The Medicine Forum
Gestational trophoblastic neoplasia (GTN) refers to a group of malignant conditions that develop due to abnormal fertilization causing abnormal proliferation of tissue. GTN is primarily treated with surgical evacuation of the underlying proliferative tissue. Approximately half of cases of GTN arise from molar pregnancy1. GTN include invasive moles, choriocarcinomas, placentalsite trophoblastic tumors and epithelioid trophoblastic tumors. The most common risk factors associated with GTN are prior molar pregnancy, advanced maternal age (>40 years of age), and Asian and Native American ancestry2-4. Following evaluation of a molar pregnancy, a post-molar GTN is diagnosed based on the …

Opioid Free Multimodal Analgesia Versus Opioid Based Analgesia In Bariatric Surgery Outcome, Zeinab Ibrahim, Gihan Eldosoky, Kamal Abosonna
Al-Azhar International Medical Journal
Background: Combined Opioid-free analgesia (OFA) and thoracic epidural (TE) by ultrasound is the technique that may have an enhanced recovery after surgery (ERAS), decrease perioperative opioid consumption and more improved positive outcomes than balanced general anesthesia with use of opioid after bariatric surgery. The Opioid free analgesia includes local anesthetics (Na channel blocker), alpha-2 receptor agonists (dexmetedomidine), NMDA antagonists (ketamine, lidocaine, magnesium sulfate), anti-inflammatory drugs (NSAID) and dexamethasone. Aim: evaluation the effect of opioid free analgesia with thoracic epidural as a multimodal analgesia versus balanced general analgesia on outcome of bariatric surgery on total opioid requirements, pain scores, and complications. …

Comparative Study Between Bipolar Electro-Cautery And Knot-Tying Technique In Total Thyroidectomy, Ahmed Hegab
Comparative Study Between Bipolar Electro-Cautery And Knot-Tying Technique In Total Thyroidectomy, Ahmed Hegab
Al-Azhar International Medical Journal
Abstract Introduction: Thyroidectomy could be considered as the commonest neck surgery in surgical practice. Proper thyroidectomy requires careful and meticulous devascularization of the gland with preservation of surrounding important structures. Haemostasis during thyroidectomy achieved by suture ligation, electro-cautery or vessel sealing devices. The traditional knot-tying method requires carful dissection which is time consuming and may lead to injury of laryngeal nerves or impairment of blood supply to parathyroid glands. Bipolar electro-cautery has precise delivery of electric current between its tips with no or limited thermal dispersion to surrounding structures. Advanced vessel sealing devices not always available and requires special disposable …

For the diagnosis of HG, there must be the existence of 3 or more vomiting episodes throughout the day, weight losing of more than 5%, and ketonuria. Studies have recently suggested that there is a correlation between emesis of gravidarum and HG with Helicobacter pylori (H. pylori) infection. Serologically positive H. pylori infections were demonstrated in hyperemesis patients. Aim and objectives; the current work aimed to detect the correlation between H. pylori infections and HG throughout gestation. Subjects and methods; …
