but not among a principally Black and Hispanic subsample, motivation. Correlation and regression coefficients were largely unchanged when the preemployment psychophysiologic symptoms scale and the Center for Epidemiologic Studies-Depression Scale (L. S. Radloff, 1977), factors that tap NA, were controlled. Findings suggest NA …
