Background: Patients are often kept NPO (nil per os), or “nothing by mouth,” longer than necessary before cardiac catheterization procedures. Local Problem: Patients scheduled for cardiac catheterization procedures receive instructions to begin fasting at midnight on the day of their procedure regardless of their scheduled procedural start time. This directive results in patient dissatisfaction as patients are fasting longer than current guideline recommendation.
Methods: The facility updated its order set to align with the American Society of Anesthesiologists (ASA) fasting guidelines for scheduled cardiac catheterizations. To evaluate patient satisfaction, a 6-item survey was administered to 169 individuals undergoing cardiac catheterization. …

Background: With half of healthcare workers reporting burnout, job dissatisfaction and burnout are nursing epidemics. Staffing shortages and heavy workloads are leading dissatisfiers. Acuity-based assignments (ABAs) can predict patient needs and equitably distribute workload among nurses.
Local problem: Nurses of a 21-bed inpatient rehabilitation facility identified burnout related to workload. There was no standard practice for creating assignments, and the existing acuity measure was not rehab-specific.

การศึกษานี้เป็นการวิจัยกึ่งทดลองแบบวัดเฉพาะหลังการทดลองมีกลุ่มเปรียบเทียบ (Posttest-only Control Group Design) โดยมีวัตถุประสงค์เพื่อประเมินผลของโปรแกรมการส่งเสริมความสุขสบายต่อคุณภาพการฟื้นตัวหลังการผ่าตัดศัลยกรรมกระดูกในเด็กวัยเรียน กลุ่มตัวอย่างเป็นเด็กอายุ 7–12 ปี จำนวน 40 ราย ที่เข้ารับการผ่าตัดใหญ่ศัลยกรรมกระดูกส่วนรยางค์และพักรักษาในหอผู้ป่วยกุมารศัลยกรรมกระดูก สถาบันสุขภาพเด็กแห่งชาติมหาราชินี โดยแบ่งเป็นกลุ่มทดลอง 20 ราย ซึ่งได้รับโปรแกรมการส่งเสริมความสุขสบาย และกลุ่มควบคุม 20 ราย ซึ่งได้รับการพยาบาลตามปกติ เครื่องมือวิจัยประกอบด้วย (1) โปรแกรมการส่งเสริมความสุขสบายแบบองค์รวมที่พัฒนาตามแนวคิดความสุขสบายของ Kolcaba (1992) (2) แบบประเมิน Children’s Comfort Daisies และ (3) แบบประเมินคุณภาพการฟื้นตัวหลังผ่าตัด 15 (Quality of Recovery–15: QoR–15) เครื่องมือทั้งหมดผ่านการตรวจสอบคุณภาพ โดยมีค่าดัชนีความตรงตามเนื้อหา (CVI) เท่ากับ 1.00, 1.00 และ 0.96 ตามลำดับ และแบบประเมิน QoR–15 มีค่าความเที่ยง Cronbach’s alpha เท่ากับ 0.86 การวิเคราะห์ข้อมูลใช้สถิติ Independent t-test ที่ระดับนัยสำคัญทางสถิติ .05 ผลการวิจัยพบว่า เด็กในกลุ่มทดลองมีคะแนนคุณภาพการฟื้นตัวหลังผ่าตัดเฉลี่ยในช่วง 48 และ 72 ชั่วโมง สูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p < .001) โดยที่ 48 ชั่วโมงหลังผ่าตัด กลุ่มทดลองมีคะแนนเฉลี่ย 130.80 (SD = 16.43) ขณะที่กลุ่มควบคุมมีคะแนนเฉลี่ย 101.45 (SD = 21.13) และที่ 72 ชั่วโมงหลังผ่าตัด กลุ่มทดลองมีคะแนนเฉลี่ย 140.90 (SD = 10.76) ขณะที่กลุ่มควบคุมมีคะแนนเฉลี่ย 118.20 (SD = 17.19) ผลลัพธ์ดังกล่าวแสดงให้เห็นว่าโปรแกรมการส่งเสริมความสุขสบายแบบองค์รวมมีประสิทธิผลในการส่งเสริมคุณภาพการฟื้นตัวหลังการผ่าตัดศัลยกรรมกระดูกในเด็กวัยเรียนอย่างมีนัยสำคัญทางสถิติ

ปัจจัยทำนายพฤติกรรมการป้องกันวัณโรคในผู้สัมผัสร่วมบ้านของผู้ป่วยวัณโรค, นงนภัส เกษรทอง

การศึกษาครั้งนี้เป็นการวิจัยแบบบรรยายเชิงทำนาย เพื่อศึกษาความสามารถของปัจจัยคัดสรรในการทำนายพฤติกรรมการป้องกันวัณโรคในผู้สัมผัสวัณโรคร่วมบ้าน โดยปัจจัย ประกอบด้วย 1) ความรู้เกี่ยวกับวัณโรค 2) ทัศนคติต่อวัณโรค 3) แรงจูงใจในการป้องกันวัณโรค 4) การรับรู้ความสามารถของตนเองในการป้องกันวัณโรค และ 5) การรับรู้อุปสรรคเกี่ยวกับการป้องกันวัณโรค กลุ่มตัวอย่างเป็นผู้สัมผัสวัณโรคร่วมบ้าน จำนวน 111 คน ซึ่งเลือกอย่างเจาะจงจากโรงพยาบาล 3 แห่งในเขตกรุงเทพมหานคร เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามชนิดตอบด้วยตนเอง วิเคราะห์ด้วยสถิติเชิงพรรณนาและการถดถอยพหุคูณ ผลการวิจัย พบว่า พฤติกรรมการป้องกันวัณโรคในผู้สัมผัสวัณโรคร่วมบ้านโดยรวม อยู่ในระดับดี (Mean = 44.2, SD = 6.5) ปัจจัยที่สามารถทำนายพฤติกรรมการป้องกันวัณโรคอย่างมีนัยสำคัญทางสถิติ ได้แก่ ทัศนคติต่อวัณโรค (b = .374, p<.05) การรับรู ้ความสามารถของตนเอง ในการป้องกันวัณโรค (b = .244, p <.05) และ การรับรู ้อุปสรรคเกี ่ยวกับการป้องกันวัณโรค (b = .166, p <.05) ปัจจัยดังกล่าวสามารถร่วมทำนายพฤติกรรมการป้องกันวัณโรคได้ร้อยละ 32.8 อย่างมีนัยสำคัญทางสถิติ (R2 = .328, p <.05) อย่างไรก็ตามในการศึกษานี้ ความรู ้เกี ่ยวกับวัณโรค และแรงจูงใจในการป้องกันวัณโรค ไม่สามารถทำนายพฤติกรรมการป้องกันวัณโรคในผู ้สัมผัสวัณ โรคร่วมบ้าน

การวิจัยแบบกึ่งทดลองนี้มีวัตถุประสงค์ เพื่อศึกษาผลของโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชันต่อพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม เบาหวาน ความดันโลหิตสูง ไขมันในเลือด 1 โรคขึ้นไป ที่มีอายุตั้งแต่ 35 – 80 ปี คัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 23 คน จับคู่ทั้งสองกลุ่มให้มีลักษณะคล้ายคลึงกันในเรื่องอายุ จำนวนโรคร่วม จำนวนกลุ่มยาที่ใช้รักษาโรค เปรียบเทียบข้อมูลพื้นฐานด้วยสถิติ Chi-square test และ Fisher’s Exact Test กลุ่มตัวอย่างทั้งสองกลุ่มมีคุณลักษณะพื้นฐานที่ไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติในทุกตัวแปร (p > .05) กลุ่มควบคุมจะได้รับการพยาบาลปกติ ในขณะที่กลุ่มทดลองได้รับโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชัน เครื่องมือที่ใช้ในการวิจัย ได้แก่ แบบสอบถามพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารของผู้ป่วยที่มีภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม และแบบสอบถามประเมินความรอบรู้ด้านสุขภาพเกี่ยวกับพฤติกรรมการจัดการตนเองด้านการบริโภคอาหาร ทดสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน ได้ค่าดัชนีความตรงตามเนื้อหาเท่ากับ 0.92 และ 1.00 และค่าความเที่ยงเท่ากับ 0.91 และ 0.73 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และสถิตทดสอบค่าที ผลการวิจัยสรุปได้ดังนี้ 1. คะแนนเฉลี่ยพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม กลุ่มทดลองหลังได้รับโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชันสูงกว่าก่อนได้รับโปรแกรม ฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. คะแนนเฉลี่ยพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม ที่ได้รับโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชันสูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง เพื่อศึกษาผลของโปรแกรมสนับสนุนการให้ข้อมูลแบบรูปธรรม-ปรนัยต่อความวิตกกังวลของสมาชิกครอบครัวของผู้ป่วยโรคหลอดเลือดสมองครั้งแรกในห้องฉุกเฉิน โดยกลุ่มตัวอย่างในการศึกษาครั้งนี้คือ สมาชิกครอบครัวของผู้ป่วยโรคหลอดเลือดสมองครั้งแรกที่นำส่งผู้ป่วยมารับบริการที่ห้องฉุกเฉินของโรงพยาบาลสรรพสิทธิประสงค์ แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 30 คน โดยคัดเลือกตามแบบเฉพาะเจาะจงตามเกณฑ์ที่กำหนด โปรแกรมรูปธรรม-ปรนัย และแบบสอบถาม ผ่านผู้ทรงคุณวุฒิ 5 ท่าน ได้ค่าความเที่ยง 0.84 โปรแกรมการสนับสนุนการให้ข้อมูลแบบรูปธรรม–ปรนัยต่อความวิตกกังวล มีเครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล ประกอบด้วย ข้อมูลทั่วไปของผู้ป่วยโรคหลอดเลือดสมอง และแบบประเมินความวิตกกังวลขณะเผชิญ (The State Anxiety Inventory (STAI) Form Y) วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติทดสอบ Independent t-test และสถิติ paired t-test ผลการวิจัย: ผลการวิจัยพบว่าสมาชิกครอบครัวของผู้ป่วยโรคหลอดเลือดสมองที่ห้องฉุกเฉิน ในกลุ่มทดลอง ภายหลังได้รับโปรแกรมคะแนนความวิตกกังวลตํ่ากว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติ (p < .05) และคะแนนความวิตกกังวลของสมาชิกครอบครัวของผู้ป่วยในกลุ่มทดลองภายหลังได้รับโปรแกรมตํ่ากว่าในสมาชิกครอบครัวของผู้ป่วยได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติ (p < .05)

This Doctor of Nursing Practice (DNP) project explores the impact of implementing a Virtual Discharge Nurse (VDN) on patient length of stay (LOS) and discharge timing at Paoli Hospital within the Main Line Health system. Hospital discharges are often complex and can negatively impact patient flow and outcomes. This study highlights the urgent need for innovative discharge practices in healthcare and supports the potential benefits of integrating technology into nursing roles to improve patient care and operational efficiency. Marginalized populations, defined as a group of individuals who experience systematic disadvantages due to various social identity factors (Oxford Review, n.d.), experience uneven barriers to cognitive and mental health assessments, including socioeconomic, linguistic, and educational inequities, that contribute to misdiagnosis (Petersen et al., 2018, Saeed & Masters, 2021). This study investigates the feasibility of hologram-based patient simulations as an educational intervention for undergraduate nursing and graduate Nurse Practitioner students. Context: The university constitutes a demanding and competitive learning environment which can impact the well-being of students. The objective of this survey is to assess the burnout of students at the faculty of dentistry.
Materials and methods: A cross-sectional study was conducted via an anonymous questionnaire with 3 sections: socio-demographic data, lifestyle and habit, and burnout through the Maslach burnout inventory-student survey (MBI-SS) . The integration of virtual simulation and self-debriefing in nursing education offers promising approaches for enhancing students' competency in medication administration and dosage calculation. This study aims to explore undergraduate nursing students' perceptions and experiences with virtual simulation and self-debriefing, building on findings from a previously published RCT that demonstrated improved learning outcomes through these approaches. The US opioid epidemic crisis led to 700,000 deaths in 2019 alone. Poor adherence to medications for opioid use disorder (MOUD) strains individuals, families, and the healthcare systems, with rising relapses at the project site highlighting the need for evidence-based intervention. Studies show that contingency management (CM), a behavioral intervention that incentivizes desired behaviors, improves adherence to MOUD. Nurse-led education to inpatient boarding patients in the emergency department is essential to improve patient’s understanding with their care and nurse’s satisfaction surrounding communicating with their patients. Inpatient boarding in the emergency department is associated with poor patient outcomes, adverse events, and reduced communication between patient and nurse.

BACKGROUND: Newborn bathing is a common task performed in a labor and delivery unit of a hospital, a newborn’s bath temperature, timing and how long it is performed, are important metrics to consider. Due to a newborn’s immature thermoregulation, there are multiple implications that come with receiving a bath that is not an optimal temperature or that is too soon after birth. Background/Introduction: Employee satisfaction and patient satisfaction/experience are largely correlated and when one declines, evidence suggests that the other will be affected similarly (Copanitsanou et al., 2017). The Great Place to Work (GPTW), a nationally recognized designation, is measured using a standardized, commercially available staff survey to evaluate nurses’ satisfaction throughout the project site organization (Boo, 2024). Incarcerated individuals have a higher incidence of type 2 diabetes (T2D) than the general population and face unique challenges in managing their condition. These include limited access to nutritious food, disruptions in the healthcare continuity, and socioeconomic barriers such as unstable housing and unemployment.
